There is no single U.S. price for GLP-1 weight-loss drugs. What you pay depends on the medicine and formulation, the reason it is prescribed, your insurance plan’s coverage and cost sharing, and whether you qualify for a manufacturer offer. As of October 4, 2026, eligible Medicare beneficiaries may have a temporary $50-a-month option for certain products, while commercial and self-pay prices vary.
What determines your cost?
Start with the exact medicine and formulation on the prescription—not just the drug’s brand name. Wegovy tablets and injections, for example, may have different prices or program terms. Zepbound KwikPen and its single-dose products also do not necessarily follow the same coverage or savings rules.

Then confirm the prescribed indication. A plan may treat weight management differently from another medically accepted use of a medicine. Your coverage type matters too: Medicare Part D, commercial insurance, Medicaid, and paying without insurance involve different rules. Finally, account for your deductible, copay or coinsurance, plan limits, and eligibility for any manufacturer offer.

The temporary Medicare GLP-1 Bridge
Medicare says eligible people with Part D drug coverage can get Foundayo tablets, Wegovy tablets or injections, and Zepbound KwikPen for a $50 monthly copayment through the Medicare GLP-1 Bridge. The program runs from July 1, 2026, through December 31, 2027. It does not include Zepbound single-dose pens or vials. See Medicare’s weight-loss-drug eligibility information and the CMS Bridge program details for current terms and exclusions.

When ordinary Part D coverage may apply instead
The Bridge is outside the normal Part D benefit payment flow. Its $50 copayment does not count toward Part D true out-of-pocket spending, and Bridge purchases do not count toward covered-drug spending in the usual way. CMS says the Bridge is not available for a drug and indication already coverable under basic Part D. Examples include Wegovy for specified cardiovascular risk reduction and Zepbound for moderate-to-severe obstructive sleep apnea in adults with obesity. Ask the plan which coverage route applies to the prescribed drug and indication; see CMS information for Part D plans.

Commercial coverage is plan-specific; there is no single copay that applies to all insured patients. Ask the insurer or pharmacy-benefit manager whether the exact product, formulation, and weight-management indication are covered on the current formulary. Find out whether prior authorization, step therapy, quantity limits, or preferred-drug rules apply, and request the expected amount for your next fill after deductible and cost sharing.

A manufacturer’s “as little as” price is a conditional offer, not proof that your insurance covers the drug or a guarantee of what you will pay. Novo Nordisk advertises Wegovy for as little as $25 a month for eligible commercially insured patients, subject to a maximum savings amount and exclusions. Check the current Wegovy cost and coverage terms, including eligibility and caps, before relying on that figure.
Eli Lilly’s Zepbound savings page describes dose-specific KwikPen card prices for certain commercially insured patients whose plan does not cover that formulation. The page says these purchases are for self-paying patients and cannot be applied to a third-party payer’s deductible or true out-of-pocket requirements. Review the current Zepbound savings terms before using an offer.

How to compare coverage and price
- Write down the exact prescription. Record the drug, formulation, and dose. A tablet, injection, KwikPen, single-dose pen, or vial may have different coverage or offer terms.
- Confirm the indication. Ask the prescriber what condition the prescription is intended to treat. Do not assume a plan’s decision for weight management also applies to a different medically accepted indication.
- Check the current plan formulary. Ask the insurer or pharmacy-benefit manager whether that exact drug, form, and indication are covered for the current benefit year. Ask about prior authorization, step therapy, quantity limits, preferred-drug requirements, and any exclusions.
- Get the amount for your actual fill. Ask what you would pay now, including any remaining deductible, copay or coinsurance, and applicable plan limits. Do not substitute a manufacturer’s advertised savings amount for the plan’s estimate.
- Compare other available prices and their consequences. Check current manufacturer offers and any discount-card price, including eligibility and caps. Medicare advises comparing discount prices with plan costs: a discount-card purchase does not count toward the Part D deductible or annual out-of-pocket limit. A lower immediate price may therefore have a different full-year cost than filling through the plan.
- If you have Medicare, check the relevant route and dates. Verify Bridge eligibility, covered formulation, and program dates on Medicare’s current page, and ask whether ordinary Part D coverage applies to your drug and indication.
What Medicaid and other coverage questions remain plan-specific
Medicaid coverage for weight-management treatment varies by state and program. CMS discusses differences between obesity or weight-management use and other medically accepted indications, but that policy discussion does not establish an individual state’s coverage. Ask the state Medicaid program or managed-care plan about the exact prescription and indication; the CMS 2026 policy fact sheet provides policy context, not a state-by-state coverage answer.
