What Neanderthal Shoulder Injuries Reveal About Hunting and Close-Range Combat

Neanderthal shoulder injuries are most consistent with a physically demanding life that included dangerous, close-range encounters with prey. They fit the use of hand-delivered thrusting spears and the risks of getting close to large animals, but they do not, by themselves, prove that Neanderthals frequently fought one another. A wound can reflect a hunting accident, an animal encounter, a fall, interpersonal violence, or later preservation effects.

The strongest evidence for close-range hunting comes not from a human skeleton but from two 120,000-year-old fallow-deer skeletons at Neumark-Nord, Germany. Their perforations match thrust-spear injuries in micro-computed-tomography and ballistic experiments. Human shoulder trauma adds context: it shows the bodily cost of dangerous activities, while rarely identifying the attacker, weapon or circumstance with certainty.

What the shoulder evidence can—and cannot—show

A shoulder lesion records force applied to bone, not the motive behind that force. The same fracture pattern may result from a thrusting weapon, a collision with an animal, a fall or a violent encounter with another person. Archaeologists therefore separate two questions: whether Neanderthals regularly undertook hazardous close-range activities, and whether a particular injury was caused by combat.

On the first question, the evidence is strong. Neanderthal bodies were capable of powerful upper-limb movements, and animal remains preserve experimentally identified thrust-spear wounds. On the second, the evidence is limited. No reliable population-wide percentage for Neanderthal shoulder injuries has been established, because the apparent frequency changes with skeletal preservation, the elements available for study, lesion definitions, age structure and the sites included.

Shanidar 1: severe injury followed by long-term impairment

Shanidar 1 provides the classic example of serious Neanderthal upper-limb trauma. The 1982 assessment by Erik Trinkaus and Pat Shipman Zimmerman documented injury to the right humerus together with hypoplasia or atrophy affecting the right clavicle, scapula and humerus. The pattern indicates a major injury to the shoulder-and-arm system and impairment that persisted during the individual’s life.

That evidence demonstrates survival with a compromised arm, not the event that caused the original damage. The bones do not identify whether the injury came from a hunting accident, a fall, an animal attack or interpersonal violence. Nor does one individual establish how common such injuries were among Neanderthals. Shanidar 1 is valuable because it shows the biological consequences of a disabling upper-limb injury and the ability to live for a substantial period afterward, while leaving the immediate cause unresolved.

Shanidar A: a spear-like perforation with an unresolved cause

A 2026 study of Shanidar A described a smooth-edged, roughly 15-millimetre circular perforation in the scapula. Radiating fractures and displaced bone fragments surround the opening. The authors interpret the morphology as compatible with a wooden spear entering from the front, probably close to the time of death.

This is a more specific injury pattern than the generalized damage in Shanidar 1, but it still cannot answer the combat question. The study explicitly states that it is impossible to know whether the lesion resulted from a hunting accident or interpersonal violence. A spear could have struck during an attempt to kill an animal, during a conflict between people, or in another circumstance that is no longer recoverable from the skeleton. The case illustrates why weapon-compatible morphology is not the same as a demonstrated human attack.

Neumark-Nord: the clearest direct evidence for close-range hunting

Two fallow-deer skeletons from Neumark-Nord in Germany were recovered from deposits dated to about 120,000 years ago and associated with abundant Neanderthal traces. Their bones contain perforations whose shape and internal damage were examined with micro-computed tomography. Researchers then compared the lesions with injuries produced in controlled ballistic experiments.

Those analyses demonstrated that the deer wounds resulted from close-range thrusting spears. This is direct evidence because the animal lesions were experimentally matched to a specific weapon action. It does not rely on assuming that a human injury must have been caused by hunting. The finding also implies a demanding hunting sequence: hunters had to approach within effective thrusting distance, coordinate their actions and manage the danger of a wounded animal.

The Neumark-Nord evidence is therefore stronger for reconstructing Neanderthal hunting tactics than any isolated shoulder wound. It establishes that at least some Neanderthals used hand-delivered thrusting weapons against large game. It does not establish that every spear wound, or every shoulder injury, came from that activity.

Why powerful shoulders matter to spear use

Churchill and Rhodes (2006) examined muscle-attachment size and leverage at the shoulder and elbow. Their conclusion was that Neanderthal upper-body strength was probably important to hunting with close-range, hand-delivered weapons. A forceful thrust requires the arm and shoulder to accelerate and stabilize a shaft while the hunter’s body resists the impact and the prey struggles.

This biomechanical interpretation explains why shoulder anatomy belongs in the hunting discussion. Strong muscle attachments and favorable leverage can make repeated, forceful actions effective. They cannot, however, identify the cause of an individual lesion. Morphology indicates what the body may have been capable of doing; it does not record whether a specific blow came from a deer, a spear-wielding person or a fall.

The “rodeo rider” analogy and its limits

In 1995, Berger and Trinkaus compared Neanderthal trauma distributions with archaeological, clinical and rodeo samples. The Neanderthal pattern resembled rodeo trauma chiefly because head and neck injuries were frequent. They proposed that repeated close encounters with large ungulates could produce that distribution, much as rodeo participants are injured while controlling powerful animals at short range.

The analogy is useful as a mechanical model, not as a diagnosis. It highlights the danger of approaching and stabbing large prey, where an animal’s head, horns, hooves or sudden movement can strike a hunter. The authors also acknowledged important qualifications: the sample was small, preservation was uneven and older individuals with restricted mobility may be under-represented. Those factors can make one activity appear more dominant than it was.

Later work by Trinkaus (2012) broadened the explanation. Hunting injuries may account for part of the pattern, but mobility, differences in skeletal susceptibility, evidence for effective throwing spears and interhuman violence could also contribute. The result is a multi-cause interpretation rather than a single behavioral signature.

Does similar cranial trauma mean Neanderthals fought less—or more?

A population-level Nature study compared cranial trauma in Neanderthals and Upper Palaeolithic anatomically modern humans. It found similar overall incidences in the two groups, with higher rates among males in both populations. That result weakens the older image of Neanderthals as uniquely and exceptionally violent.

The comparison also carries methodological warnings. Trauma prevalence varies with which skulls survive, how lesions are classified and the age distribution of the sample. Preservation bias can remove fragile evidence, while age-related exposure to risk can change the apparent rate. Similar population averages do not rule out individual fights, but they do not support a claim that Neanderthals had an unusually distinctive pattern of interpersonal violence.

How to weigh hunting and combat explanations

Researchers can compare competing explanations using several independent criteria. None is decisive alone; the combination determines how confident an interpretation should be.

Evidence axis What it can establish What it cannot establish
Experimental match to an animal lesion Whether a wound is consistent with a particular weapon action, such as a close-range thrust Who held the weapon or whether a human target was involved
Human shoulder, arm, head or neck trauma That substantial force affected a vulnerable body region Whether the force came from prey, another person, a fall or an accident
Population distributions Whether trauma rates and locations differ between groups when samples are comparable A complete census of injuries or the motive behind any lesion
Biomechanical anatomy Whether an upper body was well suited to forceful, hand-delivered actions The cause of an individual injury
Context and dating Whether injuries occur alongside evidence of hunting activity or other behavior A direct narrative of the final event

Under this framework, Neumark-Nord ranks high for direct evidence of thrusting hunts. Shanidar A is strong evidence for a spear-compatible impact but weak evidence for distinguishing hunting from combat. Shanidar 1 documents severe disability without a recoverable cause. The population studies help prevent isolated cases from being treated as a universal Neanderthal pattern.

What a close-range hunting scenario would have required

  1. Finding and approaching prey. Hunters needed to locate suitable animals and move close enough for a thrust to reach the body. Concealment and control of movement would reduce the chance of an early escape.
  2. Coordinating several hunters. The Neumark-Nord interpretation emphasizes careful planning and close cooperation. Multiple people could channel an animal, distract it or deliver successive attacks while reducing the danger to any one hunter.
  3. Absorbing impact and maintaining the weapon. A thrust transfers force into the animal and back through the shaft and the hunter’s arms. A strong shoulder and elbow system would help stabilize the weapon, but the same forces could injure the hunter.
  4. Managing the animal’s counterattack. A wounded deer or larger ungulate could turn, charge, kick or collide with a hunter. Such movements offer a plausible route to upper-body, head and neck trauma without any interpersonal fight.
  5. Finishing and recovering the carcass. Close-range weapons may require repeated approaches. Each approach creates another opportunity for an accident, even after the animal has been weakened.

This sequence explains why a hunting lifestyle can generate repeated injuries while leaving no uniquely “hunting” mark on every skeleton.

Could a spear wound distinguish combat from hunting?

Usually not. A projectile or thrust mark may reveal the direction, shape or energy of an impact, but the same weapon technology can be used against an animal or a person. A wooden spear entering a scapula from the front is compatible with a human attack, yet it is also compatible with a hunter being struck during a dangerous encounter.

Distinguishing the scenarios requires context beyond the hole itself: associated animal remains, repeated injuries, settlement or burial circumstances, the position of the body, other weapons and the chronology of the lesion. Even then, the conclusion may remain probabilistic. Shanidar A demonstrates the limit clearly: the investigators could identify a spear-like injury but could not determine whether it was a hunting accident or interpersonal violence.

Why shoulder injuries are easy to overinterpret

  • Preservation bias: Some bones survive better than others, and damaged or missing shoulders can make rates look lower than they were.
  • Classification choices: Studies may count only healed lesions, only perforations or a broader set of fractures and degenerative changes.
  • Age and mobility: Older or mobility-impaired individuals may face different risks and may be less likely to be represented in archaeological samples.
  • Small samples: A few famous skeletons can dominate public narratives even though they cannot represent an entire population.
  • Overlapping mechanisms: Falls, animal attacks, hunting accidents and violence can produce similar damage in the same anatomical region.

These constraints do not make the injuries uninformative. They define the level of claim the evidence can support: strenuous activity and dangerous encounters are plausible and, in some contexts, directly demonstrated; frequent organized combat is not established by shoulder trauma alone.

The most defensible conclusion

Neanderthal shoulder injuries reveal the costs of forceful, close-range lives more reliably than they reveal the presence of combat. The Neumark-Nord deer provide experimentally matched evidence that Neanderthals used thrusting spears at close range around 120,000 years ago. Shoulder biomechanics show why powerful upper bodies would have aided that tactic. Shanidar 1 records severe long-term impairment, while Shanidar A preserves a roughly 15-millimetre spear-compatible scapular perforation whose cause remains unresolved.

Taken together with the rodeo comparison and the finding that Neanderthal and Upper Palaeolithic modern-human cranial-trauma rates were broadly similar, the evidence supports a cautious picture: dangerous hunting was real, cooperation was important, and interpersonal violence was possible, but no shoulder lesion can by itself turn a risk-filled hunting life into proof of frequent close-range combat.
