Confirmed loss of a protein of interest proves target engagement, not safety. A degrader can be toxic because it removes that protein from healthy tissue, depletes unintended proteins, or produces prolonged, tissue-specific pharmacology at the exposure used. Distinguishing those mechanisms requires evidence beyond a target-depletion assay.
Why successful target degradation can still be harmful
Targeted protein degraders are designed to eliminate a protein rather than merely inhibit it. In the usual ubiquitin–proteasome pathway, a molecule brings a target into productive proximity with an E3 ubiquitin ligase. The ligase attaches ubiquitin chains, and the marked protein is then destroyed by the proteasome. Bifunctional PROTACs contain a target-binding ligand and a ligase-binding ligand; molecular glues promote or stabilize an interaction with a smaller molecule.

If an assay shows that the intended protein has fallen, it answers one question: the compound engaged the target and initiated degradation under those conditions. It does not answer whether the same protein was needed in healthy organs, whether other proteins were recruited to the ligase, or whether drug exposure and degradation persisted long enough to cause injury. Those questions explain how toxicity and apparent pharmacological success can occur together.
| Potential cause | What is being depleted or affected | Why target-depletion data alone are insufficient |
|---|---|---|
| On-target, off-tissue toxicity | The intended target in healthy tissue where it performs a normal function | A tumor-cell assay may not represent target abundance, function or exposure in organs at risk |
| Unintended substrate degradation | Other proteins recognized after the ligase is recruited, including neosubstrates | Measuring only the intended protein cannot reveal the broader degradation profile |
| Context-dependent selectivity | The set and rate of proteins degraded in a particular tissue, time window or cellular state | Ternary-complex geometry, ligase and substrate abundance, competition and kinetics vary between systems |
| Exposure and persistence | Pharmacological effects that continue after a transient binding event | Percent target loss at one time point does not describe tissue exposure, recovery or duration |
On-target toxicity: the right protein removed in the wrong place
Normal function can make the therapeutic target a safety liability
A target may be disease-driving in one tissue and essential elsewhere. Degrading it in a healthy cell is still “on-target” at the molecular level, but the result is an adverse effect because the target’s physiological role has been removed. For example, a protein selected for depletion in malignant cells may also support blood-cell production, cardiac function, immune regulation or tissue maintenance. The relevant question is not simply whether the target is present, but whether a given level and duration of loss disrupts a normal function in the exposed organ.

Distribution determines which tissues experience target loss
The target must be present where the degrader reaches effective concentrations. Tissue exposure can differ substantially from plasma exposure because of permeability, transport, metabolism and intracellular accumulation. A toxicity signal may therefore appear in an organ that was not prominent in the original disease model, while a tissue with high target expression may be spared if the compound does not reach it.
Ligase distribution matters as well. A 2024 review of targeted protein degradation describes a BCL-XL degrader in which lower VHL expression in platelets was associated with reduced platelet toxicity. The example illustrates a general principle: even when the target is abundant, degradation can be limited in a tissue that has little of the recruited ligase. Conversely, a ligase that is highly expressed in an organ can increase local degradation and risk.
Target engagement is not the same as a safe therapeutic window
Safety depends on the relationship between exposure, target loss and recovery. Partial depletion may be adequate for disease control but excessive in a normal tissue. A tissue may tolerate brief loss yet develop injury after sustained depletion. Conversely, a rapidly replenished target may require continuous exposure, creating a different risk from a stable protein whose removal has effects that outlast the drug.

Unintended degradation: when the recruited ligase acts on more than the target
Degradation is selective, but not automatically exclusive
PROTAC selectivity is governed by productive ternary-complex formation and ubiquitination, not by target-binding affinity alone. The orientation and surface contacts of the target–degrader–ligase complex determine whether ubiquitin can be transferred efficiently. A compound can bind its intended target tightly yet form a productive complex with other proteins or fail to degrade the intended protein equally in every cellular context.
Once a ligase is recruited, the degrader can change which proteins that ligase recognizes. Proteins that were previously poor substrates may become accessible or efficiently ubiquitinated. These induced substrates are often called neosubstrates. Their loss can produce toxicity even when the intended target is depleted exactly as designed.
Why cereblon-recruiting compounds receive particular scrutiny
Cereblon (CRBN) is a component of an E3 ligase recruited by immunomodulatory imide drugs and by some molecular glues and PROTACs. Binding to CRBN can create a new recognition surface, allowing degradation of proteins that are not normally removed at an appreciable rate. Reviews of CRBN-recruiting degraders identify neosubstrate degradation as a central safety concern and discuss possible organ-system liabilities, including developmental toxicity.

The thalidomide experience is a mechanistic example. Primary studies identified the transcription factor SALL4 as a thalidomide-dependent CRBN substrate and connected its loss with teratogenic effects. Other CRBN neosubstrates, including IKZF1, have been reported for compounds in this class. These findings demonstrate a route from ligase recruitment to unintended protein loss and a tissue phenotype; they do not show that every CRBN degrader, every adverse event or every patient risk is caused by SALL4 or another single substrate.

An attribution needs the relevant compound, substrate, tissue and phenotype to line up. A protein may be degraded in cultured cells but not reach a meaningful level of depletion in the affected human organ. Alternatively, a compound may induce several neosubstrates, making one candidate an incomplete explanation. The safest interpretation is therefore compound-specific: identify what is depleted, where, at what exposure and with what biological consequence.
Why selectivity changes with tissue, time and dose
Expression and competition alter the degradation profile
Cellular abundance of the target, recruited ligase and competing substrates all influence which interactions are productive. If a ligase is scarce, target degradation may be weak even at high drug concentration. If a competing substrate is abundant, it can occupy the ligase and change the amount of degrader available for another protein. Differences in protein complexes, post-translational modifications and subcellular location can also change ternary-complex geometry.

Kinetics matter as much as the maximum percentage depleted
Two compounds can each produce 90% target loss in an endpoint assay yet have different safety profiles. One may act briefly and allow rapid protein resynthesis; another may maintain degradation for days because the compound or the depleted state persists. The rate of target removal, the time to nadir and the rate of recovery should be measured in the tissue relevant to the toxicity signal.
The event-driven “hook effect” and exposure
Degraders are event-driven drugs: one molecule can promote repeated rounds of ubiquitination and degradation. More drug is therefore not always better. At high concentrations, separate binary drug–target and drug–ligase complexes can predominate and reduce productive ternary-complex formation, a phenomenon known as the hook effect. European Medicines Agency guidance on targeted protein degradation highlights pharmacokinetic/pharmacodynamic (PK/PD) characterization, including management of this non-linear behavior. Exposure, free concentration, tissue distribution and the duration of target or substrate loss must be interpreted together.
How to investigate a toxicity signal
No universal assay threshold assigns every adverse event to one mechanism. A defensible investigation combines molecular, tissue and exposure evidence and asks whether the findings track with dose and time.
Best Value
- Confirm depletion in the affected tissue. Measure the intended target in the organ showing injury, using a validated protein assay and an appropriate time course. A blood or tumor result cannot substitute for tissue evidence when distribution differs.
- Test the target’s normal function there. Determine whether the observed degree and duration of target loss are sufficient to disrupt a known physiological pathway in that tissue. Correlate molecular depletion with functional biomarkers and histopathology.
- Profile the proteome without assuming the answer. Use unbiased quantitative proteomics, supported by targeted assays for plausible ligase substrates. Look for neosubstrate depletion and pathway changes at exposures that produce the toxicity.
- Characterize PK/PD together. Relate free and total drug concentrations to target loss, candidate off-target loss, recovery and the onset or resolution of injury. Include multiple doses and time points so a transient peak is not confused with persistent pharmacology.
- Test ligase and substrate dependence. Compare matched cells or tissues with and without the recruited ligase, or with altered expression of a candidate substrate, when experimentally feasible. Loss of the toxicity signal after removing the relevant ligase supports a ligase-mediated mechanism.
- Seek rescue or phenocopy evidence. Restoring the depleted protein, preventing its degradation, or reproducing its loss genetically can strengthen causal attribution. Such experiments should be interpreted with exposure and tissue data rather than in isolation.
- Check species and model relevance. A laboratory species or cell line may lack the human ligase, express a neosubstrate at a different level or form a different protein complex. A negative result in that model cannot reliably exclude a human-specific liability.
Evidence that distinguishes the main mechanisms
| Evidence pattern | Interpretation it supports | Remaining question |
|---|---|---|
| Intended target is depleted in the injured organ; functional markers change in proportion to depletion | On-target, off-tissue toxicity is plausible | Whether another degraded protein contributes to the same phenotype |
| One or more proteins fall only when the recruited ligase is present, at toxic exposure | Ligase-dependent unintended degradation is implicated | Which depleted protein is causally responsible |
| Proteomic changes vary between tissues despite similar plasma concentrations | Distribution, expression or local ternary-complex biology is shaping selectivity | Whether exposure can be adjusted without losing efficacy |
| Injury tracks with prolonged drug or target-substrate loss and improves as proteins recover | Duration and persistence are important drivers | Whether the relevant driver is target loss, neosubstrate loss or both |
| Depletion is shown only in a tumor or engineered cell line | Target engagement is established in that model | Safety mechanism in human healthy tissue remains unresolved |
How to compare two degraders or proposed toxicity explanations
Use the same questions for each candidate rather than treating a high percentage of target loss as a complete ranking.
- Target biology: Is the intended protein expressed in the affected healthy tissue, and what essential function does it perform there?
- Ligase biology: Which E3 ligase is recruited? Where is it expressed, and what neosubstrates are known or induced by this specific chemistry?
- Degradation selectivity: Is there evidence for productive ternary-complex formation and ubiquitination across relevant proteins, cell types and tissues?
- Exposure and time: What dose and tissue exposure accompany the signal, how quickly does degradation occur, and how long does depletion persist?
- Causal evidence: Does the explanation rely only on target depletion, or does it include proteomics, genetic perturbation, rescue and a matching tissue phenotype?
These are mechanistic comparison axes, not a validated clinical scoring system. A candidate with less dramatic target depletion can be safer if it avoids a vulnerable tissue or limits duration, while a highly potent degrader may carry greater risk if its ligase is abundant in that tissue and its off-target profile is broad.
What target-depletion results can—and cannot—tell you
A well-controlled depletion assay can establish that a compound reaches the target, forms a productive degradation complex and lowers target abundance under specified conditions. It can support dose selection and pharmacodynamic monitoring. It cannot, by itself, establish selective degradation, rule out neosubstrates, identify the injured tissue’s exposure or prove that target loss caused an adverse event.

There is also no single population-level incidence estimate for the broad category of “toxicity despite successful target removal.” The frequency depends on the compound, target, recruited ligase, dose, tissue distribution, treatment duration and patient population. Landscape counts of degrader programs do not measure this causal category and should not be presented as a toxicity rate.
Practical interpretation
When a degrader is toxic despite confirmed target removal, start with three non-exclusive hypotheses: the intended target is being removed from a healthy tissue; the recruited ligase is degrading additional proteins; or exposure and persistence are producing a tissue effect that a single endpoint assay misses. Resolve them with tissue-matched target and substrate measurements, unbiased proteomics, PK/PD time courses and ligase- or substrate-dependent experiments. Only compound-specific evidence can determine which mechanism explains a particular adverse event.
