What Can Go Wrong With AI-Designed Proteins? Safety and Off-Target Risks Explained

AI-designed proteins can achieve a requested activity in a model or assay while still binding the wrong molecules, disturbing unintended pathways, becoming toxic in a particular tissue, misfolding, or provoking an immune response. A high design score is computational evidence—not proof of biological specificity, safety, or useful behavior in a person.

The practical answer is layered validation: define the intended target and unacceptable outcomes, test likely off-targets and cellular effects, characterize stability and breakdown products, assess immune and delivery risks, and apply stronger controls as a design moves toward animals, patients, or real-world release. There is no single, reliable failure percentage for AI-designed proteins; risk depends on the protein class, target, exposure, assay, and development stage.

Why a plausible design can still be unsafe

Protein-design systems work from incomplete representations of biology: sequence data, predicted or measured structures, functional labels, and training examples. They can optimize the objective supplied by a researcher without representing every receptor, membrane, enzyme, cell type, or physiological feedback loop that the protein may encounter. A sequence can therefore look likely to fold and perform its intended task while its behavior in a living system remains uncertain.

Structural confidence also has a narrow meaning. A model may predict a stable-looking fold or a good interface, but that does not establish that the protein reaches the right tissue, remains intact, avoids unintended partners, or produces only the desired downstream signal. EFSA has noted that toxicity is often tied to biological function that is not evident from primary sequence alone, and that biases in structure-training data can affect AI-derived conclusions.

What can go wrong with an AI-designed protein?

Off-target binding and pathway effects

A design optimized to bind one target may also interact with related proteins, receptors, membranes, nucleic acids, or components of the extracellular environment. Even a weak interaction can matter if the protein is abundant, persistent, delivered directly to a sensitive tissue, or able to amplify a signaling cascade.

The consequence may not be simple binding. An unintended interaction can block a receptor, activate it, change localization, alter a metabolic pathway, or compete with a natural ligand. Functional labels and structural predictions are incomplete proxies for these effects. Machine-learning methods learn from available sequence, structure, and function data rather than from a complete model of cellular biology.

Direct or context-dependent toxicity

A novel protein can injure cells or tissues through mechanisms that are not obvious in its primary sequence. Toxicity may depend on dose, exposure time, cell type, membrane composition, protease activity, or the presence of a cofactor. A protein that is harmless in one cell line can behave differently in primary human cells or in an organ with a distinct receptor profile.

Context also changes risk. A protein intended to alter an immune signal could produce excessive inflammation in a susceptible person; a protein that binds a membrane component could disturb barrier tissues; and a protein that is safe when localized may be hazardous if delivery spreads it to the brain, heart, liver, or reproductive organs. A negative result in one assay therefore narrows risk but does not prove universal safety.

Misfolding, aggregation, and instability

Predicted structure is not the same as correct folding during production, storage, or use. A candidate may misfold, form soluble or insoluble aggregates, or change conformation when exposed to temperature, pH, salts, surfaces, or repeated handling. Aggregates can alter potency, create new biological activities, and increase immune recognition.

Instability can also produce a moving target. Proteolysis, oxidation, deamidation, clipping, or other chemical changes may generate truncated forms, precursors, or degradation products with different pharmacology. Manufacturing and storage must therefore be treated as part of the safety case, not as administrative details after design.

Immunogenicity and anti-drug antibodies

The immune system may recognize an engineered protein as foreign even when the intended target is human. Anti-drug antibodies can neutralize activity, accelerate clearance, change tissue exposure, or form immune complexes. Carter and Quarmby describe these responses as uncommon but unpredictable in severity; in rare cases they can seriously compromise therapeutic safety or efficacy.

Risk is influenced by sequence, structure, aggregates, impurities, dose, dosing interval, route, and the patient’s immune state. Computational screening and preclinical assays can identify warning signs and reduce incidence or severity, but none predicts every clinical response. A low-risk result in an in-vitro immunogenicity assay is evidence to weigh, not a guarantee.

Delivery and biodistribution effects

The same protein can have different safety profiles depending on how it enters the body. Injection, inhalation, oral delivery, local administration, or a gene-delivery approach changes which tissues see the molecule, at what concentration, and for how long. Off-target exposure may occur even when the molecular design is specific.

Pharmacokinetics and biodistribution must therefore answer practical questions: Where does the protein go? How quickly is it cleared? Does it cross a barrier or accumulate in an organ? Do fragments persist after the active form disappears? These answers determine whether an otherwise useful design has a manageable therapeutic window.

Biosecurity misuse and accidental release

AI-assisted design can be misused to optimize known toxins or pathogen traits, improve immune escape, or propose novel harmful proteins. A screening system based mainly on sequence similarity may miss a dangerous design with little resemblance to known hazards but a similar function. Philip Hunter reported this concern in EMBO Reports in 2024, noting that creating novel sequences with similar functions and undetectable sequence homology could undermine similarity-based screening.

Risk is not limited to deliberate misuse. An improperly controlled workflow, an exposed design repository, or synthesis of an inadequately reviewed sequence could enable accidental release. Controls must cover the full chain from prompt and model access through sequence export, synthesis, handling, storage, and disposal.

How to evaluate a candidate before it advances

Use a staged process in which each new level of biological realism can stop or redirect the program. NIST describes using safe proteins as proxies in testing, evaluation, validation, and verification (TEVV) for AI-assisted protein-design risks. A practical sequence is:

  1. Define the intended use. Record the target, desired activity, dose or concentration, exposure route, duration, recipient population, and tissues that must not be affected. Write unacceptable outcomes in measurable terms, such as receptor activation, cytotoxicity, cytokine release, aggregation, or persistence beyond a specified interval.
  2. Map plausible off-targets. Identify homologous proteins, related receptors, abundant serum proteins, membrane components, enzymes, and pathways present in the intended exposure sites. Rank them by biological consequence, not just by predicted binding score.
  3. Test binding with orthogonal methods. Use more than one assay format where feasible, because immobilization, buffer, avidity, and labeling can create artifacts. Include negative controls, concentration ranges that cover expected exposure, and positive controls showing that the assay can detect a real interaction.
  4. Measure function in relevant cells. Binding alone does not reveal whether an interaction activates, blocks, internalizes, or leaves a pathway unchanged. Test primary or tissue-relevant cells when practical, measure viability and pathway markers, and look for delayed effects rather than only immediate toxicity.
  5. Characterize the material. Confirm identity, purity, folding, aggregation, potency, and degradation products after manufacture and under intended storage conditions. Re-test activity after stress studies that represent shipping, temperature excursions, agitation, and repeated use.
  6. Assess exposure and biodistribution. Establish pharmacokinetics, tissue distribution, clearance, and persistence for the proposed route. Determine whether fragments or aggregates have different distribution from the intact protein.
  7. Evaluate immunogenicity. Combine sequence and structural review with appropriate preclinical assays, including assessment of anti-drug antibodies when the candidate is therapeutic. Treat immune findings as a development decision point, not a box to check once.
  8. Use governed in-vivo studies only when justified. Animal work should follow the intended exposure and include predefined stopping criteria, relevant controls, and analysis of organs and delayed outcomes. In-vivo evidence adds context; it does not erase uncertainty about humans.
  9. Keep the evidence traceable. Store the exact sequence, model version, prompts or design constraints, assay conditions, lots, exclusions, failures, and decisions. A reproducible record makes it possible to investigate a later adverse result and prevents a favorable prediction from being mistaken for experimental proof.

How to compare candidate designs

A candidate with the highest predicted affinity is not automatically the safest choice. Compare designs across the dimensions that determine real-world risk and the strength of evidence supporting each claim.

Decision axis Questions to ask Evidence strength to record
Intended function Does the protein produce the requested activity at a feasible exposure? Computational prediction, purified-protein assay, cell assay, animal or clinical observation
Specificity Which plausible proteins, receptors, membranes, or pathways could it affect instead? Predicted interactions and measured off-target binding and function
Toxicity Are there direct, delayed, dose-dependent, or tissue-specific effects? Cell viability and pathway assays, tissue models, animal findings
Immune risk Could antibodies neutralize the product or create harmful immune complexes? Sequence review, preclinical immunogenicity assays, anti-drug-antibody observations
Material quality Does manufacture or storage create aggregates, truncations, precursors, or degradants? Analytical characterization and stressed-lot testing
Delivery Which tissues receive the protein, at what concentration, and for how long? Pharmacokinetic and biodistribution data for the proposed route
Detectability Would sequence-similarity or synthesis screening recognize a hazardous design? Screening results plus expert review for novel-function risks
Containment and reversibility Can access be restricted, activity stopped, or the material recalled? Operational controls, logs, shutoff plans, and disposal procedures
Overall confidence Are conclusions based on prediction, in-vitro work, animals, or clinical observation? Explicit stage and limitations; no universal off-target percentage is established

Controls for biosecurity and responsible access

Technical screening should be paired with organizational safeguards. Useful controls include:

  • Sequence and synthesis screening: check requested sequences and orders against hazard databases, while recognizing that similarity-only systems may miss novel sequences with similar functions.
  • Access control: restrict high-risk design capabilities, require authenticated users, and separate exploratory analysis from export or synthesis authorization.
  • Logging and review: retain prompts, generated sequences, model versions, downloads, and approvals so unusual activity can be investigated.
  • Human oversight: route designs involving toxins, pathogen traits, immune escape, or unusual biological functions to qualified reviewers before synthesis or distribution.
  • Containment: define laboratory, transport, storage, and disposal procedures appropriate to the hazard assessment, with incident reporting and emergency contacts.
  • Governance and public-health alignment: WHO emphasizes that AI used across medicine should deliver public-health benefit under appropriate governance. Safety decisions should therefore include independent review and a plan for communicating uncertainty.

David Baker and George Church argued in Science in 2024: “To enable the full benefits of this revolution while mitigating risks that may emerge, all synthetic gene sequences and synthesis data should be collected and stored in repositories that are only queried in emergencies to ensure that protein design proceeds in a safe, secure, and trustworthy manner.” The proposal illustrates a broader principle: sensitive design and synthesis records need controlled access, auditability, and an emergency-use framework rather than unrestricted circulation.

Testing only the intended target

Why it fails: a protein can perform its desired function and still activate a related receptor or pathway.

Correction: build an off-target panel from biological plausibility, tissue expression, and consequence severity.

Ignoring the manufactured product

Why it fails: aggregates and degradation products can have different potency, distribution, or immune effects.

Correction: test real production lots and stressed material, not only an idealized sequence or freshly purified sample.

Assuming one cell line represents a person

Why it fails: receptor abundance, metabolism, barriers, and immune state differ among tissues and individuals.

Correction: progress from controlled cell assays to tissue-relevant models and governed in-vivo work when the decision warrants it.

Relying on sequence similarity to detect every hazard

Why it fails: novel sequences can preserve a dangerous function without looking like known hazard sequences.

Correction: combine sequence and synthesis screening with function-based review, access controls, logging, and escalation for unusual designs.

What the evidence can and cannot tell you

Current literature supports a layered-risk approach, not a universal numerical estimate. No general off-target-failure rate has been established for AI-designed proteins across classes and applications. Results vary with the protein’s mechanism, assay design, target biology, delivery route, and stage of development.

Computational predictions can prioritize candidates and reveal obvious liabilities. In-vitro assays can demonstrate activity, binding, cytotoxicity, or pathway changes under defined conditions. Animal studies add information about exposure, distribution, and organism-level effects, while clinical observation is the strongest evidence for how a therapeutic behaves in people—but it remains population- and protocol-specific. Each level answers different questions, and none makes the others unnecessary.

Bottom line for developers and users of protein-design tools

AI can shorten the search for proteins with useful activity, but it cannot certify specificity or safety on its own. The main hazards are unintended binding and signaling, context-dependent toxicity, misfolding and aggregates, harmful degradation products, immune reactions, delivery to the wrong tissue, and deliberate or accidental biosecurity misuse.

The safest development strategy is to define failure conditions before designing, test the most consequential off-targets first, characterize the actual manufactured material, measure exposure and immune effects, and keep every design and decision auditable. Advance only when the evidence—not the model’s confidence—supports the next level of risk.
