Microrobots could one day help doctors deliver treatment or inspect areas that conventional instruments cannot reach. Their use inside the body is not risk-free, however, and the risks depend on the robot’s materials, payload, propulsion method, target anatomy, imaging system, operating time and plan for removal or clearance.
The main concerns are biological compatibility, tissue effects, reliable navigation and actuation, imaging and tracking, material stability, post-treatment fate and integration with clinical workflows. Current reviews describe mobile medical microrobotics as an emerging field with major translation challenges—not as a routine, uniformly proven human treatment. No single adverse-event rate or universal risk ranking applies to every microrobot design.

Why the risk question is harder than it sounds
“Microrobot” describes a scale and a class of technology, not one standardized medical device. A magnetically steered capsule, a biodegradable swimmer carrying a drug, and a soft robotic device deployed through a catheter can have very different failure modes. The surrounding environment also changes the risk: blood flow, mucus, tissue movement, immune activity, temperature, chemical conditions and available imaging all affect performance.
The 2024 review Medical Microrobots in the Annual Review of Biomedical Engineering and the 2024 Roadmap for Clinical Translation of Mobile Microrobotics identify safety, biocompatibility, imaging, actuation, fabrication and material issues as unresolved translation problems. A 2024 review of wireless soft robotic medical devices likewise emphasizes that a laboratory demonstration does not by itself establish clinical readiness.

That distinction matters for patients and clinicians. The available reviews support a framework for asking what could go wrong and what evidence is needed. They do not provide a general human harm rate, a complete risk-benefit estimate or a device-independent prediction of an individual patient’s risk.
Biological compatibility and tissue effects
Materials can interact with the body
Every component that enters the body—robot structure, coating, magnetically responsive material, electronics, adhesive or drug payload—must be assessed in the relevant biological environment. A material that appears inert in a bench experiment may behave differently when exposed to blood proteins, digestive fluids, immune cells or a particular tissue for a prolonged period.
Biocompatibility is therefore a design-specific question, not a property that can be assumed from the word “microrobot.” Researchers need to examine whether the material or coating provokes unwanted biological interactions and whether those interactions change with the location and duration of exposure. The reviews do not establish a general rate of immune reactions, inflammation or tissue injury across the field.
Contact, pressure and movement can matter
A device may touch, press against or move through tissue. The relevant concern is whether its shape, surface, stiffness, propulsion method and operating time are appropriate for the target anatomy. A system intended to travel through a fluid space raises different tissue questions from one designed to anchor briefly at a lesion.
Potential effects must be evaluated for the particular procedure rather than presented as established common outcomes. The evidence summarized in the current reviews supports identifying these interactions as safety questions, not claiming that a particular injury occurs at a known frequency.

Payloads add another layer of risk
Some proposed microrobots carry drugs, imaging agents or other therapeutic payloads. In that case, safety depends not only on the robot but also on dose, release timing, localization and what happens if release is incomplete or occurs away from the intended site. A payload can change the body’s response to the device, and the device can change how the payload is distributed. Evidence must therefore cover the combined system.
Navigation, actuation and loss of reliable control
Movement inside the body is not laboratory movement
Microrobots are commonly driven by external magnetic fields, light, ultrasound, chemical reactions or other actuation approaches. Inside the body, they face moving fluids, anatomical barriers, variable viscosity, tissue deformation and limited access for control equipment. Conditions that are stable in a dish or a model may be substantially less predictable in a living patient.

This is a plausible engineering consequence of losing control; the reviewed sources do not document one general patient outcome or frequency that can be applied to all systems.
Control must include recovery options
A clinically credible design needs more than a way to move forward. Operators need to know how to pause, redirect, retrieve or otherwise manage the device if conditions change. Control must also remain adequate when the patient moves, when anatomy differs from pre-procedure images or when another medical device affects the field.
When comparing a proposed system, ask what happens after a communication interruption, field-generation fault, unexpected obstruction or loss of the planned trajectory. A design that works only while every parameter is ideal has a narrower safety margin than one with a documented recovery strategy.

Imaging and tracking constraints
The operator must know where the robot is
Safe use requires a way to locate and monitor the microrobot during deployment and treatment. Imaging and tracking are repeatedly identified as central translational issues in the 2024 reviews. The robot may be too small, have insufficient contrast, or move in an environment where available imaging provides an incomplete view.
Crashes, No Sound, or Screen Glitches?
Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minutePC Slower Than It Used to Be?
A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11Uncertainty about location or behavior can complicate decisions such as whether the target has been reached, whether a payload has been released, whether the device has become stuck or whether retrieval is possible. The sources do not provide a universal misplacement or injury rate, so these concerns should be described as system-level risks to test rather than as established clinical outcomes.
Imaging can affect the procedure itself
Different imaging methods impose different constraints on equipment, timing, patient positioning and the materials that can safely be used nearby. A clinical team must determine whether the chosen tracking method can operate continuously or at appropriate checkpoints and whether it is compatible with the actuation system and the rest of the procedure.
Tracking evidence should be specific to the intended anatomy and operating conditions. Demonstrating visibility in a transparent model is not equivalent to showing reliable localization through the tissue, fluid, motion and imaging limitations of a patient.
Material stability and what happens after treatment
Stability during use
A microrobot must retain the properties that make it controllable and safe for the required duration. Corrosion, swelling, chemical changes, mechanical wear or degradation could alter its motion, payload release or visibility. Material stability is explicitly identified as a clinical-translation challenge, so the relevant evidence must cover the actual body environment and time of exposure.

Retrieval, degradation or clearance
Every design needs a credible end-of-use plan. Some systems may be retrieved, others may be designed to degrade, and others may be expected to clear through a biological route. The appropriate option depends on the device and procedure; no single fate is universally optimal.
The 2026 review Deployment and clearance of microrobots for localized therapy: past, present and future frames degradation, retrieval and clearance as safety issues that must be evaluated in context. Questions include whether the device can be recovered if treatment stops early, whether degradation products are tolerated, how long clearance takes and how clinicians would confirm that no problematic material remains.
Failure can occur between intended states
A system designed to remain intact may become unstable, while a biodegradable system may degrade too soon or too slowly for the treatment plan. These possibilities make timing and monitoring important. Evidence should show that the material follows its intended path under the specific chemical, mechanical and biological conditions of use.

The robot is only one part of a medical system. Safe deployment may require a delivery instrument, field-generating equipment, imaging hardware, software, trained operators and a procedure for retrieval or disposal. The clinical team must coordinate those elements without creating new hazards or unacceptable delays.

Microrobots may be manufactured at a scale where small variations in dimensions, coatings or magnetic properties affect behavior. Best Value
Regulatory and workflow questions are separate from whether a prototype can move. The reviews identify collaboration among clinical experts and microrobotics researchers as necessary to address microfabrication, safety and imaging. As the Advanced Materials roadmap puts it: “Clinical translation of mobile microrobots is only possible with a close collaboration between clinical experts and microrobotics researchers to address the technical challenges in microfabrication, safety, and imaging.”
How to compare microrobot designs responsibly
Performance claims are not automatically comparable across platforms. Designs may use different body environments, imaging methods, actuation systems and therapeutic aims. A useful comparison keeps the intended application fixed and asks the same questions of each candidate.
| Risk area | Question to ask | Why it matters |
|---|---|---|
| Biological compatibility | Are the materials, coatings and payload suitable for the target tissue and exposure time? | Biological response depends on the complete device and its environment. |
| Actuation and navigation | Can movement remain controllable despite barriers, flow and tissue motion? | Unreliable control could prevent the intended task or target from being reached. |
| Imaging and tracking | How will clinicians locate and monitor the device during the procedure? | Incomplete location or behavior information complicates safe decisions. |
| Material stability | Will the device retain its required properties for the full treatment period? | Unexpected change can affect motion, release, visibility or structural integrity. |
| Post-treatment fate | Is the device retrieved, degraded or cleared, and how is that outcome verified? | The plan must address retained material and degradation products. |
| Workflow integration | Can deployment, control, imaging and recovery fit the clinical procedure? | A working prototype is not necessarily a usable or safe clinical system. |
Reported results should be read with those differences in mind. A speed, targeting result or visibility claim from one model does not establish equivalent performance in another anatomy or with another control method.

What evidence should exist before human use?
A responsible development path connects each proposed benefit to a specific safety test. The exact program will vary, but the following checks identify the evidence a reader should look for:
- Define the use case. Specify the target anatomy, intended treatment, operating time, payload and whether the device is temporary or permanent.
- Characterize the complete device. Test the structure, coatings, propulsion components and payload together rather than treating each part as independent.
- Test the relevant environment. Examine motion, degradation, visibility and control under conditions that represent the intended body site, including fluid flow or tissue motion where relevant.
- Demonstrate tracking and control. Show how operators locate the device, confirm its actions and respond to loss of trajectory or communication.
- Verify the end-of-use plan. Provide evidence for retrieval, degradation or clearance and define how clinicians will detect an incomplete outcome.
- Assess the clinical workflow. Include delivery equipment, imaging, actuation, operator training, manufacturing consistency and recovery procedures.
- Interpret results narrowly. Keep conclusions tied to the tested design, environment and procedure; do not generalize a model demonstration to every microrobot.
What current reviews do—and do not—establish
The 2024 literature presents medical microrobotics as an emerging field. The Annual Reviews article describes the long-standing aim of controlling miniature robots inside the human body to help identify and treat disease, but that statement is an overview of the field’s objective, not evidence of clinical efficacy.
The clinical-translation reviews identify recurring barriers in safety and biocompatibility, imaging, actuation, material behavior, fabrication and workflow integration. The 2026 review adds the need to evaluate deployment and post-therapy fate in context. Together, these sources support careful risk categories and testing priorities.
They do not establish a universal human adverse-event rate, a complete risk-benefit estimate for a particular patient, or regulatory approval status for a specific microrobot. A claim about a named device or proposed procedure requires evidence specific to that system.
Questions to ask about a proposed procedure
- What exact microrobot design is being used, and which components remain in the body?
- What materials, coatings and payloads contact tissue, and for how long?
- How is the device propelled, controlled and tracked in the intended anatomy?
- What happens if it cannot reach the target, stops responding or becomes difficult to locate?
- Is it retrieved, degraded or cleared after treatment, and how is that verified?
- Which results come from the target body environment rather than a simplified laboratory model?
- What evidence covers manufacturing consistency, imaging compatibility and operator recovery procedures?
Bottom line
Microrobots may eventually provide valuable ways to reach difficult anatomy or localize treatment, but their risks are highly design- and procedure-dependent. The central safety questions are whether the device is biologically compatible, controllable and trackable, stable for the required time, and safely retrieved, degraded or cleared afterward. Current reviews show substantial engineering and clinical-translation work remains before broad routine use can be assumed.
