In people with lung cancer, it can be part of cancer cachexia, a broader syndrome that may also involve loss of body fat, weakness and fatigue. Tell your oncology team about unintended weight loss, appetite changes or difficulty eating; these symptoms deserve assessment, but do not by themselves prove cachexia.
What muscle wasting and cancer cachexia mean
Muscle wasting describes a reduction in muscle tissue. When it occurs alongside changes such as loss of body fat, weakness and fatigue, it may be part of cancer cachexia. Severe depletion of muscle is often called sarcopenia.

Cachexia is not simply a matter of eating too little. Cancer-related inflammation and changes in how the body uses nutrients can contribute, and the loss may continue even when someone is getting enough calories and protein. The National Cancer Institute (NCI) explains that nutrition support alone cannot reverse cachexia. NCI: Nutrition in Cancer Care.

These are clinical descriptions, not stages a person can determine from symptoms at home. NCI: Nutrition in Cancer Care.

Why it can affect people with lung cancer
Lung cancer is among the cancer types associated with increased metabolism and weight loss, which can be relevant to cachexia. But not everyone with lung cancer develops it, and the reviewed sources do not establish a lung-cancer-specific prevalence figure. An NCI feature estimates that cachexia affects “up to 80% of people with advanced cancer”; that broad estimate varies by cancer type and treatment response and is not an individual risk estimate or a lung-cancer-specific rate. NCI Cancer Currents: Cancer Cachexia Treatment Research.
Cancer and treatment can also make eating difficult. Possible barriers include appetite loss, feeling full quickly, changes in taste or smell, dry mouth, mouth sores, swallowing problems, nausea, pain, fatigue, anxiety or depression. These can occur alongside metabolic changes; the problem is not necessarily solved by simply trying to eat more. NCI: Nutrition in Cancer Care.

Changes to notice and report
Tell the oncology team about changes such as:
- Unintended weight loss or reduced appetite and interest in food.
- Visible loss of muscle, weakness, reduced strength or worsening fatigue.
- Difficulty with everyday activities that used to be manageable.
- Eating or swallowing problems, or symptoms that make meals difficult.
Weight alone may not show the full picture. A clinical assessment can consider nutritional status, body composition, symptoms and ability to function. Weight loss, fatigue, weakness or poor appetite can have several causes, including cancer, treatment effects and other health issues; none of these symptoms alone establishes cachexia. Record what has changed and when it began so you can describe it to your care team. NCI: Weight Changes.
When to contact the oncology team
Contact your oncology team promptly if you have appetite loss, trouble eating or unintended weight loss. The NCI specifically advises reporting a loss of more than 3 to 5 pounds in one week and asking about nutrition screening and a referral to a registered dietitian. NCI: Weight Changes.

For sudden severe symptoms, follow the urgent-care instructions from your oncology team or local emergency guidance. The cited sources do not provide a cachexia-specific emergency threshold.
Questions you can bring to an appointment include:
- “Could treatment side effects be making it hard for me to eat?”
- “Can I have a nutrition screening?”
- “Could I meet with an oncology dietitian?”
- “Would physical therapy or an assessment of my strength help?”
How the care team may help
Care is individualized and may address more than food intake. Depending on the person’s symptoms and situation, the team may:
Rank #4
- Assess nutrition, weight changes, symptoms and physical function.
- Treat problems that interfere with eating, such as nausea, pain, mouth sores or swallowing difficulties.
- Refer to a registered dietitian for tailored nutrition counseling.
- Consider clinician-selected medicines, such as appetite stimulants or anti-inflammatory drugs.
- Recommend physical therapy to support strength and endurance.
A dietitian may discuss high-calorie, high-protein foods or nutrition supplement drinks such as Ensure or Boost. These products can be part of nutrition support, but they are not proven standalone treatments that reverse cachexia. Ask the oncology team or dietitian whether a particular product is appropriate for you. NCI: Nutrition in Cancer Care.

Nutrition interventions are tailored to individual needs, and evidence for reversing cachexia remains limited. In an NCI feature, oncologist Eric Roeland, M.D., of Oregon Health & Science University, said of patients’ familiarity with the condition: “Most patients ‘are completely unfamiliar with the term and what [cachexia] is,’ Dr. Roeland said.” NCI Cancer Currents: Cancer Cachexia Treatment Research.

Clinical criteria used to identify cachexia can include more than 5% weight loss over six months, or specified combinations of body-mass index, sarcopenia and weight change. These are clinical criteria—not a checklist for self-diagnosis. NCI: Nutrition in Cancer Care.
