What Can Healthcare Engagement Learn From Sports Betting?

Healthcare can borrow carefully bounded engagement ideas from responsible-gambling work: give people relevant feedback, make useful next steps easy to find, and adapt delivery to their needs and setting. It should not borrow the commercial aim of keeping people betting. Evidence does not establish that sportsbook retention tactics—or messages, points, and rewards—reliably improve health behavior.

Start with a different goal

Engagement is not an outcome in itself. In a betting product, more time or repeat use may serve a commercial objective; in healthcare, engagement is valuable only when it supports a person’s health goals, self-management, or access to appropriate care. A high click rate or longer session cannot stand in for better health or a protective behavior.

This distinction changes how any borrowed design should be judged. Feedback or reminders are reasonable design hypotheses to test, not proven clinical interventions simply because they resemble a feature used in betting.

Design question Responsible-gambling context Healthcare context
Goal Support safer choices and self-management, rather than treating continued betting as success. Support patient-defined or clinically appropriate health goals and autonomy.
Outcome Protective behavior, such as use of limit-setting tools; app activity alone is not enough. Meaningful health behavior or outcome, not clicks, sessions, or time in an app.
Personalization Make feedback relevant to the person’s circumstances. Adapt information to the person’s needs without using vulnerability to pressure them.
Safety and choice Make limits and safer choices visible. Keep choices informed, allow opt-out, and avoid friction that prolongs unwanted use.
Delivery Messages or tools may be delivered digitally or through community programs. Fit the care setting, trusted relationships, and available support resources.

These are practical comparison criteria drawn from the aims and measures of the cited studies, not a validated universal framework.

What personalized feedback has—and has not—shown

A randomized trial by Nassim Tabri, Richard T. A. Wood, and Michael J. A. Wohl compared personalized and normative responsible-gambling feedback with no feedback among 4,091 Canadian adults who had gambled online or at land-based casinos in the previous three months. The feedback group had 1,940 participants and the no-feedback group had 2,151. Outcomes were assessed after randomization and again three months later. The trial record and abstract describe gambling outcomes—not healthcare patients or clinical outcomes.

Compared with no feedback, the feedback group showed greater gambling insight and stronger intentions and self-efficacy related to learning how games and odds work and using operator-provided limit-setting tools. Reported effect sizes were d = 0.32 for gambling insight, d = 0.31 for intentions to learn how games work, and d = 0.30 for intentions to learn odds. Differences were not observed for intentions and self-efficacy around limiting time or money, being open with others, or balancing gambling with other activities. Although some behavioral measures increased by three months, those differences were not statistically significant.

  • GOLD SCRATCH OFF BOXES Other betting strip cards use tape you have to peel off to reveal the numbers, which can also peel off the numbers. Our cards use commercial scratch off ink which looks professional and is easier to use. These betting cards feature gold metallic scratch off ink which completely conceal the numbers printed beneath them. The gold latex material scratches off cleanly and with ease. Once scratched off, the numbers are clear and easy to read.
  • MADE IN U.S.A. These Football Squares Cards are proudly made in America.

The authors’ conclusion was that personalized and normative feedback “appears to increase gambling insight, intentions and self-efficacy to engage in gambling literacy and use of limit-setting tools.” For healthcare, the defensible design lesson is narrower: feedback that helps someone understand their situation, paired with an accessible next step, is worth testing. The trial does not show that the same approach improves medication adherence, appointment attendance, or health outcomes.

Why a well-worded message may not change behavior

A five-week experiment on safer-gambling messages for sports and race betting found no significant differences across message conditions in commercial betting amounts, time spent gambling, gambling harm, urges, or risk-related beliefs. Its result cautions against assuming that a more persuasive prompt, by itself, will change consequential behavior. The study abstract reports the outcomes measured.

A review and focus-group study found that preferences and responses to responsible-gambling message types varied among cohorts, including groups distinguished by age and betting frequency and people who bet on skill-based games such as sports betting. That makes tailoring a plausible approach to test—not proof that personalization will change behavior for every group. The review and focus-group study describes those differences.

Trust and delivery context matter

The Football Fans and Betting (FFAB) report reflects on an eight-week early intervention delivered through community-trust coaches at Leicester, Preston North End, Accrington Stanley, and Blackpool football clubs. Its authors describe stigma and shame as recruitment barriers. Smaller, locally rooted clubs made recruitment easier, but had fewer resources and greater retention challenges. They recommend public-health education before recruitment and more localized, grassroots delivery. The report is an account of implementation lessons, not definitive evidence of improved clinical outcomes.

MoneyLine Sports Betting Dice - Novelty Gift for Sports Bettors & Gamblers
  • FUN GIFT FOR SPORTS BETTORS - A niche novelty gift for the sports bettor who has everything. Great for birthdays, holidays, stocking stuffers, game day, trips and get-togethers.
  • LET THE DICE DECIDE - When you want some action on a game but do not have a strong pick, roll the dice and leave the decision to chance. A novelty decision-maker, not a betting system.
  • FIVE PURPOSE-BUILT DICE - Includes Home/Away, Moneyline/ATS, Over/Under, Units and a Gimme-All-the-Options combo die that can make the whole pick in one roll.
  • BLACK-AND-GOLD DESIGN - Five 19 mm translucent black dice with engraved gold betting symbols create a distinctive look that stands out from ordinary dice.
  • GIFT-READY SET - Includes all five MoneyLine dice, a black logo drawstring pouch, gold presentation wrap and an illustrated guide explaining what each die and symbol means.

For healthcare teams, the useful implication is that engagement depends on more than interface design. A digital reminder cannot resolve stigma, create trust, or supply missing local support. The setting, recruitment approach, staff capacity, and relationship with participants all affect whether people can take part.

Gamification is a set of options, not proof of effectiveness

Healthcare apps can use progress indicators or levels, points, rewards, narrative or themes, personalization, and customization. These are design elements, not treatments with established effects. A systematic review of gamification in healthcare management identifies these commonly discussed mechanics while noting a shortage of systematic, structured quantitative evidence linking them to motivation, engagement, and behavior change. The review sets out that limitation.

A team considering one of these mechanics should define the intended patient benefit first, then test whether the feature contributes to it. A progress cue might help a person understand completion of a care plan; a points system could instead reward app activity without improving care. The distinction must be measured rather than assumed.

How to apply these lessons responsibly

  1. Define the patient benefit. Specify the health behavior or outcome the feature is meant to support; do not make time in app or repeat use the objective by default.
  2. Make feedback actionable. Show information that is relevant to the person and connect it to a clear, beneficial next step, such as a self-management tool or appropriate support.
  3. Offer choice and limits. Let people control reminders and participation, make relevant safety options easy to find, and avoid design friction that keeps them engaged against their wishes.
  4. Fit the message and setting. Test whether different groups understand and value the communication, and account for trust, stigma, staff, and local delivery resources.
  5. Measure behavior and outcomes. Track meaningful actions and, where appropriate, health outcomes—not just opens, clicks, or session length. Compare results with a suitable baseline or control and report uncertainty.
  6. Check for unintended effects. Look for pressure, confusion, exclusion, or engagement that does not serve the patient, and revise or stop the feature if its costs outweigh its benefit.

The cited evidence supports careful experimentation with feedback, tailoring, and delivery—not importing betting-industry retention goals. The responsible standard for healthcare is whether a feature helps people pursue worthwhile health goals with understanding and control.
