Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →Use a risk-based approval policy rather than a universal allow-or-deny list. Let the AI execute only predefined, low-impact and readily reversible actions within a documented playbook. Require a trained human to confirm actions with meaningful service, user, data or operational effects, and use authorized incident-command or dual-control approval for high-consequence or difficult-to-reverse actions. Every path needs an accountable approver, an audit trail, and a working way to pause, override or deactivate the AI.
There is no universal approval matrix
NIST does not publish a single table saying that a particular incident-response action is always safe for autonomous execution or always requires a person. The appropriate threshold depends on your systems, business impact, evidence quality, delegated authority, regulatory obligations and tolerance for disruption. The NIST AI Risk Management Framework (AI RMF) Core, released January 26, 2023, supplies outcomes and controls; your organization must translate them into action-level rules.

That translation should be written into the incident-response plan, playbooks and access-control model. An approval setting is not merely a button in an AI product. It defines who may let the system act, what evidence they must see, how a decision is recorded, and how operations recover if the recommendation is wrong.

Use three operational approval bands
The following bands are a practical policy design, not categories named by NIST. Classify each action in your environment, document the rationale, and revisit the classification after incidents and near misses.

| Band | When it fits | Illustrative actions | Required control |
|---|---|---|---|
| Pre-authorized | Low potential impact, tightly bounded scope, high confidence and a quick, tested rollback. | Collecting additional telemetry; opening or enriching a case; applying a temporary, narrowly scoped block already approved in a playbook; restarting a noncritical, stateless worker when health checks and rollback are defined. | Permit automatic execution only within explicit limits. Log the recommendation and result, enforce stop conditions, and expose a human stop or override route. |
| Human confirmation required | Meaningful service impact, uncertain blast radius, sensitive-data implications, or material effects on customers or employees. | Quarantining a production host; disabling an account used by a business process; changing a firewall rule across several systems; rotating credentials that may interrupt integrations. | Show the responder the evidence, affected assets, proposed change, expected impact, confidence or uncertainty, and rollback procedure before approval. |
| Restricted or dual control | High consequence, broad scope, safety or essential-service implications, destructive effects, or no reliable reversal. | Broad isolation or shutdown of critical services; destructive remediation; deletion or irreversible alteration of evidence; actions that could affect physical safety or essential operations. | Require an authorized incident commander or designated specialist. Add independent second-person approval when your risk assessment calls for separation of duties. |
The examples illustrate how to design a policy; an action is not inherently safe or prohibited in every environment. A tightly isolated test system may justify automation that would require approval in production, while a small change can be high risk if it touches a safety system or regulated data.
Decide approval using five risk axes
Impact
Estimate the plausible harm if the AI is wrong or the action has an unexpected side effect. Consider people, customers, revenue, business operations, critical services, confidentiality, integrity and availability. Define impact tiers in terms your responders can recognize, such as a single disposable endpoint, one customer-facing service, or an organization-wide identity system.
Reversibility
Ask whether the action can be undone quickly and reliably, not merely whether a theoretical reversal exists. A tested configuration rollback is different from restoring a database from backup. Record the rollback command or procedure, its prerequisites and the person authorized to invoke it. If evidence, data or safety conditions cannot be restored, treat the action as restricted.
Scope and uncertainty
Combine the number and sensitivity of affected assets with confidence in the detection. An AI recommendation based on incomplete logs, conflicting indicators or an unfamiliar environment should move upward in the approval bands. Limit automatic scope by asset list, time window, rate, network segment or transaction count; do not let a broad, uncertain recommendation execute as if it were a bounded one.

Authority and competence
Confirm that the AI and the human operator have delegated permission for the action. A technically capable responder may still lack authority to affect a regulated system or another team’s service. The approving person needs enough context and role-appropriate proficiency to understand the recommendation, challenge it and carry out recovery. The NIST AI RMF Playbook Govern guidance calls for defined responsible functions and personnel, as well as suitable proficiency and training.
Time sensitivity
Compare the harm caused by waiting with the harm caused by an incorrect action. A fast-spreading compromise may justify a pre-authorized containment step with strict limits, while a slowly developing anomaly may warrant a human investigation. Time pressure does not remove accountability: predefine emergency authorities, maximum scope and an automatic expiry or review point.
Define the human-AI boundary in policy
Write a role map that names the AI’s delegated tasks and the humans responsible for approval, denial, escalation, override, recovery and post-incident review. State which team owns each asset class and who may act when the primary approver is unavailable. Include separation-of-duties rules for sensitive actions and a named incident commander for major events.
NIST AI RMF Govern 3.2 says: “Policies and procedures are in place to define and differentiate roles and responsibilities for human-AI configurations and oversight of AI systems.” Under Map 3.5, it adds: “Processes for human oversight are defined, assessed, and documented in accordance with organizational policies from the govern function.” Treat those statements as design requirements for your documentation, not as a ready-made action list.

Build an approval workflow responders can use under pressure
- Register the action. Give every automated or proposed action a stable name, owner, affected asset classes, permitted scope, prerequisites, maximum duration and rollback reference.
- Set the default band. Assign pre-authorized, human-confirmation or restricted status using the five risk axes. Record why the action received that status and which conditions would move it to a stricter band.
- Present decision context. Before a human confirms, display the triggering evidence, data freshness, affected assets, proposed commands or changes, expected impact, confidence and known uncertainty. Show what the AI did not observe.
- Check authority. Verify the approver’s identity, role, on-call status and permission for the asset and action class. Escalate automatically when the person lacks authority or required context.
- Capture the decision. Store the recommendation, evidence presented, approver identity, decision, timestamp, expiry, execution result and any comments. Record rejection reasons, overrides, recovery actions and who performed them.
- Execute with guardrails. Enforce allow-lists, rate limits, timeouts, transaction limits and environment boundaries in the execution layer rather than relying on the model’s instructions alone.
- Verify and close the loop. Run health and security checks after execution. If the result differs from the expected effect, pause further actions, invoke rollback or escalate to the incident commander.
The exact record fields above are implementation guidance consistent with NIST documentation, monitoring and incident-tracking outcomes; NIST does not mandate one universal schema.
Make override, pause and deactivation real
A human-in-the-loop design fails if a person can approve but cannot stop the system. Provide an immediately reachable control to reject a pending action, pause a playbook, supersede an instruction, cancel queued work and deactivate the AI integration. Test those controls with the same permissions and failure conditions used in production.
Define what happens when the model produces inconsistent outcomes, loses access to required evidence, exceeds its scope or behaves outside its intended use. Assign a person who can disengage the system, preserve evidence, continue manually and authorize reactivation. Include a break-glass path for urgent containment, with retrospective review rather than silent bypass.

Connect approval rules to the incident-response plan
Do not operate AI actions as a separate automation experiment. The NIST AI RMF Playbook expects policies for AI monitoring and incident response, with responsible functions and personnel identified. Map each approval band to your existing preparation, detection and analysis, containment, eradication, recovery and lessons-learned activities.

Use the current cyber incident-response baseline: NIST finalized SP 800-61 Revision 3 on April 3, 2025, superseding Revision 2. Align AI-specific escalation, evidence handling and communications with that current guidance, while also applying contractual, legal and sector requirements that are more specific.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Scale governance for generative and high-impact systems
The NIST Generative Artificial Intelligence Profile, published July 2024, emphasizes oversight and evaluation proportional to identified risks and addresses incident response and containment roles in complex generative-AI systems. For an AI agent that can call tools, modify infrastructure or affect users, assess not only the model output but also the tool permissions, chained actions, prompt or data injection risks and the blast radius of a compromised account.
Use narrower scopes and more frequent human checkpoints as capability and consequence rise. A model that only drafts a ticket can usually remain in a lower band; one that can change identity policy or isolate a production cluster should face stronger authorization, independent review and continuous monitoring.

Train the approvers
Training should cover the AI’s intended use, known failure modes, confidence and uncertainty signals, asset criticality, rollback procedures, evidence preservation and escalation contacts. Exercise both an ordinary approval and a refusal during a tabletop scenario. A person who cannot explain the proposed effect should not approve it merely because the interface displays a high confidence score.
Best Value
Monitor recommendations and outcomes
Review false positives, false negatives, rejected recommendations, overrides, execution failures, rollback events and near misses. Monitor for drift in data quality, asset inventories, permissions and model behavior. Alert when the AI repeatedly reaches a scope limit or when humans approve actions without reviewing required context.
Protect the audit trail
Make decision and execution records tamper-evident, access-controlled and retained for the period required by your policy or applicable law. Synchronize timestamps, link actions to the incident identifier and preserve the model version, policy version and tool identity so investigators can reconstruct what happened.
Review after incidents and near misses
After-action review should ask whether the band was appropriate, whether evidence and rollback information were sufficient, whether authority was clear, and whether the AI or a human bypassed a safeguard. Update playbooks, scopes, training and approval assignments. NIST AI RMF Manage 4.1 specifically includes post-deployment monitoring mechanisms for appeal and override, decommissioning, incident response, recovery and change management.

- One global setting: A single “autonomous” or “manual” switch ignores differences between disposable assets and critical services. Classify actions and environments separately.
- Confidence-only decisions: A model score does not describe business impact or reversibility. Require asset context and a rollback assessment.
- Human rubber-stamping: If the interface hides evidence, scope and uncertainty, approval becomes ceremonial. Make those fields mandatory and record the decision.
- Unbounded emergency access: Break-glass authority without limits can create a second incident. Set scope, duration, logging and retrospective review.
- Unusable rollback: A rollback link that has not been tested under incident conditions is not a control. Exercise it and document prerequisites.
- Missing ownership: If nobody is named to reject, escalate, override or deactivate the AI, the organization has no operational human oversight.
- Stale classifications: New integrations, changed data sensitivity or expanded tool permissions can raise risk. Reassess the band whenever the system, asset or action changes.
A concise policy template
For each AI incident-response action, document: its purpose and owner; permitted environments and assets; default approval band; impact, reversibility, scope and uncertainty thresholds; required approver and backup; evidence the interface must show; execution limits and expiry; rollback and evidence-preservation steps; pause, override and deactivation methods; logging and retention; and the review trigger after an incident, near miss or material system change.
Apply the policy with organizational rules and applicable law. The AI RMF is voluntary guidance, so a regulated sector, customer contract or safety requirement may impose stricter approval, recordkeeping or independent-review controls than the framework alone.
