The effects and pace of recovery differ widely. Before discharge, ask the care team to explain the likely cause, follow-up, rehabilitation needs, and individual guidance for activity, driving and work.
What happens in hospital after cardiac arrest?
Survivors typically receive hospital care, which may include intensive, critical or coronary care. Clinicians monitor the person, assess effects on the heart and other parts of the body, investigate possible causes and consider treatments intended to aid recovery or reduce the risk of another arrest. Cardiac arrest is not the same as a heart attack: a heart attack can cause an arrest, but it is only one possible cause. Sometimes the cause is still unknown at discharge; Resuscitation Council UK describes an arrest with an undetermined cause as idiopathic. Resuscitation Council UK’s patient guidance explains that care and driving restrictions can depend on the cause, its effects and treatment.

Stabilization is only the first phase. The American Heart Association describes survivorship as a journey through rehabilitation, recovery and social reintegration. Its 2025 post-cardiac-arrest care guideline recommends addressing recovery needs before discharge, rather than treating hospital discharge as the end of care.
What a discharge plan should cover
The AHA guideline recommends structured assessment and treatment or referral for emotional distress after medical stabilization; multimodal rehabilitation assessment and treatment for cognitive, physical, neurological and cardiopulmonary impairments; and multidisciplinary discharge planning. A practical plan should record the medical and rehabilitation recommendations, follow-up arrangements, expected recovery and return-to-activity or work expectations. Ask who coordinates care and how to report a change in symptoms or function.

What recovery can feel like
There is no single pattern. Some people improve steadily; others have effects that persist or become more obvious after they return home or to work. The American Heart Association notes that recovery looks different for each survivor. Possible physical changes include fatigue, weakness, pain, sleep problems, swallowing difficulty, speech changes, fine-motor problems, changes in sexual desire or function, and medication side effects.
Thinking, memory and daily tasks
Some survivors notice changes in attention, short- or long-term memory, planning, organization, learning, language or problem-solving. These can make ordinary tasks or work more demanding, even when the person looks well. Tell the healthcare team about changes in physical, mental or emotional wellbeing rather than assuming they will resolve without support.
Emotional wellbeing and relationships
Fear of another arrest, anxiety, low mood, depression and loneliness can follow the event, sometimes alongside relief or gratitude at surviving. Professional support may help. Family members, friends and people who witnessed or responded to the arrest can also be distressed; they may need support in their own right.

Being home, walking or appearing outwardly well does not establish that someone is fully recovered. Community reintegration can take time and may depend on the effects of the arrest and the support available. The AHA guideline says maximal recovery in referenced research was typically assessed at three to six months after arrest; this is a study follow-up timeframe, not a deadline or promise for an individual.
Which rehabilitation and support may help?
Rehabilitation should match the person’s needs; it is not necessarily limited to exercise. The AHA recommends considering cognitive, physical, neurological and cardiopulmonary impairments. Depending on the assessment, a care team may involve doctors, nurses, physiotherapists, pharmacists, occupational therapists, counsellors, clinical psychologists or clinical exercise physiologists.
Cardiac rehabilitation
Cardiac rehabilitation is one possible component. The AHA describes it as a medically supervised program that combines exercise training, education about heart-healthy living and support to reduce stress. It may support physical function, confidence and recovery after a cardiac event. Referral and eligibility are not universal; ask whether it is appropriate for your cardiac condition and treatment.

Choosing services and peer support
When discussing a local service, ask which needs it assesses, how it coordinates with the treating team, where or how it is delivered, who is eligible, how referral works, and whether caregivers are included. Availability and cost vary by location. Peer groups and online communities can connect survivors with people who have had similar experiences, but they complement rather than replace medical assessment and treatment. The AHA provides survivor recovery guidance and support resources.

Before leaving hospital, make sure the survivor and caregiver know what appointments are arranged, which professionals are involved, and who to contact with questions. Ensure primary care knows about the arrest. Ask the team which symptoms need urgent attention and which changes should be raised at a scheduled review. New or concerning symptoms need advice from the healthcare team; for an emergency, contact local emergency services.
Some difficulties may only become clear when the survivor resumes routines at home. Keep track of changes in energy, sleep, memory, concentration, mood and ability to manage everyday activities, and share them with clinicians. Follow the individualized plan rather than judging recovery by appearance alone.

Driving, exercise and returning to work
There is no universal safe date for driving, exercising, returning to work or resuming other activities. The AHA recommends that discharge planning include individual return-to-activity and work expectations. Decisions depend on the cause of arrest, health, treatment, symptoms and the demands of the activity or job.
Driving
Do not assume that feeling better or being discharged means it is safe or legal to drive. Restrictions depend on the cause of arrest, its impact on health and treatment received. Ask the treating team and check the current rules of the licensing authority in your jurisdiction; requirements differ by location.

Work
Timing and adjustments depend on recovery and the kind of work. Discuss a gradual or modified return with the clinician and employer if fatigue, memory or concentration changes, physical demands, safety-sensitive tasks or stress may affect the job. The guidance supports individualized expectations, not a fixed timetable.
Questions to take to follow-up
- What is the likely cause of the arrest, and are more tests planned?
- Which symptoms or functional changes should prompt an urgent call, and which should be raised at a scheduled review?
- Which rehabilitation assessments or services are appropriate, and who will arrange them?
- What are my individual instructions for driving, exercise, intimacy and work?
- Which medications are short-term and which are intended for longer use?
- What follow-up appointments are arranged, and who coordinates them?
- Could relatives be at increased risk, and should they seek assessment?
- Where can the survivor and caregiver find emotional or peer support?
For caregivers and people who witnessed the arrest
Caregivers can help by attending follow-up when invited, keeping track of questions and changes, and supporting the survivor’s agreed recovery plan without pressuring them to return to normal quickly. They should also pay attention to their own emotional wellbeing. Families, friends and witnesses may benefit from professional or peer support.
This is general information, not an individual prognosis or care plan. Resuscitation Council UK estimates that CPR is attempted in approximately 40,000 out-of-hospital cardiac arrests in the UK each year; that is a UK estimate, not a global figure or the number of survivors. RCUK’s recovery report page summarizes the estimate.
