What symptoms may occur?
The National Cancer Institute (NCI) lists the following as possible signs of colorectal cancer in children:

- Abdominal pain
- Constipation or diarrhea
- Blood in the stool
- Nausea or vomiting
- Weight loss for no known reason
- Loss of appetite
- A lump in the abdomen
- Anemia, which can cause tiredness, dizziness, a fast or irregular heartbeat, shortness of breath, or pale skin
For adolescents and young adults, the American Cancer Society (ACS) also describes changes in bowel habits, thin or pencil-like stools, blood that may look bright red or dark or black, unexplained weight loss, and low energy or weakness. That is general guidance for this age group, not a rule for diagnosing colorectal cancer in a child.
Symptoms do not tell you whether it is cancer
These signs are not specific to colorectal cancer and may result from other problems. The NCI advises that a doctor determine the cause; the ACS similarly recommends medical evaluation when a sign or symptom does not improve or go away. A symptom list cannot confirm cancer, and one symptom alone does not establish that a child has it.

Patterns described in medical guidance are not a way for families to identify where a tumor might be. For example, the NCI’s professional summary discusses different symptom patterns for right-sided and descending-colon tumors, but those descriptions are clinical context, not a home diagnostic tool.
How rare is childhood colorectal cancer?
In the United States, fewer than 100 children are diagnosed with colorectal cancer each year, according to the NCI’s patient page, updated May 14, 2025. This figure describes U.S. children, not worldwide incidence. Rarity does not mean persistent or worrying symptoms should be ignored; it means symptoms need a clinician’s assessment rather than assumptions based on a checklist.

When should you contact a doctor?
Discuss symptoms that concern you with the child’s healthcare professional, particularly if they persist, do not get better, or go away and return. Share what you have observed and ask what evaluation is appropriate. The available guidance does not set a single symptom threshold that determines when an individual child needs testing; that decision depends on the clinician’s assessment.

The clinician may ask when symptoms started and how often they occur, review the child’s personal and family medical history, and perform a physical examination. Based on those findings, the clinician may recommend further tests. A colonoscopy, for example, allows examination of the colon and rectum and can allow tissue sampling; whether it is appropriate is a medical decision.

What risk factors should families mention?
The NCI identifies some inherited syndromes and inflammatory bowel disease as risk context. These factors do not mean a child will develop cancer, and cases can also occur without a known risk factor. Tell the child’s doctor if the family is concerned about a possible risk or relevant medical history.
