For a boy who has not gone through puberty and cannot produce sperm, the principal option currently offered by some specialist programs is testicular tissue cryopreservation: a sample of testicular tissue is removed and frozen before fertility-threatening treatment. It remains experimental. The stored tissue contains immature cells, not ready-to-use sperm, and no current technique has been proven to produce a future pregnancy or biological child from it.
If puberty has begun and sperm can be collected—or retrieved surgically when a semen sample cannot be provided—sperm cryopreservation is a different, established pathway. A treating clinician and pediatric fertility-preservation specialist should assess the child promptly, ideally before treatment starts.

The key question is whether sperm can be obtained
Pubertal development changes the preservation options. A prepubertal boy generally does not yet make mature sperm, so standard sperm banking is not possible. Once sperm production has started, the team may be able to freeze sperm directly or retrieve it from the testicle.
| Clinical situation | What can be collected | Potential preservation approach | Evidence status | Timing guidance |
|---|---|---|---|---|
| Prepubertal boy who cannot produce sperm | Testicular tissue containing immature germ cells | Testicular tissue cryopreservation through an approved specialist protocol | Experimental or investigational; future fertility use is unproven | Discuss and arrange referral before gonadotoxic treatment when feasible |
| Puberty has begun and a semen sample can be produced | Ejaculated sperm | Sperm cryopreservation | Established fertility-preservation approach | Usually performed before treatment that may damage fertility |
| Puberty has begun, but the boy cannot provide a semen sample | Sperm retrieved from testicular tissue | Testicular sperm extraction (TESE) followed by sperm cryopreservation | ASCO recommends offering this option in the relevant clinical setting | Offer before cancer treatment begins when possible |
The categories are not interchangeable. A specialist must determine whether puberty has progressed enough for sperm retrieval, whether the planned treatment threatens fertility, and whether an appropriate program is available locally.

What testicular tissue cryopreservation involves
The procedure preserves tissue now in the hope that future methods will be able to mature its cells into usable sperm. The process is normally organized as a coordinated clinical pathway rather than as a routine fertility test.
- Prompt referral and risk counseling. The treating team explains how the disease and proposed therapy may affect fertility and refers the family to a pediatric fertility-preservation program. Guidance from the American Society for Reproductive Medicine (ASRM) calls for prompt counseling and referral for patients facing fertility-threatening treatment.
- Assessment of pubertal status and treatment plan. The specialists determine whether the child is producing sperm, how urgent treatment is, and whether sperm collection, TESE, or tissue preservation is the appropriate route.
- Review of an approved protocol. For a prepubertal child, tissue freezing should take place under a clinical trial or another approved experimental protocol. ASRM describes the approach as being performed under institutional review board (IRB)-approved protocols, and the 2025 ASCO guideline update likewise limits this option to clinical trials or approved experimental protocols.
- Consent, assent, and procedural planning. The team discusses what tissue will be removed, the timing relative to treatment, anesthesia, procedural risks, storage arrangements, and the choices that may be needed later. Parents or guardians provide the legally required consent, while the child’s age and maturity determine what assent can be sought.
- Tissue collection and freezing. A sample of testicular tissue is obtained and cryopreserved. The exact biopsy method, anesthesia plan, laboratory process, and storage duration depend on the center and the child’s medical circumstances.
- Long-term storage and follow-up. The family receives information about where the tissue is stored, how follow-up works, and what decisions will govern its future use. Storage does not itself create a treatment that can restore fertility today.
Why the technique is still experimental
Prepubertal tissue contains immature germ cells. Researchers are investigating ways to develop those cells into sperm outside the body or to return them to a testicle so that sperm production might occur later. ASRM identifies in-vitro spermatogenesis and germ-cell transplantation as technologies under study; neither is an established clinical route to a child from a prepubertal boy’s stored tissue.
“Testicular tissue cryopreservation is an experimental technique performed under institutional review board (IRB)- approved protocols.”

ASRM, Fertility preservation in patients with medical indications: a committee opinion, 2026
The existence of frozen tissue therefore does not guarantee that sperm will ever be produced, that a pregnancy will result, or that the child will have a biological child. Families should weigh the possible future benefit against the certainty that the procedure is investigational and may never provide a usable fertility treatment.
What the available evidence actually shows
The European Society of Human Reproduction and Embryology (ESHRE) published good-practice recommendations in 2025 covering program organization, patient selection, counseling, biopsy, and cryopreservation for children receiving gonadotoxic therapies. Those recommendations help centers deliver the procedure consistently; they do not demonstrate that current clinical techniques can restore fertility using stored prepubertal tissue.

ESHRE reported that a survey of centers offering the procedure indicated that more than 3,000 patients had undergone testicular tissue cryopreservation. This is a count of people who had tissue collected and frozen, not a count of successful fertility treatments. The same discussion states that clinical applications using the stored tissue remain unproven.

No reliable live-birth or pregnancy success rate for using stored prepubertal tissue is established by these guidance documents. A center may have its own protocol-specific information, but that information should not be generalized to every child or program.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. When a specialist may discuss tissue preservation
Consideration is most relevant when all of the following are true:
- The planned disease treatment is expected to pose a meaningful risk to future fertility.
- The child is prepubertal or otherwise unable to provide mature sperm.
- An experienced pediatric fertility-preservation program can perform the procedure under an IRB-approved protocol, clinical trial, or other authorized experimental pathway.
- The collection can be coordinated without delaying medically necessary treatment.
These points do not establish eligibility. A specialist still has to review the child’s diagnosis, treatment schedule, general health, anesthesia suitability, pubertal development, and the protocol’s inclusion criteria.

Questions to ask the treating team
Bring a written list to the oncology, endocrinology, urology, or fertility-preservation consultation. Useful questions include:
- What fertility risks are expected from this child’s specific disease and planned treatment?
- Is the child producing sperm, and does his pubertal development make sperm collection or TESE possible?
- Is testicular tissue cryopreservation available through an experienced pediatric program here, and is it being offered as a clinical trial or another approved experimental protocol?
- What would tissue collection involve for this child, including timing, anesthesia, and procedural risks?
- Where would the tissue be stored, how long could it remain there, and what follow-up would be required?
- What costs might apply, what insurance or public-coverage rules govern them, and who can explain the financial arrangements?
- What assent can the child provide, what consent is required from parents or guardians, and how will later decisions about the tissue be made?
- How will preservation be coordinated with treatment so that necessary care is not delayed?
Practical limits families should plan for
Availability differs by location
Only some specialist centers have the laboratory, surgical, storage, and research infrastructure required. A referral may involve another hospital or region. The cited professional recommendations do not establish that the procedure is available in every country, state, or health system.
The procedure has immediate burdens
Tissue collection is an invasive procedure and may require anesthesia. The child’s clinicians must explain the specific surgical and anesthesia risks rather than relying on a generic estimate. Timing can be difficult when treatment is urgent, so referral should occur as soon as a fertility risk is recognized.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteStorage creates future decisions
Families should understand the storage contract, fees or coverage rules, length of storage, and what happens if the child later changes institutions or reaches adulthood. Consent and assent arrangements can affect who may authorize future use, continued storage, transfer, or disposal.
The future technology may not work
Even if research eventually enables sperm production from immature tissue, the result would still depend on the quality and quantity of the stored sample, the child’s later health, laboratory performance, and reproductive options available at that time. A preservation decision should therefore be described as keeping a possibility open, not securing future parenthood.
A decision path for families
- Ask about fertility risk before treatment. Request a prompt explanation of how the proposed therapy could affect reproductive function.
- Clarify sperm availability. Determine whether the child is prepubertal, producing sperm, or unable to provide a semen sample despite having entered puberty.
- Match the option to that finding. Use sperm cryopreservation when sperm can be collected; discuss TESE with sperm cryopreservation when clinically appropriate; consider tissue cryopreservation only through an approved experimental pathway for a child who cannot produce sperm.
- Confirm the protocol and logistics. Ask about referral timing, anesthesia, tissue handling, storage, follow-up, consent, and costs before agreeing.
- Protect treatment timing. The fertility-preservation team and the treating clinicians should coordinate so that preservation planning does not postpone necessary therapy.
How the major guidance documents fit together
ASRM’s 2026 committee opinion provides current professional guidance on male fertility preservation and explicitly labels prepubertal testicular tissue cryopreservation experimental. ASCO’s 2025 cancer guideline update distinguishes this investigational pathway from TESE and sperm cryopreservation for pubertal or postpubertal males. ESHRE’s 2025 recommendations address how centers should set up and deliver testicular tissue cryopreservation programs. Together, they support early specialist counseling while making clear that frozen prepubertal tissue is not yet a proven fertility treatment.
