Robotic-assisted surgery has the risks of the operation and anesthesia, plus possible device and workflow problems. Complications can include bleeding, infection, blood clots, injury to nearby organs or blood vessels, anesthesia reactions, or a need for another procedure. A system malfunction can force the team to pause, restart, change the plan, or continue with a non-robotic approach. The robot does not operate independently: a trained surgeon controls every instrument. Your actual risk depends on the operation, your health and anatomy, the surgeon’s experience, and the system’s authorized use.
What “robotic surgery” means
Robotically assisted surgery is still surgery. The system may provide a surgeon with a three-dimensional view, articulated instruments, and a console from which to control those instruments, but it does not decide where to cut or perform an operation on its own. The surgeon remains responsible for positioning, instrument movement, tissue handling, and clinical decisions.

The phrase “robotic surgery” covers many different operations, including procedures in general, cardiac, colorectal, gynecologic, head and neck, thoracic, and urologic surgery. A risk statement for one operation cannot automatically be applied to another. The patient’s age, medical conditions, previous operations, anatomy, medications, and the difficulty of the case also change the balance of risk.

The main medical risks
For a particular patient, the most important risks are usually the risks of the underlying operation and anesthesia rather than the word “robotic” itself. MedlinePlus describes risks of robotic surgery as comparable in kind to those of open and laparoscopic surgery, while minimally invasive access may offer benefits in appropriate cases. A possible benefit is not a guarantee of an easier recovery or better outcome for an individual.

- Bleeding: Blood loss can require transfusion, additional treatment, or an urgent change in the operative plan.
- Infection: Infection can involve the incision, the urinary tract, the lungs, the abdomen, or another treated area and may require antibiotics or drainage.
- Blood clots: A clot in a leg vein can travel to the lungs and become life-threatening. Risk is influenced by the operation, mobility, prior clotting history, cancer, and other health factors.
- Injury to nearby structures: Instruments can damage an organ, bowel, urinary tract, blood vessel, nerve, or other tissue near the target area. Repair may be possible during the same operation, but another procedure can sometimes be needed.
- Anesthesia complications: Reactions to medicines, breathing problems, heart or blood-pressure complications, nausea, and other anesthesia-related events are possible.
- Incomplete treatment or further intervention: A procedure may not achieve its intended result, or a complication may require another operation, an intervention, longer monitoring, or a longer recovery.
There is no single complication percentage for “robotic surgery.” A meaningful estimate must identify the exact operation, the patient population, the comparison group, the outcome definition, and the follow-up period.
Device and operating-room complications
A robotically assisted system adds equipment, software, instruments, cables, displays, and a team workflow to the operating room. Problems can affect the operation even when no patient injury occurs.
| Possible issue | What it can mean during surgery |
|---|---|
| Instrument or component breakage | A piece may break, become damaged, or need to be located and removed; the team may replace the instrument or alter the plan. |
| Mechanical problem | An arm, joint, drive, or other mechanical part may not move or respond as expected, requiring troubleshooting or a different approach. |
| Electrical arcing or unintended instrument operation | An electrical or control problem can create an unexpected tissue hazard and may require immediate instrument withdrawal or system shutdown. |
| System error | The team may need to pause, restart the system, change equipment, or continue without the robot. |
| Image, camera, or display problem | Loss or degradation of the operative view can interrupt the procedure until visibility is restored or another approach is used. |
The team may convert to a laparoscopic or open operation, change the operative plan, restart the system, or reschedule a procedure if a problem cannot be safely resolved. Conversion is a safety decision, not proof that the robotic approach was inherently negligent or that the operation has failed.

What device reports show—and what they do not show
The U.S. Food and Drug Administration (FDA) receives reports about robotically assisted systems. Its overview says most reports concern device malfunctions and generally are not associated with injuries, although injuries and deaths have also been reported. A report can be duplicated, incomplete, inaccurate, or unverified. It does not by itself prove that a device was defective or caused a patient’s outcome.
A retrospective analysis of FDA MAUDE reports for the da Vinci system from 2000 through 2013 identified 10,624 reports: 8,061 described device malfunctions, 1,391 described patient injuries, and 144 described deaths. The researchers estimated a mean of 83.4 injury and death events per 100,000 procedures during the period they analyzed. These are historical passive-surveillance counts and an analysis of one system, not a current complication rate for an individual or for all robotic operations. Reporting practices, procedure mix, technology, and clinical practice have changed, and the data cannot establish causation.
The same analysis described broken or burnt instrument pieces, electrical arcing, unintended instrument operation, system errors, and video or imaging problems. Some reports said the event led to a system restart, conversion to non-robotic surgery, or rescheduling. Those outcomes illustrate possible operational responses; they do not tell you how often a similar event will occur in your operation.

How strong is the comparative safety evidence?
A 2020 systematic review in BJS Open examined 103 robot-assisted surgery studies. Its percentages describe what the studies reported, not the percentage of patients who had complications:
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →| Finding in the review | Number and percentage | Correct interpretation |
|---|---|---|
| Studies that assessed safety | 71 of 103 (68.9%) | Safety was not reported in all included studies. |
| Safety-assessing studies that recorded complications | 70 of 71 (99%) | Most studies that addressed safety included a complication outcome. |
| Safety-assessing studies reporting conversion to open surgery or a changed plan | 32 of 71 (45%) | Nearly half reported this outcome, but the figure is not a conversion rate among patients. |
| Safety-assessing studies reporting further intervention | 28 of 71 (39%) | This is a study-reporting figure, not the proportion of patients needing another procedure. |
| Safety-assessing studies with at least one year of follow-up | 8 of 71 (11%) | Long-term safety information was uncommon. |
The review found that conclusions often reported no significant difference or equivalence between robotic and comparison groups. Uneven outcome definitions and limited long-term follow-up make it unsafe to turn those findings into a universal claim that robotic surgery is safer, less safe, or equivalent for every operation.
Is robotic surgery safer than open surgery?
That question cannot be answered for “robotic surgery” as a single category. Compare approaches for the same operation and a similar patient group. A fair discussion should include:

- Common and serious complications, defined the same way for each approach.
- The chance of conversion or an unplanned change in the surgical plan.
- The chance of reoperation, drainage, transfusion, or another intervention.
- Blood loss, infection, hospital stay, recovery time, and longer-term function when reliable data exist.
- The surgeon’s experience with that specific operation and the particular system model.
- Your health, anatomy, prior procedures, goals, and non-surgical or non-robotic alternatives.
A smaller incision or a technologically advanced console does not automatically produce a better outcome. Conversely, the possibility of conversion does not mean a robotic approach is inappropriate. The relevant question is which option offers an acceptable balance of benefit and risk for your condition with this team.

In the United States, the FDA has cleared robotically assisted systems for trained physicians performing specified laparoscopic procedures in general, cardiac, colorectal, gynecologic, head and neck, thoracic, and urologic surgery. A clearance is tied to an authorized procedure and intended use; it is not a blanket endorsement for every operation, disease, or patient.
The FDA states that no robotically assisted surgical device system has U.S. marketing authorization specifically for the prevention or treatment of cancer. For mastectomy and breast-cancer prevention or treatment, a separate FDA communication says safety and effectiveness have not been established and discusses FDA oversight of significant-risk clinical studies. If cancer is involved, ask whether the device is being used within an authorized indication or within an appropriately overseen clinical study. FDA clearances, warnings, and authorized uses can change, so the current FDA information and your clinician’s explanation should control.

How a team handles a malfunction or difficult case
Operating teams plan for equipment and clinical contingencies before the procedure. If a problem occurs, the response depends on what happened, the patient’s condition, and the stage of the operation. In general, the team may:
- Stop the affected instrument or pause the operative step and assess the patient and the equipment.
- Restore visualization, replace an instrument, restart the system, or use another approved component if that is safe.
- Continue with a revised robotic plan if the problem is resolved and the surgeon judges that appropriate.
- Convert to laparoscopic or open surgery, or otherwise change the plan, when that is safer than continuing robotically.
- Provide additional treatment, monitoring, or a later procedure if the event or the underlying condition requires it.
Ask your surgeon how the hospital staffs and equips the operating room for this possibility. The specific backup arrangements are local operational details, so a general description cannot substitute for the plan at your facility.
Questions to ask before consenting
- Why is a robotic approach recommended for my specific condition and operation?
- What non-robotic alternatives do I have, and what are the trade-offs for my case?
- How often do you perform this operation with this system, and what training have you completed on this model?
- Which complications are most relevant to my health, anatomy, medicines, and previous operations?
- If the system has a problem or the operation becomes difficult, what is the plan for continuing or changing the procedure?
- What could require a transfusion, another procedure, additional treatment, or a longer recovery?
- If this is cancer-related, is the device being used within its authorized indication or an appropriately overseen clinical study?
Your surgeon should discuss the risks, expected benefits, alternatives, and the option of not having the procedure when appropriate. These questions support an informed consultation; they cannot predict your individual outcome without a clinician who knows your case.

Robotic assistance changes how a surgeon reaches and manipulates tissue, but it does not remove the ordinary hazards of surgery. The most useful risk estimate is procedure-specific and patient-specific, and it includes the team’s experience and a clear backup plan for equipment or clinical problems. Treat device-report numbers as safety signals rather than personal odds, and judge robotic and non-robotic options by comparable outcomes for your own operation.
