What Is an AI Incident Response Plan and How Do You Build One?

An AI incident-response plan is an approved, tested playbook for detecting, assessing, containing, communicating, recovering from and learning from failures or abuse involving an AI system. It extends conventional cybersecurity response to cover models, training and retrieval data, prompts, outputs, tool calls, human oversight, vendors and potential social harm.

The practical goal is not a longer policy document. It is to give named people the authority, evidence and pre-approved actions needed to stop unsafe behavior quickly, protect affected people and restore a validated service.

Why AI incidents require a different response plan

A conventional security plan may assume a compromised host, stolen credential or malicious network connection. An AI incident can begin with any of those, but it can also arise from a model update, poisoned retrieval source, prompt injection, an over-permissive agent, a privacy failure or a plausible but dangerous answer. The same model can produce different outcomes when its system prompt, tools, context window, data source or human-review path changes.

AI incidents therefore include failures of the model, data, prompts, tools, vendors, oversight and outputs—not only attacks against infrastructure. A response plan must be able to stop an autonomous action, disable a retrieval source, roll back a model or prompt, preserve a conversation trace and decide whether people need to be notified.

NIST’s AI Risk Management Framework (AI RMF) organizes this work into Govern, Map, Measure and Manage. Its Core says, “Risk management should be continuous, timely, and performed throughout the AI system lifecycle dimensions.” In Manage 4.1, post-deployment monitoring plans explicitly include user feedback, appeal and override, decommissioning, incident response, recovery and change management. See the NIST AI RMF Core.

NIST guidance is voluntary and must be adapted to the system, sector and jurisdiction. The AI RMF Playbook states that it is “neither a checklist nor set of steps to be followed in its entirety.” Use it to select controls that fit your risk rather than treating it as a certification recipe.

What an AI incident-response plan should contain

Purpose, scope and authority

Start by stating which systems and environments are covered: internal copilots, customer-facing assistants, predictive models, generative applications, agents, development sandboxes, production APIs and third-party AI embedded in business software. Include jurisdictions, business services and vendors. State who may pause traffic, disable a tool, revoke credentials, roll back a model or prompt, place an application in read-only mode, or decommission it. If authority is unclear, responders lose time seeking permission during the incident.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Roles and contacts

Name a 24-hour incident commander and deputies. The roster should include the AI or model owner, data owner, security operations or incident-response lead, privacy, legal, safety or responsible-AI lead, communications, procurement or vendor management, the business owner and an executive decision maker. Record primary and backup contacts, escalation paths and the authority each role has. A model owner can explain a failure; the incident commander controls the response; legal and privacy specialists determine notification duties.

System and dependency inventory

Maintain an inventory that responders can access during an outage. For every system, record model and fine-tune versions, system prompts and policy files, training and retrieval data, embedding indexes, tools and agents, API keys and cloud accounts, monitoring, log locations, human-review points, downstream consumers and third parties. Include deployment regions and data classifications. Without this map, a team may roll back the model while leaving a poisoned retrieval index or an exposed tool credential active.

Detection and intake

Define how an incident enters the process: automated alerts, user reports, abuse reports, red-team findings, quality monitoring, vendor notices and employee escalation. Use one intake channel with required evidence such as timestamps, account or session identifiers, model and prompt versions, representative inputs and outputs, tool-call traces and the reporter’s impact assessment. Provide a safe route for reporting sensitive content without spreading it through ordinary email or chat.

Severity and activation

Rate impact across people, privacy, security, physical or operational safety, bias and discrimination, financial loss, legal exposure, service availability and reputation. Define levels and deadlines before an event occurs. For example, a critical incident may involve imminent physical harm, confirmed sensitive-data exposure at scale, unauthorized financial or administrative actions, or compromise of a production control plane; it should trigger immediate executive and legal escalation. A lower level can cover a contained hallucination with no affected person, while still requiring a ticket, owner and trend review. The exact labels are yours; the thresholds and decision authority must be explicit.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Evidence and records

Preserve immutable logs where feasible: model, prompt, policy and data versions; inputs and outputs subject to privacy controls; retrieval documents; tool-call and authentication traces; approvals; decisions; timestamps; notifications; and chain-of-custody records. Capture a snapshot before changing the system. Redact or restrict personal and confidential data, and document who can access the evidence. If logs are sampled, note the sampling method and its limitations.

Communications and reporting

Specify internal update intervals, executive briefings, customer or user notices, partner contacts, regulator engagement and when law enforcement may be appropriate. Legal review is jurisdiction-specific; do not promise a universal notification deadline. Prepare plain-language templates that explain what happened, which data or actions were involved, what users should do and how you will provide updates. Keep technical indicators and sensitive evidence in a restricted annex.

Recovery and improvement

Define restoration gates: the cause is understood enough to control recurrence, unsafe paths are disabled, fixes are reviewed, data and credentials are clean, monitoring is operating and a business owner accepts residual risk. Plan for user remediation such as correcting records, reversing an automated action, resetting credentials or offering a human review. Close with root-cause analysis, corrective actions, assigned owners and due dates, then update the plan.

How to build the plan with the NIST lifecycle

1. Govern: establish accountability

  1. Obtain executive sponsorship and name an accountable owner for the program.
  2. Set risk tolerance for privacy, safety, fairness, availability, financial and legal impacts.
  3. Connect the AI plan to existing cybersecurity incident response, privacy breach response, business continuity and vendor-risk processes.
  4. Approve the authority matrix: who can stop traffic, revoke a key, roll back a release, notify customers and accept residual risk.

NIST released AI RMF 1.0 on January 26, 2023. Its companion Playbook’s complete version is dated March 30, 2023. The Generative AI Profile, NIST AI 600-1, is listed as July 26, 2024. NIST says the AI RMF is being revised, so verify the current status when you approve or refresh your plan. The framework history and resources are at NIST AI RMF development and NIST AI RMF resources.

2. Map: understand use, misuse and dependencies

  1. List each AI use case, intended user, business owner, deployment environment and critical business function.
  2. Trace data flows from collection and training through retrieval, inference, output storage and downstream decisions.
  3. Identify foreseeable misuse, affected people, vulnerable groups and possible harms.
  4. Mark human approval points and the actions an AI system can take without approval.
  5. Document third-party models, hosting, tools, data providers, support contacts and contractual incident obligations.

Pay particular attention to systems that can send messages, change records, execute code, move money, control devices or make decisions about people. Their response plans need faster activation and stronger containment than a read-only internal assistant.

3. Measure: define telemetry and thresholds

Select indicators for the system’s risk profile. Model measures can include refusal failures, unsafe-content rates, drift and confidence calibration. Data measures can include provenance, poisoning signals, leakage and access anomalies. Security measures include prompt-injection detections, unusual token use, credential use and model-extraction patterns. Privacy and fairness measures can include sensitive-attribute exposure, disparate error rates and complaint trends. Availability measures include latency, error rates, quota exhaustion and denial-of-service indicators.

For each indicator, specify the data source, owner, threshold, alert destination and expected response. Test that logs contain enough context to reconstruct an event without collecting unnecessary personal data. Test rollback and credential rotation in a non-production environment, and document how long each action takes.

4. Manage: turn risks into decisions and actions

Write a decision tree for every high-priority scenario. Pre-authorize low-regret containment actions—such as disabling an external tool, blocking a data source, switching to a safe fallback or placing outputs behind human review—while reserving irreversible actions for the incident commander or executive authority. Include communications, evidence preservation, recovery validation and corrective-action tracking.

5. Exercise: prove the plan works

Run tabletop exercises with technical and business participants. Use realistic injects: a user reports a prompt injection that exposed internal instructions; a retrieval index begins returning confidential records; an agent sends unauthorized messages; or a vendor reports a compromised model endpoint. Record time to detect, time to decision, missing telemetry, authority conflicts, evidence-access problems and notification delays. Convert every gap into an owner and due date.

6. Maintain: treat the plan as a lifecycle control

Review it after incidents and exercises, major model or vendor changes, new tools or data sources, regulatory changes and on a scheduled cadence. A change-management ticket should ask whether the threat model, severity thresholds, logging, rollback steps, contacts and user notices still apply. Manage 4.1’s emphasis on monitoring, incident response, recovery and change management means the plan cannot remain static after launch.

Scenario playbooks responders can use

Prompt injection or jailbreak

  1. Capture the full conversation, retrieved context, system and developer instructions, model version and tool traces.
  2. Classify whether the attack only changed an answer or caused data access or an external action.
  3. Disable the affected tool or route, add a temporary content or instruction filter, and move risky requests to human review.
  4. Assess whether other sessions or tenants saw the same payload, then search logs for indicators.
  5. Patch instructions, isolation and authorization checks; test against the original and variant attacks before restoring automation.

Sensitive-data disclosure

Stop further exposure by restricting the affected index, conversation history or endpoint and rotating credentials if they may have been revealed. Preserve evidence under privacy controls. Determine whose data was exposed, the time window, recipients and whether it was downloaded or acted upon. Privacy and legal teams then assess jurisdiction-specific notification and remediation duties. Remove or quarantine the source, correct retention and access controls, and require a clean validation before reopening it.

Poisoned or corrupted data

Identify the first bad ingestion or transformation, quarantine the dataset or index and prevent retraining or retrieval from it. Compare hashes, provenance and approval records with a known-good copy. Roll back the model or index, reprocess data through validation gates and examine outputs produced during the contaminated period. Keep the contaminated artifact available for forensic analysis, but inaccessible to production workloads.

Unsafe or biased output

Preserve representative prompts and outputs, affected populations and any human decisions based on them. Reduce exposure with stricter filters, lower permissions, a fallback model or mandatory human review. Measure whether the issue is isolated or systematic across languages, groups, use cases and model versions. Correct the model, data or policy, provide an appeal or correction path, and document residual risk before resuming normal operation.

Unauthorized agent or tool action

Revoke the agent’s credentials and disable the tool or workflow first. Identify every action taken, target system, account and downstream consequence. Reverse transactions or records where possible, preserving an audit trail. Inspect authorization boundaries, confirmation prompts, sandboxing and tool schemas; do not restore autonomous access until a controlled test demonstrates that the dangerous path is blocked.

Model theft, extraction or denial of service

For suspected extraction, preserve request patterns, rate data and account activity; tighten quotas, authentication and output detail while investigating. For denial of service, use traffic controls and a safe fallback, then distinguish malicious load from a runaway application or vendor outage. Coordinate with the hosting or model provider and document what information the provider needs to investigate.

Vendor compromise, deepfake or harmful generated content

Invoke the vendor escalation path, isolate the integration and verify signed releases, domains, keys and data flows. For impersonation or harmful generated content, preserve provenance and distribution evidence, limit further generation or publication, and coordinate with affected users, platforms and legal advisers. Avoid amplifying the material in internal or public communications.

Comparison checklist for evaluating a plan

Capability Questions to ask
AI-specific coverage Does it address prompts, data, outputs, tools, agents, oversight and societal harm?
Roles and escalation Are decision makers, backups, thresholds and deadlines named?
Detection and telemetry Can responders reconstruct model, prompt, data and tool activity?
Containment and rollback Are pause, isolation, credential rotation, fallback and rollback actions tested and authorized?
Evidence quality Are logs protected, privacy-controlled and linked to a chain of custody?
Third parties Are vendor contacts, dependencies and contractual notifications included?
Communications Does it cover users, partners, regulators and public disclosure with legal review?
Recovery validation Are safety tests, monitoring and residual-risk acceptance required before reopening?
Exercises and change Are tabletop findings, incidents and major releases tracked into plan updates?

Testing cadence and evidence of readiness

Exercise the highest-risk scenario after initial deployment and whenever a major model, tool, data source or vendor changes. Run a broader tabletop on a regular schedule set by your risk owner; high-impact systems generally need more frequent exercises than low-risk internal experiments. After every exercise, retain the scenario, participants, timeline, decisions, missing data, failed assumptions and corrective actions.

A plan is ready when a responder can find the inventory and contacts, identify the incident commander, preserve a useful trace, execute an approved containment action, explain the notification decision and prove that recovery gates were met. A document that has never been exercised cannot demonstrate those capabilities.

Current NIST incident-response references

NIST published SP 800-61 Rev. 3 in April 2025. It integrates incident-response recommendations with Cybersecurity Framework 2.0 and can provide the conventional response foundation around which AI-specific scenarios, evidence and authority are added. Pair it with the AI RMF materials rather than replacing one with the other.
