Keep human ownership and approval, but change how reviewers spend their attention. AI coding assistants and agents can produce large, fast diffs whose reliable and risky parts look equally confident. Effective teams therefore require small, traceable changes; review intent and risk before reading every line; independently challenge tests; scrutinize security-sensitive code, dependencies, and trusted build systems; and measure whether the new workflow preserves quality.
The goal is not to label every AI-authored line as unsafe. It is to make uncertainty visible and ensure that the person approving a change understands what it does, what it assumes, and what could fail.

What stays non-negotiable
AI assistance does not transfer accountability from the engineering team to a model or tool. The UK Home Office engineering standard says AI-assisted code must be reviewed and approved by suitably qualified people before production, tested, traceable, and held to the same security expectations as code written by people. It also says teams must understand what they run. That is a UK departmental standard, not a law or a universal employer policy, but it is a useful concrete baseline for writing your own policy: SEGAS-00020 Use AI.

Why AI-generated diffs require a different review habit
A human author usually supplies social and contextual signals: why a shortcut was chosen, which parts are uncertain, and where the design is unusual. An AI output can present a secure function and a subtle authorization flaw with the same polished confidence. It can also produce far more code in one pass, making exhaustive line-by-line attention an unrealistic first move.
JetBrains researchers and Lund University describe this as a trust-calibration problem. Their proposed approach begins with an overview, identifies uncertain or risky areas, and then drills into selected details. It came from a participatory design study with 17 practitioners and a follow-up survey of 43 software professionals. Those are sample sizes, not population estimates or evidence of defect reduction. The October 2026 article presents a conceptual framework rather than a controlled demonstration that the method improves production outcomes: Our Framework for Reviewing AI-Generated Code.
That distinction matters. Teams should adopt staged review because it is a sensible way to manage a large information load, then verify that it works in their own process instead of claiming that a research proposal is settled policy.

Split agent work into coherent units
Ask for a small feature, fix, or refactor that can be explained in one sentence. Break a broad agent-generated task into separate commits or pull requests with a single purpose. The UK National Cyber Security Centre notes that large commits make effective review harder, while small regular commits simplify both review and rollback: Produce clean & maintainable code.
For a change that genuinely spans many components, start with a reviewed specification or acceptance criteria. A reviewer can then compare the implementation with an agreed behavior instead of reconstructing intent from thousands of generated lines.
Require an intent and traceability note
Have the author put the following information in the pull request:

- Files, services, data stores, and trust boundaries affected.
- Important assumptions and known limitations.
- Tests and security checks run, including their scope.
- Which portions were AI-assisted and how the output was verified.
Existing commits and pull requests can provide traceability. The Home Office standard gives example wording for AI-assisted commits and pull requests; treat that wording as one implementation option, not a universal disclosure mandate.
Check tool and data permissions
Apply your organization’s approved-tool and data-handling rules before an assistant sees source code, tickets, logs, credentials, or customer information. The Home Office guidance says to use approved tools and not expose restricted information without approval. Employers differ in their definitions of restricted data, retention, and permitted integrations, so your policy must state those boundaries explicitly.
Use a staged review during the pull request
1. Establish system intent before inspecting syntax
First answer: does this change solve the right problem, and is it placed in the right architectural layer? Read the acceptance criteria, call graph, data flow, and neighboring code. Check who owns validation, where authorization is enforced, and whether the patch changes a trust assumption between a client, service, queue, database, or deployment system.
Reject or return the change when its behavior is unclear, even if individual lines look idiomatic. A perfectly formatted implementation of the wrong requirement is still a failed review.
2. Map the diff to risk
Mark high-consequence areas before spending time on routine formatting or mechanical edits. Give direct human scrutiny to:

- Input validation, output encoding, deserialization, and path or query construction.
- Cryptographic operations, key handling, tokens, and secrets.
- Database migrations, data deletion, and changes to retention or privacy behavior.
- New or upgraded dependencies.
- Build scripts, package scripts, CI/CD configuration, container definitions, deployment manifests, and infrastructure code.
OWASP warns that coding agents can modify build scripts, CI/CD configuration, package scripts, and deployment infrastructure, all of which may execute in trusted contexts. A change outside the application directory is not automatically low risk: OWASP Secure Coding with AI Cheat Sheet.

3. Read selectively, then drill down
For a broad diff, begin with a file and dependency overview. Identify unfamiliar algorithms, security boundaries, generated boilerplate, and sections where the model’s assumptions are not obvious. Filter the review to those areas, then inspect the exact control flow, error handling, and data transformations. Request or write detailed explanations for behavior that remains uncertain.
This overview-to-details sequence is a proposed framework, not proof that a particular interface or tool produces better reviews. Its value is operational: it makes reviewer attention proportional to uncertainty and consequence instead of to the number of changed lines alone.
4. Challenge the implementation’s assumptions
Ask questions that a code generator cannot answer merely by restating its output:
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitches- What happens with an empty, oversized, malformed, duplicated, or expired input?
- What happens when a dependency times out, returns partial data, or changes its schema?
- Can two requests race and bypass an invariant?
- Does the code fail closed when authorization or validation cannot be completed?
- Are errors, logs, metrics, and retries safe for secrets and personal data?
- Does the patch preserve backward compatibility and rollback safety?
Treat tests as independently authored evidence
Generated tests can be useful scaffolding, but passing tests written from the same prompt or implementation are not independent evidence that the behavior is secure. Review the test suite as a separate artifact.
Look for tests that became weaker
- Deleted tests, reduced coverage, or assertions replaced with snapshots that accept any shape.
- Assertions weakened from exact security or business rules to “does not throw.”
- Mocks that replace the real authorization, parser, transaction, or external boundary.
- Tests that merely encode the implementation’s current behavior rather than the requirement.
- Fixtures that omit permissions, tenant identity, expired credentials, or failure responses.
Add cases the model is unlikely to volunteer
OWASP recommends adversarial and negative cases that the AI did not generate, and manual writing or independent verification of security-critical tests. Add invalid-input, boundary, concurrency, replay, rate-limit, permission, and expired-token cases where applicable. Test both sides of a security decision: an authorized request that must succeed and a nearly identical unauthorized request that must fail.
Passing generated tests alone is not evidence of security. A reviewer should be able to explain which requirement each important assertion protects and why the test would fail if that control were removed.

Give dependencies and trusted tooling their own review
Dependency changes
For every added, removed, or upgraded package, verify provenance, version constraints, known vulnerabilities, license compatibility, maintenance status, transitive dependencies, and architectural fit. Confirm that the package is necessary rather than a convenient replacement for a small function the project already owns. The NCSC guidance supports dependency scrutiny, and the Home Office standard includes it among the areas requiring understanding and review.
Build, CI, and deployment changes
Read shell commands, permission changes, artifact paths, secret references, and network access directly. Check whether a generated workflow runs code from an untrusted pull request with production credentials, downloads an unpinned action or binary, uploads source or logs externally, or changes the release trigger. Require the same approval and security checks for these files as for application code; their position in the repository does not make them less consequential.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Compare review organizations deliberately
| Review organization | Strength | Risk to manage | What to measure |
|---|---|---|---|
| Traditional line-by-line diff | Detailed local inspection and familiar tooling | Reviewer fatigue, missed system-level intent, and poor scalability for large generated diffs | Comprehension, missed-risk findings, review time, and cognitive effort |
| Overview → risk-based zoom/filter → details on demand | Prioritizes uncertain and consequential areas before exhaustive reading | Early triage can overlook a low-signal defect; the framework is conceptual, not proven superior | Comprehension, missed-risk exposure, useful context surfaced, and reviewer effort |
These approaches are not mutually exclusive. Use the overview to set priorities, then perform line-level inspection where the risk map demands it.
Decide how AI may assist the review
An AI system may summarize a diff, suggest locations to inspect, or propose missing tests. It should not become the final approver. The available guidance establishes human ownership and the need to qualify tools, but it does not provide a fair comparative benchmark for particular products.
| Approach | Questions to answer |
|---|---|
| Human-only review | Does the team have enough expertise and time? Which classes of defects are repeatedly missed? |
| AI-assisted triage plus human approval | Does triage improve coverage and useful findings without increasing false alarms, missed defects, privacy exposure, or un-auditable decisions? |
Keep prompts and outputs within approved data controls. Record consequential AI findings in the same issue or pull-request trail as human findings so a later investigator can understand what was checked and what was accepted.
After approval: preserve the gates and measure outcomes
Do not lower test, security, or release gates because an assistant produced the patch. Require the normal checks before merge and release, and sample production changes for escaped defects and security incidents.
Best Value
OWASP’s AI Security Verification Standard Appendix C suggests comparing AI-produced or AI-mediated work with a human-only baseline. Candidate measures include vulnerability density, mean time to detect, AI-attributable defect rate, prompt-injection detection rate, and fork-PR rejection rate: OWASP AISVS Appendix C.
These are proposed controls and metrics, not a validated universal scorecard. Define each metric before collecting it. For example, specify whether vulnerability density counts only confirmed findings, how duplicated alerts are handled, and which time window separates detection from remediation. Compare similar work with a human-only baseline, account for change size and risk, and look for trends rather than treating one number as a quality verdict.
Review the workflow when tools, model capabilities, permissions, or organizational guidance change. A prompt policy that was safe for a read-only assistant may be inadequate when an agent can edit CI files or open pull requests.
A practical team checklist
- Confirm the tool and data use are approved for the repository and information involved.
- Define the intended behavior and acceptance criteria before asking for broad implementation.
- Split generated work into coherent, reversible commits or pull requests.
- Require a traceability note covering intent, affected areas, assumptions, tests, and AI assistance.
- Start review with architecture, trust boundaries, and system behavior.
- Mark authentication, authorization, validation, cryptography, migrations, dependencies, and delivery tooling for direct scrutiny.
- Use an overview-to-details reading sequence for large diffs, then inspect high-risk lines and control flow.
- Review tests independently; restore deleted or weakened assertions and add adversarial cases.
- Check dependency provenance, licensing, maintenance, and transitive risk.
- Run the ordinary tests, security gates, and release approvals.
- Record findings, escaped defects, review effort, and security outcomes against a comparable baseline.
- Adjust guardrails when the tool’s permissions or the team’s risk profile changes.
What this evidence does—and does not—prove
The guidance supports a process change: retain qualified human approval, make AI-assisted work traceable, prioritize risk, independently validate tests, and monitor outcomes. It does not establish that AI-authored code is uniformly worse, that a specific assistant fixes review, or that an AI reviewer can replace human accountability.
NIST SP 800-218A, finalized 26 July 2024, addresses secure development of generative AI and dual-use foundation models rather than serving as a direct code-review standard for ordinary application teams: NIST SP 800-218A. OWASP’s cheat sheet and AISVS are living documents, so check their current revisions before turning wording into version-specific requirements.
The Bottom Line
AI should change the shape of code review, not who is accountable. Keep human approval, make changes small and traceable, inspect architecture and high-risk boundaries before routine lines, challenge generated tests independently, scrutinize dependencies and trusted tooling, and use measured outcomes to refine the workflow.
