Night sweating is often simple overheating from a warm bedroom or heavy bedding. True night sweats usually means repeated, substantial sweating that soaks sleepwear or sheets even when the room is cool. Hormone changes, medicines, anxiety, alcohol or other substances, low blood sugar, infections and several medical conditions can all play a role. The symptom alone cannot identify the cause.
Arrange a medical appointment when episodes are regular, unexplained, disturb sleep or concern you. Seek prompt help for heavy sweating accompanied by lightheadedness, chest pain or nausea, or for sweating with symptoms such as high fever, unexplained weight loss, persistent cough or diarrhoea.

What counts as a night sweat?
Temperature and bedding matter. Everyone may perspire after sleeping under a thick duvet, wearing insulating sleepwear or keeping the heating high. That is ordinary overheating and should improve when the environment is cooled.

Record whether the room was cool, how much bedding you used and whether the sweat actually soaked clothes or sheets. This distinction helps a clinician separate environmental heat from a symptom that warrants investigation.
Common reasons you may wake up sweating
Bedroom heat and bedding
A warm room, several blankets, a mattress that retains heat or non-breathable sleepwear can trigger sweating without an illness. If changing the room temperature and bedding stops the problem, overheating is a plausible explanation. Persistent drenching despite a cool environment deserves a different assessment.
Menopause and other hormone changes
Perimenopause and menopause commonly cause hot flushes and night sweating as oestrogen levels fluctuate. Pregnancy, the postpartum period and some menstrual-cycle changes can also alter temperature regulation. Sweats that begin months or years after menopausal symptoms had ended should be discussed with a clinician rather than assumed to be a late recurrence.
Medicines
Some antidepressants, steroids, pain medicines and other prescription or over-the-counter drugs can increase sweating. A dose change or a newly started medicine may provide an important clue, but timing alone does not prove causation. Ask the prescriber or pharmacist to review every medicine, supplement and dose; do not stop a prescribed treatment abruptly without advice.

Alcohol, drugs, anxiety and panic
Alcohol and other substances can affect sleep and the nervous system, and withdrawal may produce sweating. Anxiety or nocturnal panic can activate the body’s stress response and leave clothing damp. These possibilities can coexist with a physical illness, so mention substance use and emotional symptoms openly during an appointment.
Low blood sugar and endocrine conditions
Low blood glucose can cause sweating, shakiness, hunger, palpitations or confusion, particularly in people using insulin or other glucose-lowering medicines. An overactive thyroid may produce heat intolerance, tremor, fast heartbeat, weight change and sweating. Testing is needed to distinguish these conditions from ordinary variation.
Infections
Viral or bacterial infections may cause sweating as body temperature rises and falls. Fever, chills, cough, diarrhoea, sore throat, urinary symptoms or a localized painful area can point toward an infection. The relevant possibilities depend on recent illness, travel, exposures, immune status and other history; night sweats alone do not identify a particular infection.
Sleep apnea and disrupted sleep
Obstructive sleep apnea can repeatedly interrupt breathing and trigger stress hormones. Loud snoring, witnessed pauses in breathing, gasping, morning headaches, dry mouth and marked daytime sleepiness make this worth raising with a clinician. Sweating is not enough to diagnose apnea, and many people who sweat at night do not have it.

Hyperhidrosis
Hyperhidrosis is excessive sweating that may occur without an identifiable underlying disease. It can affect the whole body or areas such as the underarms, hands, feet or scalp and may be present during the day as well as at night. A clinician can determine whether this pattern fits primary hyperhidrosis or whether another condition or medicine is contributing.
Cancers and cancer treatment
Some cancers and cancer treatments are associated with night sweats, but this is only one item on a broad differential diagnosis. Sweating by itself does not establish cancer and should not be treated as the presumptive explanation. Persistent drenching sweats accompanied by weight loss, fever, swollen lymph nodes, pain or other new symptoms need proper evaluation rather than online self-diagnosis.
Why the pattern and accompanying symptoms matter
Before booking care, note the details that change the level of concern:
- Frequency: Is this occasional, weekly or happening most nights?
- Severity: Are sleepwear and sheets soaked, or is the skin merely warm or slightly damp?
- Environment: Was the bedroom cool, and did lighter bedding make a difference?
- Timing: Did episodes follow a new medicine, dose change, alcohol use, illness, menstrual change or menopause-related symptom?
- Associated symptoms: Record fever, chills, cough, diarrhoea, pain, breathlessness, palpitations, tremor, swollen glands, daytime sleepiness or unintentional weight loss.
- Impact: Note awakenings, repeated sheet changes, missed work or inability to return to sleep.
A short diary for one to two weeks can give your clinician a clearer timeline. Include temperature readings if you have a fever, but do not delay care while trying to collect a perfect record.

When should you see a doctor?
Arrange a routine appointment
Contact a primary-care clinician if night sweats are regular, have no obvious environmental explanation, wake you, disrupt sleep or worry you. NHS advice also supports assessment when sweating occurs with a very high temperature or feeling shivery, cough, diarrhoea or unexplained weight loss. Mayo Clinic guidance similarly recommends review for recurrent episodes, interrupted sleep, fever, weight loss, localized pain, cough, diarrhoea, other concerning symptoms, or sweating that starts long after menopause symptoms had stopped.
Use your local primary-care or out-of-hours pathway if you cannot get a timely appointment. Tell the service how often the episodes occur and list any red-flag symptoms so the urgency can be triaged accurately.
Seek immediate medical attention for a concerning combination
Mayo Clinic’s general guidance for excessive sweating—not a night-sweat-specific emergency rule—advises immediate medical attention when heavy sweating is accompanied by lightheadedness, chest pain or nausea. These symptoms can signal an acute problem, especially if they begin suddenly or occur with shortness of breath, fainting, confusion or a racing heartbeat. Call your local emergency number or use the emergency service available where you live.

What a medical appointment may involve
Questions and examination
Expect questions about when sweating began, how often it occurs, whether it soaks clothes or bedding, room conditions, fever and other symptoms. The clinician may ask about medication and supplement changes, alcohol or other substances, menstrual or menopause status, pregnancy or postpartum timing, sleep symptoms, recent infections, travel, immune conditions and previous medical problems.
An examination may include vital signs, skin, thyroid, heart and lungs, and checks for enlarged lymph nodes or an enlarged spleen when clinically appropriate. The aim is to match testing to your history rather than order every possible test.
Possible tests
A Maidstone and Tunbridge Wells NHS referral guideline illustrates tests a clinician might consider: full blood count, blood film, inflammatory markers such as ESR or CRP, thyroid-function tests, glucose and selected viral testing. It also describes history and examination, including lymph-node and spleen assessment. This is a local referral pathway, not a universal panel or a recommendation that every person with night sweats needs all of these tests.

What you can do while arranging care
Reduce heat without masking the pattern
- Use lightweight, breathable sleepwear and layered bedding that can be removed easily.
- Ventilate the room or use a fan safely; keep the bedroom comfortably cool rather than uncomfortably cold.
- Consider a cooling pillow, mattress cover or other breathable bedding as a comfort measure. No bedding product treats an infection, hormone disorder, medication effect or other underlying cause.
- Notice personal triggers such as spicy foods, caffeine or alcohol and avoid them in the evening if they reliably worsen sweating.
These steps may improve comfort, but they should not be used to postpone assessment when episodes are recurrent, drenching or accompanied by other symptoms.
Review medicines safely
Make a complete list of prescriptions, non-prescription medicines, supplements and recreational substances, including when each is taken. Bring the list to the appointment. Never discontinue antidepressants, steroids, pain medicines or other prescribed drugs on your own; a prescriber can adjust treatment safely or suggest an alternative if a medicine is contributing.
A practical decision path
- Check the environment: Cool the room, reduce bedding and see whether the sweating stops.
- Document recurrence: If episodes continue, record frequency, soaking severity, timing and associated symptoms.
- Book primary care: Arrange review for regular, unexplained, sleep-disrupting or worrying episodes, and for the NHS-listed combinations of sweats with high temperature or shivering, cough, diarrhoea or unexplained weight loss.
- Escalate acute symptoms: Seek immediate help for heavy sweating with lightheadedness, chest pain or nausea, following local emergency guidance.
Bottom line
Most sweating in a hot bed is environmental, but repeated drenching in a cool room has many possible causes, from menopause and medicines to infection, metabolic conditions, sleep apnea or hyperhidrosis. Because no single symptom identifies the cause, let the pattern and accompanying symptoms guide the urgency: document what happens, arrange clinical review when it persists or disrupts sleep, and seek emergency help for heavy sweating with lightheadedness, chest pain or nausea.
