A biological age test produces an estimate from a particular measurement and model—not a literal age stored in your body. Depending on the test, it may summarize DNA methylation, the pace of changes in health markers, or other biological measurements. Some measures are associated with later health outcomes across groups, but one consumer score cannot diagnose a condition, reliably forecast your personal future, or show by itself that a treatment has made you healthier.

What does a biological age test measure?

“Biological age” is an umbrella term, not the name of one standardized test. A test’s meaning depends on the sample it analyzes, the measurements it uses, and what its model was built to estimate. A birthday gives your chronological age; an age-like score compares selected biological measurements with a reference population or other model.

Approach What it measures What the result is intended to represent
DNA-methylation clock Patterns of DNA methylation, chemical modifications to DNA, often measured from a biological sample. An age-like estimate or, for some clocks, a pace-related measure. The specific clock determines which.
Clinical-marker or protein-based measure A combination of clinical biomarkers, proteins, or other measurements. A model-defined summary of health status or biological features; it is not interchangeable with a DNA-methylation clock.
Organ-specific measure Measurements grouped to represent one or more organ systems. A model-based estimate of the status of those systems, rather than one universal age for the whole body.

For example, NIH’s 2024 account of DunedinPACE describes a model trained on methylation patterns associated with change in 19 biomarkers of organ-system function over time. It is intended to estimate the pace of aging, not simply assign a birthday-like age. NIH reported that a faster measured pace was associated with poorer health, chronic disease, or earlier death in other long-term studies. Those are group-level associations, not guarantees for an individual test-taker.

Other approaches use different inputs and targets. An NIH report in 2025 described the Health Octo Tool, developed using data from more than 40,000 people to represent health status across 13 organ systems. That work illustrates why two products both marketed as “biological age” tests may not be measuring the same thing.

Can a biological age test tell you how healthy you are?

Not on its own. A score may capture information relevant to health, but it is not a full assessment of your symptoms, medical history, examination, or established risk factors. It also does not automatically tell you which action, if any, would improve your health.

A 2024 National Institute on Aging summary of a study published in Proceedings of the National Academy of Sciences reported that DNA-methylation estimates can help predict multiple chronic diseases, cognitive and functional limitations, and mortality among older adults. The same summary noted that demographic, socioeconomic, mental-health, and health-behavior factors were comparable and often more robust predictors. The finding supports potential value in population research; it does not establish that a consumer clock can diagnose illness or give a dependable personal prognosis.

How accurate and repeatable are the results?

There is no single accuracy figure that applies to all biological-age tests. The FDA distinguishes analytical validity—whether a test measures what it says it measures—from clinical validity—whether the result predicts a health state. Evidence for one test or claim should not be assumed to apply to another.

A peer-reviewed review of epigenetic clocks as individual biomarkers describes technical and biological variation, effects from blood-cell composition, and differences in how separate clocks behave. It also notes weak relationships between some clocks and other measures of aging, and the absence of a clear gold-standard reference for biological age. Some technical-replicate studies cited in that review reported deviations of up to nine years. That is an example from particular studies, not a universal error range for every test.

Results may also reflect social and environmental exposures, and performance may vary when the populations used to develop a clock do not adequately represent the people taking it. A score should not be read as a verdict on personal choices or as proof that the model has isolated an individual’s behavior from wider conditions.

Does lowering the score mean you have improved your health?

No—not by itself. A lower score on a particular test is a change in that test’s output. To show a health benefit, evidence needs to establish that the intervention improves meaningful outcomes, not just that it changes a clock reading.

One example helps show the distinction. In NIH’s 2024 summary of the CALERIE trial, more than 200 healthy, middle-aged volunteers were randomized to calorie restriction or usual eating. Over two years, participants assigned to restriction averaged a 12.5% reduction in daily calories and lost an average of 10% of body weight, mostly fat. NIH also reported a small but statistically significant decrease in DunedinPACE in that group. This is a trial-level result on a biomarker; it does not prove that lowering a score extends an individual’s life, nor does it validate a commercial testing program or diet.

How should you choose or use a test?

If you are considering a test, evaluate the exact service and claim rather than relying on the broad label “biological age.” These questions can help:

Best Value

  • PROVIDES RECOMMENDATIONS TO REDUCE AGING BY UP TO 7.8 YEARS - turns your biomarker results into specific regimen and product recommendations from biological age-segmented lines, so you can make smarter choices and optimize your routine over time
  • What does it measure? Check the sample type and whether the test analyzes DNA methylation, routine clinical biomarkers, proteins, or another input.
  • Which model does it use? Look for the named clock or algorithm and whether it estimates chronological-age-like status, disease-related risk, or pace of aging.
  • What evidence supports this exact claim? Find out which population was studied, what outcomes were measured, and how long participants were followed.
  • How does it explain uncertainty? Look for information about repeatability, reference populations, and limits on interpreting an individual result.
  • What is the intended use? Determine whether the report is for general wellness or clinical use and who can help interpret it.
  • Does it support ordinary care? A responsible report should not present a score as a reason to skip recommended screening or clinician evaluation.

FDA oversight varies with test type and risk. Low-risk general-wellness offerings may not receive FDA premarket review, so do not infer that a biological-age service is FDA-cleared or authorized unless that exact test and claim have been verified. The FDA also cautions that consumer test results should not be the sole basis for medical decisions or replace clinician visits and recommended screening.

If you already have a result, discuss health-related findings with a health care professional. Do not start or stop a treatment, supplement, restrictive diet, or screening schedule based on the score alone.
