Rhabdomyolysis is a potentially serious breakdown of skeletal muscle. Injured muscle cells release myoglobin, creatine kinase (CK), electrolytes and other substances into the bloodstream. Myoglobin and electrolyte disturbances can damage the kidneys, so suspected rhabdomyolysis needs prompt medical assessment rather than home treatment.
Symptoms are variable. Muscle pain, weakness, stiffness, swelling, dark urine and reduced urination may occur, but many people do not have the classic combination of muscle pain, weakness and reddish-brown urine. Fewer than 10% of patients have that full triad, according to the MSD Manual Professional Edition (reviewed February 2026).

How rhabdomyolysis affects the body
Skeletal muscle contains proteins, enzymes and electrolytes that normally stay inside muscle cells. When cells are badly injured, their contents spill into the blood. Myoglobin is then filtered by the kidneys. Its breakdown products can injure kidney tissue, while released electrolytes—especially potassium—can disturb heart and nerve function.

The amount of muscle injury, the speed of treatment and the person’s underlying health all influence the risk. Rhabdomyolysis can be limited to muscle symptoms, or it can progress to acute kidney injury, dangerous electrolyte changes, shock or other complications.

Rhabdomyolysis is not simply ordinary post-exercise soreness
Hard exercise can cause temporary muscle damage without rhabdomyolysis. The concern is substantial muscle injury in a particular context, such as unaccustomed extreme exertion, heat exposure, dehydration, trauma or another medical trigger. There is no single exercise duration, distance or CK value that a person can use at home to establish the diagnosis.
Symptoms: what it can feel and look like
Possible symptoms include:
- Muscle aching, tenderness or severe pain
- Muscle weakness or an unusual loss of strength
- Stiffness, swelling or difficulty moving an affected area
- General weakness or fatigue
- Dark, red or cola-colored urine
- Passing less urine than usual
Symptoms may affect the muscles that were injured, but some people have few or no muscle complaints. Urine can remain its usual color even when rhabdomyolysis is present because discoloration depends on the amount of myoglobin released and how it is excreted.
Why dark urine is not required
“The classic triad of symptoms in rhabdomyolysis is muscle pain, weakness, and reddish-brown urine,” the MSD Manual Professional Edition states. The same reference notes that this triad appears in fewer than 10% of patients. Therefore, clear urine does not rule out rhabdomyolysis, and urine color alone cannot confirm it.

Seek prompt professional assessment if significant muscle pain, weakness, dark urine or reduced urination develops after intense or unfamiliar exercise, a crush or other injury, heat exposure, prolonged immobility, a seizure, an infection or use of a potentially relevant medicine or drug. Worsening weakness, very little urine, confusion, fainting, chest symptoms or severe illness require urgent medical attention.
Do not wait for every symptom to appear. A clinician must decide whether testing and observation are needed, because kidney injury and electrolyte abnormalities may develop even when muscle symptoms seem modest.

What causes rhabdomyolysis?
Rhabdomyolysis has many possible triggers. Clinicians usually look for an event that damaged muscle directly, reduced its blood supply, overheated it, caused excessive contraction or interfered with muscle-cell metabolism.
Trauma, compression and loss of blood flow
- Crush injuries can destroy large amounts of muscle.
- Other major trauma may injure muscle directly.
- Prolonged pressure or immobility can reduce circulation and compress muscle, including after being unable to move for many hours.
- Ischemia, meaning inadequate blood flow, can deprive muscle of oxygen and cause cell injury.
Exertion and temperature stress
- Severe or unaccustomed exertion, including marathon running or demanding calisthenics, can trigger muscle breakdown.
- Heat extremes can increase risk, particularly when exertion and fluid loss occur together.
- Severe dehydration can worsen muscle and kidney stress.
Exercise itself does not automatically mean rhabdomyolysis. The relevant question is whether the muscle injury is excessive for the circumstances and accompanied by clinical or laboratory evidence of breakdown.

- Seizures can produce intense, sustained muscle contraction.
- Some infections are associated with muscle injury.
- Electrolyte abnormalities can contribute to muscle-cell damage.
- Inherited or acquired metabolic and genetic muscle disorders can predispose someone to episodes.
Medicines and drugs
Certain medicines or recreational drugs can injure muscle directly, alter metabolism or create conditions such as overheating or prolonged immobility. The responsible clinician may decide that a medicine should be changed or stopped. Do not stop a prescribed drug on your own; medication decisions require professional review of the risks and benefits.
How clinicians diagnose rhabdomyolysis
Diagnosis combines the history of what happened, a physical examination and laboratory testing. A clinician may ask about exercise, injuries, heat, time spent immobile, seizures, infections, medical conditions, medicines and substance exposure, then examine muscle strength, tenderness, swelling, circulation and urine output.
Creatine kinase (CK)
CK is a central blood test because damaged skeletal muscle releases this enzyme. The MSD Manual describes a CK level typically greater than five times the laboratory’s upper limit of normal as the usual diagnostic pattern, while also noting that no universal cutoff has been established. The appropriate interpretation depends on the clinical situation and the laboratory’s reference range.

CK does not identify the cause or exact location of muscle damage. MedlinePlus Medical Tests explains that CK levels vary with age, sex, race, muscle mass and activity. After some injuries, the level can take up to two days to peak, so a single early result may not show the eventual maximum. Clinicians may repeat testing when the timing or trend matters.
Other tests
- Kidney-function tests: Blood tests help identify impaired filtration and monitor whether kidney injury is developing.
- Electrolytes: Potassium, calcium and other measurements can reveal disturbances that require treatment.
- Urine testing: Urinalysis can provide evidence consistent with myoglobin release, even when urine does not look dark.
- Myoglobin: Blood or urine myoglobin may be assessed in selected situations, although it changes quickly and is interpreted with the rest of the findings.
- Additional evaluation: Depending on the suspected trigger, clinicians may investigate infection, a medication reaction, compartment syndrome, inherited muscle disease or another cause.
No home urine check, symptom checklist or isolated CK number can safely confirm or exclude rhabdomyolysis.
How rhabdomyolysis is treated
Treatment takes place under medical supervision and has two goals: limit further muscle and kidney injury, and correct complications or the underlying trigger.
Fluids and monitoring
Clinicians commonly give intravenous fluids and monitor urine output, kidney function, CK and electrolytes. The amount and rate are individualized because excessive fluid can also be harmful, especially when kidney or heart function is impaired. IV fluids are clinical treatment, not a substitute for evaluation or a do-it-yourself hydration plan.
Addressing the trigger
Care may include treating an infection, relieving dangerous pressure in a muscle compartment, managing a seizure, cooling a person with heat illness, treating trauma or correcting a metabolic problem. If a medicine or drug is suspected, the care team determines whether it should be withheld or replaced.

Correcting complications
Electrolyte abnormalities can require urgent treatment because they may affect the heart and muscles. If acute kidney injury becomes severe, dialysis may be needed to perform some of the kidneys’ filtering functions while the person recovers or receives further treatment.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Complications and kidney risk
The most important complication is acute kidney injury, which can range from a temporary reduction in kidney function to kidney failure. The MSD Manual reports acute kidney injury in 15% to 55% of rhabdomyolysis cases, citing a 2016 systematic review. This is a range observed across reported cases, not an individual prediction.
Risk is higher in situations such as dehydration, sepsis and very high CK levels. Other possible complications include acute tubular necrosis, harmful electrolyte shifts and shock. The need for hospital monitoring depends on the degree of muscle injury, laboratory results, urine output, the trigger and other health conditions.
Recovery and follow-up
Recovery is determined largely by the amount of kidney damage and the event that caused the muscle injury. “The outcome depends on the amount of kidney damage,” MedlinePlus Medical Encyclopedia says in its rhabdomyolysis entry, reviewed September 2, 2025.

MedlinePlus reports that milder cases may return to usual activities within a few weeks to a month. Fatigue or muscle pain can last longer for some people, and recovery after major kidney injury or extensive trauma may take substantially more time. These are general ranges, not a promise for any individual case.

Returning to activity
Do not use a calendar, a urine color change or one CK result to decide when to resume strenuous exercise. Follow-up testing and the treating clinician’s assessment determine when activity can safely increase. The plan may account for kidney function, CK trends, persistent weakness or pain, hydration status and the original trigger. Recurrent or unexplained episodes may prompt evaluation for an inherited or metabolic muscle disorder.
What to remember if you suspect rhabdomyolysis
- Recognize concerning context: major muscle pain or weakness, dark urine or reduced urination after exertion, heat, injury, immobility, seizure, infection or a potentially related medicine.
- Arrange prompt medical assessment rather than waiting for the classic triad.
- Tell the clinician exactly when symptoms began and describe recent exercise, injuries, heat exposure, time immobile, seizures, illnesses, medicines and substances.
- Expect blood and urine testing, including CK, kidney-function measurements and electrolytes; repeat tests may be needed because results change over time.
- Follow the care team’s instructions about fluids, medicines, observation and return to activity. Do not substitute supplements, sports drinks or retail medicines for clinical treatment.
Frequently asked questions
Can rhabdomyolysis occur without dark urine?
Yes. Dark urine is only one possible sign, and the complete classic triad occurs in fewer than 10% of patients. Normal-looking urine does not exclude the condition.
Can intense exercise cause rhabdomyolysis?
It can, particularly when exertion is severe or unaccustomed and occurs with factors such as heat or dehydration. Exercise alone does not establish a diagnosis; clinical evaluation and laboratory testing are required.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsIs a high CK level enough to diagnose the cause?
No. CK confirms muscle-cell injury in the right clinical context but does not show why the injury occurred or exactly where it is. CK also varies with personal factors and may peak up to two days after an injury.
Will everyone with rhabdomyolysis need dialysis?
No. Dialysis is reserved for some people with severe kidney injury or related complications. The need is determined by kidney function, fluid and electrolyte status and the overall clinical picture.
The Bottom Line
Rhabdomyolysis is muscle breakdown that can threaten kidney function even without dark urine or severe muscle pain. Prompt medical assessment, CK and kidney testing, clinician-directed fluids, treatment of the trigger and monitoring for complications are the safe path to recovery.
