What happens when you stop taking a GLP-1 drug? Weight regain and other changes

Stopping a GLP-1-based weight-management medicine commonly puts lost weight at risk of returning, but the amount and timing vary widely. In the 2024 SURMOUNT-4 withdrawal trial, people who switched from tirzepatide to placebo regained an average of 14.0% of their week-36 weight over the next 52 weeks, while people who continued treatment lost another 5.5%. Those figures describe a selected clinical-trial population—not a personal forecast. Stopping can also allow blood pressure, glucose regulation, lipids, or obesity-related complications to worsen again, so a prescriber-guided plan and follow-up matter.

First, identify which medicine and why you take it

“GLP-1 drug” is everyday shorthand for several different medicines and indications. Some medicines primarily activate the glucagon-like peptide-1 (GLP-1) receptor. Tirzepatide activates both the glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptors, so it is a dual GIP/GLP-1 agonist rather than a GLP-1-only drug. Approved uses, doses, and product labels differ by medicine and jurisdiction.

The reason for treatment changes the safety question. A person using a medicine for chronic weight management needs a plan for maintaining weight-related benefits. A person also using it for type 2 diabetes, or another condition, needs a plan for that condition as well. Results from an obesity trial in people without diabetes cannot be treated as a complete answer to diabetes management.

What the strongest withdrawal trial found

How SURMOUNT-4 was designed

SURMOUNT-4 was a phase 3 randomized-withdrawal trial reported in JAMA in 2024. It took place at 70 sites in Argentina, Brazil, Taiwan, and the United States. Adults with obesity, or overweight plus a weight-related complication, but without diabetes, first received open-label tirzepatide for 36 weeks. Participants also received lifestyle counseling. Of the 670 people who entered the randomized phase, one group continued tirzepatide and the other switched to placebo for 52 weeks.

By the end of the 36-week lead-in, the randomized participants had lost an average of 20.9% of their starting weight. The withdrawal comparison then measured change from week 36 to week 88.

The results, with the denominators kept clear

Measure Continued tirzepatide Switched to placebo
Mean change from week 36 to week 88 Additional 5.5% weight reduction 14.0% mean regain of week-36 weight
Participants maintaining at least 80% of their lead-in weight loss at week 88 89.5% 16.6%

The 14.0% figure is a percentage of the weight participants had at week 36; it is not 14 percentage points of everyone’s original body weight and not a guaranteed amount for an individual. For example, someone weighing 100 kg at randomization into the withdrawal phase would have a trial-average regain of about 14 kg only if that person matched the group mean; individual outcomes can be lower, higher, or absent. The trial’s conclusion was that “withdrawing tirzepatide led to substantial regain of lost weight, whereas continued treatment maintained and augmented initial weight reduction.”

What this trial cannot tell you

  • It did not study every GLP-1 medicine.
  • It did not include people with type 2 diabetes.
  • Everyone in the randomized phase had tolerated and completed a 36-week tirzepatide lead-in, so the participants were not a cross-section of everyone who starts treatment.
  • It tested a planned switch to placebo, not every form of abrupt or unplanned stopping in routine care.
  • Lifestyle counseling continued throughout, meaning the results do not isolate medication from all other support.

These limits are why the trial is useful evidence that regain is a substantial risk, not a formula for predicting one person’s result or deciding whether to stop.

Why weight may return after treatment is withdrawn

While treatment is active, its effects can help a person maintain the lower weight achieved during the trial. When the medicine is removed, that pharmacologic support is no longer being provided. Appetite, eating patterns, and other weight-regulation pressures may change, but the available sources do not establish a single timetable or a uniform response for everyone.

Weight recurrence does not mean that a person has failed or that all lost weight must return. Some people regain part of the loss, some regain more, and some maintain substantial loss. The SURMOUNT-4 averages show the direction and scale of risk in one carefully defined group; they do not assign an inevitable outcome to an individual.

Other health measures can drift as weight returns

The American Diabetes Association (ADA) states in its 2025 Standards of Care in Overweight and Obesity that clinicians should “Continue obesity medications after reaching treatment goals to maintain health benefits, as discontinuation often results in weight recurrence and worsening or reemergence of cardiometabolic risk factors or obesity-related diseases and complications.”

In practical terms, improvements associated with weight loss may weaken if weight returns. Depending on a person’s underlying health, that can include blood-pressure control, glucose regulation, blood-lipid levels, or complications linked to obesity. The guideline describes a risk of worsening or reemergence; it does not say that every person will experience every change or that a particular measurement will deteriorate by a predictable amount.

If the medicine is treating diabetes, stopping may also change the person’s glucose-management needs independently of the scale. The diabetes prescriber should decide how glucose will be monitored and whether another treatment is needed. An obesity-trial percentage should never be used as a substitute for a diabetes care plan.

Appetite, stomach symptoms, and other effects after stopping

People often ask whether nausea, diarrhea, constipation, vomiting, abdominal discomfort, reflux, or appetite changes disappear immediately. The reviewed sources do not quantify how quickly these effects resolve, how often they persist, or whether everyone experiences a rebound in appetite after stopping.

The current US prescribing information for tirzepatide injection (Zepbound), revised August 2026, lists nausea, diarrhea, vomiting, constipation, abdominal pain, dyspepsia, fatigue, hair loss, and gastroesophageal reflux disease among common on-treatment adverse reactions. That label describes effects while taking the medicine; it does not provide a reliable post-discontinuation timetable. Do not assume that a symptom will stop on a specific day, and contact the prescribing clinician about symptoms that are severe, persistent, or concerning.

What can be inferred about semaglutide versus tirzepatide?

It is reasonable to expect that withdrawal evidence may differ by molecule, dose, treatment duration, population, and follow-up. Those details must be kept separate when comparing studies.

Comparison factor Why it matters
Molecule Tirzepatide is a dual GIP/GLP-1 agonist; a GLP-1-only medicine such as semaglutide is not the same intervention.
Indication and population Results in adults with obesity without diabetes cannot be assumed to apply to people taking a medicine for diabetes.
Lead-in duration The amount of weight lost and the time spent on treatment before withdrawal influence what happens afterward.
Withdrawal design A randomized switch to placebo is different from an unplanned interruption or a clinician-guided switch to another treatment.
Outcome definition A percentage of week-36 body weight is not interchangeable with a percentage of the weight previously lost.

The cited SURMOUNT-4 paper references a 2022 extension of the STEP 1 semaglutide trial on weight regain and cardiometabolic effects after withdrawal. The full extension results are not used here to give a numerical semaglutide prediction. Until the original study details are checked, the safe conclusion is that semaglutide and tirzepatide should not be assigned one shared regain percentage.

How to plan a possible stop with your clinician

Do not create a taper or replacement schedule from an online article. A prescriber can weigh the original indication, current dose, response, side effects, other medicines, pregnancy plans where relevant, and access to follow-up.

  1. Contact the prescribing clinician before changing the dose. Explain whether you are considering a planned stop, have missed doses, or are stopping because of side effects, cost, supply, or another concern. The reason affects the safest next step.
  2. Separate each treatment goal. Ask what will replace the medicine’s role in weight management and, separately, how diabetes or another treated condition will be managed.
  3. Agree on measurements to follow. The plan may include body weight, symptoms, blood pressure, glucose data, or laboratory tests selected by the care team. The ADA recommends close monitoring when medication is stopped.
  4. Set a review date. A scheduled check-in makes it easier to respond to a trend before a small change becomes a larger clinical problem.
  5. Discuss maintenance options. The ADA recommends continuing obesity medication after treatment goals when appropriate because stopping often leads to weight recurrence and return of cardiometabolic or obesity-related problems. Whether continuation, a switch, or discontinuation is right is an individualized decision.

Using a home scale without overinterpreting it

A digital bathroom scale can be an optional way to observe a weight trend after a medication change. It is not a diagnostic device, does not treat obesity, and cannot replace follow-up. If you use one, record readings in a consistent way and share the trend—not a single day’s number—with your clinician. The purpose is early communication, not self-directed dose changes.

If weight begins to return

Contact the care team rather than waiting for a predetermined amount of regain. The clinician may review whether the medication was stopped for a reversible reason, whether another indication needs attention, and whether a maintenance or alternative strategy is appropriate. The response should account for blood pressure, glucose, lipids, symptoms, and obesity-related complications, not only the scale.

Avoid treating the SURMOUNT-4 average as a target, a deadline, or proof that all lost weight will come back. Likewise, do not assume that a short-term stable weight means cardiometabolic risk is permanently protected. Monitoring is useful precisely because the direction and speed of change differ among people.

The practical bottom line

Stopping a GLP-1-based medicine is often followed by some weight recurrence, and the risk can be substantial. In SURMOUNT-4, planned withdrawal of tirzepatide produced a 14.0% mean regain of week-36 weight over 52 weeks, while continued treatment produced another 5.5% loss; those findings apply to the trial population, not automatically to you. Other benefits can weaken as well, particularly when the medicine was helping control diabetes or obesity-related cardiometabolic risk. Discuss any change with the prescriber, make a monitoring plan, and treat the specific medicine and indication—not the generic label “GLP-1”—as the starting point for the decision.
