What Causes Narrow Arteries in Young Adults, and Can It Be Reversed?

Narrow arteries in a young adult can result from atherosclerotic plaque, but plaque is not the only explanation. Fibromuscular dysplasia (FMD) and blood-vessel inflammation called vasculitis can also narrow arteries, and the likely cause depends on which vessel is affected and the person’s symptoms and medical history. Whether narrowing can be reversed depends on its cause and severity: early atherosclerosis may sometimes improve with treatment, while FMD has no cure but can be managed.

What “narrow arteries” means

“Narrow arteries” describes a finding, not a diagnosis. It means that the channel through which blood flows has become smaller, but it does not explain why. The mechanism matters: plaque inside an artery, abnormal thickening of its wall, and inflammation of blood vessels are different problems and are not treated the same way.

The location also matters. Narrowing in an artery supplying a kidney, for example, may be considered when evaluating high blood pressure, while an artery supplying the brain raises different concerns. A clinician has to establish whether narrowing is present and identify its cause; symptoms or a blood-pressure reading alone cannot show the anatomy.

It is also important not to assume that a young person with a narrowed artery has the same condition as an older adult with atherosclerosis. Atherosclerosis can begin early, but clinically important narrowing is not implied by that fact. FMD and vasculitis affect the artery wall through different processes, and the appropriate evaluation depends on the individual findings.

What can cause arterial narrowing in young adults?

Atherosclerosis: plaque inside the artery

Atherosclerosis develops when plaque—made up of substances including cholesterol, fat, blood cells, and calcium—builds up inside an artery. Plaque can harden and restrict the flow of oxygen-rich blood. The process can begin in childhood and worsen over time, although the risk of atherosclerosis rises with age. That does not mean clinically important narrowing is typical in young adults.

Risk can reflect several factors accumulating together. The National Heart, Lung, and Blood Institute lists high blood pressure, high cholesterol, diabetes, smoking, and family history among relevant risks. An inherited condition such as familial hypercholesterolemia can raise cholesterol and increase risk. Inflammatory disease may also be relevant. These factors are not proof that a particular person has plaque, but they can help a clinician assess risk.

Family history can be especially useful context when a young adult has high cholesterol or other cardiovascular risks. It does not establish a diagnosis on its own. Similarly, having one or more risk factors does not confirm that a particular artery is narrowed; it helps guide a clinician’s assessment alongside symptoms, examination, blood tests, and any indicated imaging.

For an overview of the condition and how plaque can affect arteries, see MedlinePlus on atherosclerosis and the NHLBI’s atherosclerosis causes and risk factors.

Fibromuscular dysplasia: abnormal growth in the artery wall

Fibromuscular dysplasia is not cholesterol plaque. In FMD, muscle and fibrous tissue in an artery wall thicken, narrowing the vessel. It most often affects arteries supplying the kidneys or brain, though other arteries may be involved. FMD can occur at any age and is more often diagnosed in women; its cause is not known.

This distinction helps explain why risk factors associated with plaque do not tell the whole story. FMD is a disorder of artery-wall tissue, not a buildup of cholesterol. A person’s age, sex, affected artery, symptoms, and other health conditions may all inform the clinician’s evaluation, but no single clue is enough to diagnose it.

Because FMD affects the wall itself, it is a different condition from atherosclerosis even though both can reduce blood flow. Treatment is selected according to the affected artery, symptoms, and other health conditions. FMD has no cure, but symptoms and complications can be managed. Mayo Clinic’s FMD overview describes the condition and possible evaluation and management.

Vasculitis: inflammation that changes the vessel wall

Vasculitis means inflammation of blood vessels. Inflammation can thicken a vessel wall and narrow or close the affected vessel. Vasculitis includes different conditions, with different causes, risks, and age patterns; the cause is not fully known. It should not be assumed simply because an artery is narrow. The NHLBI explains more in its overview of vasculitis causes and risk factors.

Thus, the main comparison is the process involved: atherosclerosis is plaque buildup inside the artery; FMD is abnormal thickening of artery-wall tissue; and vasculitis is inflammation that can thicken the vessel wall. They can all narrow a vessel, but the finding alone does not identify which process is responsible. Vasculitis is not one uniform disease, so care depends on the particular type identified.

Can high blood pressure at a young age point to renal artery narrowing?

It can be one clue, but it is not a diagnosis. Narrowing of a renal artery can contribute to renovascular hypertension. High blood pressure at a young age, a sudden worsening, or blood pressure that is difficult to control may prompt a clinician to consider this and other possible causes. FMD is one possible cause of renal artery stenosis and is typically seen in women under 50.

Those patterns are reasons to seek medical evaluation, not a checklist for self-diagnosis. High blood pressure has multiple possible explanations, and only a clinician can decide whether investigation for renal artery disease is appropriate. A blood-pressure reading shows pressure, not the condition or appearance of the renal arteries. MedlinePlus provides an overview of renovascular hypertension.

If hypertension has already been diagnosed, follow the care team’s instructions about monitoring and follow-up. A sudden change or difficulty controlling readings is worth reporting, but it does not prove that a renal artery is narrowed. The clinician considers the pattern in context and decides whether further assessment is warranted.

Can narrowed arteries be reversed?

Sometimes, depending on what caused the narrowing and how advanced it is. “Reversal” does not mean the same thing for every cause: atherosclerosis may sometimes improve, FMD is managed rather than cured, and treatment for vasculitis is specific to its type.

Atherosclerosis may improve, but reversal is not guaranteed

The NHLBI says atherosclerosis can sometimes be reversed over time, especially in earlier stages when major risk factors are controlled and medicines are used to achieve low LDL cholesterol. Severe atherosclerosis may not be fully reversible. The practical goal is a clinician-guided plan to reduce risk and treat the person’s condition—not a promise that an artery will return to normal.

What that plan involves depends on individual health and test results. The research supports risk-factor control and medicines as part of the approach; it does not establish a universal reversal diet, supplement, or self-directed treatment. Do not treat a diet, supplement, or medication as a universal cure, and do not start or stop a prescribed medicine based on a general article. Read the NHLBI’s atherosclerosis treatment guidance for its discussion of treatment.

FMD can be managed, not cured

There is no cure for FMD. Depending on the case, management may involve blood-pressure medicines, follow-up, or a procedure such as angioplasty to improve blood flow in a narrowed artery. Surgery is uncommon. A procedure is not appropriate for everyone with FMD; decisions depend on the artery involved, symptoms, and clinical circumstances.

In other words, finding FMD does not mean that everyone needs angioplasty or surgery. The clinician weighs where the narrowing is, what symptoms or complications are present, and the person’s overall health before recommending management. Follow-up may also be part of care. These decisions cannot be made from the diagnosis label alone.

Mayo Clinic advises discussing aspirin with a health care team rather than starting it on your own. Do not assume that aspirin, another medicine, or a supplement is suitable just because FMD or atherosclerosis has been mentioned.

Vasculitis treatment depends on its type

For vasculitis, care is specific to the type and aims to control inflammation and prevent flares. The appropriate treatment cannot be determined from the fact of narrowing alone; the clinician first needs to identify the condition involved. Different vasculitis types have different age patterns and risks, so a generic plan cannot substitute for diagnosis.

How clinicians investigate a suspected narrowing

Evaluation is tailored to the artery and the person’s symptoms and history. It can include a medical history and examination, blood-pressure assessment, blood tests, and imaging. For suspected FMD, Mayo Clinic lists blood tests and ultrasound among possible tests. A clinician decides which evaluation is warranted; this article is not a reason to order a scan or start medication independently.

The purpose of evaluation is not simply to confirm that a vessel looks narrow. The care team needs to consider the location and likely cause, because plaque, FMD, and vasculitis have different mechanisms and management. A blood test or ultrasound may be part of an FMD assessment, but no one test or result listed here should be treated as a do-it-yourself diagnosis.

Questions to ask include: Which artery or arteries are suspected? What evidence shows narrowing? What cause is being considered? What follow-up or treatment, if any, is appropriate for this artery and these symptoms? These questions help clarify whether the finding is confirmed and what it means for the individual rather than assuming a single explanation.

If you have diagnosed high blood pressure, a clinician may ask you to record readings at home. A home blood-pressure monitor measures pressure; it does not show the arteries or identify a stenosis. Use readings as requested and share them with your care team rather than treating them as an anatomical test. A monitor is an optional way to record clinician-requested readings, not a test for narrowed arteries or a treatment.

When to seek urgent help

Mayo Clinic advises immediate medical help for sudden symptoms of stroke in people with FMD. Seek emergency care for sudden vision changes, difficulty speaking, or new weakness in an arm or leg. Do not wait for a routine appointment to assess these symptoms.

Frequently Asked Questions

What causes narrow arteries in young adults?

Possible causes include atherosclerotic plaque, fibromuscular dysplasia (FMD), and vasculitis. Atherosclerosis is plaque buildup inside an artery; FMD is abnormal thickening of artery-wall tissue; vasculitis is inflammation that can thicken or close vessels. The likely cause depends on the affected artery and the person’s history, symptoms, examination, and any indicated testing.

Can narrowed arteries be reversed?

Sometimes, but it depends on the cause and severity. Atherosclerosis may sometimes improve over time, particularly earlier in the disease when risks are controlled and medicines are used to achieve low LDL cholesterol; severe disease may not be fully reversible. FMD has no cure but can be managed, and vasculitis treatment is specific to its type.

What is the difference between plaque and fibromuscular dysplasia?

Atherosclerotic plaque builds up inside an artery and includes substances such as cholesterol, fat, blood cells, and calcium. FMD is not plaque: muscle and fibrous tissue in the artery wall thicken. Both can narrow an artery, but they are different conditions and are managed according to the affected vessel and the individual case.

Can high blood pressure at a young age mean renal artery narrowing?

Renal artery narrowing is one possible contributor to renovascular hypertension. High blood pressure at a young age, sudden worsening, or difficult-to-control readings may prompt a clinician to consider it, but none of these clues proves that an artery is narrowed. FMD is one possible cause of renal artery stenosis.

What tests might a clinician use to evaluate suspected FMD or arterial narrowing?

The evaluation is tailored to the suspected artery and the person’s symptoms and history. It may include a medical history and examination, blood-pressure assessment, blood tests, and imaging. Mayo Clinic lists blood tests and ultrasound among possible tests for FMD. A clinician decides what is warranted; a home blood-pressure monitor records pressure but cannot show stenosis.

Should I start aspirin or a supplement if I think I have narrowed arteries?

No. Do not start aspirin or supplements based on suspicion or this article. Mayo Clinic advises people with FMD to discuss aspirin with their health care team, and treatment choices should be individualized. Ask a clinician before starting, stopping, or changing medication.

The Bottom Line

Narrow arteries in young adults can reflect plaque, FMD, or vasculitis, and the cause cannot be inferred from age or blood pressure alone. Atherosclerosis may sometimes improve when recognized earlier and treated, while FMD can be managed but has no cure. A clinician can determine whether narrowing is present, identify the likely cause, and recommend evaluation or care based on the affected artery and the person’s circumstances.
