What Happens When an Underwater Robot Loses Its Tether? Safety and Recovery Basics

If an underwater robot physically separates from its tether, it can lose the path used for control and communication. It may sink, drift, or surface; which outcome occurs depends on the vehicle’s buoyancy and operating conditions. A lost control link or telemetry signal alone does not prove that the tether has parted. Treat suspected separation as an incident, alert the responsible supervisor and vessel team, and follow the recovery plan for that specific vehicle and operation.

What “loses its tether” can mean

People use “tether loss” to describe different failures. An ROV can stop responding to commands or stop sending telemetry while its tether remains physically connected. Alternatively, the tether can be damaged or completely separated. These are not interchangeable situations: the appropriate response depends on what failed and on the ROV’s design.

Control or telemetry loss is not proof of separation

A control or telemetry failure means the crew has lost some expected communication or feedback. It does not, by itself, establish that the cable has detached. Depending on the system, other signals or fail-safe behavior may remain available. The ADCI consensus document, edition 6.3 (2020), describes a work-class configuration in which fail-safe behavior maintains depth and can assist with TMS latching. That is an example for a particular configuration, not a feature to assume on every commercial or small inspection ROV.

Use the system’s indicators, available communications and tracking information, and the operation’s emergency plan to assess what has happened. Avoid turning a missing signal into an unsupported conclusion about the vehicle’s location or condition.

Physical separation can leave the vehicle uncontrolled

A parted tether can remove the ROV’s control and communication path. What happens next is not universal: the vehicle may sink, drift with conditions, or surface, depending on its buoyancy and circumstances at the time. The available incident and guidance material does not establish one outcome for all ROVs, or a general probability that a separated vehicle will be recovered.

Physical damage can start before a cable parts. In an IMCA-reported platform-jacket inspection incident, abrasion against the structure caused the tether to part; the ROV drifted away and communication was lost. That case shows a plausible failure mechanism, not a statistical measure of how often tethers fail.

What the crew should prioritize after suspected separation

Once separation is suspected, treat it as an incident rather than improvising a recovery. The initial priorities are to notify the people responsible for the operation, establish the last known position, and use the tracking and communications resources available under the applicable emergency plan. The correct actions depend on whether this is a small inspection ROV or a work-class system operated from a vessel with a tether management system (TMS).

Immediate priorities for any operation

  1. Raise the alarm through the operation’s chain of responsibility. Alert the responsible supervisor and, where applicable, the vessel team. Make clear whether the crew has confirmed physical separation or only lost control or telemetry.
  2. Record the last known position and relevant status. Preserve available position, survey, transponder, and vehicle information so the team is working from the same last-known picture. Follow the site’s incident and emergency procedures.
  3. Use tracking resources under the plan. Coordinate with the personnel responsible for tracking or survey information. Do not assume a beacon is available, heard, powered, or functioning just because the system normally uses one.
  4. Keep recovery controlled and planned. Do not apply a procedure written for another vehicle or vessel setup. The team should use the instructions for the exact ROV and tether arrangement, with suitable recovery equipment and an agreed recovery area where relevant.

These are priorities, not a guarantee of recovery. The location, current, vehicle buoyancy, cable arrangement, vessel setup, available lifting or tracking equipment, and circumstances of the failure can all affect what is possible.

Work-class ROVs and vessel/TMS operations

ADCI’s consensus guidance for commercial diving and work-class ROV operations describes a vessel-based response to suspected tether separation. In that context, the guidance includes raising the TMS, alerting the captain, posting lookouts, and coordinating with survey personnel using beacon data when available. If the beacon is not heard, it discusses raising a hydrophone pole. If the vehicle is located, it describes planning a surface recovery using the vessel’s recovery zone and appropriate lifting points.

Those details belong to the work-class and vessel context addressed by the guidance. They are not a universal checklist for a consumer or small inspection ROV, and they should not replace the applicable operation-specific emergency plan. A TMS, survey team, beacon, hydrophone pole, vessel recovery zone, or suitable lifting arrangement may not exist in another setup.

Small inspection ROVs

Small ROV operators should use the manufacturer’s directions for their exact model and configuration. For BlueROV2, Blue Robotics says to disarm before recovery. Its operating guidance also says that if power or the connection to QGroundControl is lost while diving, the tether should be pulled in at a moderate pace, not as fast as possible. That is model-specific guidance; it should not be generalized to every ROV or interpreted as permission to pull a cable that may be snagged or physically damaged.

Blue Robotics also advises keeping the tether clear of propellers, boats, and sharp objects, and avoiding excess tether in the water. These are preventive practices in its BlueROV2 guidance, not a substitute for an incident response when separation is suspected.

How recovery differs between systems

“ROV recovery” covers different equipment and risks. A small inspection vehicle may be retrieved by managing its tether in accordance with its manufacturer’s instructions. A work-class ROV may be operated through a TMS from a vessel and require coordination among the pilot, captain, lookouts, and survey personnel. Applying the wrong procedure can make a difficult situation worse.

Operating context What the cited guidance addresses Important limit
BlueROV2, small inspection ROV Disarm before recovery; after loss of power or QGroundControl connection during a dive, retrieve the tether at a moderate pace. Keep it clear of propellers, boats, and sharp objects, and avoid excess tether in the water. These are Blue Robotics’ instructions for BlueROV2, not a general instruction for other models or a response to every type of damage.
Commercial/work-class ROV operation ADCI edition 6.3 (2020) describes a vessel/TMS context that can include raising the TMS, notifying the captain, posting lookouts, coordinating with survey personnel and beacon data, and planning a surface recovery if the ROV is located. The document concerns commercial diving and work-class operations. It is not consumer ROV instruction, and its described resources may not be present in other operations.
VideoRay Pro 4 suspected cut tether The Pro 4 Operator’s Manual, version 2.00.00, gives specific steps for a cut or completely separated tether and distinguishes field repair from more permanent repair. Use the manual for the Pro 4 system; its advice does not establish compatibility or repair suitability for another model or tether.

Vehicle design and buoyancy matter, as do the tether and TMS arrangement, location, conditions, vessel configuration, and available tracking and lifting equipment. A method that is appropriate for one setup may be unsafe or ineffective in another. The IMCA incident also illustrates why emergency procedures need to match the actual operation rather than rely on a generic lost-vehicle checklist.

What the VideoRay Pro 4 manual says about a cut tether

For a suspected cut tether on a VideoRay Pro 4, the manufacturer’s Operator’s Manual, version 2.00.00, gives a model-specific sequence. It says to turn off power, retrieve the ROV if possible, mark the spot if the tether is completely separated to help search for the vehicle, and remove the tether from the water promptly to prevent water ingress. The manual notes that tether may often be spliced.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For field repair, the manual identifies hot glue or silicone/RTV glue as possible materials. It also says a more permanent repair should be carried out at the first opportunity. A field repair is therefore not equivalent to a permanent repair, and this passage does not establish that those materials are suitable for every tether, damage type, or ROV. Follow the instructions and compatibility requirements for the exact system.

What an incident report teaches about preparation

The IMCA platform-jacket case provides a practical example of how a tether can fail and why recovery planning matters. Abrasion against the jacket structure led to tether separation; the ROV was lost and communication with it ceased. The report identified shortcomings that were specific to that operation: emergency procedures did not describe the actual scenario in sufficient detail, the crew complement was inadequate for the operation’s complexity, and transponder interrogation depleted the beacon battery prematurely.

The report says procedures were revised to include environmental limits and recovery detail, and current information was added to the pilot interface. The transferable lesson is not that every incident has the same cause or remedy. It is that teams should plan for credible failure scenarios, account for the operation’s complexity, keep relevant current information available, and manage beacon power according to the tracking plan.

This is a single reported incident, not a failure-rate study. No suitable population statistic for ROV tether failures or recovery success is established by the sources cited here.

What to check in the ROV’s manual and emergency plan

Before deployment, operators should know which instructions apply to their particular vehicle, cable, and operating context. The manufacturer’s operating or service manual and the operation’s emergency plan answer different questions: the manual covers system-specific behavior and handling, while the plan coordinates the people and resources involved in an incident.

  • Identify the failure being addressed. Know how the system indicates loss of control or telemetry, and how the crew distinguishes that from confirmed physical separation.
  • Confirm recovery instructions for the exact model. Check the manufacturer’s directions for disarming, retrieval, damaged tether handling, and any model-specific fail-safe behavior.
  • Establish the last-known-position and tracking process. For operations using survey or beacon equipment, confirm who monitors it and how information is shared. The IMCA incident shows why beacon power management and the interrogation plan matter.
  • Match the plan to the operating environment. For vessel and work-class operations, ensure responsibilities, communications, environmental limits, recovery equipment, and the recovery area are addressed in the applicable plan.
  • Know what not to assume. Do not assume another model’s buoyancy, fail-safe behavior, tether repair method, or recovery procedure applies to your vehicle.

If a tether must be replaced rather than repaired, confirm the replacement procedure and part against the exact ROV model. For example, Blueye’s support listing identifies a service manual that covers tether replacement; that establishes the subject of that manual, not universal compatibility among tethers or an all-model replacement method. The right parts and service instructions are system-dependent.

ADCI’s cited consensus document is edition 6.3 (2020) and concerns commercial diving and work-class ROV operations. IMCA’s catalog lists “Remotely operated vehicle intervention during diving operations” as Revision 1, October 2026, with a scope covering combined ROV and diving operations, including risk assessment, communications, responsibilities, and emergency preparedness. That catalog description establishes scope and revision, not specific tether-separation steps; follow the actual applicable procedures for the operation.

Frequently Asked Questions

Does a lost control signal mean the ROV tether has broken?

No. Loss of control or telemetry is not proof of physical separation. Depending on the system, other signals or fail-safe behavior may remain available. Use the vehicle’s indicators, available tracking information, and the operation’s emergency plan to assess the failure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Will an ROV sink if its tether separates?

Not necessarily. A separated ROV may sink, drift, or surface depending on its buoyancy and the operating conditions. The available guidance does not support one prediction for every vehicle, and recovery is not guaranteed.

What should a crew do first if tether separation is suspected?

Treat it as an incident: alert the responsible supervisor and vessel team where applicable, establish the last known position, and use survey, transponder, or other tracking information under the operation’s emergency plan. Specific steps depend on the vehicle and operating setup.

Can I use the same recovery procedure for a small ROV and a work-class ROV?

No. BlueROV2 guidance, for example, says to disarm before recovery and retrieve its tether at a moderate pace after certain power or QGroundControl connection losses. ADCI’s work-class context instead describes vessel and TMS coordination, lookouts, survey personnel, and other resources. Follow the instructions for the exact system and operation.

Can a cut ROV tether be repaired in the field?

It depends on the system and the damage. VideoRay’s Pro 4 Operator’s Manual, version 2.00.00, identifies hot glue or silicone/RTV glue as possible field-repair materials and says a more permanent repair should follow at the first opportunity. Do not assume those materials or methods are suitable for another model or tether.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is there a known ROV tether failure or recovery-success rate?

The sources cited here do not provide a suitable population statistic for tether failure frequency or recovery success. The IMCA platform-jacket report is a single incident case, not a rate.

The Bottom Line

A lost signal does not automatically mean a tether has parted, and a separated ROV may sink, drift, or surface depending on its design and conditions. Treat suspected separation as an incident, use the exact vehicle’s manual and operation-specific recovery plan, and do not assume recovery or a repair method is universal.
