Security teams should require explicit human approval before an AI agent performs a high-impact or irreversible operation, show the reviewer the exact action that will run, and have an independent policy or execution layer verify the request before it reaches a tool. Approval is only one control: agents also need least-privilege access, deterministic safeguards, complete and accessible audit records, reliable pause and stop mechanisms, and fail-closed behavior when a critical check cannot be completed.
The right threshold depends on impact, reversibility, data sensitivity, privilege and external visibility. Low-risk work can run within pre-approved boundaries; destructive, financial, administrative, externally visible or sensitive-data operations generally require a person in the loop.

Start with a risk-based approval policy
Do not use a single “autonomous” or “manual” switch for every agent task. Classify each proposed operation by what could happen if it is wrong, delayed, replayed or manipulated. The classification should consider:
- Impact: Could the action affect safety, finances, legal obligations, customers or business continuity?
- Reversibility: Can the result be reliably undone, or is it permanent?
- Sensitivity: Does it expose personal, confidential, regulated or security-sensitive data?
- Privilege: Does it change identity, access, infrastructure, code or security controls?
- External visibility: Will it send information or make a change outside the organization?
OWASP’s AI Agent Security Cheat Sheet states: “Require explicit approval for high-impact or irreversible actions.” That principle is more useful than a universal list because organizations can map it to their own systems and consequences.

| Risk tier | Typical characteristics | Control response |
|---|---|---|
| Low | Read-only, reversible work with limited data and no external side effect | Run under a documented policy boundary; retain normal monitoring and logs |
| Medium | Limited changes, sensitive internal data or actions that are reversible with effort | Require stronger policy checks; use approval for operations that exceed preset limits |
| High | Privileged changes, sensitive-data transfer, financial or externally visible actions | Require explicit, operation-specific human approval and step-up authentication where appropriate |
| Irreversible | Deletion, destructive configuration, non-recoverable publication or other permanent effects | Require explicit approval, short-lived authorization and replay protection before execution |
These tiers are a starting model, not a mandatory industry taxonomy. A seemingly routine task can become high risk when its target, data set or permissions change.
Make approval specific to the action
A reviewer must see what the agent intends to do, not merely a natural-language summary such as “I fixed the issue.” The approval screen or workflow should present the normalized operation in terms a person can verify:
- the tool or API that will be called;
- the identity under which it will run;
- the exact target, account, resource or recipient;
- all material parameters, filters, quantities and data fields;
- the expected effect and whether it can be reversed;
- the policy decision that caused approval to be requested; and
- the time limit after which the approval expires.
Approval should be bound cryptographically or transactionally to those details. If the agent changes the target, parameters, tool, scope or data after approval, the previous decision must not carry over; the operation should be re-evaluated and, when required, sent back for approval.
Recommended Free Tools
Use an independent enforcement point
The component that executes the action should independently verify permissions, scope and approval state. Do not let the model decide that its own request is authorized. A policy service, gateway or tightly controlled tool runner can compare the proposed call with the approved action, reject extra parameters, check that the approval is still valid and record the final decision.

This separation also limits prompt-injection damage. Untrusted instructions may influence an agent’s reasoning, but they should not be able to grant a new permission, alter an approved target or bypass a deterministic deny rule.
Keep permission controls separate from approval
Human review cannot compensate for excessive standing access. Give each agent only the tools, data and operations needed for its assigned job. Deny unapproved actions by default, and use deterministic allowlists and parameter validation for sensitive tools.
Least-privilege requirements
- Use a distinct workload identity for each agent or narrowly defined agent role.
- Limit accessible data sets, fields, environments and tenants.
- Separate read, write, delete, publish and administrative capabilities.
- Constrain destinations, record counts, monetary amounts and command arguments.
- Use short-lived credentials or authorization artifacts rather than reusable, broad secrets.
For critical operations, require step-up authentication for the human approver. The additional factor should be requested at the point of approval, not assumed from an earlier low-risk login.
Design a review workflow that people can actually use
Approval is a security control only when the reviewer can understand the request and stop the agent in time. A practical workflow has these stages:

- Classify: The agent or orchestration layer assigns a risk level using impact, reversibility, sensitivity, privilege and external visibility.
- Prepare: The agent produces a structured action proposal with the exact tool, target, parameters, data scope and expected outcome.
- Check policy: A separate policy component verifies that the operation is permitted and determines whether approval is required.
- Request approval: The reviewer receives the full proposal, relevant context and any warnings, with an explicit approve, reject or request-change decision.
- Bind and expire: The system binds approval to the normalized operation, approver identity and timestamp, then assigns a short validity period.
- Recheck at execution: The execution component verifies that the operation is unchanged, the permissions remain valid and the approval has not expired or been used already.
- Execute and report: The tool runs within the approved scope, while progress and the final outcome remain visible to the reviewer.
For long-running plans, approval should be requested for each consequential step or for a tightly bounded batch whose scope cannot expand. A blanket approval for an open-ended objective is difficult to audit and easy to misuse.
Provide a dependable pause and stop path
Microsoft Learn describes meaningful oversight as “giving users meaningful control to guide, correct, and interrupt autonomous behavior—especially when input is ambiguous, actions are high‑impact, or adversarial manipulation is possible.” Implement that control as an operational capability, not a message the agent is expected to obey.
- Expose a prominent pause or stop control to authorized operators.
- Make the control reachable outside the agent’s own conversation or user interface.
- Ensure the execution layer can cancel queued work and revoke short-lived authorization.
- Show whether the agent is planning, awaiting approval, executing, paused, stopped or completed.
- Display which actions have finished and which remain queued after an interruption.
Stopping an agent may not undo an already completed external action. The status view and audit record should distinguish cancellation from rollback and show the last confirmed outcome.
Build an audit trail that reconstructs events
Logs should let an investigator answer, without relying on the model’s memory: What did the agent try to do? Which tools and data did it use? Who authorized it? What policy decisions were made? What actually happened?

Minimum event record
| Record element | What it establishes |
|---|---|
| Agent and workload identity | Which software identity initiated the request |
| Human actor and approval evidence | Who reviewed the action, what decision they made and how identity was verified |
| Tool, operation and normalized parameters | The exact capability invoked and the arguments that were authorized |
| Target and data scope | Which resource, account, recipient or records were in scope |
| Risk and policy decisions | Classification, rules evaluated, allow or deny result and any escalation |
| Times, expiry and replay state | When the proposal, approval, execution and expiration occurred, and whether the authorization was reused |
| Execution outcome | Success, partial completion, error, cancellation and the external system’s response |
| Progress and interruption events | Pauses, stops, resumptions and changes in execution state |
Keep records accessible after execution for security review, incident response and compliance work. Protect logs against unauthorized alteration, but do not make them so opaque that investigators cannot understand the sequence of events. Preserve the action proposal and the final executed call when they differ; that difference can reveal a policy failure or an attempted scope change.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Fail closed when safeguards fail
An agent should not continue with a risky action when a critical control is unavailable. Fail closed if risk classification cannot be completed, the policy service cannot be reached, approval cannot be validated, authorization has expired, replay protection cannot be checked or the audit event cannot be written.
For low-risk, read-only work, an organization may define a separate degraded mode, but that exception must be explicit, bounded and logged. It must never silently extend to destructive, privileged or externally visible operations.
Prevent replay and scope drift
Irreversible actions need more than a timestamp in a log. Use single-use or otherwise short-lived authorization artifacts, bind them to the exact operation and reject duplicate execution attempts. Include an idempotency or transaction identifier where the destination system supports one, and verify that the target state has not changed in a way that invalidates the approval.
Best Value
Re-evaluate an approval when a plan is edited, a tool version changes, the target changes, the data set expands or the agent resumes after a long pause. Treat these events as a new authorization decision rather than an implementation detail.
Test the controls against realistic failure modes
Security teams should test the workflow before granting production access. Scenarios should include:
- A prompt injection instructs the agent to send confidential data to a new destination.
- The model proposes a safe target, then changes a parameter after approval.
- An approval expires while a job is queued.
- The same authorization is submitted twice.
- The policy service or audit sink is unavailable.
- A reviewer presses stop while several tool calls are in flight.
- A tool returns partial success or an ambiguous timeout.
- A privileged credential is valid but the requested operation is outside the approved scope.
For each test, verify that the action is blocked or safely bounded, the reviewer receives an understandable status, and the audit record contains enough evidence to reconstruct the decision.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesUse these criteria when evaluating an agent implementation
Whether you are assessing a commercial platform or an internal build, compare implementations on the controls that determine real oversight:
- Can risk thresholds be configured by impact, reversibility, sensitivity, privilege and external visibility?
- Does the reviewer see exact operation details, and is approval bound to those details?
- Is authorization enforced independently at execution time?
- Are identities, permissions and credentials least-privileged and short-lived?
- Are action, tool, decision, approval and outcome records complete and accessible?
- Can operators pause, stop, cancel and determine what has already completed?
- Are replay, expiry, scope changes and control outages handled safely?
NIST’s National Cybersecurity Center of Excellence announced on February 5, 2026, a proposed project to apply identity standards and best practices to software agents, including identification, authorization, auditing, non-repudiation and prompt-injection controls. The announcement described a concept paper open for public comment through April 2, 2026; it should not be treated as a finalized universal agent-control standard.
Quick Recap
A concise control checklist
