Most people infected with West Nile virus (WNV) never develop symptoms. When illness does occur, it is usually a short flu-like illness with fever, headache, body or joint aches, vomiting, diarrhea, or a rash. A small proportion develop a dangerous infection of the brain or the membranes around it. High fever, neck stiffness, muscle weakness, confusion, or tremors require immediate medical attention.
Symptoms alone cannot confirm WNV. A clinician weighs your symptoms and possible mosquito exposure, examines you, and decides whether laboratory testing for WNV or another infection is needed. There is no medicine that cures WNV itself, so treatment ranges from rest and fluids for mild illness to hospital monitoring and supportive care for neurologic disease.

What West Nile virus symptoms look like
West Nile virus is spread by infected mosquitoes. Anyone living in an area where the virus circulates can become infected, although a mosquito bite does not necessarily transmit WNV. The Centers for Disease Control and Prevention (CDC) estimates that 80% of infected people develop no symptoms. About 20% develop a febrile, flu-like illness, and less than 1% develop severe disease affecting the central nervous system, requiring hospitalization, or resulting in death. These percentages describe groups of infected people; they cannot predict what will happen to one individual.

| Illness pattern | Possible features | What it usually means for care |
|---|---|---|
| Asymptomatic infection | No noticeable symptoms | No symptom treatment is needed, but a person may not know they were infected. |
| Mild febrile illness | Fever, headache, body aches, joint pain, vomiting, diarrhea, rash, fatigue or weakness | Contact a healthcare professional for advice, especially if symptoms are worsening or the person is at higher risk. |
| Severe neurologic illness | High fever, severe headache, neck stiffness, confusion, tremors, muscle weakness, seizures, disorientation, stupor, vision loss, numbness or paralysis | Seek immediate medical attention; hospitalization and supportive care may be necessary. |
A rash, headache, or fever is not specific to WNV. Many viral and bacterial illnesses cause similar symptoms, so a symptom list is not a diagnosis.

When symptoms start
Symptoms commonly begin two to six days after the bite of an infected mosquito. The CDC gives a broader range of two to 14 days. In people with weakened immune systems, onset can take longer. Because people often do not notice the bite and may have several mosquito exposures, the timing cannot by itself prove that WNV caused an illness.
How long mild symptoms can last
Most people with mild WNV illness recover completely. Fatigue and weakness can nevertheless continue for weeks or months after the fever and other acute symptoms improve. A gradual recovery does not rule out WNV, and persistent or worsening symptoms warrant medical advice.
Warning signs that need immediate medical attention

Why these signs are urgent
Severe WNV can inflame the brain (encephalitis) or the membranes around the brain and spinal cord (meningitis). Neurologic illness may also cause disorientation, stupor, coma, convulsions, vision loss, numbness, or paralysis. These problems can progress quickly and require observation for complications that cannot be managed safely at home.
Caregiver checklist
- Note the person’s temperature, mental state, strength, balance, and ability to drink.
- Ask about new neck stiffness, shaking, severe headache, vision changes, numbness, or difficulty moving.
- Tell emergency staff about possible mosquito exposure, when symptoms began, medical conditions, and medicines that weaken immunity.
- Do not drive someone who is confused, having a seizure, or too weak to sit safely; request emergency help.
Who has a higher risk of severe West Nile disease?
Severe illness can occur in anyone, but risk rises with older age, certain chronic conditions, and weakened immunity. CDC identifies cancer, diabetes, high blood pressure, and kidney disease as examples of conditions associated with greater risk. Some medicines that suppress the immune system can also increase risk.
Adults aged 65 and older are three times as likely to develop neurologic illness as people younger than 65, according to CDC guidance. For an older adult or someone with a listed condition, call a healthcare professional early when fever or flu-like symptoms follow possible mosquito exposure rather than relying on home observation alone.

When to call a doctor if symptoms are not an emergency
The CDC consumer guidance does not set a universal number of hours or days for a routine call. Contact a clinician for individualized advice if you develop a fever or flu-like illness after possible exposure, if symptoms are not improving, or if you are caring for someone in a higher-risk group. A clinician can decide whether an examination or testing is appropriate and can distinguish WNV from other infections circulating in your area.

Call sooner if the person cannot keep fluids down, becomes markedly weaker, or develops any neurologic change. Those developments can require urgent assessment even if the initial illness seemed mild.
What to expect at a medical evaluation
There is no reliable home pathway for confirming WNV. CDC describes diagnosis as a combination of symptoms, a history of possible mosquito exposure, and laboratory testing. The provider chooses tests based on the presentation, timing, local illnesses, and medical history.
- History and exposure review: The clinician asks when symptoms began, whether mosquito exposure is plausible, where the person has been, and whether immune-suppressing medicines or chronic conditions are present.
- Physical and neurologic assessment: The examination looks at temperature, hydration, mental status, neck stiffness, strength, coordination, sensation, and other signs of central-nervous-system involvement.
- Laboratory testing: The provider may order testing for WNV and for other infections that produce similar fever, headache, rash, gastrointestinal, or neurologic symptoms. The specific test and timing are clinical decisions, not something to select from a home kit.
- Escalation when needed: Confusion, weakness, seizures, severe headache, neck stiffness, or other neurologic findings can lead to emergency evaluation or admission for monitoring and supportive treatment.
A negative or inconclusive result does not make symptoms irrelevant. Your clinician interprets results alongside the examination and the timing of illness, and may investigate another cause when WNV is not established.
Treatment: what helps and what does not
Mild illness
No medicine is available to treat WNV itself. For an uncomplicated illness, CDC describes rest, staying hydrated, and over-the-counter medicine for symptom relief, such as acetaminophen. Use medicines only as directed on the label or by a healthcare professional, taking into account age, pregnancy, liver disease, kidney disease, allergies, and other medicines.

Do not apply a blanket recommendation for ibuprofen, naproxen, or other nonsteroidal anti-inflammatory drugs (NSAIDs). CDC cautions people living in areas with dengue, another mosquito-borne illness, to avoid ibuprofen and other NSAIDs. Because dengue and WNV can initially look alike, ask a clinician or pharmacist which pain or fever medicine is appropriate for your location and circumstances.
Severe illness
Severe WNV commonly requires hospitalization. Treatment is supportive: clinicians may provide intravenous fluids, pain medicine, nursing care, and close monitoring for neurologic or other complications. The cited CDC guidance does not identify a consumer-available antiviral cure. Hospital teams determine the level of breathing support, seizure management, fluid management, and observation needed for each patient.

After a mild infection, most people recover fully, but fatigue or weakness may linger for weeks or months. Recovery from encephalitis, meningitis, or another severe neurologic infection can also take weeks or months. Some neurologic effects may be permanent, including persistent weakness, movement problems, cognitive changes, or other loss of function. Follow-up care is important when symptoms interfere with walking, work, memory, sleep, mood, or daily activities.

Preventing additional mosquito bites
Prevention belongs alongside medical care, not in place of it. CDC recommends an EPA-registered insect repellent, long loose-fitting clothing, and avoiding outdoor exposure between dusk and dawn, when mosquitoes that spread WNV are most active. Follow the repellent label, and use additional local public-health measures when mosquito activity is high.
Repellent may reduce the chance of future mosquito bites; it does not treat an existing WNV infection or guarantee that infection will not occur. A person with current symptoms should seek medical advice rather than trying to treat the illness with a prevention product.
How common is West Nile disease?
CDC says about 2,000 people in the United States are diagnosed with West Nile each year, and notes that this is an underestimate. Reported counts vary by season and location, and local mosquito activity can change the likelihood of exposure. Regional health departments may issue advice that reflects other mosquito-borne illnesses as well as WNV.
