A large 2026 study found that many laboratory measures appear to shift abruptly around the final menstrual period (FMP), rather than changing at a perfectly steady pace. The authors call this population pattern a step-like physiological “cliff.” It does not mean every person has a sudden medical crisis, that routine blood tests can diagnose menopause, or that one treatment is needed. Menopause remains a highly variable transition whose symptoms and health implications require individual assessment.
What the new study found
The Nature Medicine study by Pridham, Hayut, Lavi-Shoseyov and colleagues reconstructed biological changes around the FMP using retrospective laboratory records. According to Nature Portfolio’s 13 October 2026 summary, the analysis covered more than 300 million laboratory tests from over one million women in United States and Israeli datasets collected between 1999 and 2024.

| Study measure | Reported result | What it means |
|---|---|---|
| United States laboratory tests | 52 of 90 tests showed a statistically significant abrupt change | The shifts spanned several body systems, not just reproductive hormones. |
| Data scale | More than 300 million tests from over one million women, 1999–2024 | This is a population-level reconstruction from existing records, not a prospective experiment. |
| Cross-country pattern | Nature Portfolio reports concordant abrupt changes in the United States and Israel | Similar statistical patterns appeared in both datasets, but they still cannot predict an individual’s course. |
| Male comparison | Comparable shifts were not observed in male individuals | The finding is consistent with a menopause-associated pattern, although it does not prove the cause of each laboratory change. |
The affected measurements included sex hormones, indicators related to bone health, liver and kidney tests, blood chemistry and blood fats. Nature Portfolio reports that the authors describe the changes as a “step-like physiological ‘cliff.’” In this context, “cliff” means that a statistical curve becomes steeper around the FMP; it does not mean that everyone’s body changes overnight.
When the hormone changes occurred
The study’s timeline does not show every hormone changing precisely at the final period. Its computational reconstruction placed some changes years earlier:

| Measure | Timing reported by the study | Important qualification |
|---|---|---|
| Anti-Müllerian hormone | Declined more rapidly around 15 years before the FMP | This is a population estimate and is not a personal countdown to menopause. |
| Estradiol | Declined more rapidly around 12 years before the FMP | Individual hormone levels fluctuate and do not follow an identical schedule. |
| Follicle-stimulating hormone | Rose more rapidly around 14 years before the FMP | The timing describes the reconstructed dataset, not a recommended screening interval. |
| Other laboratory measures | Several showed abrupt changes near the FMP | The study did not establish that each shift was caused solely by menopause or that treating a number prevents an outcome. |
Because the researchers worked backward from recorded FMP information, these dates should not be used to forecast an individual’s menopause or to decide when to order broad hormone panels.
What menopause and perimenopause are
The World Health Organization defines natural menopause retrospectively: 12 consecutive months without menstruation, when there is no other obvious physiological or pathological cause and no medical intervention responsible. Natural menopause generally occurs between ages 45 and 55 worldwide. Menopause can also be induced by surgery, chemotherapy, radiation or other medical treatment.

Perimenopause is the transition that begins with signs such as changing cycles and continues through the first year after the FMP. It commonly lasts several years. Since the FMP can only be confirmed after a year without a period, irregular bleeding during the transition does not by itself establish that menopause has occurred.

- Irregular, skipped or unusually heavy or light periods during perimenopause
- Hot flushes and night sweats
- Difficulty falling asleep or staying asleep
- Vaginal dryness, discomfort or pain during sex
- Urinary urgency, recurrent urinary symptoms or other genitourinary changes
- Mood changes, anxiety or depression
- Changes in body shape or composition, skin, joints and muscles
- Differences in memory or ability to focus
These symptoms are possible rather than universal, and not every midlife change is caused by menopause. Thyroid disease, medication effects, sleep disorders, depression, anemia and other conditions can produce overlapping symptoms, so persistent or severe problems deserve a clinical assessment.
Health areas that deserve attention after the transition
Bone health
Bone loss accelerates in the first two years after menopause, according to NIH information summarized for this topic. Reduced bone density can contribute to osteoporosis and fractures, but a person’s risk also depends on age, genetics, activity, nutrition, medications and other illnesses. A clinician can determine whether a bone-density evaluation is appropriate.

Heart and metabolic health
Menopause is associated with changes in blood lipids and cardiovascular risk factors. The new study found abrupt patterns in some blood-fat and blood-chemistry tests, but it did not show that every person will develop an abnormal result or that correcting a single laboratory value prevents a later heart event.
Pelvic, urinary and vaginal health
Lower estrogen exposure can coincide with vaginal and urinary symptoms and changes in pelvic support. These problems are treatable, yet they should not be dismissed as something a person must simply tolerate.

Body composition and weight
The International Menopause Society’s 2025 recommendations distinguish chronological aging from menopause. They describe midlife weight gain as mostly related to aging, while declining estrogen is a major contributor to increased abdominal adiposity. Menopausal hormone therapy can treat menopause symptoms, but the recommendations do not indicate it for weight management or improving body composition.

The study’s enormous dataset makes the population pattern notable, but its design has limits. The records were collected retrospectively, menopause timing was self-reported, and the primary analysis reconstructs temporal associations rather than observing each participant prospectively. Nature Portfolio also says apparent associations involving menopausal hormone therapy need confirmation in future studies.
For otherwise healthy people aged 45 or older with typical symptoms, NICE recommends identifying perimenopause or menopause from symptoms and menstrual history without laboratory tests. NIH likewise says hormone testing is usually unnecessary because levels fluctuate substantially during the transition. Testing can be considered in specific situations, such as suspected menopause at an unusually early age or when another diagnosis is possible. The new study is therefore not a reason to request a large screening panel.

- Record patterns. Note bleeding changes, hot flushes, sleep, mood, urinary or vaginal symptoms and factors that make them better or worse. A written or digital record is a discussion aid, not a diagnostic test.
- Describe impact and timing to a clinician. Explain which symptoms interfere with daily life, when they began and any medicines or medical conditions that could contribute.
- Discuss options matched to the problem. WHO and NIH describe lifestyle measures, nonhormonal medicines and menopausal hormone therapy as possible approaches. Choice depends on the symptom, expected benefit, contraindications, medical history, route and burden of treatment, and personal preference.
- Ask about longer-term health. Depending on individual risk, a clinician may address bone health, cardiovascular risk, physical activity, muscle strength, sleep and pelvic or urinary care.
- Seek prompt evaluation for warning signs. Bleeding after 12 months without periods, very heavy bleeding, chest pain, unexplained weight loss, severe depression or new neurological symptoms should not be assumed to be ordinary menopause.
Putting the finding in demographic context
WHO reported that women aged 50 and older represented 26% of women and girls globally in 2021, up from 22% a decade earlier. That statistic describes population age structure, not the percentage experiencing symptoms. It does help explain why clearer evidence about midlife health matters to more people and health systems.
What this means for an individual
The “cliff” is best understood as a new description of how several measures clustered around the FMP in very large datasets. It supports taking menopause-related bone, cardiovascular, pelvic and symptom concerns seriously, while leaving diagnosis and treatment individualized. A person’s symptoms, menstrual history, other health conditions and preferences remain more useful for care decisions than a population curve or an isolated laboratory result.
