Both are caused by the bacterium Yersinia pestis, but they differ in where the infection is concentrated, how it is usually acquired, which symptoms stand out, and whether respiratory droplets can spread it between people.
Bubonic plague most often follows an infected flea bite and produces a painful, swollen lymph node called a bubo. Pneumonic plague causes rapidly developing pneumonia, with symptoms such as cough, shortness of breath, chest pain, and sometimes bloody or watery sputum. Suspected plague is a medical emergency: seek immediate professional care, and clinicians should begin appropriate antibiotics promptly when suspicion is strong rather than waiting for final test results.

The essential difference: lungs versus lymph nodes
The word pneumonic refers to the lungs. In pneumonic plague, Yersinia pestis infects lung tissue and causes rapidly developing pneumonia. The word bubonic refers to buboes, the painful swollen lymph nodes that characterize bubonic plague.

These are clinical forms of the same bacterial disease, not different bacteria. Bubonic plague can sometimes spread through the bloodstream or into the lungs, producing secondary pneumonic disease. Pneumonic plague can also begin as a primary lung infection after a person inhales infectious droplets.

The distinction matters because pneumonic plague can spread from person to person through infectious respiratory droplets during close contact. Ordinary bubonic plague is not the respiratory-droplet-spreading form.
Bubonic and pneumonic plague compared
| Dimension | Bubonic plague | Pneumonic plague |
|---|---|---|
| Main site | Lymph nodes, where painful swollen nodes called buboes develop | Lungs, with rapidly developing pneumonia |
| Common route of exposure | Usually the bite of an infected flea; handling an infected animal is another possible route | Inhalation of infectious droplets, or spread to the lungs from bubonic or septicemic plague |
| Typical distinguishing symptoms | A painful swollen lymph node with fever, headache, chills, and weakness | Cough, shortness of breath, chest pain, and sometimes bloody or watery sputum, with fever, headache, and weakness also possible |
| Person-to-person respiratory spread | Ordinary bubonic plague does not spread between people through respiratory droplets | Possible through infectious droplets during close contact |
| Reported incubation | Usually 2–8 days, according to CDC guidance | After droplet exposure, typically 1–3 days, with a reported range of 1–6 days |
| Response to suspected illness | Immediate medical evaluation and prompt clinician-directed treatment | Immediate medical evaluation and prompt clinician-directed treatment |
The incubation figures describe reported timing, not a diagnostic test. A person can neither confirm nor exclude plague solely by counting days since an exposure.
What causes each form?
How bubonic plague usually begins
An infected flea can transmit Yersinia pestis when it bites. The bacteria then multiply and travel to nearby lymph nodes. The affected node becomes markedly painful and swollen, forming a bubo. Fever, headache, chills, weakness, and other systemic symptoms commonly accompany the swelling.
Handling an infected animal is another possible route. The relevant exposure may involve contact with infected animal tissue or fluids, so a person with a compatible illness should tell a clinician about animal contact as well as insect bites.
How pneumonic plague begins
Primary pneumonic plague follows inhalation of infectious respiratory droplets. The bacteria establish infection in the lungs, and pneumonia can worsen quickly.

Secondary pneumonic plague develops when infection reaches the lungs from another form, usually untreated bubonic or septicemic plague. In that situation, the initial illness may have begun outside the lungs, but the patient can later develop the respiratory symptoms that define pneumonic involvement.
Symptoms and warning signs
Bubonic symptoms
- Fever
- Headache
- Chills
- Weakness
- One or more painful, swollen lymph nodes
The swollen node is often the clearest clue, but its location depends on where bacteria entered and which lymphatic drainage area is involved. If bubonic plague is untreated, it may spread into the bloodstream or lungs.
Pneumonic symptoms
- Rapidly developing pneumonia
- Cough
- Shortness of breath
- Chest pain
- Bloody or watery sputum in some cases
- Fever, headache, and weakness
Pneumonic plague may not produce an obvious sign that identifies it specifically as plague. A person can have serious lung disease without a visible bubo, and the absence of a bubo does not rule plague out. Symptoms overlap with many other infections, so they cannot establish the diagnosis by themselves.
Why the form cannot be self-diagnosed
A painful node after a possible flea or animal exposure raises a different concern than a rapidly worsening cough after close contact with a coughing patient, but neither pattern proves the cause. The type of exposure, examination, laboratory testing, imaging, and the illness’s progression all inform a clinician’s assessment.
How pneumonic plague spreads between people
Person-to-person transmission is associated with pneumonic plague because infected lungs can produce infectious droplets when the patient coughs. The CDC’s April 1, 2026 fact sheet describes close contact as being within six feet of a patient with pneumonic plague while that patient was coughing.

This is not the same as saying every person nearby will become infected. Transmission requires the relevant infectious exposure, and the public-health team evaluates the duration and circumstances of contact. Ordinary bubonic plague does not spread person to person through respiratory droplets; a patient with bubonic disease would need to develop pneumonic involvement for that respiratory transmission concern to arise.
The same 2026 CDC fact sheet reports that, after droplet exposure, pneumonic plague symptoms typically begin in 1–3 days, with a range of 1–6 days. It states that someone exposed to infected droplets more than seven days earlier who remains healthy is very unlikely to develop infection. That statement applies to the specified exposure scenario and is not individualized medical advice.
Incubation and disease progression
| Situation | Reported timing | How to interpret it |
|---|---|---|
| Bubonic plague after the usual exposure | Usually 2–8 days | A typical CDC-reported interval, not a rule that confirms or excludes illness |
| Pneumonic plague after infectious-droplet exposure | Typically 1–3 days; range 1–6 days | A reported interval for that exposure route; symptoms and clinical assessment still determine concern |
| Droplet exposure followed by more than seven healthy days | CDC says infection is very unlikely in the specified scenario | Do not use this population guidance to override symptoms or a clinician’s instructions |
Because pneumonic symptoms can develop rapidly, someone who becomes ill after a relevant close exposure should not wait for the incubation window to end before seeking care.
What to do if plague is suspected
If you have compatible symptoms
- Seek immediate medical attention, especially for rapidly developing pneumonia, shortness of breath, chest pain, cough with bloody or watery sputum, fever, or a painful swollen lymph node.
- Tell the clinician about the timing and type of possible exposure, including flea bites, animal handling, travel or residence in an area where plague is present, and close contact with someone who was coughing.
- Follow instructions about evaluation and contact with other people while the cause is being assessed.
The CDC’s public guidance says, “If you are experiencing symptoms like those listed here, seek immediate medical attention.” Do not attempt to confirm plague at home or rely on leftover or consumer-purchased antibiotics.
If you were a close contact of pneumonic plague
Contact a healthcare professional or the relevant public-health authority promptly and describe exactly when, where, and how close the contact was. The six-foot definition in the 2026 CDC fact sheet is specifically tied to being near a coughing patient; public-health officials use the full exposure circumstances to decide what follow-up is needed.

How clinicians diagnose plague
Diagnosis combines the clinical picture with exposure history and laboratory testing. Samples may be collected for testing, but a strong suspicion should not be placed on hold while final results are pending.
The form of illness helps guide the evaluation: a painful bubo supports concern for bubonic disease, while rapidly developing pneumonia after a relevant exposure supports concern for pneumonic disease. Neither finding is exclusive, and secondary spread can change the presentation.
Why treatment must start promptly
Plague is serious but treatable. CDC clinical guidance states: “Begin appropriate therapy as soon as plague is suspected.” Clinicians choose the antibiotic and route according to the patient’s illness severity and other clinical factors. Diagnostic samples can be collected at the same time, but treatment should not be delayed for final confirmation when suspicion is strong.
Because pneumonic plague can involve rapidly worsening lung disease and can expose close contacts, urgent evaluation also allows clinicians and public-health teams to assess people who may have been exposed.
Common misconceptions
“A bubo is required for plague.”
No. A bubo is characteristic of bubonic plague, but pneumonic disease may present primarily as pneumonia and may have no obvious plague-specific sign.
Best Value
“All plague spreads through coughs.”
No. Respiratory-droplet spread is associated with pneumonic plague. Bubonic plague usually follows a flea bite and does not ordinarily spread person to person through respiratory droplets.
“A symptom-free week proves there was no exposure.”
The CDC’s statement about being very unlikely to develop infection after more than seven healthy days applies to the specified droplet-exposure scenario. It is not a substitute for professional advice, particularly if symptoms appear or the exposure details are uncertain.
“Waiting for a laboratory result is safer than starting treatment.”
When plague is strongly suspected, CDC clinical guidance calls for appropriate therapy to begin promptly while testing proceeds.

Pneumonic plague is Yersinia pestis infection in the lungs, usually acquired by inhaling infectious droplets or by spread from another form of plague. Bubonic plague usually follows an infected flea bite and centers on painful swollen lymph nodes. The lung involvement and potential for close-contact respiratory transmission make suspected pneumonic plague especially urgent, but both forms require immediate professional medical evaluation and prompt clinician-directed treatment.
