A supplement is sold to support nutrient intake; a prescription drug has product-specific medical uses, directions and safety requirements. The right choice depends on a person’s circumstances and, for prescription treatment, the particular medicine and clinician’s instructions.
What differs: product category, purpose and oversight
The word “vitamin D” describes a nutrient, not one uniform product. Supplements and medicines may use related forms of it, but their labeling and intended use differ. A supplement’s role is nutrient intake. A prescription drug is a labeled medicine used for specific clinical purposes, with directions and warnings that apply to that product.

| Point of comparison | Dietary supplement | Prescription vitamin D drug |
|---|---|---|
| Form | Commonly ergocalciferol (D2) or cholecalciferol (D3), according to the NIH Office of Dietary Supplements. | Depends on the medicine. The cited example is ergocalciferol (D2), according to its DailyMed label. |
| Purpose | Provides vitamin D as part of nutrient intake; whether an individual needs one depends on diet, risk and clinical circumstances. | Uses are specific to the product. The cited ergocalciferol label lists hypoparathyroidism, refractory rickets and familial hypophosphatemia. |
| Directions | Follow the product label and clinician advice; there is no universally appropriate dose for every reader. | The cited label says dosage must be individualized under close medical supervision. |
| Monitoring and safety | Consider total intake, possible adverse effects and interactions with medicines. | The cited label includes calcium and phosphorus monitoring and lists product-specific contraindications. |
The table’s prescription details describe one labeled ergocalciferol product, not every prescription vitamin D formulation. Having vitamin D as an ingredient does not make a supplement a substitute for a prescribed medicine.
D2 and D3, and how the body uses vitamin D
Ergocalciferol is vitamin D2; cholecalciferol is vitamin D3. Both can raise blood levels of 25-hydroxyvitamin D [25(OH)D]. The NIH Office of Dietary Supplements summarizes evidence that D3 generally raises the level more and maintains it longer, but that comparison does not establish one form as the right choice for everyone.

Vitamin D from sunlight, food and supplements is biologically inert until the body processes it. It undergoes hydroxylation first in the liver and then primarily in the kidney. Blood 25(OH)D is the main marker used to assess vitamin D status.
What the cited prescription label says
The DailyMed label for ergocalciferol capsules, 1.25 mg (50,000 USP Units), identifies hypoparathyroidism, refractory rickets and familial hypophosphatemia as indications. These are the listed uses for that product, not a list of indications for every vitamin D prescription.

The label states, “DOSAGE MUST BE INDIVIDUALIZED UNDER CLOSE MEDICAL SUPERVISION,” and warns, “THE RANGE BETWEEN THERAPEUTIC AND TOXIC DOSES IS NARROW.” It also describes monitoring calcium and phosphorus. Do not use those label statements to choose a dose or to self-treat; the prescribing clinician and the label for the actual product govern its use.

More vitamin D is not automatically better. The ODS fact sheet discusses potential interactions with orlistat, statins, corticosteroids and thiazide diuretics. Anyone taking medicines, managing a relevant medical condition or using multiple vitamin D sources should review the combination with a healthcare professional.

How to decide what to do
- If considering a supplement for nutrient intake, account for diet, other vitamin D sources and personal health circumstances; follow its label and seek clinician advice when appropriate.
- If you have a prescription, do not replace it with a supplement, combine products or change how you take it without checking with the prescriber or pharmacist.
- If you are unsure why a prescription was issued, ask the prescriber what condition it treats and what monitoring is needed for that specific product.
