In a searchable-image PDF, the original page image remains intact and an invisible text layer is placed over it, so you can search, copy, and often improve accessibility without changing the page’s appearance. Accuracy depends mainly on the quality and layout of the image you give the OCR engine.
The most reliable process is: capture at an appropriate resolution, straighten and clean the pages, select the correct language and layout assumptions, run OCR, then inspect high-risk text such as numbers, names, headings, footnotes, tables, and columns. Keep the untouched scan as your master and correct the text layer or a separate transcript rather than repeatedly editing the image.

What OCR actually does to a scanned PDF
A scanned PDF page is usually a raster image, similar to a photograph. You can see the words, but the file may contain no characters that a search engine, screen reader, or copy-and-paste operation can identify. OCR analyzes those pixels and produces a character-and-word representation.
The recognition pipeline
Most OCR systems perform several linked tasks:
- Page analysis: The software identifies the page boundary, margins, text blocks, illustrations, rules, and likely reading regions.
- Text-line detection: It finds baselines and separates lines, even when the page contains multiple columns or mixed elements.
- Character and word recognition: Image patterns are matched to letters, digits, punctuation, and spaces using the selected language model.
- Layout reconstruction: Recognized text is assigned coordinates so it can be searched, copied, selected, or read in an approximate visual order.
- Text-layer embedding: The recognized characters are stored in the PDF as a hidden or selectable layer aligned with the original image.
The output is not a new scan. It is an interpretation laid over the scan, and the interpretation can be wrong even when the page looks perfect.

Searchable-image output versus replacing the page
Adobe’s searchable-image workflow keeps the original page image, may clean or deskew it, and adds an invisible text layer. Adobe describes the cleanup as modifying the image “for example, deskews it” before placing that layer over the page (Adobe PDF Services OCR documentation). The result normally looks like the scan but supports search and text selection.
Some workflows create a text-only PDF, a word-processing file, or a sidecar text file instead. Those formats can be easier to edit, but they may lose the original typography, page geometry, stamps, handwriting, or table alignment. If preserving the legal or visual record matters, retain the image-based PDF and treat extracted text as a separate representation.
What OCR cannot infer reliably
OCR does not know the author’s intended spelling, whether a faint mark is a comma or a speck, or whether a number is a date, account identifier, or page number. It estimates from visual evidence. A plausible-looking result can therefore contain silent errors in a name, decimal point, minus sign, superscript, or column order. OCR is a recognition step, not proof that every character is correct.

Why OCR accuracy varies
Capture resolution and focus
Resolution determines how many pixels describe each stroke. A low-resolution scan can merge adjacent letters, erase thin punctuation, and make serifs resemble noise. Blur, motion, camera perspective, and compression artifacts create similar problems. More pixels cannot restore detail that was never captured, but an adequate scan gives the recognizer enough evidence to distinguish characters.
Skew, rotation, and page geometry
If lines are slanted, upside down, curved, or photographed at an angle, line detection becomes harder. OCRmyPDF and Tesseract both identify skew and orientation as common recognition problems. Deskewing rotates the page so baselines are horizontal; orientation correction turns an upside-down or sideways page into a normal reading direction. Keep the original image because aggressive geometric correction can crop edges.
Background, contrast, and print defects
Yellowed paper, shadows near a book spine, bleed-through, stains, halftone photographs, and speckles can be mistaken for text. Conversely, over-aggressive thresholding can erase light characters. Cleaning should increase the distinction between foreground and background without deleting thin strokes. Adobe exposes deskew, descreen, background removal, and text sharpening controls. OCRmyPDF documents background cleanup, while Tesseract discusses binarization methods such as Adaptive Otsu and Sauvola in its quality guide (Adobe scanned-PDF settings; OCRmyPDF cookbook; Tesseract quality guide).

Language, type, and layout
Recognition models are language- and script-dependent. Selecting the wrong language can turn accented characters into different letters and reduce dictionary-based corrections. Mixed-language pages may need multiple language models, if the software supports them.
Typography also matters. Clear, consistently printed text is easier than decorative fonts, tightly joined characters, very small type, handwriting, or faded carbon copies. Layout assumptions matter just as much: a newspaper-style page, a form with boxes, a two-column report, and a table should not be interpreted as one uninterrupted paragraph.
Margins and borders
Tesseract warns that missing, excessive, or unusual borders can reduce recognition quality. A tightly cropped page may lose characters at the edge; a large dark scanner border may be mistaken for a region of text. Preserve a modest, even margin and crop only obvious scanner artifacts.
What DPI should you use for OCR?
DPI (dots per inch) describes the scan’s sampling density. The figures below are guidance for capture, not guarantees of a particular accuracy percentage.
| Situation | Guidance | Qualification |
|---|---|---|
| Adobe OCR input minimum | 72 dpi | Adobe states this as a minimum for its text-recognition options; it is not an ideal capture target. |
| Grayscale or color document | 300 dpi | Adobe’s 2025 settings documentation recommends 300 dpi for these scans. |
| Black-and-white document | 600 dpi | Adobe’s 2025 recommendation for black-and-white scans. |
| Tesseract practical baseline | At least 300 dpi | The Tesseract project says it works best on images with a DPI of at least 300 dpi. |
These values come from different products and purposes. A clean 300-dpi grayscale page can outperform a blurred 600-dpi photograph. If a scanner offers a true optical resolution, use it rather than a software-interpolated setting. For tiny type, degraded microfilm, or characters that remain indistinct at 300 dpi, a higher optical setting may help, but test a representative page before scanning an entire archive.
Adobe’s documented settings are available at its scanned-PDF settings reference, and Tesseract explains its resolution recommendation at Improving the quality of the output.

A reliable OCR workflow, step by step
- Preserve the master scan. Make a working copy of the PDF before deskewing, cleaning, resampling, or adding a text layer. Store the untouched file with a clear name and, where appropriate, a checksum or document ID. This gives you a visual authority to consult when a recognition result is disputed.
- Inspect a representative sample. Choose pages containing ordinary body text plus the hardest features in the set: tables, columns, stamps, faint pages, small type, and non-Latin or accented characters. A short pilot reveals whether the problem is capture quality, layout, language, or the OCR engine.
- Capture or recapture at a suitable DPI. Use 300 dpi for most grayscale or color documents and 600 dpi for black-and-white documents when following Adobe’s recommendations. Keep the page flat, in focus, evenly lit, and free of camera glare. Do not assume a higher number compensates for motion blur.
- Correct orientation and skew. Rotate sideways pages and deskew small angular deviations before recognition. Check the outer edges after correction; a page that was rotated or cropped too aggressively can lose characters. OCRmyPDF’s cookbook and Tesseract’s quality guide both describe skew and orientation as issues worth correcting.
- Clean conservatively. Remove shadows, uneven backgrounds, speckles, and halftone patterns when they interfere with letters. Apply sharpening only where text is genuinely soft. Compare a cleaned preview with the original: if hairlines, punctuation, or light ink disappear, reduce the cleaning strength or use the original for OCR.
- Set language and layout assumptions. Choose every language actually present on the page, and select a layout mode that matches the document if your software provides one. Keep reasonable margins. For forms and tables, test whether the tool preserves blocks and cells or whether you need a structured transcription after OCR.
- Run OCR while retaining the page image. Prefer searchable-image output when visual fidelity matters. In a desktop application, choose the scan-recognition command and enable the invisible text layer. In an automated OCRmyPDF workflow, an option such as
--oversample 300can resample low-resolution images before recognition; use it only on a working copy and verify the result. - Review the text layer against the image. Start with headings, names, dates, amounts, measurements, footnotes, page numbers, tables, and column boundaries. Acrobat can mark uncertain words as suspects and lets you compare each item with the page image before accepting or correcting it (Acrobat’s scanned-text correction guide).
- Record corrections without overwriting evidence. Correct the OCR layer or export a separate text file, but retain the original page image. Keep a change log when the document is contractual, financial, medical, or archival. A reviewer should be able to see both the source pixels and the final corrected text.
- Test search and reading order. Search for a word near the beginning, middle, and end of the file; copy text from a single column and from a table; and test a screen reader or accessibility checker if accessibility is a requirement. A PDF can be searchable while still having a poor reading order.
How to fix OCR errors without changing the original scan
Use a layered file strategy
Keep three logical versions when the document matters: the untouched scan, a searchable-image PDF, and a corrected text representation. The searchable-image file supplies convenience while preserving the visible page. The corrected representation supplies clean text for analysis or reuse. Naming them distinctly prevents an edited transcript from being mistaken for the source.
Correct the text layer, not the pixels
When an OCR editor shows a suspect, compare the highlighted word with the image and replace only the recognized characters. This changes what users search and copy, not what the page looks like. If editing the PDF causes visible reflow or changes the image, undo that operation and work in a text export or sidecar file instead. Adobe also provides a scan-editing workflow, but any visible edit should be treated as a new document version and reviewed separately (Adobe’s edit-scans guidance).
Use annotations for uncertain readings
If the image is genuinely ambiguous, do not silently guess. Add a note, editorial mark, or change-log entry that identifies the page and the uncertainty. For a legal or historical record, preserving an uncertain reading is more defensible than replacing it with a confident but unsupported character.

Re-OCR only the problem pages
When a few pages fail, isolate them and try a different preprocessing path rather than rasterizing the entire document repeatedly. Keep the best output for each page and merge it only after checking page order, metadata, and search behavior. Repeated rasterization can soften text and remove vector content, especially when a force-OCR mode is used.
Troubleshooting common OCR symptoms
| Symptom | Likely cause | Targeted fix |
|---|---|---|
| Search finds almost nothing | No text layer, wrong OCR mode, or OCR failed on the page | Confirm that the PDF is image-based, run recognition with searchable-image output, and test a known word after saving. |
| Every line is shifted or scrambled | Skew, rotation, perspective, or incorrect layout mode | Deskew and rotate first; preserve margins; choose a layout mode matching columns or blocks. |
| Letters vanish in pale areas | Over-aggressive thresholding, background removal, or sharpening | Reduce cleanup strength and compare with the untreated scan; use adaptive binarization only when it preserves thin strokes. |
| Speckles become punctuation | Dirty background, bleed-through, or halftone dots | Despeckle or descreen cautiously, then inspect commas, decimal points, and quotation marks. |
| Columns read across each other | Page treated as a single text block | Set a multi-column layout option or OCR each region separately, then verify reading order by copying a paragraph. |
| Tables become meaningless text | Recognition detected words but not cell relationships | Use a table-aware mode if available and manually verify every row and column; export a structured table only after checking the image. |
| Accented or non-English characters are wrong | Language model does not match the page | Select the correct language set and rerun a sample; verify names and diacritics individually. |
| Numbers look credible but are wrong | Confusion among 0/O, 1/l/I, 5/S, decimal marks, or minus signs | Prioritize numeric fields during review and compare against surrounding labels, totals, and the image. |
| OCR gets worse after “improving” resolution | Interpolation or repeated rasterization introduced blur | Return to the original scan, use optical resolution when recapturing, and apply oversampling only to a working copy. |
Choosing an OCR tool or workflow
No engine is universally most accurate. The right choice depends on the document set, the amount of automation, and how much human review you can provide. Compare tools on the following dimensions rather than relying on a single headline accuracy claim.
| Capability | Desktop Acrobat workflow | OCRmyPDF with Tesseract | Other OCR service or application |
|---|---|---|---|
| Preserves original page image | Searchable-image output is designed to retain it | Designed to add a text layer while retaining page images, subject to selected options | Varies; confirm whether output is image-plus-layer or text-only |
| Deskew and cleanup | Exposes deskew, descreen, background removal, and sharpening settings | Documents deskew and cleanup options; exact behavior depends on command and version | Varies by product and configuration |
| Language support | Choose document language in OCR settings | Uses Tesseract language data and configuration | Check supported scripts and mixed-language handling |
| Batch and automation | Convenient for interactive review; automation depends on edition and deployment | Command-line workflow suited to repeatable batches | May offer APIs, queues, or cloud processing; verify data-handling terms |
| Suspect-word review | Provides visual comparison and suspect correction | Usually requires an external review or text-diff process | Varies; test whether uncertain words are exposed |
| Layout and tables | Visual PDF editing and reading-order tools are available, but complex tables still require checking | Recognition can preserve a PDF layer without guaranteeing structured table extraction | Do not assume table structure without a document-specific test |
| Cost and licensing | Depends on the Acrobat edition and license | OCRmyPDF and Tesseract are open-source projects; installation and support are your responsibility | Varies by license, page volume, hosting, and data-retention terms |
For a small set of difficult pages, an interactive application with suspect review may save time. For a repeatable archive, a scripted pipeline can provide consistent settings and logs, but it needs sampling and human quality control. Before committing, process the same representative pages through each candidate and compare character accuracy, layout, output size, search behavior, and review effort.

Resampling, oversampling, and force-OCR cautions
OCRmyPDF’s --oversample DPI option can resample a low-resolution image before OCR. This is useful when the embedded image advertises a low DPI but contains enough detail to benefit from a larger working raster. It cannot recreate missing strokes, and it may increase file size or processing time.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Repeated conversion between PDF and raster images can compound blur, alter colors, and discard vector elements. Force-OCR modes may rasterize pages that already contain useful text or vector content. Use those modes only when you have confirmed that the existing text layer is absent, corrupt, or untrustworthy. Always compare the working output with the master scan and retain the original.
Validation: deciding whether the result is trustworthy
Validation should be risk-based rather than a quick glance at the first page. A practical review order is:
- Identity fields: names, addresses, dates, document numbers, and signatures.
- Numbers: prices, quantities, decimals, percentages, units, page references, and negative signs.
- Structure: headings, lists, footnotes, captions, columns, and table boundaries.
- Rare characters: accented letters, symbols, ligatures, superscripts, and scientific notation.
- Search tests: words from early, middle, and late pages, including a term near a scan edge.
- Copy tests: one ordinary paragraph, one multi-column passage, and one table or form field.
For high-stakes collections, sample pages from every scanner, date range, paper type, and language. Record the OCR settings and software version so a failed batch can be reproduced. No authoritative percentage applies to every scanned PDF; resolution, blur, skew, language, typography, compression, and layout can change the result substantially.
Accessibility and downstream use
An invisible text layer enables searching and copying, but it does not automatically create a fully accessible PDF. Screen readers also depend on a sensible reading order, document language, tags, heading structure, and meaningful descriptions for non-text content. A two-column page may be searchable while being read across columns in the wrong sequence. If accessibility is a requirement, perform a separate reading-order and tagging review after OCR.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteLikewise, extracted text is not automatically suitable for database import. OCR may preserve words while losing table cells, superscripts, or relationships between labels and values. Use OCR as the first extraction stage, then apply document-specific parsing and validation.

A decision framework for improving a difficult scan
If you can recapture the pages
Fix the physical cause first: flatten the page, clean the scanner glass, improve lighting, focus the camera, and capture at the appropriate optical DPI. Recapturing usually produces a larger improvement than trying many filters on a blurred image.
If you cannot recapture the pages
Work from the highest-quality original available. Test deskew, orientation correction, background cleanup, and language settings on a sample. Compare the untouched and processed images, then select the least destructive preprocessing that improves recognition.
If visual fidelity is mandatory
Use searchable-image output and correct only the text layer or a sidecar transcript. Never discard the source image, and document any ambiguous reading.
If throughput is the priority
Use a scripted batch with fixed settings, logs, and representative-page sampling. Automate the routine parts—conversion, OCR, and file naming—but reserve human review for high-risk fields and pages that trigger quality checks.
OCR succeeds when the software receives a legible, correctly oriented image and when a person verifies the places where a small character error changes meaning. Treat the visible scan as the evidence, the OCR layer as a searchable aid, and any corrected transcript as a separately controlled derivative.
