An Alzheimer’s diagnosis is made by putting together a person’s history, changes in everyday abilities, examination findings and selected test results—not by relying on one test. A clinician may use cognitive assessments, lab work or brain imaging, and, in some settings, cerebrospinal-fluid or blood biomarker tests. The mix depends on the person and the questions the clinician needs to answer.
What happens at the first appointment?
The clinician will usually ask about the changes that prompted the visit: when they began, how they have progressed, and how they affect everyday life. Questions may cover general health, past medical problems, prescription and over-the-counter medicines, diet, and changes in behavior or personality. A family member or friend who has observed the changes may also be asked for their perspective.

If you are preparing for an appointment, consider bringing an up-to-date medication list and, if the person being evaluated agrees, someone familiar with the changes. These are practical suggestions, not requirements.
Which assessments and tests may be used?
There is no universal checklist. The clinician chooses assessments based on the person’s symptoms, health and risk factors, then interprets the findings together. The Alzheimer’s Association explains that “There is no single test that can determine if a person is living with Alzheimer’s or another dementia.”

- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
Cognitive and functional assessment
Mental-status or cognitive tasks can assess aspects of thinking and memory. The clinician may also ask about everyday activities and how independently the person manages them. An abnormal result is one piece of evidence; by itself, it does not establish Alzheimer’s disease.
Physical and neurological examination
A physical and neurological examination adds clinical findings to the history and cognitive assessment. It contributes to the overall evaluation rather than independently proving Alzheimer’s.

Laboratory tests
Blood or other laboratory tests may help identify other explanations for cognitive symptoms. The sources do not establish a fixed panel for every patient, so the clinician selects tests in light of the individual’s health and symptoms.

Imaging can also provide biomarker information. Alzheimer’s-related measures described by the Alzheimer’s Association include amyloid or tau on PET, glucose metabolism assessed with PET, and brain atrophy assessed with structural MRI. Some biomarkers are not specific to Alzheimer’s alone, so clinicians interpret them in context.
Cerebrospinal-fluid tests
A cerebrospinal-fluid (CSF) test can measure disease-related proteins. Obtaining CSF requires a lumbar puncture, sometimes called a spinal tap. Whether it is appropriate depends on the clinical situation; it is not a routine requirement for everyone being evaluated.

Blood biomarkers
Some blood tests look for biomarkers associated with Alzheimer’s-related brain changes. The Alzheimer’s Association’s 2026 Alzheimer’s Disease Facts and Figures reports two U.S. FDA clearances in 2025 with different intended settings and roles:

| Test | Reported clearance and intended role |
|---|---|
| Lumipulse G pTau217/β-Amyloid 1-42 Plasma Ratio | Cleared in 2025 for specialty-care use; the report describes use for early detection of amyloid plaques in the population specified by the test’s clearance. |
| Elecsys pTau181 plasma test | Cleared in 2025 for primary-care use; the report describes it as designed to rule out Alzheimer’s-related brain changes in the population specified by the test’s clearance. |
These are not home tests, population-screening tests or stand-alone diagnoses. As the National Institute on Aging notes, blood test results alone should not be used to diagnose dementia, though they may be considered alongside other tests. The specific test’s labeling determines who it is for and where it may be used; eligibility and availability can vary.

Do I need an MRI, PET scan or spinal tap?
Not necessarily. These are possible parts of an evaluation, not a mandatory package. A clinician may recommend imaging or CSF testing when it can help answer a specific clinical question. Ask what a proposed test is intended to clarify and how its result would fit with the history and examination.
What happens if a primary-care clinician suspects Alzheimer’s?
A primary-care clinician may refer the person to a specialist for a more detailed diagnosis or further assessment. The next steps depend on the person’s circumstances; there is no single referral timeline, and specialist appointments or advanced tests may not be equally available in every location.
How the results fit together
The clinician combines reported changes and day-to-day functioning with cognitive, physical and neurological findings, plus any laboratory, imaging or biomarker results that were appropriate to obtain. A memory complaint or a screening result alone is not the same as an Alzheimer’s diagnosis. The Alzheimer’s Association’s 2026 report notes that revised diagnostic and staging criteria published in 2024 incorporate biomarker evidence; biological evidence still needs to be understood alongside a person’s symptoms and functioning.
For a plain-language overview of the evaluation, see the National Institute on Aging’s guide to how Alzheimer’s disease is diagnosed and the Alzheimer’s Association’s diagnosis overview.
