Cloudflare traced the failure to an unexpectedly enlarged Bot Management feature file. A database permissions change altered how that file was generated, and the resulting file exceeded a size limit in software that routes traffic across Cloudflare’s network.
What caused the Cloudflare outage?
Cloudflare’s postmortem identifies an internal configuration and data-generation failure, not excessive demand. A change to database permissions affected the process that generated information for the Bot Management feature. The resulting feature file grew beyond its expected size.
That file was distributed to the software responsible for handling and routing traffic across Cloudflare’s network. The software had a fixed size limit; once the expanded file exceeded it, the software began failing. Because Cloudflare sits between many users and the websites or applications they visit, a failure in that shared routing layer could surface as errors at many unrelated services.
Early reports raised the possibility of an attack, but Cloudflare’s postmortem says the incident was not an external cyberattack. The more specific explanation is an internal permissions change that propagated bad or oversized feature data into network-serving software.

Which services were affected?
The disruption appeared differently depending on which service path a user took. Cloudflare reported effects to its own network and customer-facing systems, while downstream websites could show different errors or remain reachable through unaffected routes.

When did OpenAI’s web access fail?
OpenAI reported website access errors for some users from approximately 3:30 a.m. to 6:40 a.m. Pacific time on November 18, 2025. That window applies to the OpenAI web properties covered by its incident report; it should not be treated as a universal start or recovery time for every Cloudflare customer.

Why could an internal Cloudflare change affect unrelated websites?
Cloudflare provides shared infrastructure: its edge software receives requests, applies security and performance rules, and forwards traffic to customer origins. A broadly deployed feature file can therefore affect many customers at once even when those customers did nothing wrong.

- Traffic-routing software could not accept a file above its built-in size limit.
- Requests passing through affected Cloudflare paths began failing or returning errors.

Was Cloudflare hacked?
Cloudflare’s postmortem does not attribute the November 18 incident to a cyberattack. It describes an internal failure triggered by the permissions change and the oversized feature file. That conclusion replaces the initial public speculation about an attack with a documented systems explanation.
Did the outage take down all of OpenAI?
No. OpenAI’s account distinguishes web access from other product paths. Some users could not reach ChatGPT Web, platform.openai.com, Sora.com or openai.com, receiving HTTP 403 or 504 errors. OpenAI said the ChatGPT and Sora iOS and Android apps were unaffected, and that API traffic and backend services remained healthy.

“OpenAI was down” is therefore too broad: the documented impact was concentrated on specified websites and affected users, not every OpenAI product or access method.

Cloudflare later introduced a resilience program called “Code Orange: Fail Small.” The plan discusses the November 18 incident together with another outage on December 5, 2025 and focuses on limiting the reach of broadly deployed configuration changes.

What the plan is intended to address
- Reduce the blast radius when configuration or feature data is distributed across the network.
- Make changes fail in smaller, more contained groups instead of affecting a large share of traffic at once.
- Improve safeguards around systems that generate and deploy operational configuration.
What the announcement does not prove
“Code Orange: Fail Small” is a stated resilience direction, not evidence that every measure was already complete or that another outage has been ruled out. The plan describes intended engineering work; its publication alone does not demonstrate a guaranteed outcome.
What this incident shows about shared internet infrastructure
- A provider-side outage can originate in an ordinary administrative or data-generation change rather than malicious traffic.
- A single software limit can become a network-wide dependency when the affected file is deployed broadly.
- Different products behind the same provider can have different results: a website may fail while a mobile app or API remains available.
- Error codes such as 403 and 504 describe what a user saw at the web-access layer; they do not by themselves identify the underlying Cloudflare failure.
The Bottom Line
The November 18, 2025 Cloudflare outage was caused by an internal Bot Management data-generation problem: a permissions change produced a feature file larger than the routing software could handle. It disrupted access to several websites, including parts of OpenAI’s web presence and services such as X, while OpenAI’s mobile apps, API traffic and backend services remained unaffected according to the company’s incident report.
