An AI lab should give an external auditor a scoped, version-specific and traceable evidence package—not an unstructured data dump. The package should show what system was examined, how it was built and tested, what risks and incidents were found, which mitigations were adopted, who approved key decisions, and what the evidence cannot establish.
The exact obligation depends on the jurisdiction, model classification, contract and auditor’s authority. A regulator, customer-appointed assurance provider, independent researcher and private consultant do not automatically have the same access rights. Agree the audit question, legal or contractual basis, systems and versions, methods, confidentiality controls and deliverables before transferring sensitive material.

Start with the audit scope and authority
Evidence is useful only when its purpose and boundaries are clear. Begin with a written scope sheet that an auditor can test against the supplied records.

State the decisions the audit must support, such as whether a model meets a contractual control, whether a deployment followed an approved risk process, or whether a general-purpose model has documented systemic-risk controls. Identify the systems, model families, deployed versions, interfaces and material changes included in scope. Record what is explicitly out of scope.

Confirm who is entitled to request what
Record the authority behind the request: a statute, regulator notice, customer contract, board mandate, certification engagement or research agreement. Under EU AI Act Article 92, a Commission access request has a defined public-law basis. The article says the request must state its legal basis, purpose, reasons and deadline; that does not give every private auditor an automatic right to source code, model weights or raw training data. See the European Commission AI Act Service Desk text for Article 92.
Agree methods and outputs
Specify whether the auditor will review documents, interview staff, inspect a controlled environment, run tests through an API, examine source code, or use a combination. Define the reporting format, evidence-retention period, permitted copies, escalation route and treatment of unresolved findings. If the auditor cannot access an artifact, agree an alternative procedure and require the final report to disclose that limitation.
Use an evidence index to make the package traceable
An evidence index is a practical control, not a statutory form. Give every artifact an owner, creation or review date, system and version identifier, source location, integrity or provenance information, related audit question, access classification and known limitation. A reviewer should be able to move from a control claim to the exact record, test result or decision that supports it.

Document the system, purpose and design history
System identity
Provide the model or system name, immutable release or build identifier, configuration, serving software, relevant API version and deployment date. Describe intended users, permitted tasks, operating environments, dependencies and system boundaries. Link each evaluation and incident to the precise version it concerns. Explain superseded versions and whether a later update invalidated an earlier result.
Problem specification and design decisions
Keep the original problem statement, success criteria, prohibited uses and design constraints. Include architecture descriptions at a level appropriate to the audit, design alternatives considered, approval records and change tickets. A short rationale for decisions such as data exclusions, safety filters, human review or release thresholds helps an auditor distinguish deliberate controls from undocumented behavior.
Development and change control
Show who approved training runs, fine-tuning, post-training changes, safety-policy changes and production release. Preserve dates, reviewers, test gates and rollback decisions. If a model was adapted by a downstream team, identify the boundary between the lab’s evidence and the integrator’s responsibilities.

Show where the data came from and how it was managed
For training, validation and test data, provide descriptions rather than assuming an auditor can infer meaning from filenames. Describe sources, collection methods, licensing or permission basis where relevant, preprocessing, filtering, deduplication, labeling, curation and retention. Identify whether a dataset is representative of the intended population and use case, and document material exclusions.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsRecord provenance for datasets and major transformations: source location, collection period, version, processing code or procedure, responsible owner and integrity information. Explain data-quality checks, contamination checks and known gaps. If privacy, copyright, confidentiality or security rules prevent disclosure of raw records, provide a controlled description, sampling evidence, redacted material or an independent test procedure that addresses the audit question without implying that the auditor inspected data it did not receive.
Make evaluation evidence reproducible and candid
Test protocol and conditions
For every material evaluation, retain the protocol, objective, criteria for passing or failing, test population or dataset description, sampling method, prompts or inputs, environment, software and hardware configuration, random seeds where relevant, and the model identifier. Record who ran the test and when. A result without its conditions cannot establish whether a claim holds in production.
Results and uncertainty
Supply raw or suitably detailed outputs, aggregation rules, error categories, confidence or uncertainty information and deviations from the approved protocol. Separate internal benchmarks from evidence collected in the intended deployment context. State where a result is exploratory, where coverage is incomplete and which populations, languages, modalities or operating conditions were not tested.
The European Commission’s guidance for covered general-purpose AI providers describes evaluation using standardised protocols and state-of-the-art tools. It also distinguishes authority-facing technical documentation from information supplied to downstream providers. Read the current Commission FAQ on obligations for general-purpose AI providers alongside the applicable law; the Commission notes that its guidelines are not themselves legally binding.

Retests and reproducibility checks
Keep the exact configuration used for a reported result, including evaluation code or a controlled execution method. When a mitigation changes the model or system, retain the before result, the mitigation decision, the after result and the reason for any changed test conditions. If independent reproduction is impossible because of confidentiality or compute limits, document the substitute procedure and its limitations.

Include adversarial testing, risks and mitigations
An external auditor should see how the lab looked for harmful, deceptive, insecure or otherwise unacceptable behavior—not only ordinary performance. Provide the threat or risk taxonomy, assumptions, attack scenarios, test plans, tools, coverage and severity criteria. Include failures as well as successful defenses.
Connect findings to decisions
For each material finding, identify the affected version, likelihood or severity rationale, owner, mitigation, residual risk, approval and deadline. Show whether the mitigation was implemented, verified and retested. A risk register that contains only open labels, without evidence of decisions and follow-up, is not a complete control record.
Systemic-risk documentation for covered providers
Where the EU AI Act applies to a general-purpose AI provider, the Commission describes duties concerning systemic-risk assessment and mitigation, adversarial testing, serious-incident reporting and cybersecurity. Applicability, exemptions and timing depend on the model and provider’s status. Do not present those duties as universal requirements for every AI lab or every private audit.

Report performance, limitations and uncertainty
State the conditions under which performance claims hold: task definition, languages or modalities, user population, environment, data freshness, latency or resource limits and human-in-the-loop assumptions. Include failure examples and known out-of-distribution behavior where disclosure is safe and relevant. Explain what the evidence does not show—for example, a benchmark score does not by itself establish reliability in a customer’s workflow or absence of social, privacy or security harms.
Keep limitation statements versioned. If monitoring reveals a new limitation, link the observation to the incident, evaluation, product change or risk decision that followed. Auditors need to see whether limitations were communicated before release and updated when evidence changed.
Preserve incident and corrective-action records
Provide a chronological incident file for material safety, security, privacy, reliability or compliance events. Each record should identify the affected system and version, discovery date, reporter, impact assessment, containment, investigation method, root-cause analysis where established, communications, corrective action, owner and current status.

Keep open findings visible. Mark an issue as closed only when the responsible owner, approval and verification evidence are recorded. If an incident was reported to a regulator or customer, include the report and any subsequent clarification subject to lawful confidentiality restrictions.
Show governance and accountability
Map decision rights across the lifecycle: who can approve data use, training, release, risk acceptance, exceptions, monitoring changes and retirement. Supply committee or review records, escalation paths, conflict-of-interest controls and evidence that decisions were made by the designated roles.
Explain how independent challenge worked in practice. An auditor should be able to distinguish a technical team’s recommendation from the authority that accepted residual risk. Include exceptions, compensating controls and expiration dates instead of presenting policy as if every control operated perfectly.
Make access meaningful without exposing sensitive material indiscriminately
Access should be proportional to the audit question. Define who may see each artifact, whether it can be copied, where it may be stored, how activity is logged, how long it is retained and how it will be destroyed or returned. Use controlled environments, redaction, secure APIs or supervised execution when those measures still let the auditor perform the agreed procedure.
The NTIA Artificial Intelligence Accountability Policy Report identifies lack of access to system components as an accountability barrier and discusses appropriate access for eligible evaluators while controlling access-related risks. It also notes that evaluators may need associated software and technical artifacts to assess a system in its functional environment. These are policy findings, not a blanket handover rule.

“The request for access shall state the legal basis, the purpose and reasons of the request and set the period within which the access is to be provided, and the fines provided for in Article 101 for failure to provide access.”
This sentence concerns a Commission request under the EU AI Act. It is not a universal term for private audit engagements.
EU AI Act documentation: separate authority and downstream audiences
The Commission’s general-purpose AI provider FAQ describes technical documentation for authorities and separate information for downstream AI system providers. Authority-facing material includes architecture, development, training, testing and validation information, computational resources and energy use, along with public training-content summaries and copyright policies where applicable. Downstream documentation is intended to help integrators understand intended tasks, capabilities, limitations, integration requirements and relevant data information.
Check the model’s classification, provider status, open-source conditions and effective dates before treating any item as a legal duty. The Commission page was updated 11 November 2025, while the Article 92 service-desk text identifies a consolidated AI Act version dated 27 July 2026; legal interpretation remains subject to the applicable regulation and authoritative decisions.
A practical assembly workflow
- Appoint an evidence owner. Give one person authority to coordinate engineering, data, security, legal, product and governance records.
- Freeze the scope. Record the audit questions, systems, versions, environments, authority, dates and excluded areas.
- Map claims to artifacts. For each control or claim, identify the test, decision, record or interview that supports it and note any gap.
- Collect immutable copies. Preserve source location, timestamp, version, integrity metadata and chain of custody before normal retention processes change the files.
- Run a quality review. Check that results identify conditions, failed tests are present, incidents have owners and every artifact has a limitation statement where needed.
- Set the access plan. Classify materials, configure secure transfer or a review environment, record permitted uses and test that the auditor can actually execute the agreed procedures.
- Log the handover. Record what was delivered, when, by whom, to whom, under which terms and what remains unavailable or pending.
Common evidence failures to prevent
- Delivering current policy documents while omitting the version and configuration that was actually deployed.
- Providing benchmark scores without protocols, test conditions, data descriptions or failed cases.
- Calling a risk “mitigated” without a retest, owner or approval.
- Claiming an auditor had access when it received only a summary or a redacted extract.
- Mixing regulatory requirements with voluntary good practice without identifying the legal scope.
- Over-disclosing raw data or source code when a controlled procedure would answer the audit question more safely.
- Deleting superseded records, incident drafts or exception decisions that explain why a control changed.
What the final audit record should say
The handover and final report should identify the evidence set, versions, methods, people consulted, tests performed, artifacts not provided, access restrictions, unresolved gaps and reliance on third parties. A transparent limitation is stronger than an unsupported assurance. The lab’s objective is not to prove that an AI system is flawless; it is to let an appropriately authorized reviewer trace claims, decisions, failures and mitigations to credible records.
