What Happens to the Body After Returning From Space?

Returning to Earth exposes an astronaut to a force the body has not felt normally for months: gravity. The first hours or days can bring a heavy feeling, wobbliness, nausea, weakness, sore feet or back, and difficulty standing without becoming lightheaded. Balance organs, blood-pressure control, muscles, bones, and sometimes vision all have to readjust, but no single recovery timetable applies to every astronaut.

NASA’s postflight programs therefore separate immediate ability to function from slower tissue recovery. Some crew members feel nearly normal within days, while strength, aerobic conditioning, and bone mineral density can continue improving for weeks or months under an individualized rehabilitation plan.

Why landing feels so different

In orbit, a spacecraft and everything inside it are in continuous free fall. Astronauts experience microgravity rather than the ordinary loading that makes a person support body weight, stabilize the head, and keep blood circulating against Earth’s pull. The nervous system adapts to that environment. When the spacecraft lands, gravity returns abruptly and the old signals no longer match the body’s recent experience.

NASA astronauts have described Earth as unexpectedly heavy. Andreas Mogensen, an ESA astronaut who returned with NASA’s SpaceX Crew-7 in March 2024, told NASA that, with his eyes closed, it was almost impossible to walk in a straight line during an early postflight test (NASA postflight recovery accounts). Jasmin Moghbeli reported feeling wobbly for the first two days. Those accounts illustrate common possibilities, not a universal experience.

What astronauts may feel immediately after landing

Heavy limbs and poor coordination

Muscles that moved a floating body now have to support it. Standing, turning, climbing steps, or carrying equipment can require more effort than expected. The brain must also reconcile visual information with signals from the inner ear and pressure sensors in the feet and joints. Until that recalibration occurs, an astronaut may sway, veer while walking, or feel nauseated when looking down.

Dizziness when standing

During weightlessness, body fluids shift toward the chest and head. The body responds by reducing total blood volume. On Earth, gravity pulls blood toward the legs as soon as a person stands. If the cardiovascular system cannot tighten blood vessels and raise heart output quickly enough, blood pressure at the brain can fall. The result is orthostatic intolerance: difficulty maintaining blood pressure while upright, which can cause lightheadedness or fainting (NASA Cardiovascular and Vision; NASA The Human Body in Space).

NASA’s cardiovascular and vision laboratory says more than 60% of astronauts could be affected if no countermeasures were used. That conditional figure is not the percentage who faint after every mission; exercise, fluid and salt strategies, mission duration, and individual physiology alter the risk.

Soreness and fatigue

Feet, calves, hips, and the lower back resume bearing loads that were largely absent in orbit. Soreness can therefore appear even when an astronaut completed regular in-flight workouts. Frank Rubio, after a 371-day mission, was reported by NASA to feel mostly recovered within a couple of weeks but still had sore feet and a sore lower back. Loral O’Hara, after 204 days in space, was reported to feel almost completely back to normal about a week after returning. These are individual observations rather than promises for a future crew member.

How each body system readapts

Balance and the vestibular system

The inner ear’s vestibular organs normally detect head motion and gravity. In microgravity, their signals are altered, so the brain relies more heavily on vision and other cues. After landing, the old gravity-related signals return before the brain has fully recalibrated. Astronauts can feel as if the room or their body is moving, have trouble walking a straight line with their eyes closed, or become nauseated when looking toward the ground. NASA postflight accounts show that some people improve in a few days and others need longer (NASA postflight recovery accounts).

Circulation and blood pressure

Fluid redistribution in orbit and the associated reduction in blood volume make the first upright hours especially challenging. Crews are monitored and assisted during egress rather than being assumed to walk unaided. A person can be alert and able to move while still needing time, hydration and carefully staged posture changes to prevent a blood-pressure drop. Orthostatic intolerance is a physiological response to returning gravity, not evidence that an astronaut is injured.

Muscles and exercise capacity

Microgravity removes much of the resistance normally imposed by body weight. Without countermeasures, muscles used for posture and locomotion lose conditioning, and cardiovascular capacity can decline. Astronauts exercise in flight to limit these effects, but in-flight workouts do not make postflight rehabilitation unnecessary.

NASA’s exercise guidance describes a phased, individualized program beginning at landing. Its rehabilitation standards set a goal of returning to preflight aerobic capacity and muscle strength within 45 days after flight. That is a program objective, not a guarantee for every person or mission (NASA OCHMO-TB-031 Exercise Overview).

Bones

Weight-bearing bones remodel in response to mechanical loading. NASA says astronauts lose an average of 1% to 1.5% of mineral density per month in weight-bearing bones during spaceflight. The rate is an average reported by NASA, not a prediction for an individual, and exercise can reduce but not necessarily eliminate the loss (NASA The Human Body in Space).

Bone recovery is slower than the return of basic mobility. NASA’s exercise overview notes that many crew members may need about a year to recover bone mineral density. An astronaut can therefore walk, work and pass functional tests while bone remodeling continues under medical follow-up.

Eyes and vision

Fluid moving toward the head can affect the eye and optic nerve. NASA describes a group of findings called spaceflight-associated neuro-ocular syndrome (SANS), which can include optic-nerve swelling or flattening at the back of the eye and may require a changed glasses or contact-lens prescription. Approximately 70% of crewmembers on long-duration International Space Station missions of about six months develop SANS findings, according to NASA’s Cardiovascular and Vision page. The direct cause and the long-term significance are not fully understood, and the statistic does not mean that every astronaut has permanent vision loss (NASA Cardiovascular and Vision).

A practical recovery timeline

Period What may happen What determines progress
Landing and egress Heavy limbs, unsteadiness, nausea, weakness or a blood-pressure drop when upright. Fluid status, cardiovascular response, vestibular readaptation and mission duration.
First two days Balance and coordination can remain noticeably impaired; some astronauts report wobbliness. Individual nervous-system adaptation and supervised mobility.
About one week Some crew members feel close to normal for ordinary activities. Personal response; a quick recovery in one astronaut does not set a standard for another.
Several weeks Strength, aerobic fitness and tolerance for demanding work continue to rebuild. Phased rehabilitation and repeated functional assessments.
Up to roughly a year Bone mineral density may still be recovering even after everyday function returns. Bone biology, loading, exercise and the length of the mission.

NASA’s published examples include rapid readaptation, feeling nearly normal after a week, and being mostly recovered within a couple of weeks. The agency does not present those anecdotes as a universal schedule. Recovery can be faster for one body system and slower for another.

How NASA tests readiness after landing

Standard postflight measurements

NASA collects health and performance data before, during and after missions through its CIPHER program. Assessments can include vision examinations, cognitive and spatial testing, cardiovascular imaging, exercise measurements, sensorimotor tasks and MRI. The purpose is to understand adaptation and prepare crews for future missions, not to provide a consumer diagnostic checklist (NASA CIPHER).

Functional tasks that mimic real work

One CIPHER postflight egress-fitness study asks astronauts returning from missions of up to a year to leave a mock capsule and perform a simulated suited spacewalk. Such tasks test whether a crew member can stand, walk, climb, manipulate equipment and work while still experiencing the effects of gravity.

Artemis II assessments

NASA’s Artemis II postflight research included blood pressure, heart rate, eye-health and motor-control measurements, plus a small obstacle course within a day of splashdown. These tests connect medical changes with practical abilities needed after landing (NASA’s Artemis II postflight research).

Why recovery differs between astronauts

  • Mission duration: Longer exposure generally means more time for fluid, muscle, bone and sensory systems to adapt, but duration alone cannot predict an individual outcome.
  • Body system: Balance, blood-pressure control, muscle strength, bone density and vision follow different recovery curves.
  • Immediate function versus tissue recovery: Passing an egress task does not prove that bone density or cardiovascular conditioning has returned to baseline.
  • Countermeasures: In-flight exercise and other mission countermeasures influence how much reconditioning is needed afterward.
  • Personal physiology: Age, prior fitness, genetics, fluid regulation and susceptibility to motion sickness all contribute to variation.

Earth is not the same as the Moon or Mars

Readapting to Earth’s full gravity is only one case. The Moon has about one-sixth of Earth’s surface gravity, and Mars has about three-eighths, so a crew landing there would face a different balance between body weight, blood-pressure demands and locomotion. NASA studies how astronauts perform in partial-gravity environments because an ability that is adequate for a lunar or Martian landing may not equal immediate readiness for Earth. The body still has to interpret altered vestibular and loading signals, but the magnitude and pattern of those signals differ.

What astronauts do during rehabilitation

  1. Stabilize and assess: Medical teams monitor vital signs, symptoms, vision, balance and the ability to sit, stand and walk safely after egress.
  2. Restore upright tolerance: Posture and activity are increased in stages while clinicians watch for lightheadedness or an abnormal blood-pressure response.
  3. Rebuild movement: Supervised aerobic, resistance and sensorimotor exercise target the muscle groups and skills needed for mission duties.
  4. Track objective recovery: Strength, aerobic capacity, coordination, cardiovascular measures and eye findings are compared with preflight data.
  5. Continue longer-term follow-up: Bone density and other slower-changing systems are monitored after the astronaut can already perform ordinary tasks.

The exact schedule, exercise loads and medical restrictions are tailored to the astronaut and mission. The published 45-day fitness objective and approximately one-year bone-recovery description are planning benchmarks, not self-treatment instructions.

What the evidence does—and does not—show

Postflight symptoms are real but not identical for everyone. It is inaccurate to say that all astronauts must be carried from a capsule, that every astronaut faints, or that every reported vision change persists permanently. NASA’s percentages describe specified populations or conditions: more than 60% refers to potential orthostatic intolerance without countermeasures, while approximately 70% refers to SANS findings among long-duration ISS crews of about six months. Neither number is a universal rate for all space travelers.

For the same reason, a quick personal account should not be used to promise a quick recovery to the next astronaut. Functional readiness can return in days or weeks while bone, muscle and vision changes continue to be evaluated. NASA’s combination of medical measurements and realistic tasks is designed to reveal those differences before a crew resumes demanding work.
