After stopping a GLP-1-based weight-loss medicine, many people regain some weight over the following months, and some health improvements may fade. The amount varies: clinical-trial averages describe groups, not what any one person will experience. If you are considering stopping—or take the medicine for type 2 diabetes—work with your prescriber on a plan for treatment and monitoring.
What the studies found after treatment stopped
The best-known withdrawal results come from two trials of different medicines and designs. Both followed adults with overweight or obesity who did not have diabetes, so their results do not establish what will happen to people with diabetes.

| Evidence | Study design and follow-up | Reported weight change |
|---|---|---|
| Semaglutide: STEP 1 extension | An exploratory extension of a subset of STEP 1 participants. After 68 weeks of treatment with once-weekly semaglutide 2.4 mg or placebo, both study medication and trial lifestyle support stopped; the extension followed participants for another 52 weeks. | The semaglutide group lost an average 17.3% of body weight during treatment, then regained 11.6 percentage points by week 120. At that point, the group remained an average 5.6% below its starting weight. Placebo participants regained 1.9 percentage points. These are group averages, not individual forecasts. STEP 1 extension, 2022. |
| Tirzepatide: SURMOUNT-4 | A randomized withdrawal trial. Participants first took their maximum tolerated tirzepatide dose (10 mg or 15 mg) for 36 weeks. Those who completed this lead-in were randomized to continue treatment or switch to placebo for 52 weeks. | From randomization at week 36 to week 88, those switched to placebo had a mean 14.0% weight increase; those who continued tirzepatide lost a further 5.5% on average. The 14.0% is the change during the withdrawal period, not 14% of the weight previously lost. SURMOUNT-4, 2024. |
Why the numbers are not a head-to-head comparison
The semaglutide result comes from an exploratory off-treatment extension that followed a representative subset of trial completers after medication and trial lifestyle support ended. SURMOUNT-4 randomized people who completed a 36-week tirzepatide lead-in to continue treatment or receive placebo. The studies also report changes against different reference points. Their figures show a similar direction—weight regain after stopping—but should not be treated as a direct comparison of the medicines or as a prediction for an individual.
Can other health benefits change?
In the STEP 1 extension, improvements in cardiometabolic measures moved toward baseline for most measured variables during the year after semaglutide and trial lifestyle support ended. That does not mean every measure changed in every participant, or that the same course applies to everyone. The withdrawal studies cited here do not establish an individual’s likely response, particularly for people with diabetes.

Is there an immediate withdrawal syndrome?
The cited trials report longer-term weight and cardiometabolic outcomes; they do not provide a complete account of short-term symptoms after a final dose or establish one universal immediate withdrawal syndrome. They also do not establish a single tapering or restarting approach for every GLP-1 medicine. Product instructions differ, so do not infer a dosing plan from trial results.
Plan a stop or treatment interruption with your prescriber
The American Diabetes Association’s 2025 obesity-care standards recommend continuing obesity medication after treatment goals are reached to maintain health benefits. The guidance notes that discontinuation often results in weight recurrence and worsening or reemergence of cardiometabolic risk factors or obesity-related conditions. It calls for an individualized decision that considers efficacy, benefits, and tolerability, with options that may include a maintenance dose, a different treatment, or stopping with close monitoring. Questions to take to your clinician
- What should I expect, and which health measures should we monitor?
- Could a maintenance dose or another treatment better fit my goals and tolerability?
- If treatment is interrupted, what should I do and when should I contact you?
If you take semaglutide for type 2 diabetes, do not use these trials to estimate your glucose response: both withdrawal populations described above excluded people with diabetes. Agree on a diabetes treatment and monitoring plan with your prescriber. The Wegovy label’s glucose-monitoring advice for people with diabetes during treatment is not a substitute for individualized advice about stopping. If you have missed Wegovy doses
The U.S. Wegovy label says that after two or more consecutive missed doses, reinitiating dosage escalation at a lower dosage may reduce gastrointestinal adverse reactions. This is a missed-dose instruction for Wegovy, not a universal stopping or restarting rule for other medicines. Check the current product label and ask your prescriber before restarting after a gap.
