A scanned PDF is often just a set of page images. Optical character recognition (OCR) examines those pixels and adds a machine-readable text layer, making words selectable, searchable, copyable and, in some workflows, editable. It does not recover an original word-processing file or guarantee that every character is correct: the engine infers text from visual shapes, so ambiguous pixels produce uncertain results.
The most dependable approach is to preserve the scan, check whether text already exists, choose every language used in the document, improve weak images, run OCR, and verify important results against the page image.

What OCR changes in a scanned PDF
A scan can contain images but no text
When paper is scanned directly to PDF, each page is normally stored as raster imagery. Adobe Acrobat Help, updated in 2025, describes the result plainly: “When you scan a paper document to PDF, the resulting file contains only image data, not searchable text.” A viewer can display the page perfectly while find, copy and text-selection commands return nothing because there are no characters for the application to read.

OCR adds a searchable text layer
OCR analyzes the page image, identifies likely characters and words, and writes that recognition into the PDF. The original image and the recognized text can coexist. In a typical searchable PDF, the scan remains the visible page while an invisible or lightly aligned text layer supplies search, selection and copying. This design preserves the appearance of the paper document while making its contents usable by search tools and accessibility features.

Recognition is an inference, not source-file recovery
The engine never sees the original typewriter, printer or authoring file. It sees pixels and estimates which characters best explain them. If a mark could be “0” or “O,” or a faint decimal point disappears, OCR must choose or flag a possibility. The text layer is therefore a useful interpretation of the image, not proof that every character matches the source.
Why OCR recognition errors happen
Weak or damaged input images
Blur, compression artifacts, faint ink, bleed-through from the reverse side, stains, shadows and low contrast all make character boundaries uncertain. A dark gutter can be mistaken for a letter; a missing stroke can turn an “m” into “rn.” Adobe recommends starting with a high-quality, well-lit scan and correcting skew when recognition fails. Better source pixels generally help more than changing a later export setting.
Resolution and rasterization
OCR engines operate on raster images. If the scan resolution is too low, thin strokes and punctuation are already lost. Conversely, a PDF may be rasterized during processing, and the rasterization or preprocessing choices determine what the engine receives. OCRmyPDF explains that its workflow rasterizes PDF pages before sending them to Tesseract; any information lost at that stage cannot be reconstructed by the recognizer.

Language selection affects character choices, dictionaries and word patterns. OCRmyPDF documents that results can be poor when a document’s languages are not specified in the language argument. Select every language actually present, including bilingual sections, rather than selecting only the language used on most pages. A name or technical term may still need manual checking even with the correct language model.
Fonts, ligatures and unusual typography
Decorative typefaces, tightly spaced letters, ligatures and degraded typewriter characters can resemble other symbols. A ligature may be interpreted as separate letters or as one unfamiliar character. Marginal notes, stamps and overprinted corrections introduce shapes that do not follow the main text’s typography. Mathematical notation is especially vulnerable because a superscript, minus sign or variable can be mistaken for ordinary punctuation or omitted.

Handwriting
Handwritten text is not simply a lower-quality printed font. Individual writing styles, connected strokes and irregular baselines require a handwriting-capable recognition system. OCRmyPDF documents that its Tesseract-based workflow is incapable of recognizing handwriting reliably. A handwriting-oriented system may produce useful results, but handwritten pages still require close checking rather than automatic acceptance.
Columns and reading order
OCR must decide not only what each character is but also which text comes next. On a two-column page, it may read across the row, join the end of the left column to the beginning of the right, or place a sidebar between paragraphs. Headers, footers, captions and pull quotes can appear in the middle of a sentence. OCRmyPDF warns that Tesseract may join text across columns and does not create paragraph or heading structure. A document can therefore contain mostly correct words in an unusable order.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsTables and non-text marks
Ruled lines, punch holes, speckles and page borders may be interpreted as characters. A diagram can be mistaken for text or ignored entirely. Tables add another problem: even when every cell is recognized, the reading sequence may run across rows or columns incorrectly. Decimal points, minus signs and column headings deserve particular attention because a single misplaced mark can change a financial or scientific meaning.
Already-text PDFs
Not every PDF needs OCR. Acrobat distinguishes image-only pages from pages containing renderable text. If selectable text is already present, running OCR may be unnecessary and can trigger an error about renderable text. Some files mix page types, so inspect several pages instead of assuming the whole document is image-only.
How to check whether a PDF needs OCR
- Open the file and drag across a sentence. If individual characters can be selected, text already exists on that page.
- Use the viewer’s Find command for a word that is visibly present. A failure to find it suggests an image-only page, but test more than one page in a mixed document.
- Copy a short passage into a plain-text editor. If the paste is empty or produces only an image, the page needs a text layer for searching and copying.
- Check whether the file contains renderable text before launching an OCR operation. Re-OCRing an already-text page can be redundant or can cause a renderable-text error.
A reliable OCR workflow
1. Keep an untouched master
Make a working copy before deskewing, cleaning or OCRing. The original image is your evidence when a recognition result is doubtful, and it gives you a recovery path if preprocessing removes a thin stroke or damages page layout.

The Tool Desk
Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →3. Improve the image conservatively
Deskew tilted pages, crop dark borders and correct uneven contrast when necessary. Remove noise carefully: aggressive cleaning can erase commas, accents and hairline strokes. If the scan is faint, compare the cleaned preview with the master before continuing. Image improvement should make characters clearer without changing the document’s meaning.
4. Run OCR while preserving the page image
Use a workflow that adds a text layer rather than replacing the visible scan wherever possible. OCRmyPDF is designed to produce searchable archival PDFs with minimally altered page imagery, while Acrobat provides an integrated desktop process. Select the document’s languages and the relevant page range, then allow the software to process the images.
5. Search for predictable failure points
Do not rely only on a general visual impression. Search the output for names, account or reference numbers, dates, decimal values, negative signs, negations such as “not,” uncommon words and table headings. These terms expose errors that ordinary prose can conceal.
6. Compare suspect text with the scan
When a word looks strange or a search produces an unexpected result, zoom the recognized text and the original image together. Acrobat’s correction workflow marks uncertain words as suspects and displays the page image beside the recognized text so you can accept or replace a correction. In other software, use the same principle: every material change should be grounded in the visible page.

- FAST SPEEDS - Scans color and black and white documents a blazing speed up to 16ppm (1). Color scanning won’t slow you down as the color scan speed is the same as the black and white scan speed.
- ULTRA COMPACT – At less than 1 foot in length and only about 1. 5lbs in weight you can fit this device virtually anywhere (a bag, a purse, even a pocket).
- READY WHENEVER YOU ARE – The DS-640 mobile scanner is powered via an included micro USB 3. 0 cable allowing you to use it even where there is no outlet available. Plug it into you PC or laptop and you are ready to scan.
- WORKS YOUR WAY – Use the Brother free iPrint&Scan desktop app for scanning to multiple “Scan-to” destinations like PC, Network, cloud services, Email and OCR. (2) Supports Windows, Mac and Linux and TWAIN/WIA for PC/ICA for Mac/SANE drivers. (3)
- OPTIMIZE IMAGES AND TEXT – Automatic color detection/adjustment, image rotation (PC only), bleed through prevention/background removal, text enhancement, color drop to enhance scans. Software suite includes document management and OCR software. (4)
7. Verify material passages manually
For legal, financial, medical, archival or publication use, check every material passage against the image. Verify names, dates, quantities, units, addresses, citations, instructions and table totals line by line. OCR is an aid to reading and retrieval, not independent proof.
8. Retain both versions
Store the untouched scan and the searchable derivative with clear filenames. Keeping both lets you reprocess the document with another language model or preprocessing choice without losing the original record.
Fixing common OCR problems
The text is gibberish or nearly empty
- Confirm that the page is not already text-based and that OCR was actually applied to the intended page range.
- Inspect resolution, blur, contrast and shadows in the source image.
- Deskew and crop borders, then run OCR again from the untouched master rather than repeatedly processing a degraded derivative.
- Choose the correct language models, including all languages present.
Words are individually correct but the paragraph order is wrong
- Expect problems on multi-column pages, sidebars and pages with prominent headers or footers.
- Review the extracted text against the visual reading order before importing it into another system.
- For critical documents, reconstruct the order manually or use a layout-aware workflow and verify the result page by page.
Numbers, symbols or table cells are wrong
- Zoom the original image and check decimal points, minus signs, superscripts, units and row or column boundaries.
- Search for every occurrence of high-risk fields such as dates, totals and identification numbers.
- Do not assume that a plausible-looking number is correct; compare it directly with the scan.
Handwriting is missing or nonsensical
A printed-text OCR workflow is the wrong tool for reliable handwriting. Use a system designed for handwriting if available, then perform human verification of the resulting transcription. Preserve the image because it remains the authoritative record.
OCR reports that renderable text already exists
Inspect the page with selection and search tools first. The file may be born-digital or may contain a text layer on some pages. OCR only the image-only pages, or create a derivative after determining which layers are already usable.

Neither tool has a universal accuracy percentage that applies to every scan. Performance depends on image quality, language, typography, handwriting and layout, so a percentage from one document would not predict the result for another.
How much trust should you place in OCR text?
Use the text layer confidently for discovery when the scan is clear and the language and layout are straightforward. Treat it more cautiously when the page is faint, damaged, handwritten, multilingual, table-heavy or mathematically complex. Search is often the first benefit even when perfect extraction is impossible: a near-match can lead you to the correct page, where you then read the original image.
For ordinary notes, a few corrected words may be enough. For a contract, prescription, invoice, historical record or published quotation, establish a verification rule before release: compare all names, numbers, dates, negations and instructions with the image, record corrections, and keep the original scan available to anyone relying on the transcription.
What a searchable PDF can and cannot do
- It can: support find-in-document searches, selectable copying, text indexing and text-layer-based accessibility tools.
- It can sometimes: support editing or reflow, although the resulting structure may not match the original page design.
- It cannot: prove that an ambiguous character was recognized correctly, recreate missing ink, reliably understand every handwritten note, or guarantee the original reading order of a complex layout.
The visible scan remains the reference. OCR makes that reference easier to search and reuse, but its output must be judged against the pixels whenever accuracy matters.
