What Is Testicular Tissue Transplantation? How It Works, Evidence, and Limits

Testicular tissue transplantation is an experimental attempt to restore sperm production using testicular tissue frozen before fertility-damaging treatment. A surgeon removes a small biopsy, the laboratory freezes tissue containing immature germ cells, and—if future use is judged safe—the tissue may be thawed and grafted back. The graft must reconnect to its blood supply and support sperm development. It is not an established treatment, and a 2026 human case report describes sperm retrieval but no pregnancy or live birth.

What the procedure means

Two separate steps are often confused. Testicular tissue cryopreservation (tissue banking) is the collection and long-term storage of testicular fragments before chemotherapy, radiotherapy, or another treatment that could destroy fertility. Transplantation is a possible later use of that stored tissue. Banking does not mean that transplantation will be possible or successful.

The approach is mainly considered for prepubertal or peripubertal boys who cannot yet produce a semen sample for conventional sperm banking. Their testes contain spermatogonial stem cells and other immature germ cells, but not necessarily mature sperm. The hope is that preserved tissue can later generate sperm after being returned to the body. In some situations, adolescents or adults who cannot provide sperm may also be discussed for tissue preservation, depending on the diagnosis, treatment schedule, pubertal development, and specialist expertise.

Professional bodies continue to classify the technique as investigational. The American Society for Reproductive Medicine (ASRM) states in its 2026 committee opinion that testicular tissue cryopreservation remains investigational in prepubertal boys. The American Society of Clinical Oncology (ASCO) says tissue cryopreservation and reimplantation or grafting should occur only in a clinical trial or an approved experimental protocol.

How testicular tissue transplantation works

A potential treatment pathway has several stages. Each stage has its own uncertainties, and storing tissue does not guarantee that every later step will be offered.

1. Referral and counselling before treatment

Fertility-preservation planning should begin as soon as a gonadotoxic treatment is proposed. The oncology team usually refers the patient to a reproductive urologist, pediatric urologist, fertility-preservation service, or a program with an approved tissue-banking protocol. Timing matters because cancer treatment may need to start quickly.

Counselling should cover the experimental status, the possibility that tissue may never be used, surgical and anesthesia considerations, storage arrangements, future costs, and what outcomes are realistic. For a child, the discussion should explain that tissue can be stored now even though no proven method currently guarantees future sperm production, pregnancy, or a healthy child.

2. Surgical biopsy

A trained surgeon removes a small portion of testicular tissue, generally through a biopsy procedure performed with anesthesia appropriate to the patient. The laboratory divides the specimen into small fragments so that multiple units can be stored and, potentially, assessed separately. The amount removed must be balanced against the need to preserve as much healthy testicular tissue as possible.

The tissue is labelled, transported under controlled conditions, and processed by a laboratory with validated procedures. ESHRE’s 2025 good-practice recommendations emphasize that programs should define responsibilities for transport, chain of identity, consent, storage, and quality control.

3. Cryopreservation and storage

Laboratories use cryoprotective solutions to reduce ice damage. ESHRE discusses controlled slow freezing and cryoprotectants, while noting that protocols and post-thaw quality assessment are not fully standardized between centers. Internal validation is required; there is no universally accepted viability assay that proves a stored fragment will later produce sperm.

Storage preserves the tissue at very low temperature until a future decision is made. The more than 3,000 patients cited in ESHRE’s 2025 recommendations represent people who underwent tissue cryopreservation in surveyed centers. That figure is a banking count, not a number of successful grafts, sperm retrievals, pregnancies, or live births.

4. Safety assessment before any graft

Before tissue from a patient with cancer is returned to the body, specialists must consider whether it contains malignant cells. Reimplanting contaminated tissue could reintroduce the disease. ESHRE recommends assessment for malignant infiltration and advises against retransplantation when relevant markers are positive, with particular concern in hematologic and metastatic cancers.

Testing is not perfect. The small sample examined may not represent every fragment in storage, and a negative result cannot eliminate every possibility of contamination. Detection methods may also change as laboratories adopt new technologies. The patient’s original diagnosis, treatment history, disease status, and the sensitivity of available testing all influence the decision.

5. Thawing and grafting

If a specialist team determines that use is ethically and medically acceptable, selected fragments are thawed and grafted back to the patient. The graft must survive the procedure, develop a blood supply (vascularization), and maintain the cellular environment needed for germ-cell development. An intratesticular graft places tissue inside the testis; other technical choices depend on the protocol and the patient’s anatomy.

Graft survival alone is not the desired endpoint. Researchers must establish whether the tissue resumes organized sperm development, whether sperm can be collected, and whether any sperm can be used safely in assisted reproduction. These outcomes may require monitoring and, in some protocols, a later procedure to retrieve sperm from the graft.

6. Monitoring after transplantation

Follow-up can include physical examination, hormone testing, imaging, assessment of graft function, and attempts to retrieve sperm. The schedule and tests are not uniform because the procedure remains experimental. A successful biological signal at one time point does not establish durable sperm production or prove that conception is possible.

What the human evidence shows as of 2026

A July 2026 Human Reproduction conference abstract describes the first reported human autologous transplantation of immature cryopreserved testicular tissue. In that single patient, the report found graft survival, vascularization, focal spermatogenesis, and sperm retrieval from an intratesticular graft one year after transplantation.

Those findings are important proof-of-concept observations, but they do not provide a success rate. The abstract does not report a pregnancy, live birth, long-term graft function, offspring health, or the frequency of complications. One case cannot show that the technique will work for children with different cancers, treatment exposures, ages, or tissue quality.

Evidence or guidance What it establishes What it does not establish
ESHRE good-practice recommendations, 2025 Forty-four recommendations covering counselling, eligibility, biopsy, transport, cryopreservation, and program quality; clinical use remained unproven when prepared. A standardized transplant protocol, reliable viability test, pregnancy rate, or live-birth rate.
ASCO guideline update, 2024 Testicular tissue cryopreservation in prepubertal males and human tissue grafting should be limited to clinical trials or approved experimental protocols. Routine clinical availability or evidence of comparative effectiveness.
ASRM committee opinion, 2026 Testicular tissue cryopreservation is investigational in prepubertal boys; sperm cryopreservation is an established option when sperm can be obtained. A guarantee that stored tissue will restore fertility.
Human Reproduction conference abstract, July 2026 One autologous recipient had graft survival, vascularization, focal spermatogenesis, and sperm retrieval after one year. Generalizable efficacy, safety, pregnancy, live birth, or long-term offspring outcomes.
NICE research priorities, 2026 Identifies safety and clinical and cost effectiveness of tissue cryopreservation for prepubertal and peripubertal boys as questions requiring research. An established cost-effective standard of care.

Why the technique remains experimental

Biological uncertainty

Immature germ cells must remain viable through biopsy, freezing, thawing, grafting, and revascularization. Even if some cells survive, they must progress through the complete sequence that produces functional sperm. The proportion of tissue capable of doing this may vary widely between patients.

Unstandardized laboratory methods

Centers differ in freezing rates, cryoprotectants, fragment size, storage systems, thawing methods, and quality checks. ESHRE recommends internal validation but notes that no standardized viability assay has been established. Results from one laboratory therefore cannot automatically be applied to another.

Unknown reproductive outcomes

Sperm detected in a graft is an intermediate laboratory outcome. It is different from retrieving sperm that can be used in treatment, achieving a pregnancy, delivering a baby, and demonstrating healthy development of children born after the procedure. The available human report does not answer those later questions.

Long-term safety

Researchers still need to understand how long graft function lasts, whether abnormal cell growth can occur, whether the procedure affects testosterone production or testicular structure, and whether genetic or epigenetic effects could affect resulting sperm. These questions require longer follow-up and more patients.

Cancer-cell contamination: the central safety issue

For patients treated for malignancy, the tissue may contain cancer cells that are not visible in every fragment. Returning such tissue could theoretically reintroduce disease. The concern is especially serious in blood cancers and metastatic disease, where malignant cells may circulate or infiltrate the testis.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assessment may include microscopic examination and marker-based testing, but a negative test is not an absolute guarantee. Sampling error is possible because the tested piece may differ from the piece selected for grafting. The decision must therefore be made by a multidisciplinary team that understands both the cancer diagnosis and reproductive-tissue testing. If markers indicate contamination, retransplantation should be avoided under ESHRE’s recommendations.

How it compares with sperm cryopreservation

Sperm cryopreservation is the established male fertility-preservation strategy when a patient can provide a semen sample. ASCO recommends collecting sperm before antineoplastic treatment, and ASRM describes sperm freezing as established. Some patients need medical or surgical assistance to obtain sperm, and urgent treatment schedules can limit the number of collections.

Consideration Sperm cryopreservation Testicular tissue cryopreservation
Who can use it Patients able to produce or obtain sperm before treatment. Often considered for prepubertal boys or patients unable to provide sperm.
Clinical status Established fertility-preservation option. Investigational; use should follow an approved experimental protocol or clinical trial.
Procedure Semen collection; medical or surgical assistance may be needed. Surgical testicular biopsy under anesthesia, followed by laboratory freezing.
What is stored Mature sperm that can potentially be used in assisted reproduction. Testicular fragments containing immature germ cells and supporting tissue.
Main additional concern Whether enough usable sperm can be obtained before treatment. Whether tissue can later produce sperm and whether it is free of malignant cells.

The appropriate choice depends on age, pubertal development, diagnosis, treatment urgency, the ability to obtain sperm, and the expertise available locally. A specialist should explain whether more than one preservation method is feasible.

What patients and parents should ask

  • Is the proposed procedure tissue banking, transplantation, or both? These are different interventions.
  • Is the program operating under a clinical trial, ethics approval, or another approved experimental protocol?
  • How many biopsies and anesthesia procedures are expected, and what are the immediate surgical risks?
  • Which freezing and thawing method does the laboratory use, and how has it been validated?
  • How will the center test stored tissue for malignant contamination, and what are the limits of that testing?
  • Who controls the tissue if the patient moves, changes hospitals, reaches adulthood, or dies?
  • What future procedures might be required to attempt grafting or sperm retrieval?
  • Which outcome is being measured: tissue survival, sperm production, sperm retrieval, pregnancy, or live birth?
  • What costs are expected for biopsy, storage, future transplantation, monitoring, and assisted reproduction?
  • Is sperm cryopreservation also possible before treatment?

Availability, cost, and realistic expectations

There is no broadly established routine service with a predictable success rate for testicular tissue transplantation. Access is generally limited to specialist centers participating in research or approved experimental programs, and availability varies by country, age group, diagnosis, and institutional expertise.

The sources summarized here do not establish a reliable comparative cost, insurance rule, or cost-effectiveness estimate. Families should request a written explanation of biopsy, storage, annual maintenance, transport, future grafting, and assisted-reproduction charges rather than assuming that one quoted fee covers the entire pathway.

The most accurate expectation is that tissue banking preserves a possibility, not a promise. The stored fragments may never be suitable for transplantation, the cancer diagnosis may make retransplantation unsafe, and future technology or clinical evidence may determine whether use is offered.

When to seek advice

Anyone facing treatment that could impair fertility should ask the oncology team for an urgent referral to a reproductive urologist or fertility-preservation program. Referral should happen before treatment whenever medically possible, but treatment for a serious illness should not be delayed without agreement from the oncology team.

For children, parents or guardians should ask for age-appropriate counselling and should include the child in decisions as much as development allows. Consent and future control of stored tissue should be documented clearly, because the patient may become the decision-maker later.
