What Menstrual Health Wearables Can Measure—and What They Can’t

Menstrual health wearables measure body signals—not reproductive hormones or ovulation itself. Rings and watches may record overnight skin-temperature trends, heart rate (HR), and heart-rate variability (HRV); some research also examines respiratory rate. Software combines those readings with cycle dates and other inputs to estimate periods, cycle phases, ovulation timing, or a fertility status. What those estimates mean depends on the device, algorithm, evidence, and intended use.

What does a menstrual health wearable actually measure?

A wearable’s sensor records a physical signal. Its app may then interpret changes in that signal as a possible sign of a cycle event. Those are different steps: a temperature trend is a measurement; an ovulation estimate is an algorithmic inference.

Skin-temperature trends

Some rings and watches track skin temperature overnight. Oura says it measures temperature once per minute at night and uses nighttime trends to help determine cycle events. Apple describes overnight wrist-temperature data on Apple Watch Series 8 and Apple Watch Ultra. The sensor location and method differ, so wrist and finger readings are not interchangeable with each other or with oral basal body temperature. Oura’s Cycle Insights explanation

Heart rate and heart-rate variability

HR and HRV are cardiovascular measurements, not hormone readings. They can vary across menstrual phases, and researchers have studied them alongside temperature and other signals to distinguish phases. A 2024 systematic review says more validation is needed before these patterns can be treated as reliable cycle markers across users and devices. 2024 systematic review in JMIR

Respiratory rate

Respiratory rate appears in some wearable research and feature sets, but the review describes evidence for its use as a cycle-staging proxy as scarce. It should not be treated as an established standalone menstrual marker. 2024 systematic review in JMIR

Cycle information entered by the user

Period dates and other manually entered details can be combined with sensor data. FDA submission materials for Natural Cycles describe temperature and menstrual-cycle information, with optional ovulation- or pregnancy-test results, as inputs to its proprietary algorithm. A cycle log is user-provided information, not a sensor measurement. FDA submission summary, K250561

What can the app infer from those signals?

Depending on the product, software may display cycle phases, an expected period start, an ovulation estimate, a fertile window, or a fertility status. These outputs are not direct observations of ovarian activity. Wearables do not directly assay estrogen, progesterone, luteinizing hormone (LH), or other reproductive hormones.

Temperature often changes after ovulation, which can make a pattern useful for retrospective interpretation. But identifying a shift after it appears is not the same as forecasting ovulation in advance. Apple says the wrist-temperature feature on Apple Watch Series 8 and Apple Watch Ultra estimates the likely day of ovulation after it has occurred, while also improving period predictions. How reliable are wearable cycle estimates?

There is no universal accuracy figure that applies to menstrual wearables as a category. Results depend on the sensor and its placement, how consistently it is worn, the algorithm, the reference method, the study design, and the outcome being evaluated. The 2024 systematic review found promising results but also small or pilot studies, incomplete validation, and a need for larger comparative prospective studies. Its findings do not establish that every ring or watch can accurately identify every user’s cycle events. 2024 systematic review in JMIR

What individual studies can—and cannot—show

In a 2019 pilot involving 22 participants using an Oura Ring, finger skin temperature was 0.30 °C higher on average in the luteal phase than in the follicular phase (standard deviation 0.12 °C). The review also reports 83.3% ovulation-detection sensitivity over a specified window in that study. These are findings from a small study, not a guarantee of performance for the ring, another device, or an individual user. 2024 systematic review summarizing Maijala et al. (2019)

Natural Cycles reports an evaluation of its Apple Watch temperature input involving 272 participants and 505 complete cycles. The company compared watch readings with two-decimal oral thermometer inputs and used ovulation-test results as an independent reference. It reports that the number of green days differed by less than 2%, or 0.6 days per cycle, between Apple Watch and oral thermometer inputs. This is a company-reported result for that app evaluation, not an independent or universal wearable-accuracy statistic. Natural Cycles research-library evaluation

Why can temperature patterns be misleading?

Skin temperature is context-sensitive. The systematic review notes that environmental conditions and perspiration from heat or activity can affect readings. Illness and individual circumstances can also alter temperature patterns. A change in the graph may therefore have more than one possible explanation.

Applicability varies, too. Oura says Cycle Insights is designed for people who menstruate and have a natural hormonal cycle, and notes that health factors, life stages, or medications may affect temperature patterns and limit what the feature shows. Menopause, hormonal contraception, pregnancy, irregular cycles, illness, and medication changes are examples of circumstances that can make an algorithm’s usual assumptions less applicable; the feature’s own guidance matters. Oura’s Cycle Insights explanation

Can a wearable tell you when you are fertile?

A fertile-window display is an estimate, not a direct test of fertility. The fertile window begins before ovulation, and wearable methods may detect a temperature shift only after it occurs. The review cautions that some detection windows may miss days before ovulation. A reading that suggests ovulation has occurred should not be treated as proof that earlier fertile days did not occur. 2024 systematic review in JMIR

If you are considering a wearable-based method to avoid pregnancy, check the specific product’s intended use, instructions, age and mode restrictions, compatible devices, and availability where you live. FDA submission materials for Natural Cycles describe a product for users 18 and older with distinct contraception and conception modes and specified cycle and temperature inputs. That product-specific regulatory scope does not make ordinary period-tracking features contraceptives or establish the status of other wearables. FDA submission summary, K250561

How to judge a wearable or tracking method

Before relying on a feature, check what it measures, when it reports an estimate, and what purpose it is designed to serve. A ring’s overnight finger-temperature trend, a watch’s wrist-temperature estimate, an oral basal thermometer reading, and an LH test are different signals with different timing and roles.

  • Signal and location: Look for whether the product uses wrist or finger skin temperature, HR or HRV, or information you enter yourself. Do not assume those measurements are equivalent to oral basal temperature or an LH test.
  • Timing: Find out whether the result is retrospective pattern recognition, a prediction, or a fertility status meant for a specific use.
  • Evidence: Check the number of participants and cycles, whether data were collected prospectively, what reference method was used, and whether findings were independently replicated.
  • Intended use and scope: Distinguish general cycle insights from a product with a specifically described contraception or conception indication; check age, device compatibility, and country availability.
  • Practical fit: Consider comfort during sleep, consistent wear, missing data, manual logging, and whether the method works with devices you already use.

A digital basal body thermometer supplies a temperature input but does not, by itself, provide a fertility algorithm. An at-home LH test measures a different signal and may be used as an optional input or reference in some workflows. The right comparison is therefore not simply “wearable versus test”; it is whether the method’s signal, timing, evidence, and intended use fit the question you need answered.
