Stopping semaglutide commonly leads to some weight regain, and improvements in cardiometabolic measures may move back toward a person’s previous levels. In the STEP 1 withdrawal extension, adults without diabetes who had used weekly semaglutide 2.4 mg for 68 weeks regained an average of about two-thirds of the weight they had lost during the following year without treatment.

That result is a study average, not an individual forecast. The trial involved Wegovy-dose semaglutide for obesity in people without diabetes; it did not directly test what happens to blood glucose when a person with type 2 diabetes stops Ozempic. Anyone considering a change should make a monitoring and treatment plan with the prescribing clinician.

The short answer

Weight regain after stopping semaglutide is common, but the amount and timing differ from person to person. Some people retain part of their loss, while others regain most of it. In the STEP 1 extension, average weight remained below the original starting point one year after withdrawal, yet most of the medication-associated loss had returned.

Stopping can also affect health measures linked with weight. The trial reported that improvements in several cardiometabolic measurements moved toward baseline by the end of follow-up. That does not mean every measure returned completely to its starting value or that every participant followed the group average.

Ozempic and Wegovy both contain semaglutide, but they are used in different treatment contexts. Evidence from a Wegovy-dose obesity study should not be presented as a precise prediction for a person taking Ozempic for type 2 diabetes.

What the STEP 1 extension actually measured

The most informative withdrawal data come from the exploratory extension of STEP 1, reported by Wilding and colleagues in 2022. The original randomized trial enrolled 1,961 adults with obesity, or with overweight plus at least one weight-related condition, who did not have diabetes. Participants received weekly subcutaneous semaglutide 2.4 mg or placebo, together with a structured lifestyle intervention, for 68 weeks. Only 327 participants were included in the subsequent extension analysis.

For the extension, both the study medication and the structured lifestyle intervention were stopped. Participants were followed to week 120, approximately one year after the 68-week treatment period. The design therefore shows what happened after this particular treatment-and-support program ended; it does not isolate medication withdrawal from the loss of structured lifestyle support.

Measure Former semaglutide group Former placebo group
Mean weight change through week 68 17.3% loss from baseline 2.0% loss from baseline

Mean weight relative to original baseline at week 120 5.6% below baseline 0.1% below baseline

The authors summarized the semaglutide group as having regained about two-thirds of its prior weight loss within the year after withdrawal. “Two-thirds” describes the average proportion across the analyzed group; it is not a schedule or a guaranteed amount for an individual.

The extension was exploratory and analyzed a subset of the original participants. Its population, dose, duration and lack of diabetes limit how broadly the findings can be applied. The full publication is available in the STEP 1 trial extension record.

How weight change can unfold after the last dose

During treatment

By week 68, the semaglutide group had lost an average of 17.3% of baseline weight in STEP 1, compared with 2.0% in the placebo group. That difference reflects the treatment period, when participants also received structured lifestyle support. It is the starting point for understanding the later withdrawal results.

During the following year

By week 120, the former semaglutide participants had regained an average of 11.6 percentage points of their earlier loss. Because they had lost 17.3% on average by week 68, the group still averaged 5.6% below its original baseline. The former placebo group averaged 0.1% below baseline at the same time.

Why an individual timeline cannot be predicted

The study reports group means rather than a fixed pattern such as a specific number of pounds per month. It does not establish that regain starts on a particular day, proceeds at a constant rate or reaches the same point for everyone. Starting weight, treatment response, medical conditions, other medicines, eating patterns, activity and the availability of follow-up can all differ, but STEP 1 was not designed to calculate an individualized forecast for those factors.

Why Ozempic and Wegovy should not be treated as interchangeable evidence

Both products contain semaglutide, but the labeled purposes and dosing contexts differ. FDA review material describes Wegovy in the chronic weight-management setting and Ozempic at marketed doses for improving glycemic control in adults with type 2 diabetes. Current prescribing information should be checked before making a treatment change; the FDA review material is available at FDA.gov.

Question What the available evidence supports What it does not establish
What was withdrawn in STEP 1? Weekly semaglutide 2.4 mg used for 68 weeks in adults without diabetes, followed by one year without the study drug or structured lifestyle program. A direct discontinuation study of every Wegovy user, every semaglutide dose or every treatment duration.
What does this say about Ozempic? It confirms that semaglutide withdrawal in a specific obesity trial was followed by substantial average weight recurrence. An exact weight or blood-glucose prediction for a person using Ozempic for type 2 diabetes.
What should a person with diabetes plan? A clinician-guided glucose-monitoring and diabetes-treatment plan before any change. A universal glucose outcome after stopping, because the cited withdrawal trial excluded people with diabetes.

If Ozempic is being used for type 2 diabetes, the immediate concern is not only weight. Stopping may require changes to glucose monitoring and to other or ongoing diabetes treatment. The cited sources do not provide an individualized prediction of how a particular person’s blood glucose will respond.

What may happen to cardiometabolic improvements

Weight is only one outcome. In the STEP 1 extension, improvements in most measured cardiometabolic variables moved toward baseline by week 120. This means the favorable changes seen during treatment were less pronounced after withdrawal on average.

The wording matters: “moved toward baseline” is not the same as “all benefits disappeared.” The extension did not show that every participant lost every improvement, and it was not powered to provide an individual risk forecast. A person’s blood pressure, lipids, glucose and other measures need to be interpreted using their own clinical history and follow-up tests.

For someone with type 2 diabetes, blood-glucose management has an additional safety dimension. The STEP 1 population did not have diabetes, so its results cannot supply a post-Ozempic glucose number or tell a patient whether another diabetes medicine will be needed.

What current guidance says about long-term treatment

The American Diabetes Association’s Standards of Care in Diabetes—2026 describes obesity as a chronic, relapsing disease and notes that stopping chronic obesity medication often results in weight recurrence and worsening or reemergence of cardiometabolic risks. The document supports shared decision-making about continuing treatment at an individualized effective dose or stopping with close weight monitoring. Read the guidance at the ADA Standards of Care in Diabetes—2026.

“Obesity is a chronic, relapsing disease, similar to hypertension, and typically requires continuation of pharmacotherapy after weight reduction goals are achieved to sustain weight loss and health benefits.”

This is guidance for a chronic condition, not an instruction that every person must remain on a medicine indefinitely. The appropriate choice depends on the treatment goal, benefits, side effects, access, cost, other conditions and the person’s preferences. If stopping is chosen, the ADA emphasizes close monitoring rather than assuming that the result will remain stable without follow-up.

The ADA also identifies regular physical activity, self-monitoring and a nutrient-dense eating pattern as strategies reported by successful long-term weight-loss maintainers. It explicitly notes that these strategies have not been validated specifically for preventing regain after GLP-1 therapy. They should therefore not be advertised as a proven post-semaglutide replacement protocol.

How to plan a medically supervised stop

A prescriber can help decide whether stopping, continuing or changing treatment best fits the reason the medicine was prescribed. A practical discussion can follow this sequence:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Define the treatment goal. Clarify whether the medicine is being used primarily for chronic weight management, glucose lowering in type 2 diabetes, or both. The goal determines which outcomes must be monitored.
  2. Review the current product and dose. Confirm whether the prescription is Ozempic, Wegovy or another semaglutide product, the current dose, the duration of use and the latest official labeling. Product indications and dosing should not be inferred from the STEP 1 withdrawal dose.
  3. Discuss the alternatives. Shared decision-making can include continuing at an individualized effective dose, changing treatment, or stopping. Compare expected benefit, tolerability, treatment burden, cost or access and the monitoring required for each option.
  4. Set a weight-monitoring method. If the clinician recommends it, a home scale can help reveal a trend. It is an optional monitoring aid, not a treatment for regain and not a substitute for clinical review. Agree on how often to record measurements and what change should prompt contact.
  5. Create a glucose plan when relevant. A person with diabetes should know when to check blood glucose, which readings require a call and how other diabetes medicines will be continued or adjusted. The prescriber should provide the individualized instructions.
  6. Schedule follow-up. Arrange clinical review after the change so weight, glucose when relevant and other cardiometabolic measures can be assessed. Do not rely on the STEP 1 average to decide that a personal result is acceptable or expected.

What has not been proven to prevent regain

The cited guidance and withdrawal trial do not establish a universal tapering schedule, a specific diet, a particular exercise prescription or a guaranteed medication switch that prevents weight recurrence after semaglutide cessation. The study stopped treatment and structured lifestyle support together, so it cannot identify a single intervention that would have preserved the treatment-period loss.

That evidence gap does not mean lifestyle care is irrelevant. It means a clinician should avoid promising that one plan will reliably reproduce the medication’s effect after stopping. Any nutrition, activity or behavioral strategy should be individualized, feasible and monitored as part of ongoing care rather than presented as a validated antidote to withdrawal-related regain.

Questions to settle before changing treatment

  • What condition is the semaglutide treating, and which outcome matters most: weight, blood glucose, or both?
  • What amount of regain or change in another measurement should trigger a follow-up?
  • For diabetes, which glucose readings require urgent advice and what medicines continue during the transition?
  • Would continuing treatment at an individualized effective dose provide benefits that outweigh burden, side effects, cost and access concerns?
  • Which current prescribing details need to be checked before the next dose or a replacement treatment?

The STEP 1 extension shows why follow-up matters: after a strong average response during treatment, substantial average regain occurred during the next year without semaglutide. The ADA’s current approach is to make that decision jointly with a clinician and to monitor closely if stopping is selected.
