Common side effects of GLP-1-based weight-loss drugs are often gastrointestinal, but product warnings also cover less common, potentially serious problems. The evidence does not support one side-effect rate or a simple safety ranking for the whole drug class. The right interpretation depends on the specific medicine, dose, indication, patient group and type of evidence.

Which side effects are common?

U.S. prescribing information for Wegovy (semaglutide) and Zepbound (tirzepatide) lists gastrointestinal reactions among common adverse reactions. These products have separate labels and study programs, so their listed reactions should not be combined into a single class-wide frequency.

Wegovy

The U.S. Wegovy prescribing information lists gastrointestinal reactions among common adverse reactions and also discusses severe gastrointestinal reactions. The label is specific to Wegovy and its clinical evidence; it does not establish that every person taking semaglutide will have the same effects or experience them at the same rate.

Zepbound

The U.S. Zepbound prescribing information lists nausea, diarrhea, vomiting, constipation, abdominal pain and dyspepsia among common adverse reactions, along with other reactions. These findings come from Zepbound’s product-specific trials and should not be treated as a direct comparison with Wegovy unless the populations, doses and study methods are aligned.

“Common” is a label category, not a prediction of what one individual will experience. The product label and a clinician can help put a particular medicine, dose and medical history in context.

What serious risks do labels and regulators flag?

Gallbladder disease and pancreatitis

Wegovy and Zepbound prescribing information identifies acute gallbladder disease and pancreatitis as safety considerations. The American Diabetes Association’s Standards of Care in Diabetes—2026, published in 2025, also discusses pancreatitis and biliary disease and advises evaluation or monitoring in relevant contexts. These warnings mean the issues merit clinical attention; they do not show that every product or patient has an identical risk.

On 29 January 2026, the UK Medicines and Healthcare products Regulatory Agency (MHRA) strengthened pancreatitis warnings across GLP-1 and dual GLP-1/GIP agonists. It cited rare reports of severe cases, including necrotising and fatal pancreatitis. The agency cautioned that early symptoms can be hard to distinguish from more common effects: “Pancreatitis may be challenging to recognise in its early stages, as initial symptoms such as abdominal pain, nausea or vomiting may be attributed to other causes such as common gastrointestinal side effects of GLP-1 and GLP-1/GIP treatment or infection.”

Because symptoms can overlap, do not try to diagnose pancreatitis yourself. Seek assessment from a health professional about concerning or persistent symptoms, particularly if you are taking one of these medicines.

Severe gastrointestinal reactions and dehydration-related kidney problems

U.S. product information also discusses severe gastrointestinal reactions and kidney problems associated with dehydration. These are not interchangeable with ordinary nausea or other common reactions: the concern is that severe gastrointestinal symptoms can contribute to dehydration, which may affect the kidneys. For guidance on symptoms or treatment changes, use the current product information and contact a qualified clinician.

Thyroid-tumor warning

A 9 September 2025 FDA warning letter to Eli Lilly discussed the boxed warning and stated that available data were insufficient to establish or exclude a causal relationship between tirzepatide and medullary thyroid carcinoma. That regulatory discussion is not a complete clinical evidence review, so it should not be stretched into a broader conclusion about the medicine or all GLP-1-based drugs.

What does a clinical trial tell us—and what does it not?

In the STEP 1 randomized trial of semaglutide in adults with overweight or obesity, serious adverse events were reported in 9.8% of semaglutide participants and 6.4% of placebo participants. The New England Journal of Medicine’s 2021 report described the difference as mainly due to serious gastrointestinal and hepatobiliary disorders.

Those figures describe the participants and conditions in that particular trial. They are not a class-wide estimate, a lifetime risk, or a forecast for a particular person taking a different product or dose. The trial also reported a small number of mild acute pancreatitis cases; the report noted relevant histories and gallstones among those cases.

Why do warnings, trial results and adverse-event reports differ?

Different evidence sources answer different questions. A warning identifies a safety issue that warrants attention; a clinical trial records outcomes in a defined study population; and a spontaneous adverse-event report records a suspected or associated event. None should be read as if it were the same kind of measurement.

  • Product labels summarize safety information for a specific medicine and its clinical program. A listed reaction or warning is not, by itself, a personal risk estimate.
  • Randomized trials compare outcomes in enrolled participants under specified study conditions. Their results apply most directly to those participants and conditions, not automatically to other drugs, doses or longer use.
  • Regulator communications can strengthen warnings when safety concerns merit attention. They may describe rare reports without establishing an identical risk for all patients.
  • Spontaneous reports can alert regulators to possible problems, but they generally cannot establish how often an event occurs in all users or prove that a medicine caused it.

What is known about compounded drugs and dosing errors?

Reports involving compounded injectable products are a separate issue from controlled-trial results for approved products. The FDA describes dosing errors involving compounded semaglutide, including reports associated with hospitalization, and adverse-event reports associated with compounded semaglutide and tirzepatide. It cautions that a report may not establish that the drug directly caused an event and that such reporting can be incomplete.

As of 31 May 2026, the FDA had received 990 adverse-event reports associated with compounded semaglutide and more than 730 associated with compounded tirzepatide. These are report counts, not incidence rates: they do not show what proportion of users were affected, and the FDA says causality is not always determinable. Do not use them as a direct comparison with trial rates for approved medicines.

Are there signs that risks differ by medicine?

A 2025 systematic review and meta-analysis reported an increase in gallbladder-related disorders, particularly cholelithiasis, in its semaglutide analysis. Its tirzepatide analysis did not find a statistically significant biliary risk. That is a review-level result within the studies it included—not proof that tirzepatide has no biliary risk, that semaglutide has the same risk at every dose, or that findings transfer unchanged to different populations.

Product-specific labels, trial data and reviews do not establish a simple universal ranking of overall safety. A useful comparison must keep the medicine, dose, indication, patient group and evidence type aligned; comparing isolated numbers across different studies can mislead.

What remains unsettled?

Rare events and long-term safety questions are not resolved by a warning, one trial or a collection of spontaneous reports alone. Evidence may differ by medicine, dose, indication, duration and patient characteristics, while reports that raise a concern do not necessarily establish causation or frequency. The practical takeaway is to treat common effects, formal warnings, trial outcomes and suspected-event reports as distinct evidence—not as one definitive estimate of risk.

For a decision about starting, continuing or changing a treatment, discuss your health history, symptoms and other medicines with a qualified clinician and consult the current prescribing information for the specific product.
