It is called a “driver” because it helps propel cancer behavior; simply finding a mutation in tumor tissue does not prove that it is driving the disease.
Drivers can turn growth signals on, disable the genes that restrain division or trigger cell death, or weaken DNA-repair systems. Cancer usually develops through a changing combination of alterations rather than one mutation acting alone. Other mutations in the same tumor are passengers: they may be biologically neutral or have little effect on the cancer’s growth.

What “driver” means in cancer biology
The National Cancer Institute (NCI) defines a driver mutation as a DNA change that causes cells to become cancer cells and helps them grow and spread. The important word is causes. A tumor may contain many DNA changes, but a mutation is not a driver merely because a laboratory test detects it.

If an altered cell divides more often, survives a condition that would kill a normal cell, or accumulates descendants more successfully than neighboring cells, that alteration can give its cell population a selective advantage. The descendants carrying it may expand into a larger clone within the tumor.

“Driver” describes a mutation’s functional role in a tumor. It does not mean that the alteration is sufficient by itself, that every cancer cell carries it, or that a treatment targeting it necessarily exists. NCI notes that a person’s cancer has its own combination of genetic changes, and a particular alteration can affect the likelihood of responding to treatment without determining the outcome on its own.
By contrast, a passenger mutation is carried along as cancer cells multiply but contributes little or nothing to their competitive advantage. A tumor can also contain changes with no clearly detectable effect. Separating these categories matters because sequencing can reveal a long list of alterations, while only a smaller subset is helping the tumor grow.
The three main ways a driver mutation changes a cell
Most introductory explanations fit driver mutations into three functional patterns. The categories describe what normal cellular job is disrupted, not a complete list of every possible cancer mechanism.
1. Growth signals become permanently active
Proto-oncogenes normally help regulate cell growth and division. Their activity is controlled so that a cell receives a growth instruction at the appropriate time and then stops. A DNA change, or an increase in the activity of the gene, can convert a proto-oncogene into an oncogene.
An oncogenic change can make a growth pathway behave as if an “increase production” signal is always present. The cell may enter the division cycle too readily, continue receiving survival instructions, or fail to respond normally when growth should pause. The result is not simply faster copying of DNA; it is a shift in the decision rules that govern whether the cell grows, divides, or remains quiescent.

2. Growth restraints or cell-death controls are lost
Tumor suppressor genes normally act as restraints. They help control cell division, monitor cellular stress, and in some circumstances activate cell death when damage cannot be safely repaired. A DNA change that inactivates such a gene can remove a checkpoint that would otherwise hold a damaged cell back.
TP53 is a well-known example of a tumor suppressor gene involved in controlling cell division and cell death. If this kind of control is impaired, cells with serious abnormalities may continue dividing instead of stopping for repair or being eliminated. The driver is therefore a loss of a protective function rather than an extra growth command.
3. DNA repair is impaired
DNA-repair genes help correct damage that occurs as DNA is copied or exposed to damaging conditions. When repair is weakened, errors and chromosome changes can accumulate more readily. That instability can create additional alterations affecting growth, survival, or cellular controls.
A repair defect may therefore contribute in two stages: the original change removes part of the cell’s maintenance system, and the resulting accumulation of further changes increases the chances that another driver will appear. The repair change is not necessarily a direct “grow faster” signal; it can make the evolving cell population more likely to acquire changes that produce that behavior.
| Normal function affected | Typical driver effect | Resulting cell behavior |
|---|---|---|
| Growth signaling through a proto-oncogene | Activity becomes excessive or stuck on, creating an oncogene | Division or survival signals continue when they should stop |
| Growth restraint and cell-death control through a tumor suppressor | Protective function is lost or disabled | Cells ignore division limits or survive damage that should remove them |
| DNA maintenance through a repair gene | Repair capacity is reduced | More mutations and chromosome changes accumulate, increasing the chance of additional drivers |
How a driver mutation can lead to a growing tumor
Cancer development is better understood as an evolving cell population than as a single event. A simplified sequence looks like this:

- An initial cell acquires a DNA change. The alteration may affect a growth signal, a restraint, cell-death control, or DNA repair. At this point, the cell is not automatically a cancer cell; NCI notes that one genetic alteration is rarely enough.
- The altered cell gains an advantage under its local conditions. If it divides more successfully or survives better than nearby cells, it leaves more descendants. Those descendants retain the driver as the clone expands.
- Some descendants acquire additional changes. A growing population has more opportunities for further DNA alterations. If repair is impaired, this accumulation can be accelerated. Different branches of the population may gain different combinations.
- Subclones compete within the tumor. Cells with changes that improve growth or survival can become more abundant, while other branches remain small or disappear. The tumor therefore may contain several genetically distinct groups of cells rather than one uniform population.
- Later alterations can support spread. NCI’s definition includes growth and spread because some combinations of changes can help cells persist and establish disease beyond the original site. Which changes matter differs among cancers.
This model explains why a mutation can be important without being the first event, and why a tumor’s behavior cannot always be inferred from one laboratory result. The effect of a driver depends on the other alterations present and on the cell in which it occurs.
Driver mutations, passenger mutations and neutral changes
| Category | What it means | Why it matters |
|---|---|---|
| Driver | The alteration contributes to a selective advantage or to cancer behavior such as inappropriate growth, survival, or spread. | It may help explain how the tumor developed and could be relevant to treatment decisions. |
| Passenger | The alteration is present because it was inherited by a multiplying cancer cell but confers little or no advantage. | It can add to the tumor’s mutation list without being a useful treatment target. |
| No clear effect | The available evidence does not show that the change alters the cell’s behavior. | Detection alone should not be treated as proof of cancer-driving activity. |
Passengers are not necessarily meaningless. They can record the history of the tumor’s evolution and may help scientists study how the cancer changed. For a patient-facing interpretation, however, the key question is whether a change contributes to the disease’s behavior, not how many total mutations were detected.
Why one mutation rarely explains a cancer
NCI states that a single genetic alteration is rarely enough to make a cell cancerous. A cell normally has overlapping systems that regulate growth, division, death, and DNA maintenance. Disrupting one control may create an advantage, but additional changes can be needed to overcome other safeguards and support continued expansion.
That is why two tumors with the same broad diagnosis can have different combinations of drivers. One may be dominated by an overactive growth signal, another by loss of a tumor-suppressor function, and another by a repair defect followed by several later changes. The combinations also influence how the tumor behaves and whether a treatment aimed at one alteration is relevant.
A 2020 NCI research report described mapping genomic alterations across 38 cancer types. That number is the scope of that study, not a count of every cancer type or a claim that all cancers share one driver profile. Its broader lesson is that cancer genomes contain patterns of both recurring functional changes and many additional alterations, with the balance varying by cancer.

What tumor testing can tell you
Testing tumor tissue can identify DNA changes that may be drivers, passengers, or changes whose significance is not established. The test result is therefore an input to clinical interpretation, not a diagnosis by itself. A report may list a mutation because it was detected, while separate evidence is needed to determine whether it contributes to the tumor’s growth.
The NCI explains the potential clinical use this way:
“Checking tumor tissue for driver mutations may help plan treatment to stop cancer cells from growing, including drugs that target a specific mutation.”
That statement describes a possibility, not a guarantee. A driver mutation may have no approved targeted medicine, a treatment may work only in certain cancer contexts, and the tumor may contain other changes that affect response. Treatment choices also depend on the cancer’s location and extent, previous therapy, overall health, and current clinical guidance.

Pick the symptom - the matching free tool is one click away.
- A detected alteration is not automatically the cause of the cancer.
- A driver designation does not promise that a targeted drug is available.
- A potentially targetable change does not guarantee that treatment will work.
- Clinical decisions require the full pathology and medical context, not a mutation name alone.
Because targeted-treatment availability changes, an individual result should be discussed with the treating oncology team using current guidance for the relevant cancer type.

Common misunderstandings
“Every mutation in a tumor is a driver.”
No. Tumors commonly contain many passenger changes, and some detected changes have no known effect. Functional contribution is what separates a driver from a passenger.
“A driver mutation always causes cancer by itself.”
Usually not. NCI’s explanation that one alteration is rarely sufficient reflects the multistep nature of cancer. A driver can provide an early or later advantage while other changes are still needed.
“Driver means the mutation is inherited.”
Not necessarily. The term describes what a DNA change does in the tumor’s development. Whether a particular alteration arose in the tumor or was present earlier is a separate question that requires clinical interpretation.

“If a driver is found, the treatment is obvious.”
No. Testing may help plan treatment, but not every driver has a matching therapy, and the same alteration can have different implications in different cancers. A result must be interpreted alongside the complete clinical picture.
The practical definition to remember
A cancer driver mutation is a DNA change that functionally pushes a cell toward cancer by activating growth, removing restraints on division or death, weakening DNA repair, or otherwise giving the cell a selective advantage. As that cell and its descendants expand, additional changes can accumulate, producing a tumor made up of multiple related but not identical populations. Passenger mutations travel with this process without materially driving it, which is why identifying the functional drivers—not merely listing every mutation—is central to understanding a cancer and considering treatment.
