What Are the Warning Signs of a Stroke, and When Should You Call 911?

Call 911 immediately for even one possible stroke symptom. Use B.E. F.A.S.T.—Balance, Eyes, Face, Arms, Speech, Time—note when the symptoms began (or when the person was last known well), and use an ambulance rather than driving. A symptom that disappears can still be a transient ischemic attack (TIA) and needs emergency evaluation.

Recognize a stroke with B.E. F.A.S.T.

Most stroke symptoms begin suddenly. The B.E. F.A.S.T. checklist helps a bystander identify the most important warning signs quickly. You do not need to confirm every letter before calling: one possible sign is enough to activate emergency services.

Letter Check What you may notice
B — Balance Look for a sudden change in coordination. New dizziness, loss of balance, trouble walking or difficulty coordinating movements.
E — Eyes Ask whether vision changed abruptly. Sudden blurred or double vision, or loss of sight in one or both eyes.
F — Face Ask the person to smile. One side of the face may droop or feel numb, making the smile uneven.
A — Arms Ask the person to raise both arms. One arm may be weak or numb, or may drift downward instead of staying raised.
S — Speech Ask for a simple sentence. Speech may be slurred, unusual, difficult to understand or impossible. The person may also struggle to understand you.
T — Time Treat the situation as an emergency. Call 911, say that you suspect a stroke, and record the symptom start time or last-known-well time.

The checklist is a recognition tool, not a diagnostic test. A person can have a stroke without showing every B.E. F.A.S.T. sign. If any one sign appears suddenly, call 911 rather than waiting for another sign to develop.

Stroke can affect movement, sensation, language, vision and thinking. In addition to the checklist, watch for:

  • Sudden numbness or weakness of the face, arm or leg, particularly on one side of the body.
  • Sudden confusion, trouble speaking or trouble understanding speech.
  • Sudden trouble seeing in one or both eyes.
  • Sudden trouble walking, dizziness, loss of balance or loss of coordination.
  • A sudden, severe headache with no known cause.

These signs can be subtle, especially if the person is alone or has difficulty describing what is happening. A witness who notices a new facial asymmetry, an arm that will not stay raised, or speech that suddenly sounds different should treat that observation as a possible stroke.

What to do when a sign appears

  1. Call 911 now. Tell the dispatcher that you suspect a stroke and describe the symptoms you observed. Do not wait to see whether the person improves.
  2. Record the timing. Write down the exact time symptoms began if known. If nobody saw the onset, record the last time the person was definitely normal (the “last-known-well” time). This information helps clinicians decide which treatments can be considered.
  3. Use an ambulance. Emergency medical services (EMS) can begin care during transport, gather critical information, alert the receiving hospital and help route the patient to specialized stroke care. Do not drive the person yourself.
  4. Keep the person safe. Stay with them, keep them as calm and still as practical, and follow the dispatcher’s instructions while waiting. Tell EMS about the person’s medical history, medicines and the sequence of symptoms if you know those details.
  5. Do not cancel because the person looks better. Temporary improvement does not rule out a serious event. Tell the dispatcher if symptoms have stopped, and keep the recorded timing available.

Even when the diagnosis is uncertain, calling is the safer decision. Dispatchers and clinicians can sort out other causes after emergency assessment; a bystander cannot reliably distinguish a stroke from a condition that merely resembles one.

Why every minute matters

Brain tissue is vulnerable when blood flow is blocked or bleeding occurs. The American Stroke Association publishes an estimate that about 1.9 million brain cells die every minute a stroke goes untreated. That is why the “T” in B.E. F.A.S.T. is an action, not a reminder to monitor symptoms.

For many ischemic strokes—strokes caused by a clot—NINDS states that the most effective clot-busting treatments can be used only within about four and a half hours for many patients. Some people may qualify for treatment later, and eligible patients with a large clot may benefit from mechanical thrombectomy as soon as possible within a window extending up to 24 hours. These are not do-it-yourself deadlines: eligibility depends on the examination, the exact last-known-well time, brain imaging, the location and size of the clot, bleeding risk and other clinical factors.

The practical implication is simple: an apparent delay can remove options. Calling promptly gives EMS and the hospital the maximum time to identify the stroke type and determine whether a time-sensitive treatment is appropriate.

When symptoms disappear: treat it as a TIA emergency

A transient ischemic attack has stroke-like symptoms that resolve because the temporary blockage clears or blood flow is restored. The American Stroke Association notes that TIA symptoms may last only minutes and can continue for as long as 24 hours. The short duration makes people dismiss the event, but it can signal a high risk of a subsequent stroke.

Call 911 even if the face looks normal again, strength returns, speech clears or vision comes back. Tell EMS exactly what happened and how long it lasted. The fact that symptoms stopped does not establish that the episode was harmless, and a clinician still needs to evaluate it urgently.

What EMS and the hospital do next

Emergency care is designed to answer two questions quickly: Is this a stroke, and is it caused by a clot or bleeding? EMS can start the process on the way to the hospital and notify staff so the stroke team is ready on arrival.

Situation clinicians assess Why it changes care
Ischemic stroke (a clot blocks blood flow) Some patients may be eligible for clot-dissolving medicine within a short, clinician-determined window. A removable large clot may make mechanical thrombectomy an option, potentially up to 24 hours for selected patients after imaging.
Hemorrhagic stroke (a blood vessel bleeds) Clot-busting medicine is not the treatment. Imaging and specialist care are needed to control bleeding and pressure and to address the source.
Possible TIA (symptoms resolved) The immediate signs may be gone, but urgent assessment is still required because the event can precede a disabling stroke.
Timing and eligibility The last-known-well time, examination and brain imaging determine which interventions are safe and available. A consumer cannot choose thrombolysis or thrombectomy.

Because these decisions depend on imaging and specialist judgment, do not delay while trying to identify the stroke subtype at home. The safest information a bystander can provide is the symptom pattern and timing.

Common mistakes that cost time

Waiting for a second symptom

A person may show only speech trouble, only vision loss or only sudden imbalance. B.E. F.A.S.T. is not a requirement to check every letter. One sudden, plausible sign warrants the call.

Driving instead of calling EMS

Private transport can delay assessment and prevents medical care from beginning during the trip. EMS can communicate with the hospital and help direct the patient to an appropriate stroke center.

Assuming improvement means the danger has passed

Resolution can indicate a TIA, not a benign episode. Keep the emergency call active and report that symptoms have changed or stopped.

Guessing the onset time

Do not substitute the time you noticed a person for the last time they were known to be normal. If the person woke with symptoms, the relevant time may be when they were last seen well before sleep; tell EMS exactly what is known and what is uncertain.

Trying to make a diagnosis first

Other medical problems can resemble stroke, but sorting them out requires professional assessment. Delaying the call to search for an alternative explanation can close treatment windows.

A practical bystander checklist

  • Look for a sudden change, not just a chronic symptom the person has had for a long time.
  • Check for balance, vision, facial symmetry, arm strength and speech while someone else calls, if another adult is present.
  • Write down the start time or last-known-well time.
  • Tell the dispatcher the person’s location, symptoms and whether they are still present.
  • Unlock the door or arrange clear access for responders if it is safe to do so.
  • Keep your phone available and follow dispatcher instructions until EMS arrives.

If you are alone with the person, make the call first. The priority is activating EMS, not completing a lengthy examination.

How common is recognition failure?

Knowing the signs remains a public-health challenge. A Centers for Disease Control and Prevention facts-and-statistics page reports that only 38% of surveyed people recognized all major stroke symptoms and knew to call 9-1-1 when someone was having a stroke. That gap is why a short checklist, an immediate call and accurate timing information matter more than confidence in making a diagnosis.

Bottom line for a suspected stroke

Sudden facial drooping, one-sided weakness or numbness, speech or comprehension difficulty, vision change, loss of balance, dizziness, confusion or an unexplained severe headache can be a stroke. Call 911 for one possible sign, note when it started or when the person was last known well, and use an ambulance. Continue to treat the situation as an emergency even if the symptoms disappear.
