What Are the Risks of Off-Target DNA Insertions in Genome Editing?

Off-target DNA insertions are unintended sequence changes that occur away from the genomic site an editor was designed to modify. They are a potential safety concern because an insertion, deletion, rearrangement or translocation could alter gene function or genome integrity. However, no single percentage describes the risk for every CRISPR system, guide RNA, cell type, delivery method or patient. Risk has to be measured for the specific product and the cells it is intended to treat.

What “off-target DNA insertion” actually means

Two terms are being combined in this phrase. Off-target describes location: the change happened somewhere other than the intended genomic target. Insertion describes one type of sequence change: extra DNA has been added at that location. An off-target event might instead be a small deletion, a substitution, a larger rearrangement or a chromosome-level translocation.

CRISPR literature also uses “DNA bulge” for a mismatch configuration in which the guide RNA and genomic DNA are not the same length. A bulge is a feature of recognition between the guide and DNA; it is not the same thing as a DNA fragment being inserted into a chromosome. Analyses that search only for ordinary base mismatches can therefore miss potential sites with bulges.

The main categories scientists separate

Event What happens Why it matters
Small insertion or deletion (indel) Repair after a cut adds or removes a few bases, often near a cleavage site. It can disrupt a coding sequence or regulatory element, although many indels have no measurable functional effect.
Off-target cleavage and repair The editor cuts a similar but unintended genomic sequence and the cell repairs it inaccurately. The resulting mutation may affect a gene or control region.
Large structural variant A deletion, duplication, inversion or other change spans hundreds or thousands of bases. Small-variant assays may miss it, and several genes or regulatory regions can be affected at once.
Chromosomal translocation Broken DNA ends from different chromosomes join together. It can create abnormal gene fusions or destabilize the genome.
Donor or vector integration Intended donor DNA or delivery-vector DNA integrates at an unintended location. The inserted sequence, its regulatory elements and the integration site may have biological effects.

Regulators treat off-target edits and translocations as possible risks and expect chromosomal integrity to be evaluated. The available evidence does not establish a general incidence for unintended donor- or vector-DNA integration, so that possibility should not be presented as a universal event rate.

Why an off-target change could be harmful

An off-target edit is not automatically dangerous. Its consequence depends on the affected genomic region, the exact sequence change, the cell in which it occurs and what happens to that cell afterward. A change in a nonfunctional stretch of DNA may have little observable effect. The same-sized change inside a tumor-suppressor gene, a growth-control gene or an important regulatory element could be consequential.

Possible biological consequences

  • Loss of gene function: an insertion or deletion can interrupt the coding sequence or alter how a gene is spliced.
  • Abnormal gene regulation: a structural change can move a promoter or enhancer next to a different gene.
  • Genome instability: multiple breaks or faulty repairs can produce translocations, inversions or other rearrangements.
  • Clonal expansion: if an edited cell gains a survival or growth advantage, it may become more common than other treated cells.
  • Loss of normal cell performance: a change in a specialized cell could impair the function the therapy is meant to restore.

These are potential outcomes, not predictions that every off-target event will cause cancer, treatment failure or another clinical injury. A safety assessment must connect a detected variant to its location, biological effect and persistence in the treated cell population.

Somatic treatment is different from germline editing

Most clinical gene-therapy discussions covered by United States regulatory guidance concern somatic cells: cells treated in one person, with changes not intended to be passed to future children. Editing an embryo, egg, sperm or early reproductive cell raises separate questions because a change could be inherited. The National Human Genome Research Institute describes distinct safety and ethical concerns for reproductive editing. Those concerns should not be folded into a risk estimate for a somatic treatment.

What studies have found—and what their numbers do not mean

Published results demonstrate that off-target activity and large unintended changes can be detected under some conditions. They do not provide a universal clinical incidence.

Study or program Finding How to interpret it
U.S. Food and Drug Administration regulatory-science project, completed 2023 Nine off-target sites were found among more than 2,000 sites assayed in a study using clinically relevant transient Cas9 delivery in primary human cells and established guide RNAs. A high-fidelity Cas9 reduced off-target activity compared with wild-type Cas9 in the tested conditions. This is a result for those editors, guides, cells and delivery conditions—not a rate for all products, patients or treatments.
CRISPR/Cas9 bulge study, indexed in PubMed, 2014 Fifteen confirmed off-target sites were identified among 114 putative loci. The sites contained a single-base bulge and one to three mismatches to the guide strand. The finding supports searching for DNA and RNA bulges as well as ordinary mismatches. It is not a clinical event rate.
Structural-variant study, indexed in PubMed, 2023 Whole-genome analysis identified deletions of 91.2 kilobases and 136 kilobases at atypical, nonhomologous off-target sites in two edited cell lines. Large changes can be missed by assays limited to small local edits. Two cell lines cannot establish how often such deletions occur in people.
Nature Communications zebrafish study, 2022 Structural variants of at least 50 base pairs represented 6% of editing outcomes in founder larvae. The guides had been selected for in-vitro off-target activity, and outcomes were followed across generations. This is an embryo and animal-model observation under deliberately sensitive conditions, not a human treatment risk.

The FDA project’s comparison of wild-type and high-fidelity Cas9 illustrates why editor choice matters, but it does not make a high-fidelity nuclease universally safe. Performance still depends on guide sequence, exposure, delivery and the relevant cell type.

Can CRISPR edit the wrong part of DNA?

Yes. Cas9 and other editors use a guide sequence to recognize a target, but recognition can tolerate some mismatches. Sites with similar sequences, unusual DNA structures or bulges may be cut or edited. The cell’s repair pathways then determine whether the result is a small indel, a larger rearrangement or another outcome.

Factors that change the risk profile

  • Editing modality: a nuclease that makes a double-strand break has different failure modes from a base editor or prime editor.
  • Guide sequence: some guides have many plausible near-matches; others have fewer. Prediction tools trade sensitivity against specificity.
  • Editor fidelity and exposure: engineered high-fidelity nucleases and transient delivery can reduce activity at some unintended sites, while prolonged exposure may provide more opportunity for mis-editing.
  • Cell source and state: primary cells, stem cells and immortalized cell lines can differ in DNA repair, growth and clonal behavior.
  • Delivery conditions: dose, route, duration and whether DNA, RNA or a protein complex is delivered affect the number and duration of cells exposed.
  • Repair environment: the same cut can produce different products in different cells or at different stages of growth.

Because these variables interact, a result obtained with one guide or cell line cannot simply be transferred to another therapy.

How scientists test for off-target edits

A credible assessment combines prediction, targeted measurement and broader genome-integrity testing. No single assay can prove that every possible unintended change has been excluded.

  1. Define the product and intended edit. Investigators document the editor, guide, donor template if used, delivery format, dose, exposure time and the relevant manufacturing or treatment material.
  2. Predict candidate sites. Computational methods search for sequences resembling the guide, including mismatch and bulge configurations. These methods help prioritize testing; a predicted site is not evidence that editing occurred.
  3. Sequence high-priority sites deeply. Targeted next-generation sequencing can measure low-frequency variants at nominated loci. Assay design must account for amplification artifacts, read errors and the detection limit.
  4. Use genome-wide methods when justified. Genome-wide sequencing or other unbiased approaches can reveal changes outside a predefined candidate list. Their value depends on coverage, read quality, bioinformatics and the type of variant being sought.
  5. Look for structural and chromosomal changes. Long deletions, inversions, copy-number changes and translocations require methods capable of detecting them; a small-amplicon assay alone is insufficient.
  6. Test relevant samples. Samples should represent the actual edited cell product or treated tissue, with appropriate controls and enough independent material to detect low-frequency events.
  7. Investigate biological significance. Variants that are detected should be mapped to genes and regulatory regions, evaluated for persistence or clonal expansion and, where appropriate, followed with functional assays.

The FDA’s sequencing recommendations address off-target assessment, on-target analysis, chromosomal integrity, sample selection, sequencing, bioinformatics and reporting. They emphasize that feasible candidate-site lists and analytical methods depend on the particular editing conditions.

Why prediction and sequencing each have limits

A prediction algorithm can miss an unusual site or overpredict a site that is never edited. Sequencing a finite list of loci cannot show that all other loci are clean. Conversely, a genome-wide assay may have insufficient sensitivity for rare variants or may not reliably detect every large rearrangement. The relevant question is not whether one test was negative, but whether the combined strategy could detect the classes of change that matter for that product.

Does a negative test mean there is no risk?

No. A negative result means that no change was detected above that assay’s validated detection limit in the samples tested. It does not establish that the event is impossible, absent from every treated cell or incapable of appearing later through clonal expansion.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Confidence in a negative finding is stronger when the study uses the intended clinical material, sufficient sequencing depth, validated error controls, independent samples and tests that cover both small variants and structural or chromosomal abnormalities. Reporting should state the detection limit and the regions or variant classes that were not assessed.

How to interpret an off-target percentage

Percentages are meaningful only with their denominator and conditions attached. “Nine sites among more than 2,000 assayed” refers to sites detected in one FDA project, not nine percent of patients or nine percent of all possible sites. “Six percent of outcomes” in zebrafish refers to founder larvae under that study’s guide-selection and model conditions, not six percent of human treatments.

When comparing studies, check:

  • the editor and guide sequence;
  • the cell type, species and developmental stage;
  • the delivery format, dose and exposure duration;
  • whether the assay was targeted or genome-wide;
  • the smallest variant it could detect and whether it captured large structural changes;
  • the number and origin of samples; and
  • whether the result came from a model system or the intended therapeutic material.

What United States FDA guidance says

The FDA’s January 2024 guidance is final. It covers product design, manufacturing and testing, nonclinical safety assessment and clinical-trial design for human gene-therapy products that incorporate genome editing.

On April 14, 2026, the FDA issued Safety Assessment of Genome Editing in Human Gene Therapy Products Using Next-Generation Sequencing as a draft guidance labeled “Draft – Not for Implementation.” It describes nonbinding recommendations unless a specific legal requirement applies; it is not a finalized rule. Its publication status should be checked before relying on it for a filing.

“Therefore, an adequate assessment to evaluate off-target editing risk and impact on chromosomal integrity of human GE products is crucial to minimize unintended changes in the target cells or tissues.”

— U.S. Food and Drug Administration, Safety Assessment of Genome Editing in Human Gene Therapy Products Using Next-Generation Sequencing; Draft Guidance for Industry, April 2026

Questions to ask about a specific genome-editing treatment

Patients and research participants cannot infer a treatment’s risk from a headline percentage in another study. Useful questions for the treating team or sponsor include:

  • Which editor, guide and delivery system are being used?
  • Were the tests performed on the actual manufactured cell product or treated tissue?
  • Which mismatch and bulge sites were evaluated, and how were they selected?
  • What was the validated detection limit for rare edits?
  • Did testing cover large deletions, inversions, copy-number changes and translocations as well as small indels?
  • How will edited cells be monitored for persistence or clonal expansion?
  • Which results are established for this product, and which are extrapolated from cell lines or animals?

These questions turn the broad phrase “off-target risk” into a product-specific assessment of what was tested, what was detected and what remains uncertain.
