What Is a Child Protection Review, and What Should It Cover?

A child protection review conference checks whether a child remains at risk of significant harm and whether the protection plan is making the child safer. It examines the child’s current safety, wellbeing and development against the plan’s intended outcomes, then decides whether the plan should continue, change or end. The process and timetable below are specific to Wales; procedures differ by jurisdiction.

What a child protection review is for

A review conference brings together relevant practitioners and others involved in the child’s plan to assess what has changed for the child and what needs to happen next. The central question is not simply whether everyone completed assigned tasks. It is whether the evidence shows that the child is safer and that the risks identified at the initial conference have reduced.

In Wales, each review should consider the child’s safety, wellbeing and development against the outcomes in the care and support protection plan, and determine whether the child remains at risk of significant harm. Wales Safeguarding Procedures set out this purpose.

What the conference should examine

The child’s experience and current risk

Consider the child’s circumstances now: their safety, wellbeing and development, what they experience day to day, and whether the concerns that led to the plan remain. Compare current information with the risks and outcomes identified earlier. A list of completed actions is not, by itself, evidence that risk has reduced.

Progress and the effect of the plan

Review what family members and practitioners have done, what has changed as a result, and whether those changes have helped safeguard the child. Use current information alongside relevant completed assessments, including specialist assessments where applicable. The discussion should test whether the plan’s actions are producing the intended outcomes for the child.

Whether the plan and partnership still fit

Decide whether the existing plan remains adequate or needs specific revisions. Identify what must change to safeguard the child and how progress will be monitored. Consider whether agencies are coordinating effectively, whether the core group has the right membership, and whether responsibilities are clear. Wales guidance addresses both the plan and its coordination in the Wales Safeguarding Procedures.

Participation, support and communication

The child’s views should be sought in a way suited to their age and understanding. The child and family should be involved in planning and implementation as appropriate, with support or advocacy identified where needed. Scottish Government guidance separately emphasizes taking account of a child’s “reactions, perceptions, wishes and feelings”; this is a child-centred principle, not a statement of Wales procedure. See National Guidance for Child Protection in Scotland.

Plan how the decision and next steps will be communicated to the child and family. Also consider contingency arrangements for significant changes or non-cooperation, and whether further health, care or legal action should be considered.

What decision should come out of the review?

The conference should reach a reasoned decision about continuing risk and whether the protection plan remains necessary. In Wales, that decision should reflect analysis of available information, assessments, reports and the discussion among conference members.

  • If the plan continues: specify any changes, who is responsible for each action, how progress will be monitored and when the next review will take place.
  • If the plan ends: record why the evidence supports ending it and how the child’s safety will be maintained afterwards.

How often reviews happen in Wales

Under the Wales procedure, the first child protection review conference should take place within three months of the initial conference. Subsequent reviews should be held at regular intervals no more than six months apart while the plan remains in place. Care Inspectorate Wales also describes the first review within three months and later reviews at intervals no greater than six months. These are Wales-specific timings, not a universal timetable; check the applicable current rules in your jurisdiction.

How to tell whether a review is useful

  • It uses current, child-centred evidence rather than relying only on activity reports.
  • It tests whether the child’s safety and development have improved against the plan’s outcomes.
  • It considers the child’s and family’s involvement and whether they had suitable support.
  • It checks that agencies are coordinating and that responsibilities are understood.
  • It reaches a clear decision and, where action remains necessary, assigns responsibilities and a next review date.
