Seek immediate medical attention if you develop symptoms that could indicate plague, especially after a flea bite, rodent exposure, contact with a sick or dead animal, or recent travel to an area where plague occurs. Bubonic plague usually causes fever with a rapidly swollen, tender lymph node. Septicemic plague can cause profound weakness, abdominal pain, shock, bleeding, or darkening tissue. Pneumonic plague causes rapidly worsening breathing symptoms, chest pain, and cough and is the only form that can spread from person to person. Do not wait for a test result: clinicians should begin appropriate treatment as soon as plague is suspected.

Why possible plague requires urgent medical care

Plague can progress quickly and may appear in different forms, so there is no single symptom that rules it in or out. The Centers for Disease Control and Prevention (CDC) advises that anyone experiencing symptoms consistent with plague should seek immediate medical attention. Symptoms and an exposure history can raise concern, but they cannot establish a diagnosis on their own.

Plague occurs naturally in parts of the western United States and in certain regions of Africa and Asia. A recent stay in one of those areas, a flea bite, contact with rodents, or handling a sick or dead animal makes an otherwise unusual illness more relevant to tell a clinician. The CDC’s public overview is available at Signs and Symptoms of Plague, updated and reviewed May 15, 2024.

Symptoms by form of plague

Bubonic plague

Bubonic plague most often begins with a combination of general illness and a characteristic lymph-node swelling. Typical symptoms include fever, headache, chills, and weakness, followed by one or more lymph nodes that become markedly swollen, tender, and painful. These swollen nodes are called buboes.

The usual incubation period for bubonic plague is two to eight days after exposure, according to CDC clinical guidance. It commonly follows the bite of an infected flea. Without treatment, infection can spread beyond the lymph nodes and develop into another form, so a painful bubo accompanied by fever is not something to watch at home.

A bubo can occur in the groin, armpit, neck, or another lymph-node region, depending on where the infection entered the body. The presence of a swollen node does not prove plague—many other infections can do this—but the pattern becomes urgent when it follows a plausible exposure or occurs with sudden fever and chills.

Septicemic plague

Septicemic plague affects the bloodstream and may be the first recognizable manifestation of infection or may develop after untreated bubonic plague. Reported symptoms include fever, chills, extreme weakness, and abdominal pain. Severe illness can lead to shock.

Bleeding into the skin or internal organs may occur. Fingers, toes, or the nose can develop darkened tissue as circulation is damaged and tissue dies. These changes, particularly when accompanied by fever, weakness, abdominal pain, confusion, faintness, or signs of shock, warrant emergency care immediately.

The incubation timing for septicemic plague is not well defined; CDC guidance says it is likely to occur within days of exposure. Because this form may not produce an obvious bubo, the absence of a swollen lymph node does not make plague safe to dismiss.

Pneumonic plague

Pneumonic plague involves the lungs and can cause rapidly developing pneumonia. Symptoms include fever, headache, weakness, shortness of breath, chest pain, and cough. The cough may produce bloody or watery mucus.

CDC clinical guidance gives a usual incubation period of one to three days. Pneumonic disease can result when untreated bubonic or septicemic plague spreads to the lungs, or after inhalation of infectious droplets. It is the most serious form and the only form of plague identified by the CDC as capable of spreading from person to person.

Rapidly worsening breathing, chest pain, or a new cough after a plausible exposure should be treated as an emergency rather than monitored for improvement. If pneumonic plague is possible, tell the healthcare facility before arrival so clinicians can arrange appropriate infection-control measures and public-health coordination.

How the three forms differ

Form Prominent symptom pattern Likely route or context Usual incubation timing Person-to-person respiratory spread
Bubonic Fever, headache, chills, weakness, and one or more rapidly swollen, tender, painful lymph nodes (buboes) Usually follows an infected flea bite; can spread to other body sites if untreated Usually 2–8 days No
Septicemic Fever, chills, extreme weakness, abdominal pain, shock, bleeding, or darkening and dying tissue May be the first manifestation or develop from untreated bubonic plague Poorly defined; likely within days of exposure No
Pneumonic Fever, headache, weakness, rapidly developing pneumonia, shortness of breath, chest pain, cough, and sometimes bloody or watery mucus Spread from untreated bubonic or septicemic plague to the lungs, or inhalation of infectious droplets Usually 1–3 days Yes

The timing and patterns in this table come from CDC clinical information at Clinical Testing and Diagnosis for Plague. They are guides for clinical suspicion, not a self-diagnosis checklist. Some cases, especially septicemic and pneumonic plague, may lack an obvious identifying sign.

When to seek urgent care

Go now for symptoms that fit any plague form

Get immediate professional medical care for fever with a painful, swollen lymph node; sudden severe weakness; abdominal pain with signs of collapse; unexplained bleeding or darkening tissue; or rapidly worsening shortness of breath, chest pain, or cough. The CDC’s instruction is direct: “If you are experiencing symptoms like those listed here, seek immediate medical attention.”

Do not wait to see whether symptoms settle overnight, and do not wait for a laboratory result. Plague is treatable with antibiotics, but treatment is most effective when started promptly. CDC clinical-care guidance says, “Begin appropriate therapy as soon as plague is suspected.”

  • A recent flea bite, particularly in an area where plague occurs.
  • Handling or close contact with a sick or dead animal.
  • Contact with rodents or other wildlife that could carry infected fleas.
  • Residence in or travel to a region where plague is naturally present, including parts of the western United States and certain regions of Africa and Asia.

Exposure alone does not prove infection, and symptoms can have many other causes. Its importance is that it changes how quickly a clinician should consider plague and arrange testing and treatment.

Breathing symptoms need the fastest response

Pneumonic plague can worsen rapidly and can spread through infectious droplets. Anyone with possible exposure who develops shortness of breath, chest pain, or a rapidly worsening cough should seek emergency medical care immediately. Avoid delaying care to arrange a routine appointment. When contacting a clinic or emergency department, describe the respiratory symptoms and the possible exposure before you arrive so staff can advise you on precautions.

What to do while arranging care

  1. Contact emergency or urgent medical services immediately. Describe the symptoms, when they began, and whether they are worsening.
  2. State the exposure history clearly. Mention flea bites, rodents, wildlife, sick or dead animals, and recent travel or residence in a plague-endemic area.
  3. Report respiratory symptoms before arrival. A cough, chest pain, or shortness of breath may require infection-control arrangements because pneumonic plague can spread through droplets.
  4. Do not self-diagnose or self-treat. Symptoms and exposure cannot confirm plague, and medication selection is a clinician’s decision.
  5. Do not postpone evaluation for testing. Clinicians can collect appropriate samples and start treatment when suspicion is high.

Information to give the clinician

Write down the approximate time each symptom started and how it has changed. Tell the clinician whether the illness began with a flea bite, a swollen lymph node, fever, breathing symptoms, abdominal pain, weakness, bleeding, or tissue color changes. Include every relevant animal or rodent contact and the dates and locations of recent travel.

Also mention whether another person who shared the same environment is ill, particularly if anyone has a cough. This information helps clinicians assess the possibility of pneumonic spread and involve public-health authorities when appropriate. The CDC’s diagnostic guidance is available at Diagnosing Plague, updated and reviewed May 15, 2024.

How clinicians diagnose and treat suspected plague

Diagnosis relies on clinical assessment, exposure history, and laboratory testing of patient samples. The CDC specifically identifies blood and material from a swollen lymph gland as useful samples. A clinician may choose additional specimens based on the symptoms, including respiratory illness.

When plague is strongly suspected, clinicians should collect samples and then begin appropriate treatment rather than wait for final laboratory confirmation. This approach matters because the illness can become severe quickly, while antibiotics can treat plague when given promptly. Treatment decisions, drug choice, route, monitoring, and duration depend on the patient’s form of illness and medical condition; they must be made by healthcare professionals.

Suspected pneumonic cases require more than individual treatment. Clinicians use infection-control precautions and coordinate with public-health authorities because respiratory droplets can transmit this form. Patients and household members should follow the instructions given by the treating team rather than attempting to manage possible exposure independently.

Why symptoms cannot confirm or exclude plague at home

Fever, headache, weakness, cough, abdominal pain, and swollen lymph nodes occur in many illnesses. Conversely, the most dangerous forms of plague may not present with the classic combination people expect. Septicemic plague can lack a visible bubo, and pneumonic plague may initially resemble another rapidly developing pneumonia.

For that reason, a normal temperature at one moment, the absence of a visible flea bite, or a swollen node in an unusual location cannot safely rule plague out. A home test or internet symptom checker cannot replace professional evaluation and laboratory testing.

Key points to remember

  • A painful, rapidly swollen lymph node with fever and chills is a classic warning pattern for bubonic plague.
  • Extreme weakness, abdominal pain, shock, bleeding, or darkening fingers, toes, or nose can signal septicemic plague.
  • Rapidly worsening shortness of breath, chest pain, or cough—especially with bloody or watery mucus—can signal pneumonic plague.
  • Seek immediate medical attention when symptoms are suggestive, particularly after a plausible exposure or travel to an endemic area.
  • Do not wait for confirmatory test results; clinicians should begin appropriate therapy as soon as plague is suspected.
