Fraud prevention is an enterprise operating capability, not a score or a queue owned by one team. A CISO should connect cybersecurity, fraud, financial-crime, threat-intelligence, data, and business operations around a common attack lifecycle; use scores as evidence for accountable decisions; and tie loss scenarios to mission impact, risk appetite, privacy obligations, and customer recourse.
Why fraud operations belong in the CISO’s enterprise risk agenda
Cyber-enabled fraud routinely crosses organizational boundaries. The people who detect a malicious login may sit in security operations, while the team that sees an unusual payment sits in fraud, and the investigators who understand laundering or sanctions evasion sit in financial-crime operations. Customer support, payments, identity, and product teams may each hold another piece of the story.

FS-ISAC’s 2025 Leveling Up: A Cyber Fraud Prevention Framework for Financial Services describes the problem directly: “To succeed, an institution requires data, intelligence, and specific capabilities — all of which may be siloed in cybersecurity, fraud, financial crimes/anti-money laundering, and other teams.” The framework supplies a shared structure and vocabulary for those groups, plus a way to exchange indicators, controls, and lessons learned. It is an industry framework for financial services, not a universal regulatory standard.
For a CISO, the practical implication is that fraud loss should appear in the same enterprise conversations as availability, confidentiality, safety, and regulatory risk. A blocked attack that creates intolerable customer friction can still damage the mission. Conversely, a low-volume fraud pattern may deserve urgent investment if it threatens a mission-essential function or a risk appetite boundary.

Use one lifecycle to connect cyber and fraud teams
FS-ISAC organizes cyber-fraud activity into five phases. An institution can begin with whichever phase first exposes an indicator, then ask other teams what they know about adjacent phases. That approach avoids waiting for a perfect end-to-end data set and helps teams place preventive controls earlier.
| Phase | What the actor is doing | Useful defensive questions |
|---|---|---|
| Recon | Selecting targets, collecting information, setting up infrastructure, and planning. | Which accounts, employees, devices, vendors, or public data are being profiled? What indicators can threat intelligence and privacy teams share before a login or transaction occurs? |
| Initial access | Obtaining a foothold against a consumer, financial institution, or related entity. | Are phishing, credential theft, malware, social engineering, or weak recovery processes involved? Which identity and email controls can interrupt entry? |
| Positioning | Changing or collecting account information so funds or data can be sent to infrastructure controlled by the actor. | Are contact details, beneficiaries, payout accounts, devices, or authentication factors being altered? Can change events trigger step-up verification or a cooling-off period? |
| Execution | Converting stolen access or data into money, often through business procedures that send unauthorized funds. | Which payment, lending, account, refund, or approval workflows are being manipulated? Who can pause, review, or reverse a high-impact action? |
| Monetization | Using a payment method or transfer through which the actor receives stolen funds. | What recipient, instrument, mule account, exchange, or cash-out behavior links cases? How quickly can fraud, payments, and financial-crime teams contain proceeds? |
The framework applies this lifecycle to scenarios including application fraud, account takeover, money laundering, and sanctions avoidance. It also argues that sector information sharing matters when threat actors attack several institutions in close succession. A single firm may see only one indicator; shared intelligence can reveal the campaign.
Build a cross-functional fraud operating model
Give the group a common mission and authority
Set a written objective such as reducing unauthorized loss while keeping legitimate customers able to complete critical transactions. Define which decisions the group can make immediately, which require a business owner, and which must be escalated to executive risk committees. Include customer harm, regulatory exposure, operational disruption, and recovery cost in the definition of loss.

FS-ISAC recommends bringing together cybersecurity, threat intelligence, financial-crime or anti-money-laundering specialists, data analytics, and fraud representatives. Add identity, payments, product, legal, privacy, customer operations, and incident response when the scenario requires them. Name an accountable incident or case owner so a case does not stall between queues.

Start with indicators you already have
- Collect a recent confirmed fraud case, suspicious transaction, compromised account, or threat-intelligence indicator.
- Place the observation in one of the five lifecycle phases and record the confidence, timestamp, source, and affected asset or customer journey.
- Ask each adjacent team what earlier or later signals it can see, including failed controls, customer reports, payment events, device data, and investigative findings.
- Map existing preventive, detective, and recovery controls to those signals. Mark controls that operate only after money has moved.
- Agree on a small set of indicators to share internally and, where lawful and useful, with trusted sector partners. Record handling restrictions and retention limits.
- Run a retrospective after containment. Update playbooks, detection logic, training, and ownership rather than treating the case as closed when funds are recovered.
This operating pattern turns a score or alert into a coordinated decision. It also creates a feedback loop: investigators explain why an alert was useful or misleading, while security and engineering teams learn which controls reduced exposure earlier in the lifecycle.
Govern risk scores as decision inputs, not verdicts
There is no universal fraud-score formula or threshold. A score is useful only in relation to a defined decision, a loss scenario, and the evidence behind it. NIST’s digital identity guidance says relying parties should periodically review provider fraud checks, thresholds, investigations, and control effectiveness. It cautions that effectiveness varies with data sources, technologies, and the applicant population.
Document the decision the score supports
For each score, state whether it informs identity proofing, account opening, login step-up, payment release, employee review, case prioritization, or another action. Specify the permitted actions at each band: allow, allow with additional verification, hold for human review, or deny. A score should not silently become a blanket customer rejection.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsConnect the score to impact and tolerance
Describe the loss scenario represented by the score and the business impact if it is missed. A payment decision may have direct financial loss; an account-opening decision may expose the institution to synthetic identity, money laundering, or later account takeover; an aggressive challenge policy may create abandonment and accessibility harm. Set thresholds in line with leadership-approved risk appetite and tolerance, not merely a vendor default or a historical alert rate.

Evaluate data quality and drift
- Inventory every input, its provenance, collection purpose, retention period, and known error modes.
- Track changes in fraud patterns, customer mix, products, geographies, channels, and attack techniques.
- Compare performance for relevant applicant or customer populations; NIST notes that results depend on the population being assessed.
- Separate confirmed fraud, suspected fraud, customer disputes, operational errors, and investigator overrides in outcome data.
- Reassess thresholds after material product, data, model, or threat changes, and on a recurring schedule.
Make overrides and investigations auditable
Record who changed a decision, why, what evidence was considered, and whether the override was temporary or permanent. Investigators need a way to attach supporting indicators and outcomes. Model or rules owners should be able to reproduce the inputs and logic used at the time of the decision, subject to privacy and security controls.
Measure both loss and customer cost
Review confirmed loss, prevented loss, time to detection, time to containment, recovery, false positives, false negatives, manual-review volume, customer abandonment, complaint rates, and successful redress. No single metric proves effectiveness. A lower fraud-loss figure accompanied by a large increase in legitimate declines may represent an unacceptable trade-off.
Turn loss prevention into enterprise risk management
NIST IR 8286D, Update 1 (2025), summarizes the executive task this way: “The management of enterprise risk requires a comprehensive understanding of mission-essential functions (i.e., what must go right) and the potential risk scenarios that jeopardize those functions (i.e., what might go wrong).”
Apply that logic to fraud by linking each material scenario to the function it could disrupt, the assets and services supporting that function, an impact value, and the controls that reduce likelihood or consequence. For example, unauthorized changes to a payout account can threaten a disbursement function; a coordinated account-takeover campaign can threaten customer access and payment operations; a compromised administrator can threaten multiple services at once.

Use risk appetite and tolerance as operating boundaries
Leadership should state the amount and type of fraud, customer friction, interruption, and regulatory exposure the organization is willing to accept. System and process owners then translate those directives into control requirements, escalation triggers, and recovery objectives. A threshold change that improves detection but exceeds the approved customer-friction tolerance requires an explicit risk decision, not an informal tuning exercise.
Feed technology risk into the enterprise profile
NIST SP 800-221 recommends integrating information and communications technology risk into enterprise risk management through risk registers and the enterprise risk profile. Include fraud-related dependencies such as identity providers, payment processors, data feeds, case platforms, and critical service providers. Record residual risk, accepted exceptions, owners, due dates, and links to mission impact so the board sees more than a technical alert count.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Privacy, transparency, and redress are control requirements
Fraud checks can involve identity data, behavioral signals, device information, transaction history, and information about other people. NIST’s identity guidance calls for a privacy-risk assessment of fraud checks and mitigation technologies. The assessment should examine data minimization, purpose limitation, access, retention, sharing, security, and the consequences of incorrect decisions.

Credential service providers are expected to document their fraud-check practices and provide redress procedures for applicants who fail checks. In practice, define a channel for a person to challenge an outcome, a way to submit additional evidence, a human review path for consequential cases, and a target time for resolution. Explain enough about the reason for the action to make correction possible without disclosing detection logic that would enable evasion. Track redress outcomes as a quality signal for the score and the process.

Regulatory obligations depend on scope and jurisdiction
U.S. organizations should not assume that one fraud framework or rule covers every company. The Federal Trade Commission’s Safeguards Rule guidance applies to covered financial institutions. It describes a written information-security program sized to the institution’s business, complexity, activities, and information sensitivity, with risk assessment, safeguards, testing, staff training, service-provider oversight, change management, incident response, and written reporting.
For covered entities, the FTC guidance states that a notification event involving the unencrypted information of at least 500 consumers must be reported as soon as possible and no later than 30 days after discovery. It also states that the Qualified Individual must provide a written report to the board or governing body at least annually. Confirm current applicability, definitions, exceptions, and overlapping federal or state requirements with qualified counsel before relying on these details.
NIST SP 800-39 frames information-security risk as organization-wide, involving mission, operations, assets, individuals, and other organizations. NIST SP 800-30 Rev. 1 structures risk assessment through preparation, conduct, and maintenance. These publications remain useful foundations, but verify their status in the current NIST catalog before describing them as the latest editions.

FBI cyber-resiliency guidance recommends maintaining an asset inventory, managing vulnerabilities according to risk, setting remediation timelines tied to that risk, and running quarterly cross-functional tabletop exercises. It notes that email remains a favored initial-access vector for intrusions and fraud. Treat this as general resilience guidance rather than a fraud-scoring standard.
Include fraud scenarios in exercises alongside ransomware and data-exfiltration scenarios. Test who can freeze or recall funds, who contacts customers, how evidence is preserved, how service providers are engaged, and how executives decide whether to accept operational disruption to stop a campaign. Exercise dependencies such as identity, email, payment, and customer-support systems, not only the security operations center.
A practical implementation roadmap
First 30 days: establish visibility and ownership
- Name an executive sponsor and accountable fraud-operations owner.
- Inventory major fraud journeys, critical assets, data sources, scores, rules, queues, and service providers.
- Select one recent loss or near miss and map it across the five FS-ISAC phases.
- Document current thresholds, override rights, investigation steps, and customer redress.
Days 31–90: connect controls and measures
- Form the cross-functional working group and agree on an indicator-sharing format.
- Map scenarios to mission-essential functions, impact values, risk appetite, and tolerance.
- Define score-performance reviews, population checks, privacy assessments, and change approvals.
- Run a tabletop covering detection, account or payment containment, communications, recovery, and post-incident learning.
After 90 days: institutionalize improvement
- Put fraud risk scenarios and residual risk in the enterprise risk register.
- Automate evidence capture for model inputs, rule changes, analyst decisions, and redress.
- Review metrics at a cadence appropriate to the threat and materiality; escalate breaches of tolerance immediately.
- Repeat lifecycle mapping when products, channels, providers, or attack patterns change.
- Share validated indicators and lessons through approved industry or public-sector channels where lawful.
Common failure modes and corrections
| Failure mode | Why it fails | Correction |
|---|---|---|
| A single team owns all fraud signals | Relevant evidence remains siloed and attacks are seen only after loss. | Use a cross-functional operating group and the shared five-phase lifecycle. |
| A vendor score is treated as an answer | The threshold may not match the institution’s population, impact, or tolerance. | Document the decision, review efficacy periodically, and retain accountable human governance. |
| Optimization targets only prevented loss | False positives, abandonment, privacy harm, and inaccessible recourse can rise unnoticed. | Measure customer and operational outcomes with loss and detection metrics. |
| Controls begin at payment execution | Earlier recon, access, and positioning signals are missed, leaving fewer recovery options. | Map controls across all lifecycle phases and add pre-transaction interventions. |
| Redress is an afterthought | Incorrect decisions persist and the organization cannot learn from disputed outcomes. | Provide documented challenge and human-review procedures and feed results into tuning. |
| Regulatory language is applied broadly | Rules often depend on covered entity, jurisdiction, data, and event definitions. | Verify scope and current obligations with legal and compliance specialists. |
What good looks like for a CISO
A mature program can show, for each material fraud scenario, the mission function at risk, the lifecycle phase, the indicators and data owners, the preventive and detective controls, the score or rule used, the threshold rationale, the human decision rights, the privacy assessment, the customer-redress path, the residual risk, and the executive who accepted or escalated it. It can explain not only how many alerts were generated, but which losses were avoided, which customers were inconvenienced, how quickly cases were contained, and what changed after investigation.
