What Health Screenings Are Recommended for Large-Breed Dogs as They Age?

Large-breed dogs do not need an automatic battery of X-rays simply because they are getting older. The useful approach is a veterinarian-led senior-care plan: regular physical examinations, laboratory screening at defined intervals, and additional tests when the dog’s breed, history, symptoms, or examination findings justify them. The American Animal Hospital Association (AAHA) describes senior care as a combination of ongoing assessment and targeted diagnostics, not a one-size-fits-all checklist.

When is a large-breed dog considered a senior?

AAHA defines the senior life stage by longevity rather than by one universal birthday: a dog is considered senior during the last 25% of its estimated lifespan, continuing through the end of life. Because expected lifespan varies among breeds and individual dogs, two large dogs of the same chronological age may not enter the senior stage at exactly the same time. Your veterinarian can estimate the transition using the dog’s breed, size, medical history, and current condition.

This definition matters because screening should become more deliberate as the risk of age-associated disease rises, but the schedule still has to reflect the individual animal. AAHA’s 2023 senior-care guidance states that “old age is not a disease.” A senior label should prompt closer observation and appropriate testing, not an assumption that every abnormality is inevitable or that every available test is necessary.

The routine senior screening schedule

The following table summarizes the frequency framework in AAHA’s senior-dog recommendations. It is a discussion guide for a veterinary appointment, not an order for owners to arrange tests independently. The veterinarian decides which tests are appropriate and how to interpret them together.

Screening or assessment AAHA frequency What the recommendation includes
Comprehensive complete blood count (CBC) Every 6–12 months Cell counts, morphology, and differentials; a pathologist may review results when necessary.
Chemistry panel Every 6–12 months Total protein, albumin, ALT, glucose, BUN, creatinine, sodium, potassium, calcium, and SDMA when available.
Urinalysis Every 6–12 months Urine specific gravity, sediment, glucose, ketones, bilirubin, protein, and occult blood.
Thyroxine (T4) Annually AAHA’s senior-dog table lists a yearly T4 check.
Blood pressure Annually A yearly blood-pressure measurement is listed for senior dogs.
Heartworm and tick-borne disease testing Annually The table lists annual testing; the exact approach still depends on veterinary advice and exposure risk.
Fecal parasite testing 1–4 times per year Frequency depends on preventive products and the dog’s lifestyle.
Electrocardiogram (ECG) Annually for high-risk breeds such as Boxers and Dobermans, or when clinically indicated Not a blanket annual test for every large-breed dog; it is used when breed risk, an arrhythmia concern, or examination findings support it.
Urine protein:creatinine (UPC) ratio When proteinuria or persistent microalbuminuria is identified A follow-up measurement for a urinary abnormality, not routine testing of every senior dog.
Imaging When a region of interest is clinically indicated; breed-specific screening may be appropriate AAHA provides no universal imaging timetable for all senior dogs.

AAHA also describes a senior medical workup about once or twice a year. The visit frequency can be adjusted when a dog has an established illness, abnormal results, rapidly changing signs, or a veterinarian’s concern that warrants closer monitoring.

What the routine laboratory tests can reveal

Complete blood count

A comprehensive CBC evaluates the circulating blood-cell populations and includes morphology and differentials. Comparing results over time can help the veterinarian recognize a new change rather than treating one isolated value as the whole story. If the laboratory result is difficult to interpret or unusual, AAHA’s table allows for pathologist review when necessary.

Chemistry panel

The senior chemistry recommendation covers proteins, liver-associated ALT, glucose, kidney-associated BUN and creatinine, electrolytes, calcium, and SDMA when that marker is available. The veterinarian reads these values alongside the physical examination, hydration status, medications, and urinalysis. A single result outside a reference interval does not by itself establish a diagnosis or determine treatment.

Urinalysis

Urinalysis adds information that bloodwork cannot provide. The listed assessment includes concentration through urine specific gravity, sediment, glucose, ketones, bilirubin, protein, and occult blood. Results are interpreted with the chemistry panel and the dog’s drinking, urination, appetite, and other changes at home.

Thyroxine and blood pressure

AAHA lists T4 and blood-pressure checks as annual parts of senior screening. Whether a result needs confirmation or additional testing depends on the clinical context, medications, examination findings, and the veterinarian’s interpretation.

Infectious and parasite testing

Heartworm and tick-borne disease testing appear on the annual senior schedule. Fecal testing is more variable: AAHA gives a range of one to four tests per year because preventive products and lifestyle alter exposure. A dog that travels, hunts, visits dog parks, or has other exposure may receive different advice from a primarily indoor companion.

Tests that are conditional rather than automatic

Some diagnostics are valuable only when a risk factor or abnormal finding makes them relevant. Treating these as universal requirements can expose a dog to unnecessary procedures, expense, or confusing incidental findings.

Electrocardiogram

AAHA specifically cites annual ECG screening for high-risk breeds such as Boxers and Dobermans, or use when an arrhythmia is suspected or another clinical indication exists. That example should not be generalized to every large breed. A veterinarian may recommend an ECG after detecting an irregular rhythm, fainting episode, exercise intolerance, or another cardiovascular concern, but the decision belongs to the clinical evaluation.

Urine protein:creatinine ratio

The UPC ratio is a follow-up test when screening identifies proteinuria or persistent microalbuminuria. It helps the veterinarian investigate whether urinary protein loss is meaningful and whether additional monitoring is needed. It is not listed as a routine test for every otherwise unremarkable senior dog.

Imaging

AAHA recommends imaging a region of interest when clinical findings make it appropriate and notes that breed-specific screening may be indicated. The guidance does not establish a universal schedule of radiographs, ultrasound examinations, or advanced imaging for all aging large-breed dogs.

Does an older large dog need routine X-rays?

Not automatically. AAHA’s life-stage guidance distinguishes a consideration for orthopedic radiographic screening in large-breed dogs as young adults from the approach for mature adults and seniors. For older dogs, the evidence does not support a general imaging recommendation for every individual; imaging is directed by symptoms, examination findings, a known disease, or a breed-specific reason.

Examples of information that can justify an orthopedic study include a new change in gait, reluctance to rise, reduced willingness to exercise, localized pain, or a finding on the physical examination. The veterinarian may choose a radiograph, ultrasound, or another modality based on the body region and question being investigated. A normal screening image would not replace an examination or ongoing observation at home.

Why the physical examination still matters

Laboratory values are only one part of senior care. AAHA recommends taking a detailed history and performing a physical examination that considers eating and drinking, exercise, movement, play, elimination, attitude, grooming, vision, hearing, and changes noticed at home. Owners often see gradual changes before they become obvious in a clinic, so specific observations are clinically useful.

Short videos of the dog walking, climbing a familiar step, getting up from rest, or playing at home can help the veterinarian assess mobility and movement. A video also preserves a baseline that can be compared if the dog’s function changes later.

Skin and other masses deserve direct assessment rather than reassurance based only on appearance. AAHA says skin masses should be measured, aspirated, and examined cytologically instead of being assumed benign. The veterinarian may recommend further diagnostics if cytology, growth, location, or the examination raises concern.

How breed, history, and lifestyle change the plan

“Large breed” is a useful starting point, not a diagnosis. AAHA notes that senior dogs may continue to be screened for cancer and orthopedic, kidney, liver, heart, gland, and eye abnormalities that can be breed-related, but it does not provide one test list that fits every large breed.

Bring the dog’s exact breed or mix, previous diagnoses, medication and supplement list, vaccination and parasite-prevention history, and prior laboratory reports. Ask whether the breed has a recognized screening recommendation from an appropriate breed-health authority, and let the veterinarian decide whether that recommendation applies to this individual. Size alone should not be used to infer a disease or to order a test without a clinical reason.

Lifestyle also affects the plan. Travel, contact with other dogs, hunting, swimming, tick exposure, diet, exercise level, and access to preventive products can change the veterinarian’s advice about infectious or fecal testing. A dog with a chronic condition may need testing more often than the senior interval shown in the table, while a healthy dog with stable results may remain on the routine framework.

Preparing for a senior screening appointment

  1. Keep a change log. Note appetite, water intake, urination, stool, activity, sleep, grooming, hearing, vision, mood, and ability to rise, walk, play, or use stairs.
  2. Record timing. Write down when each change began, whether it is worsening, and whether it appears after exercise, meals, medication, or rest.
  3. Bring evidence. Take videos of unusual movement and bring medication containers, supplement details, previous test reports, and parasite-prevention dates.
  4. Ask for a complete plan. Confirm which routine tests are due, which findings would trigger a UPC ratio, ECG, or imaging, and when results should be rechecked.
  5. Follow preparation instructions. If fasting, a urine sample, or medication timing is needed, follow the clinic’s directions rather than changing food or medication on your own.

What to do after results return

Ask the veterinarian to explain each abnormality in relation to the dog’s baseline and examination. Some findings lead to a repeat sample, a more specific test, or a shorter recheck interval; others may simply be monitored. The appropriate response depends on the magnitude and persistence of the change, the dog’s symptoms, and the interaction among CBC, chemistry, urinalysis, blood pressure, and other findings.

Keep copies of reports so trends are visible at later visits. A result that is normal today does not guarantee that a new sign months later is unrelated, and a mildly abnormal value does not automatically mean serious disease. The veterinarian’s interpretation and follow-up plan are more useful than a reference-range comparison made in isolation.

Changes that should prompt an earlier appointment

Do not wait for the next routine screening if you notice a persistent or marked change in drinking or urination, appetite, elimination, exercise tolerance, movement, play, attitude, grooming, vision, hearing, or a new or enlarging mass. These observations do not identify a cause by themselves, but they give the veterinarian a reason to examine the dog and decide whether testing should occur sooner than the routine schedule.

Common misconceptions to avoid

  • “Every senior large dog needs annual X-rays.” AAHA does not set a universal senior imaging schedule; imaging is clinically or breed indicated.
  • “One blood panel is enough for the rest of the dog’s life.” Senior screening is periodic because trends and new findings matter.
  • “Large breed automatically means the same diseases and tests.” Breed, individual history, lifestyle, and examination findings determine relevance.
  • “A normal-looking lump can be watched without testing.” AAHA recommends measuring, aspirating, and examining skin masses cytologically rather than assuming they are benign.
  • “Home observations are subjective and unhelpful.” Specific notes and videos can reveal mobility or behavior changes that are not present during a short clinic visit.

Bottom line

For an aging large-breed dog, start with a veterinary senior examination and a plan built around the dog’s expected lifespan, breed, history, and current signs. AAHA’s framework uses CBC, chemistry, and urinalysis every 6–12 months, annual T4 and blood-pressure checks, annual heartworm and tick-borne disease testing, and fecal testing one to four times yearly according to preventive use and lifestyle. ECG, UPC testing, and imaging are targeted tools, not automatic requirements. Regular observation at home and prompt discussion of changes allow the veterinarian to adjust that schedule when the dog’s health changes.
