What Is Transcranial Magnetic Stimulation (TMS), and How Does It Work?

Transcranial magnetic stimulation (TMS) is a noninvasive treatment that uses a coil held near the head to produce rapidly changing magnetic fields. Those fields pass through the scalp and skull and induce weak electrical currents in a limited area of brain tissue, stimulating neurons and neural circuits. Repetitive TMS (rTMS) delivers many such pulses in a treatment session. It is used for certain depression cases and other device-specific indications, but its complete therapeutic mechanism is not yet settled.

What TMS is

TMS is a clinical brain-stimulation technique. A clinician positions an external electromagnetic coil next to the patient’s head, rather than implanting an electrode or stimulator inside the body. When the device rapidly turns its magnetic field on and off, the changing field induces a weak electrical current in nearby brain tissue. That current can activate neurons and connected neural circuits.

The most familiar form is repetitive transcranial magnetic stimulation, usually abbreviated rTMS. “Repetitive” means that the coil delivers a sequence of pulses instead of a single pulse. The pulses are generated by a console and coil system and are directed at a limited cortical volume. The treatment is therefore targeted, but it is not a way to stimulate the entire brain at once.

The National Institute of Mental Health describes the basic action this way: “The magnetic field is turned on and off very rapidly, inducing weak electrical currents that stimulate neurons and neural circuits in the brain.” That sentence explains the physical process, not a complete explanation of why a particular patient improves.

Why it is called noninvasive

TMS does not require a surgical opening in the skull and does not involve implanting a stimulator. The coil remains outside the head while the magnetic pulses are delivered. This distinguishes rTMS from implanted stimulation procedures. It also differs from electroconvulsive therapy (ECT), which is designed to induce a controlled seizure and is delivered as a medical procedure under anesthesia.

“Noninvasive” does not mean risk-free or suitable for everyone. The magnetic pulse is powerful enough to stimulate brain tissue, and the device’s electromagnetic field can interact with certain implants or leads. A qualified clinician must review the person’s medical history and the exact device labeling before treatment.

How TMS works, step by step

1. The coil creates a changing magnetic field

The treatment system sends rapidly changing electrical current through a coil positioned close to the scalp. That current produces a magnetic field. The field is switched on and off in brief pulses rather than left continuously active.

2. The field passes through the scalp and skull

Magnetic fields can pass through the scalp and skull without a surgical incision. As the field changes, it reaches nearby cortical tissue. The effect is strongest in a limited region close to the coil, which is why coil placement and the selected device protocol matter.

3. Induction produces a weak electrical current

Electromagnetic induction converts the changing magnetic field into a weak electrical current in the brain. That current can alter the activity of neurons and neural circuits. The device is not injecting electricity through the skin; the electrical effect is induced inside the tissue by the changing magnetic field.

4. Repeated pulses stimulate circuits over a treatment course

In rTMS, the system repeats the pulse pattern specified by the clinician. Repetition is intended to influence activity in the targeted circuit, but the precise chain from pulses to symptom improvement remains an active scientific question. It is accurate to say that rTMS stimulates brain tissue; it is not accurate to present one proposed long-term biological explanation as proven.

What TMS is used for

Depression

NIMH describes rTMS as a treatment option for moderate-to-severe depression when medication has not helped enough or has been intolerable. In clinical practice, brain stimulation is often considered after psychotherapy and medication, although it may be discussed earlier when depression is severe or life-threatening. The appropriate sequence depends on the person’s symptoms, preferences, medical history and the judgment of a mental-health or medical professional.

Other indications

NIMH also describes FDA-cleared rTMS uses that include treatment-resistant depression, obsessive-compulsive disorder (OCD), migraines, anxiety occurring with depression and smoking dependence. These categories should not be read as a universal list for every machine. A clearance applies to a particular device, indication and labeling, and those details can change.

Clearance is device-specific

In the United States, rTMS systems are Class II medical devices reviewed through the FDA’s 510(k) pathway. One system may have a different cleared indication, protocol or labeling from another. Before agreeing to treatment, ask the clinic which device it uses, which indication is being considered and whether the proposed use matches that device’s current US labeling. Regulations and indications outside the United States can differ.

What a treatment visit involves

Clinical assessment before stimulation

A clinician first determines whether TMS is appropriate. The assessment should cover the diagnosis and current severity, previous treatment response, medications, neurological history, implanted devices and any factors that could alter seizure or hearing risk. For depression, the clinician may use a validated symptom scale to establish a baseline and monitor change. FDA guidance recommends validated depression scales and reporting response and remission in clinical studies; a scale score is a monitoring tool, not a guarantee of benefit.

Positioning and equipment

The patient sits in a treatment chair with head support while the operator positions the coil near the selected area of the scalp. FDA’s device classification describes systems that can include a treatment chair and head support, console, gantry, coil, and system electronics and software. The operator uses the console to deliver the prescribed pulse pattern.

During the pulses

The machine delivers a series of magnetic pulses while the patient remains awake and able to communicate. The sensations and sounds depend on the device and protocol. A clinician or trained operator observes the patient and can stop the session if a problem occurs. Ask in advance how the clinic handles discomfort, unexpected symptoms and breaks.

After the session

The treating team should explain what symptoms to watch for and how to report them. Monitoring is particularly important when depression is worsening or suicidal thoughts or behavior appear. Do not change psychiatric medication or stop other treatment on your own because TMS is being considered or started; coordinate changes with the prescribing clinician.

Safety, side effects and contraindications

Common effects to discuss

FDA guidance identifies headache and pain or discomfort at the application site among adverse events that device labeling should address. These effects may occur where the coil contacts or approaches the scalp. Ask what level of discomfort is expected, what measures the clinic uses, and when pain should prompt a call.

Serious events requiring monitoring

Safety information also calls attention to seizure, worsening depression, suicidal ideation or behavior, and unusual behavioral changes. A clinic should have a plan for observing symptoms and responding if mood deteriorates. Anyone experiencing an immediate risk of self-harm should seek emergency help rather than waiting for the next TMS appointment.

Implanted devices and leads near the head

FDA guidance identifies active or inactive implanted devices, including leads, in or near the head as a major safety concern. Examples cited in the guidance include deep-brain stimulators, cochlear implants and vagus-nerve stimulators. Under the scope of that guidance, these situations are contraindications. This is not a complete personal clearance rule: other implants, medical conditions and device-specific warnings may also matter. Give the clinic a complete list of implanted hardware before evaluation.

Hearing protection

The pulse sounds can be loud enough that FDA guidance recommends hearing protection for patients and operators. Ask the clinic what protection it supplies or requires, whether it fits correctly and whether any hearing condition changes the plan. Earplugs are an accessory for managing sound exposure; they are not the treatment itself.

This hardware description is why rTMS should be understood as a supervised clinical treatment, not a consumer gadget or a general wellness device.

TMS compared with other treatment choices

Choosing a brain-stimulation treatment is not a contest in which one option is universally best. NIMH frames treatment selection around a person’s needs, preferences and medical situation. The clinician should consider the prior response to psychotherapy or medication, current severity, contraindications, the device’s indication, side-effect burden, access and the proposed schedule.

Consideration rTMS ECT Medication or psychotherapy
How stimulation is delivered External coil produces magnetic pulses near the head; no implanted stimulator. A medical procedure that intentionally induces a controlled seizure. No brain-stimulation coil; treatment uses medication, psychotherapy or both.
Anesthesia Protocols generally allow the patient to remain awake; confirm the clinic’s procedure. Delivered with anesthesia and medical monitoring. Usually not applicable to psychotherapy; medication decisions depend on the prescription.
Who may be considered People whose diagnosis and device-specific indication fit, including some depression cases after inadequate or intolerable medication response. Considered according to the person’s illness severity, urgency and medical circumstances. Often considered earlier in care, depending on symptoms, preferences and clinical history.
Key safety questions Implants or leads near the head, seizure risk, mood changes, application-site pain and hearing protection. Anesthesia and seizure-related medical risks. Medication adverse effects, therapy fit and the person’s medical situation.
Comparative success rate Not established by the cited sources for this overview. Not established by the cited sources for this overview. Not established by the cited sources for this overview.

The table does not imply that the options are interchangeable or equally effective. The authoritative material used here does not provide a population-level response percentage, so a clinic’s claimed rate should be examined for the device, diagnosis, study design and time period behind it.

Questions to ask a TMS clinician

  • Which device will be used, and what indication is it cleared for under its current labeling?
  • What clinical factors in my history affect whether TMS is appropriate?
  • Do any implanted devices, leads or other hardware make treatment unsafe?
  • What side effects are expected, and which symptoms should prompt an immediate call?
  • How will the clinic monitor depression, suicidal thoughts or unusual behavioral changes?
  • What hearing protection will patients and operators use?
  • How will my response be measured, and which validated scale will establish a baseline?
  • How does the proposed treatment plan fit with my current medication and psychotherapy?

How to evaluate TMS claims

Be cautious with claims that a single pulse pattern “rewires” the brain in a fully understood way, works for every diagnosis, or guarantees remission. The established physical description is narrower: changing magnetic fields induce weak currents that stimulate neurons and circuits. The complete biological mechanism of therapeutic improvement is not settled.

Ask whether a statement refers to a specific FDA-cleared device, a research protocol or an off-label use. Ask which country’s regulator applies, because clearances and labeling are jurisdiction-specific. Also distinguish a clinic’s internal outcome report from a controlled clinical study. The FDA guidance recommends standardized scales and response or remission reporting, but it does not supply a universal outcome rate for all TMS systems.

A practical preparation checklist

  1. Bring a complete list of medications, diagnoses, prior treatments and implanted devices or leads.
  2. Tell the clinician about any history that could affect seizure risk or hearing safety.
  3. Confirm the device name, intended indication and current labeling.
  4. Ask where the coil will be positioned and what sensations and sounds to expect.
  5. Confirm the hearing-protection procedure before the first pulse.
  6. Agree on how symptoms and safety will be monitored between visits.
  7. Keep the prescribing and mental-health teams informed rather than making treatment changes independently.

TMS is best understood as a targeted, externally delivered form of brain stimulation whose immediate physical action is well described but whose full therapeutic pathway remains under investigation. Its appropriateness depends on the individual, the device and the current clinical labeling.
