Short answer: Google’s AMIE research shows that a research AI system performed strongly in a controlled, text-chat diagnostic evaluation against primary-care physicians. It does not show that AMIE replaces doctors, works as well in ordinary visits, or is ready for unsupervised clinical use.
The main study used standardized cases and trained patient actors, not routine face-to-face appointments. A later supervised clinic feasibility study provides useful real-world context, but it was small in scope, single-center, and missing key clinical inputs such as EHR access and physical examination.

What AMIE is and what the study was designed to test
AMIE is a research system, not a commercial primary-care service
AMIE stands for Articulate Medical Intelligence Explorer. Google describes it as a research AI system built on a large language model and optimized for diagnostic reasoning and medical dialogue. The system is intended to gather information conversationally, reason through possible diagnoses, explain its thinking, and suggest next steps.

AMIE was compared with physicians in a particular research task, under a particular communication format, with standardized cases. The result is evidence about performance on that task—not a finding that an AI is generally superior to a clinician in primary care.

The main evaluation used simulated consultations
The peer-reviewed study, published in Nature in 2025 under the title “Towards conversational diagnostic artificial intelligence,” used a randomized, double-blind crossover design. AMIE and board-certified primary-care physicians (PCPs) conducted synchronous text chats with trained patient actors who followed standardized clinical scenarios. The scenarios came from providers in Canada, the United Kingdom and India.
The design borrowed the standardized, structured logic of an objective structured clinical examination (OSCE), but it was not a conventional in-person OSCE. Google’s researchers specifically said the setup was not intended to reproduce normal face-to-face care or the usual clinical use of text, email, chat or telemedicine. The physicians were communicating through a text interface that was unfamiliar to them.
What the published Nature study found
The indexed Nature abstract reports 159 case scenarios involving 20 PCPs. Specialist physicians and the patient actors rated the consultations across multiple performance “axes”—for example, diagnostic reasoning, communication and management-related qualities.

|---|---|---|

| Primary-care physicians | 20 | Board-certified PCPs who took part in the crossover evaluation |
| Specialist-rated axes | AMIE rated higher on 30 of 32 | AMIE received higher ratings on most specialist assessment dimensions in this study setting |
| Patient-actor-rated axes | AMIE rated higher on 25 of 26 | Actors preferred or rated AMIE higher on most of their assessment dimensions |
Those are striking results within the experiment. They suggest that AMIE could produce coherent diagnostic conversations and that its responses were often judged favorably by both medical specialists and the people role-playing patients.
The earlier Google report used different figures
Google Research’s original post, dated January 12, 2024 and updated October 6, 2026, described an earlier reporting snapshot: 149 OSCE-provider scenarios, with AMIE rated higher on 28 of 32 specialist-rated axes and 24 of 26 patient-actor-rated axes. The later peer-reviewed abstract reports 159 scenarios and the 30-of-32 and 25-of-26 results.

These figures should not be blended into one statistic. They are different reporting versions. For the published-study headline, the Nature abstract’s numbers are the appropriate ones; the earlier Google figures document how the reported sample and analysis changed before publication.
What “higher on an axis” does not mean
An axis rating is not the same as a proven improvement in patient health. The study did not show that AMIE reduced missed diagnoses, lowered complications, shortened treatment time, or improved long-term outcomes in ordinary practice. It showed that blinded evaluators gave one system higher scores on specified dimensions of simulated conversations.
Nor does “higher” mean AMIE won every case. The reported totals summarize ratings across axes, not a guarantee of correct diagnosis in each scenario. A system can receive strong overall communication or reasoning scores and still make a consequential error in an individual case.
Did AMIE beat primary-care doctors?
The narrow answer is yes, in the study’s text-chat comparison
Within the randomized crossover evaluation, AMIE received higher ratings than the participating PCPs on most specialist- and patient-actor-rated axes. That is a legitimate comparative finding, and it should not be minimized simply because the study was simulated.
The broad answer is not established
The experiment did not test whether AMIE is more accurate than PCPs during normal appointments. It did not compare an AI-only visit with a doctor’s complete workflow, including vital signs, physical examination, medication reconciliation, prior records, laboratory results, referrals, follow-up and safety-netting. It also did not establish that patients would have better outcomes if a health system substituted AMIE for clinicians.

The Nature abstract says the text-chat interface was unfamiliar to the clinicians and that further research is needed before translating the findings to real-world care. Google’s authors likewise wrote: “Our research has several limitations and should be interpreted with appropriate caution.” That caution applies to the comparison itself, not merely to minor technical details.

A primary-care encounter is an information-and-action workflow, not just a conversation. The AMIE experiment intentionally controlled many variables so that the dialogue could be evaluated consistently. Those controls make the study interpretable, but they also limit what can be inferred from it.
- Patients were trained actors. Actors followed case scripts and standardized scenarios. Real patients may describe symptoms incompletely, misunderstand questions, omit sensitive information, change their story, or present several problems at once.
- The interaction was synchronous text chat. Typing changes the pace, turn-taking and cognitive burden for both sides. It is not equivalent to speaking in a clinic, using a telephone, or exchanging asynchronous portal messages.
- No physical examination was available. A clinician can inspect, palpate, auscultate, measure and observe a patient’s appearance and movement. A text model cannot perform those actions.
- The study did not reproduce a full EHR workflow. Routine care involves prior diagnoses, medication lists, allergies, test results, imaging, specialist notes and documentation requirements. Those sources of context were not part of the simulated comparison.
- Standardized cases reduce messiness. A controlled case makes it easier to compare systems, while real practice includes uncertainty, interruptions, language differences, health-literacy barriers, comorbidities and resource constraints.
- Ratings are not the same as clinical accountability. A doctor must decide when to escalate, order a test, arrange follow-up or send someone to emergency care, and remains responsible for the plan. The simulation did not establish how AMIE would perform those duties independently.
Google’s broader research discussions identify fairness, equity, privacy, robustness and oversight as areas requiring additional work. Those are not side issues: a system that performs well on average can still be unsafe if it fails disproportionately for certain languages, symptoms, populations or care settings.
What the later clinic feasibility study adds
A subsequent prospective study moved AMIE closer to a real workflow. Google Research described the work with Beth Israel Deaconess Medical Center (BIDMC) as a supervised, single-center feasibility study in an academic urgent-care setting. It was not a head-to-head replacement trial.
How the workflow worked
Adults with new, non-emergency episodic complaints completed an AMIE interaction before their visit. A physician monitored the session and could intervene under preset criteria. AMIE generated a transcript and summary for the PCP, who then conducted the clinical visit. The Google Research account was updated October 8, 2026 to note publication in The Lancet.

| Participants | Trained patient actors and 20 PCPs | 100 adults completed the pre-visit interaction; 98 attended appointments |
| Setting | Randomized, double-blind crossover text-chat study | Single academic urgent-care center |
| Clinical oversight | Study comparison with blinded ratings | Physician supervision with preset intervention criteria |
| AMIE’s role | Conducted the simulated diagnostic conversation | Collected information and produced a transcript and summary for the PCP |
| Reported outcomes | Ratings on diagnostic and communication axes | Feasibility, safety monitoring, differential-diagnosis and management-plan assessments |
| Important constraints | Text interface; standardized cases | No EHR access, physical examination or multimodal patient input |
What happened in that study
Google’s summary says all 100 participants completed the AMIE interaction and 98 attended their appointments. No safety stops were needed in the supervised interactions. Blinded evaluators rated AMIE and PCPs similarly for the overall quality of differential diagnoses and management plans.
However, PCPs scored higher for the practicality and cost-effectiveness of their management plans. That distinction is important. A plan can be medically plausible yet difficult to carry out, too expensive, poorly matched to available services or impractical for a particular patient. The clinic study therefore adds evidence about workflow feasibility, not proof that autonomous AI care is safe or effective at scale.
Why this is not a direct confirmation of the Nature result
The two studies answer different questions and should not be treated as a single head-to-head comparison. The first used simulated cases and compared AMIE directly with PCPs in text chat. The later study observed a supervised pre-visit process in one clinic, where PCPs still examined patients and made care decisions. It used a single-arm workflow rather than randomizing patients to AI-only versus doctor-only care.
The later sample was also narrowly defined: adults with new, non-emergency episodic complaints at one academic center. It does not establish performance for children, pregnancy, chronic disease management, emergencies, complex multimorbidity, preventive care or patients who cannot use the interface.

Supported by the studies
- AMIE can conduct structured diagnostic dialogue in a text interface.
- In the published simulated comparison, specialist physicians and patient actors rated AMIE higher than the participating PCPs on most measured axes.
- A supervised clinic workflow can collect a patient’s pre-visit history and provide a transcript and summary to a PCP.
- In the reported feasibility sample, 100 adults completed the interaction, 98 attended visits and no safety stop was required.
- PCPs remained important to the workflow and were rated higher for practical and cost-effective management planning in the later study.
Not established by the evidence
- That AMIE is generally more diagnostically accurate than doctors in routine primary care.
- That it can safely operate without clinician supervision.
- That it improves patient outcomes, reduces total costs or saves clinician time across a health system.
- That it performs equally well across populations, languages, disabilities, socioeconomic groups or uncommon conditions.
- That it can replace physical examination, EHR review, testing, clinical judgment or responsibility for follow-up.
- That the later feasibility findings generalize beyond one center and a limited set of non-emergency complaints.
How patients should interpret the findings
For a patient, the sensible interpretation is that conversational AI may become a useful information-gathering or preparation tool, especially before a clinician visit. The studies do not justify treating an AI conversation as a diagnosis or as permission to delay urgent care.
Best Value
A patient should still seek emergency help for severe or rapidly worsening symptoms and should tell the clinician about symptoms, medications, allergies, home measurements and changes that an automated questionnaire may miss. If an AI-generated summary is used in a clinic, the patient should have an opportunity to correct it before decisions are made.
How clinicians and health systems should interpret it
For clinicians
AMIE’s strongest near-term role, based on the evidence available, is assistive: structured history-taking, preparation of a visit summary and generation of diagnostic possibilities for review. Any deployment would need clear escalation rules, a way to identify missing or contradictory information, and a clinician who can inspect the underlying transcript rather than relying only on a generated summary.
For health-system leaders
A responsible evaluation should measure more than conversational ratings. It should track diagnostic errors, delayed escalation, equity across patient groups, privacy, clinician workload, follow-up completion, patient understanding, resource use and the effect of EHR and examination data. The system should be tested prospectively in the actual population and workflow where it would be used.
Those requirements follow directly from the gap between the studies: one demonstrates strong performance in a controlled comparison; the other demonstrates a limited, supervised workflow. Neither supplies the evidence needed for unsupervised primary-care replacement.
The bottom line
Google’s AMIE study is meaningful evidence that a research AI can produce highly rated diagnostic conversations under controlled text-chat conditions. The Nature results—159 standardized scenarios, 20 PCPs, and higher ratings on 30 of 32 specialist axes and 25 of 26 patient-actor axes—deserve to be taken seriously.
They should also be read exactly as designed. The participants were actors, the interface was text, and the physicians worked without the normal advantages and responsibilities of an in-person clinical workflow. The later supervised BIDMC feasibility study shows that AMIE can be incorporated into a narrow pre-visit process, while also showing that PCPs remained more practical and cost-effective in management planning. Together, the studies support continued clinical research and carefully supervised assistance—not the claim that AI has already replaced primary-care doctors.
