Data-center outages usually result from interacting failures, not one dramatic fault. In the Uptime Institute’s 2024 Global Data Center Survey, power was the primary cause of respondents’ most recent impactful incident (54%), followed by cooling (13%), network problems (12%) and IT hardware or software (11%). Those figures describe a survey of 97 responses, not universal industry-wide probabilities. Preventing outages therefore requires an engineered and operated system: resilient power and cooling, controlled IT changes, documented procedures, dependency awareness, tested recovery and continual maintenance.
What the outage data actually shows
Two recent Uptime Institute publications measure different things, so their numbers should not be combined. The 2024 Global Data Center Survey asked about the cause of each respondent’s most recent impactful incident. Its excerpt reports power at 54%, cooling at 13%, network at 12% and IT hardware/software at 11% (n=97). Read the survey excerpt.

Uptime’s 2025 annual analysis, reporting on its 2024 survey work, says IT and networking issues together represented 23% of impactful outages in 2024. It associates their growing importance with more complex environments, change-management weaknesses and misconfiguration. The report also warns that outage frequency and cost are difficult to estimate because public reporting is incomplete and definitions and samples vary. See the 2025 analysis.
Cost can be substantial, but it is another separate survey result. In the 2024 annual survey reported in 2025, 54% of respondents said their latest significant, serious or severe outage cost more than $100,000; 20% said it cost more than $1 million. These are reported respondent estimates, not a universal average or forecast.

1. Power failures: the first protection priority
Utility interruptions are only one part of a power event. Failures can also occur in switchgear, distribution panels, automatic-transfer equipment, batteries, generators, fuel systems, maintenance work or the paths connecting those components to the IT load. A redundant component does not help if a shared bus, transfer sequence or control system fails.
How the power layer is supposed to work
- UPS: An appropriately sized uninterruptible power supply bridges the instant between a utility disturbance and longer-duration generation. NIST says sizing should cover the system components that must remain powered, including environmental and safety controls, not only servers.
- Generators: Generators provide sustained backup when the utility is unavailable. Fuel storage, delivery, starting systems, load-bank testing and safe maintenance are part of the effective backup system.
- Distribution and transfer: Automatic-transfer switches, breakers, power paths and controls must be coordinated and tested. A failure in distribution can take down equipment even when both utility and generator capacity are available.
NIST’s Contingency Planning Guide for Federal Information Systems identifies UPS equipment and generators as preventive or recovery controls, but it does not prescribe a universal runtime, topology or equipment size. Those decisions depend on measured load, growth, environmental systems, safety requirements, local utility conditions and the business impact of downtime. The guidance is official federal material from 2010, not a current universal regulation for every operator. Consult NIST SP 800-34 Rev. 1.
How to verify power resilience
- Inventory every load that must ride through an interruption, including cooling controls, fire systems, monitoring and access systems.
- Test transfer sequences under realistic load and confirm that controls do not create a single point of failure.
- Maintain batteries, generators, fuel systems and breakers according to their manufacturer and site programs.
- Exercise a utility-loss scenario while observing voltage, frequency, cooling availability, alarms and operator actions.
2. Cooling and environmental conditions
Cooling accounted for 13% of the most recent impactful incidents in the cited 2024 survey. A cooling outage can become an IT outage quickly: temperatures rise, equipment throttles or shuts down, and operators may need to remove load before hardware is damaged.

Typical cooling failure points
Chillers, pumps, fans, controls, valves, power feeds and water systems can fail independently or through a shared dependency. A control-system error or maintenance mistake may disable otherwise healthy mechanical equipment. Heat loads can also exceed the design assumption when density, utilization or airflow changes.
Controls that reduce exposure
- Provide adequate excess air-conditioning capacity so specified component failures can be tolerated, as NIST recommends.
- Monitor temperature and other environmental conditions at locations that reveal developing hot spots rather than relying only on room-level averages.
- Maintain mechanical, electrical and control equipment, and validate that alarms reach an accountable operator.
- Test cooling performance during the failure and load scenarios that matter to the facility, including maintenance states.
The cited sources establish the need for capacity, monitoring and maintenance; they do not establish a universal temperature threshold, sensor brand or maintenance interval. Set those values through the equipment design, manufacturer requirements and the organization’s risk analysis.
3. IT systems, networks and configuration changes
The 2024 cause excerpt assigns 11% of impactful incidents to IT systems and 12% to networks. Uptime’s 2025 analysis places the combined category at 23% in 2024 and points to complexity, change management and misconfiguration as likely contributors. A physically healthy facility can therefore be unavailable because software, routing, storage, identity, virtualization or a configuration change breaks a service dependency.

4. Third-party providers expand the failure domain
Cloud, colocation, telecommunications, managed-network and software providers can remove some tasks from an enterprise, but outsourcing does not remove outage risk. A provider can experience a facility, network, cyber or software failure that affects many customers simultaneously. Uptime notes that provider failures still occur and can have serious consequences.
Make provider boundaries explicit
- Record every critical provider, service, region, circuit, credential and data dependency.
- Identify which party owns detection, escalation, failover, data restoration and customer communication.
- Check whether contracts and service-level commitments match the recovery time and recovery point the business requires.
- Practice a provider-loss scenario without assuming that a nominally independent service is truly independent.
5. Human factors are usually process failures
Operators often describe an outage as “human error,” but Uptime reports that most human-error-related incidents involve staff not following procedures or procedures that are inadequate. Its 2025 announcement says nearly 40% of organizations experienced a major human-error-caused outage in the preceding three years, and that 85% of those incidents arose from procedure noncompliance or flawed procedures. Attribute those figures to Uptime’s release; they are not a universal workforce statistic.
Uptime suggests rapid industry growth and staffing shortages may be connected to the rise in procedure-related causes, while acknowledging that the reason is unclear. Treat that as a possible explanation, not a proven causal finding.
Recommended Free Tools
Build an operation that makes the safe action practical
- Keep procedures current, searchable and specific about prerequisites, hold points, verification and rollback.
- Use formal change control for facility, network, software and security work, with an independent reviewer for high-risk steps.
- Train staff on normal operations and abnormal scenarios; ensure shifts have enough qualified coverage.
- Capture near misses and incidents, then correct the procedure or system rather than assigning blame to one person.
- Check that alarms, diagrams, labels and access permissions match the current physical and logical configuration.
6. Fire, water, cyber and external events
Fire and water
NIST identifies fire suppression, fire and smoke detection, and water sensors as preventive controls. They address specific hazards; none guarantees that a facility will avoid every incident. Place detection where it can reveal a developing condition, route alarms to responders, maintain suppression systems and test response procedures. Water monitoring should cover likely leak paths and be tied to a clear isolation and escalation plan.

Cyber incidents
Uptime reports that cyber incidents are rising and can have severe, lasting impacts. Physical redundancy alone cannot restore a compromised identity system, management plane or backup environment. Cyber resilience should include protected administrative access, segmentation appropriate to the architecture, monitored changes, recoverable data and rehearsed restoration. Facility controls and cyber controls must be coordinated without treating one as a substitute for the other.
Weather and grid constraints
Extreme weather and power-grid constraints can affect utility supply, fuel delivery, transport, staff access, telecommunications and cooling-water availability at the same time. Model the combinations that are credible for the site, and verify that emergency access, communications and staffing plans work when normal services are disrupted.
Redundancy is useful only as a complete system
“Redundant” does not mean “outage-proof.” A second UPS can fail to help if both units share a distribution path; a backup generator is irrelevant if its transfer gear or fuel system is unavailable; and a second network circuit may share a conduit or provider. Additional failover mechanisms can reduce exposure while increasing configuration, testing and ownership complexity. Uptime specifically warns that modern distributed-resiliency tools can complicate responsibility and root-cause analysis.

- Establish business impact and priorities. Define which services are critical, acceptable downtime, data-loss tolerance, safety obligations and the order in which systems must be restored. Use those priorities to justify controls and recovery capacity.
- Identify and maintain preventive controls. Select appropriately sized UPS equipment, generators, cooling capacity, fire protection and detection, and water sensing according to the site configuration and cost-effectiveness. Keep an asset and maintenance register.
- Control changes and procedures. Apply review, approval, testing and rollback to facility and IT changes. Inspect whether procedures are usable and followed, especially where complexity is increasing.
- Prepare recovery, not just prevention. Document recovery strategies, dependencies, roles, communications and restoration order. For operational technology specifically, NIST SP 1339 recommends integrating backups into change management, making them regularly, testing them and reviewing them in recovery exercises; its scope is OT, not a universal data-center backup standard. Read NIST SP 1339.
- Test, train and exercise. Run realistic utility-loss, cooling-loss, network, provider, cyber and environmental scenarios. Record observed recovery times, decision points and failures, then update the plan.
- Review incidents and dependencies. After incidents and near misses, assign corrective actions to owners and dates. Reconcile facility diagrams, configuration records, provider responsibilities and recovery documentation.
How to choose a UPS without pretending one size fits all
An appropriately sized data-center UPS is a legitimate prevention category because NIST explicitly identifies UPS equipment as short-term backup for system components, including environmental and safety controls. It is engineered facility equipment, not a consumer power strip. Before specifying a data center UPS uninterruptible power supply, establish the measured and forecast load, required ride-through, voltage and distribution characteristics, cooling and safety loads, bypass and maintenance requirements, battery environment, generator coordination and the consequences of a failed module. No generic product category can determine those values for a particular site.
What operators should measure after each exercise
- Time from event detection to acknowledgement and escalation.
- Whether every intended load remained powered and cooled.
- Transfer, failover and restoration times compared with business requirements.
- Alarms that were missing, misleading, delayed or routed to the wrong team.
- Steps that depended on undocumented knowledge, unavailable credentials or an absent person.
- Provider, network and data dependencies that behaved differently from the documented design.
- Corrective actions with named owners, deadlines and a date for verification.
Limits of the available outage evidence
Uptime’s reports synthesize surveys, member and partner input, and publicly reported incidents. Public reporting is incomplete, sample sizes vary, and definitions and collection methods introduce uncertainty. Treat the percentages as attributed findings for specific questions and populations, not as definitive global failure rates. No reviewed source provides a validated comparison proving that one redundancy architecture prevents outages best, nor a current universal failure-rate figure for all data centers.
Frequently Asked Questions
Is power always the cause of most data-center outages?
Power was the leading cause in Uptime Institute’s cited 2024 survey of 97 respondents, at 54% of most recent impactful incidents. That is a survey result, not a universal failure-rate claim; other populations and definitions may produce different distributions.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Does outsourcing infrastructure eliminate outage risk?
No. Cloud, colocation, network and software providers can fail, and their incidents may affect many customers. Operators still need dependency maps, clear recovery ownership and tested provider-loss procedures.
What is the most important outage-prevention exercise?
Start with a business-impact analysis, then test the failure scenarios that threaten the highest-priority services. Include power, cooling, IT/network changes, provider dependencies and recovery—not just component failover.
