What Is Immunotherapy for Bowel Cancer, and Who May Be Eligible?

Immunotherapy is an option for some people with bowel (colorectal) cancer, mainly when tumour testing shows high microsatellite instability (MSI-H) or deficient mismatch repair (dMMR). In England, current NICE recommendations cover specific situations involving unresectable or metastatic disease, both before and after chemotherapy. A positive biomarker is important, but it does not by itself decide treatment: stage, whether surgery is possible, previous medicines, how quickly the cancer needs to be controlled, general health and local funding all matter.

What immunotherapy means in bowel cancer

Immunotherapy uses medicines that help the immune system recognise and attack cancer. The treatments discussed in current colorectal-cancer guidance are immune checkpoint inhibitors. They work by releasing inhibitory signals that can prevent immune cells from responding effectively to a tumour.

The key medicines in the NICE pathway are nivolumab, ipilimumab and pembrolizumab. They are prescription treatments given and monitored by an oncology team; they are not consumer products, supplements or home treatments. The choice of medicine depends on the clinical setting and the person’s test results and health.

The tumour features doctors look for

MSI-H and dMMR

Microsatellites are short, repeated DNA sequences. “High microsatellite instability” (MSI-H) means the tumour shows many errors in these sequences. “Deficient mismatch repair” (dMMR) means the tumour’s DNA-repair system is not working normally. These are related findings that can identify colorectal cancers more likely to respond to particular checkpoint inhibitors.

NICE recommends testing people with colorectal cancer for mismatch-repair deficiency or microsatellite instability when the cancer is first diagnosed. Your pathology or molecular report may use one term, the other, or both. The treating team interprets the result alongside the cancer’s stage, location, resectability and your previous treatment.

A biomarker is not a treatment prescription

An MSI-H or dMMR result can open an immunotherapy option, but it does not guarantee that treatment is suitable. The oncology team must also establish whether the disease can be removed surgically, whether it has spread, which treatment line you are in, how urgently a response is needed, and whether there are health issues that affect safety. Funding and authorisation rules may limit what is available in a particular health system.

Who may be eligible under current NICE guidance in England?

The following table summarises the settings covered by the cited NICE recommendations. “Untreated” refers to the advanced disease being considered, not necessarily to having received no treatment of any kind at any point.

Clinical setting Biomarker Medicine named by NICE Important qualification
Untreated, unresectable or metastatic colorectal cancer High MSI or MMR deficiency Nivolumab with ipilimumab Recommended as an option for adults when the disease cannot be removed surgically or has spread.
Untreated metastatic colorectal cancer High MSI or MMR deficiency Pembrolizumab Listed by NICE as an option; the oncology team determines suitability and the precise regimen.
Metastatic colorectal cancer after fluoropyrimidine-based combination chemotherapy High MSI or MMR deficiency Nivolumab with ipilimumab A separate, previously treated pathway from first-line treatment.
Metastatic colorectal cancer after that chemotherapy when nivolumab with ipilimumab cannot be used High MSI or MMR deficiency Pembrolizumab NICE specifies a maximum of two years of uninterrupted treatment, or stopping sooner if the cancer progresses.

NICE’s recommendation for first-line nivolumab plus ipilimumab appears in technology appraisal TA1065, published on 28 May 2025. The recommendations describe NHS practice in England, not a universal rule for every country.

Untreated advanced disease: how eligibility is considered

When the cancer is unresectable or metastatic

For an adult whose colorectal cancer is untreated in the relevant advanced setting and whose tumour is MSI-H or dMMR, NICE recommends nivolumab with ipilimumab as an option. “Unresectable” means the disease cannot be removed completely by surgery; “metastatic” means it has spread to distant parts of the body. The two terms can overlap, but they describe different aspects of the disease.

NICE’s colorectal-cancer guidance also lists pembrolizumab for untreated metastatic disease with the same biomarker findings. This does not mean a patient can select whichever medicine they prefer. The team weighs the extent and pace of disease, symptoms, other illnesses, possible toxicities, treatment goals and local arrangements.

Why treatment may start before every result is available

The NICE committee noted that treatment decisions depend on genomic test results being available. If testing is delayed, or if the team believes chemotherapy is preferable because rapid disease control is needed, chemotherapy may be used first. This is a clinical decision, not advice to postpone testing or to choose a regimen without specialist guidance.

After previous chemotherapy

Nivolumab plus ipilimumab after fluoropyrimidine-based combination treatment

For metastatic colorectal cancer that is MSI-H or dMMR after fluoropyrimidine-based combination chemotherapy, NICE recommends nivolumab with ipilimumab. Your team will verify what medicines you have already received and whether the current disease fits this treatment line.

When pembrolizumab is used later

Pembrolizumab is recommended in this previously treated setting only when nivolumab with ipilimumab cannot be used. NICE specifies treatment for no more than two years without interruption, with earlier stopping if the cancer progresses. The two-year limit is a condition of this recommendation; it should not be assumed to apply identically to every immunotherapy regimen in every country.

What happens from diagnosis to a treatment decision

  1. Confirm the diagnosis. The pathology team establishes that the tumour is colorectal cancer and records its location and other relevant features.
  2. Request or review MSI/MMR testing. NICE recommends this testing at diagnosis. Ask whether the report says MSI-H, dMMR, another result, or that testing is still pending.
  3. Stage the cancer and assess resectability. Scans and specialist review determine whether disease is localised, locally advanced, unresectable or metastatic, and whether surgery could remove it.
  4. Map previous treatment. The team records chemotherapy, surgery, radiotherapy and any other medicines. A first-line plan is different from a plan after fluoropyrimidine-based combination chemotherapy.
  5. Assess urgency and overall health. Symptoms, the pace of disease, organ function, other medical conditions and medicines can influence whether immunotherapy or chemotherapy is safest and most practical.
  6. Check access and authorisation. NICE recommendations apply to England’s NHS. Hospital policies, funding decisions and rules in other countries can change which option is available.
  7. Discuss benefits, risks and alternatives. The oncology team explains the expected purpose of treatment, monitoring, possible immune-related complications, alternatives and when treatment would stop.

What if the cancer is localised?

The routine NICE recommendations described above concern unresectable or metastatic colorectal cancer. They should not be read as a recommendation for every person whose tumour is MSI-H or dMMR at an earlier stage or after surgery.

Clinical trials may investigate immunotherapy before surgery, after surgery or in other earlier-stage situations. Each trial has its own inclusion and exclusion criteria, which may specify stage, biomarker confirmation, previous treatment, laboratory results and general health. A trial record cannot be converted into a general eligibility rule. Ask your colorectal team whether a suitable, locally available study exists.

How doctors balance the main decision factors

Stage and resectability

A biomarker result has different implications in a tumour that can be removed surgically compared with disease that has spread or cannot be removed. Surgery, systemic treatment or a combination may be considered in different sequences.

Previous treatment

The distinction between untreated disease and disease after fluoropyrimidine-based combination chemotherapy is central to the NICE recommendations. Bring an accurate list of medicines and dates to the appointment, including treatments given at another hospital.

Need for rapid control

If symptoms or the pattern of spread means a rapid response is important, the team may consider chemotherapy first, particularly while biomarker testing is pending or when chemotherapy is judged more suitable. The decision belongs to the treating specialists.

Health, medicines and preferences

Other illnesses, current medicines, ability to attend monitoring visits, personal priorities and willingness to accept different risks all matter. A treatment that is technically listed in guidance may still be unsuitable for an individual.

Local rules

NICE guidance describes England. Approval, reimbursement, hospital protocols and trial access differ elsewhere. A result that supports an option in one jurisdiction may not produce the same access in another.

Questions to take to your cancer appointment

  • Has my tumour been tested for MSI or mismatch-repair deficiency, and does the report show MSI-H or dMMR?
  • What test established the result, and is any confirmatory testing still needed?
  • Is my cancer localised, unresectable or metastatic, and can surgery remove it?
  • Am I being considered for first-line treatment, or have I already had the chemotherapy relevant to a later-line recommendation?
  • Does the biomarker result change the options for my stage and treatment line?
  • How quickly does treatment need to work, and does that affect the choice between immunotherapy and chemotherapy?
  • Which regimen, schedule and monitoring plan are you recommending, and why?
  • What symptoms should I report urgently, and what happens if side effects occur?
  • What alternatives are available if this treatment cannot be used or the cancer progresses?
  • Is the proposed treatment funded and available where I am receiving care?
  • Would a clinical trial be relevant, and what eligibility rules would it use?

How common is the relevant biomarker?

In a 2021 NICE committee discussion for pembrolizumab, high MSI or DNA mismatch-repair deficiency was reported in around 4% of metastatic colorectal cancer. That figure is tied to the population and appraisal context described by NICE; it is not a percentage for all bowel cancers, all stages or every country.

There is no single percentage that can determine an individual’s eligibility. The pathology result, stage, resectability, treatment history and local rules are more useful for personal decision-making than a population estimate.

What immunotherapy can and cannot tell you

An MSI-H or dMMR result is a reason to have a specialist discussion about checkpoint inhibitors. It is not proof that a medicine will work, a promise that treatment will be funded, or a substitute for staging and medical assessment. Conversely, a result that is not MSI-H or dMMR means the specific NICE options discussed here may not apply; it does not by itself describe every possible colorectal-cancer treatment.

Only a colorectal cancer specialist with your pathology report, scans, treatment history and current health can determine whether an immunotherapy option is appropriate. If you are outside England, ask your team to explain the guidance and funding rules that apply in your country.

The NICE recommendation in plain language

“Nivolumab with ipilimumab is recommended as an option for untreated unresectable or metastatic colorectal cancer with high microsatellite instability (MSI) or mismatch repair (MMR) deficiency.”

That sentence captures the central eligibility pattern, but the surrounding conditions still matter: the tumour must have the relevant biomarker, the disease must fit the stated setting, and an oncology team must confirm that treatment is suitable and available.
