Extrachromosomal DNA (ecDNA) in cancer is circular DNA found outside a cell’s chromosomes. It can carry cancer-driving genes and regulatory DNA, and its uneven inheritance as tumour cells divide may help cancers change and diversify. ecDNA is a feature of tumour biology—not a cancer diagnosis or, on its own, a guide to treatment.
What ecDNA is—and what it is not
Most of a cell’s DNA is organized into chromosomes. Cancer-associated ecDNA consists of circular DNA molecules that exist separately from those chromosomes, inside the cell nucleus. They can carry oncogenes—genes that promote cancer when abnormally active or present in excess—as well as regulatory regions that influence gene activity. A 2024 study described individual ecDNA elements as typically larger than 500 kilobases, though that does not mean every ecDNA molecule has that size. The study in Nature describes ecDNA in cancer cells.

Not every kind of extrachromosomal circular DNA should be treated as cancer-associated ecDNA. The term here refers to the gene-containing and regulatory DNA structures studied in tumours, not every circular DNA molecule found outside chromosomes.

How ecDNA can influence a tumour
It can increase cancer-gene activity
Because ecDNA can carry oncogenes, tumour cells may have extra copies of genes that help drive growth. The amount of a gene and the way it is regulated can affect its activity. ecDNA’s circular structure may also bring genes and regulatory elements into new interactions, including through structures known as ecDNA hubs. These features can alter gene expression; the details of how they work are still an active area of research. A 2024 review surveys ecDNA formation, maintenance, transcription and its role in tumour biology.
#1 Best Overall
It can create differences between tumour cells
Chromosomes are normally copied and distributed to daughter cells in an organized way. ecDNA is inherited outside the chromosomes and may be distributed unevenly when a cell divides. As a result, daughter cells—and different cells within one tumour—can end up with different numbers of ecDNA copies. That variation can contribute to tumour heterogeneity, meaning that cancer cells in the same tumour are not all alike.
Such diversity can give a tumour ways to evolve as conditions change, but ecDNA does not make every tumour behave the same way. Its presence, gene content and effects vary.
Rank #2
It may involve more than oncogenes
A 2024 analysis also found ecDNA carrying immunomodulatory and inflammatory genes, and reported an association between this type of ecDNA and reduced T-cell infiltration in tumours. This is a finding from that study, not an established clinical test or a rule for choosing immunotherapy. The study’s results should be understood in the context of its cohort and methods.
How common is ecDNA in cancer?
Bailey and colleagues analyzed samples from 14,778 patients across 39 tumour types in the Genomics England 100,000 Genomes Project. In that 2024 study, ecDNA was detected in 17.1% of tumour samples. This percentage applies to that cohort, its tumour types and the study’s detection approach; it is not an estimate for every cancer population or every cancer type. The 2024 Nature paper reports the analysis.
Rank #3
The same study reported that ecDNA detection was associated with tumour stage, metastases, prior targeted or cytotoxic treatment, and shorter overall survival. These are population-level associations. They do not show that ecDNA alone caused an outcome or predict what will happen to a particular patient.
Can ecDNA make cancer resistant to treatment?
EcDNA’s ability to alter gene copy number and expression, and to vary among tumour cells, provides plausible routes for cancer evolution under changing conditions, including treatment. The 2024 cohort analysis found ecDNA more prevalent after targeted and cytotoxic treatment, but that association does not establish that treatment caused ecDNA or that ecDNA is the sole reason a cancer resists therapy.
Research has also explored ways to exploit vulnerabilities in cancer cells that contain ecDNA. In an account dated December 5, 2024, the U.S. National Cancer Institute described BBI-355 as being tested in a small human clinical trial. That report establishes its status at that time only; it does not establish current trial enrollment, approval or standard-of-care use. NCI’s dated report explains the experimental strategy.

What an ecDNA finding means for a patient
EcDNA is a research finding about the biology of a tumour. It cannot be identified from symptoms, and the sources cited here do not establish a universal clinical testing standard. An ecDNA result by itself does not diagnose a cancer, determine prognosis for an individual or select a treatment. Patients should discuss the meaning of any tumour-genomics result with their oncology team, which can interpret it alongside the cancer type, stage and other clinical findings.
