What Is Tuberculosis Infection, and How Is It Different From Active TB Disease?

Tuberculosis (TB) infection means TB bacteria are present in your body. In inactive TB infection—also called latent TB infection—the bacteria are alive but controlled, so you do not have TB disease and generally cannot spread TB to other people. Active TB disease develops when the bacteria multiply and damage tissue. It can cause illness and may be contagious, especially when the lungs are affected.

A positive TB blood test or skin test shows evidence of infection, but it does not by itself tell you whether the infection is inactive or active. A health professional determines that by combining your symptoms, medical history, examination, chest imaging and, when needed, sputum or other laboratory tests.

TB infection is not the same diagnosis as TB disease

TB bacteria can remain in the body in two clinically different states. During inactive infection, the immune system keeps the bacteria contained. During active disease, the bacteria are multiplying and causing an illness that can affect the lungs or another organ.

The terms latent TB infection and inactive TB infection describe the first state. They are used interchangeably in current U.S. public-health guidance. “Active TB” or “TB disease” describes the second state. A person can move from inactive infection to active disease, but a positive test alone cannot show that this has happened.

Inactive (latent) TB infection

  • TB bacteria are present but inactive.
  • Evaluation finds no TB disease.
  • The person generally has no TB-related symptoms.
  • The person does not spread TB to other people.

Active TB disease

  • TB bacteria are multiplying and causing disease.
  • Symptoms depend on the organ involved.
  • Pulmonary TB can make a person infectious; whether someone can spread TB is a clinical determination.
  • Treatment must eradicate active bacteria, prevent complications and reduce transmission.

Inactive infection and active disease compared

Question Inactive (latent) TB infection Active TB disease
What is happening? Bacteria are present but controlled; no disease is found on evaluation. Bacteria are multiplying and causing disease.
Symptoms No TB disease symptoms; the person generally feels well. Symptoms may occur and vary with the affected body site.
Can it spread? No. It may spread, particularly when the lungs are involved; clinicians determine infectiousness.
What does a positive blood or skin test mean? It supports infection, while evaluation finds no disease. It usually supports infection, but evidence of disease comes from the broader evaluation.
Why treat it? To prevent future TB disease. To cure disease, limit harm and reduce the chance of transmission.

These distinctions follow the U.S. Centers for Disease Control and Prevention (CDC) clinical overviews of latent TB infection, active TB disease and TB symptoms.

What a positive TB test does—and does not—tell you

“A positive TB test result means you have TB germs in your body.”

— CDC, Diagnosing Tuberculosis

That statement establishes infection, not its activity. A TB blood test (an interferon-gamma release assay) or a TB skin test detects an immune response to TB bacteria. Neither test can determine on its own whether bacteria are inactive or causing disease, how infectious a person is, or whether a particular organ has been affected.

A positive result therefore needs medical follow-up. Conversely, a negative result does not automatically rule out TB in every situation; the clinician considers symptoms, immune status, recent exposure and the type and timing of testing when deciding what to do next.

How clinicians distinguish infection from active disease

CDC diagnostic guidance describes a combination of history, examination, imaging and laboratory work. The exact combination depends on the person’s symptoms and risk factors.

  1. Review the test and history. The clinician confirms which blood or skin test was used, asks about previous TB exposure or treatment, and reviews medical conditions and medicines that may affect immune defenses.
  2. Check for symptoms and examine you. A symptom review looks for the pulmonary and general signs listed below, while the examination considers where disease might be occurring.
  3. Obtain chest imaging when indicated. A chest X-ray is commonly used to look for findings consistent with pulmonary TB. Imaging helps assess disease; it does not replace microbiological testing when that testing is needed.
  4. Collect respiratory or other samples when indicated. For suspected pulmonary disease, sputum may be examined by smear and culture. Laboratory testing can also include drug-resistance testing so treatment can be selected safely.
  5. Classify and treat the condition. The clinician decides whether the evidence supports inactive infection, active disease or another explanation, then discusses the appropriate regimen and follow-up.

If you have TB symptoms or a positive TB blood or skin test, contact a health care provider rather than trying to interpret the result yourself. The CDC’s step-by-step overview is available at Diagnosing Tuberculosis.

Symptoms that suggest active TB disease

Pulmonary TB

When TB affects the lungs, symptoms can include:

  • A cough lasting three weeks or longer
  • Chest pain
  • Coughing up blood or sputum
  • Fatigue
  • Weight loss
  • Loss of appetite
  • Chills
  • Fever
  • Night sweats

These signs are not specific to TB, and having one does not prove that you have it. Their persistence or combination—especially a prolonged cough with systemic symptoms—warrants prompt clinical evaluation.

TB outside the lungs

TB can affect organs other than the lungs. The resulting symptoms depend on the site, so the absence of a cough does not exclude active TB. A clinician may investigate an unusual swelling, pain, neurological symptom or other organ-specific problem when the person’s history and tests make extrapulmonary TB possible. The CDC symptom guidance explains why presentation varies by affected body site: Signs and Symptoms of Tuberculosis.

Can inactive TB become active?

Yes. Without treatment, the CDC estimates that about 5%–10% of infected people develop TB disease over their lifetimes. This is a population estimate, not a prediction of what will happen to one individual. Progression is most common during the first two years after infection or when immune defenses are weakened.

Risk is higher for people with weakened immune systems and for people with certain conditions, including HIV infection, diabetes and cancer. Other personal factors also matter, so a clinician should assess risk rather than applying the percentage mechanically. Inactive infection can progress at any time, which is why preventive treatment is offered even when a person feels healthy.

The CDC’s clinical overview, updated April 17, 2025, explains progression and risk factors in more detail: Clinical Overview of Latent Tuberculosis Infection.

How treatment differs

Treating inactive TB infection

The purpose is prevention: eliminate contained bacteria before they cause disease. CDC and the National Tuberculosis Coalition of America prefer short-course, rifamycin-based three- or four-month regimens over six- or nine-month isoniazid monotherapy when clinically appropriate. The right option depends on other medicines, medical conditions, possible drug interactions, pregnancy or other individual circumstances. A clinician must select the regimen and monitor it; do not start, stop or substitute TB medicines without medical advice.

Details and regimen considerations are provided in Treating Inactive Tuberculosis.

Treating active TB disease

Active disease requires a clinician-directed combination of multiple medicines. The exact drugs and duration depend on the disease site, laboratory results, drug susceptibility, other health conditions and response to therapy. Completing the prescribed course is essential: stopping early can allow disease to return and can contribute to drug resistance. Monitoring may include clinical reviews and laboratory tests, with public-health involvement when infectious pulmonary disease is suspected.

The CDC treatment overview is at Treating Tuberculosis. A person with suspected active disease should follow the treating team’s instructions about appointments, testing and any temporary measures intended to protect household or workplace contacts.

Is inactive TB contagious?

No. Inactive infection does not spread TB to other people. A positive test does not mean that friends, family or co-workers have been exposed by casual contact with you.

Active TB may be transmissible, particularly when disease involves the lungs. Infectiousness cannot be determined from a blood or skin test alone; clinicians use symptoms, imaging, sputum results and the response to treatment. Do not assume that every person with active TB is equally contagious or that extrapulmonary disease behaves like pulmonary disease.

What to do after a positive test or concerning symptoms

  • Arrange evaluation promptly. Bring the test report, a list of medicines and information about prior TB treatment or exposure.
  • Describe symptoms precisely. Note when a cough began, whether there is sputum or blood, fever, night sweats, weight change or pain, and whether symptoms are worsening.
  • Ask what testing is planned. Depending on the situation, this may include a physical examination, chest X-ray, sputum smear and culture, or drug-resistance testing.
  • Take prescribed treatment exactly as directed. Preventive and active-disease regimens have different goals and must not be interchanged.
  • Keep follow-up appointments. Follow-up confirms that inactive infection has not progressed and that active disease is responding safely to treatment.

Seek urgent medical attention if you are seriously unwell, have significant breathing difficulty or are coughing substantial amounts of blood. For routine questions about a positive result, contact a health professional or local TB program rather than relying on a home kit, supplement or online interpretation.

Common misunderstandings

“A positive test means I have active TB.”

Not necessarily. It means TB bacteria are likely present. Active disease is established—or excluded—through the clinical evaluation that follows.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“I feel fine, so I do not need treatment.”

Feeling well is consistent with inactive infection, but untreated infection can later progress. Preventive treatment is a medical decision based on your risk and test results.

“No cough means no active TB.”

TB outside the lungs can produce different symptoms, and even pulmonary symptoms vary. A clinician evaluates the whole picture rather than relying on one symptom.

“Any TB medicine plan will work for anyone.”

Regimens differ for inactive infection and active disease, and drug interactions or resistance can change the safest choice. Use only a plan prescribed for you.
