Utah has not given AI a general license to practice medicine. Instead, the state is testing specific healthcare tasks through time-limited agreements with named organizations, defined safeguards and regulatory oversight. An October 5, 2026 announcement described pilots for acne assessment and treatment, pelvic-health physical therapy, and staged prescription refills. How much human review occurs depends on the pilot: the announcement does not establish that any of them currently operates without clinician involvement.
What Utah’s AI healthcare pilot actually is
Utah’s Office of Artificial Intelligence Policy, within the Department of Commerce, can coordinate regulatory mitigation agreements with the state agency responsible for a field. For prescribing pilots, the Division of Professional Licensing must review and approve the arrangement. The agreements provide temporary flexibility for a named product and task; they do not rewrite the underlying rules or grant an AI company a professional license. Utah also says participation is not state endorsement.

On October 5, 2026, the Department of Commerce announced five healthcare agreements: three pilots focused on particular services and two umbrella agreements that create expedited review paths for future pilots. Those future proposals are to be reviewed individually, not automatically approved.

What each announced agreement covers
| Organization or agreement | Task or pathway | Human review and known limits |
|---|---|---|
| Nolla Health | AI-assisted visual evaluations and treatment pathways for mild-to-moderate acne. | The announcement describes specialist oversight. It does not state the pilot’s current phase, enrollment, detailed exclusion rules or outcomes. |
| Expect Fitness | An AI-enabled physical-therapy pathway for postpartum recovery, urinary incontinence and other pelvic-health issues. | The announcement does not specify the exact division of work between AI and clinicians, the current phase, enrollment or outcomes. |
| August AI | A stage-gated pilot for routine refills of non-controlled chronic-disease medications. | Physician review is required before authorization in the described pilot. The announcement does not establish that it has advanced to a later stage. |
| University of Utah Health | An umbrella agreement providing an expedited review pathway for future healthcare AI pilots. | Each future pilot is to be individually vetted; the announcement does not identify a particular clinical task or patient pathway. |
| Intermountain Health | An umbrella agreement providing an expedited review pathway for future healthcare AI pilots. | Each future pilot is to be individually vetted; the announcement does not identify a particular clinical task or patient pathway. |
The state announcement also says participating organizations face patient disclosures about AI use, specialty medical malpractice insurance, 24-hour incident-response plans, and multi-layered state oversight. It says traditional legal remedies remain available to patients.

Can AI prescribe medication in Utah?
Utah’s prescription pilots are limited to refills of medication already prescribed by a licensed physician, not new prescriptions. In the first phase described in the state’s FAQ, a licensed physician reviews each AI-recommended refill before it reaches a pharmacist. An AI system may send a refill directly to a pharmacist only if the pilot meets its benchmarks and the state approves movement to that phase. A pharmacist can escalate a refill to a licensed physician.
The exact formulary depends on the agreement. Utah says the pilots it describes exclude controlled substances, and the October announcement characterizes August AI’s pilot as covering routine, non-controlled chronic-disease medications. The state’s account of the Doctronic pilot says it handles 30-, 60- or 90-day renewals for medications previously prescribed by a licensed provider; it does not issue new prescriptions, handle controlled or addictive substances, or change treatment plans.

These are not interchangeable descriptions of one program. The August AI announcement describes a staged refill pilot with physician review before authorization. Utah’s separate Doctronic page describes its own refill process and phase structure. The state’s general FAQ says direct-to-pharmacist refills are possible only after benchmarks and state approval; it does not establish that either named program has reached that stage.

Utah says the specific formularies and agreement terms vary by pilot. Its policy framework therefore should not be read as a single identical workflow for every company.
How Utah says it will evaluate clinical AI
Utah’s provisional clinical AI standards, authored by Alice Schwarze, PhD, and Zach Boyd, PhD, in August 2026, organize evaluation around three questions:
Rank #4
- REMOTE HEALTH MONITORING - Audar E2 features built-in sensors to track heart rate, blood pressure, blood oxygen (SpO₂), and body temperature automatically at customizable intervals of 30 minutes, 1 hour, 2 hours, or 4 hours, or on demand. View clear daily, weekly, monthly, and yearly trend charts on the Audar Health Platform. Keep your wrist still during measurements and calibrate the device using a blood pressure cuff (not included) for maximum reliability
- AI WELLBEING WEEKLY REPORTS - Reports can be generated and viewed anywhere in the world on Audar Health Platform. Created from the wearer's health data captured by Audar E2, the reports summarize weekly trends and may help identify unusual changes that warrant continued monitoring or further attention. Each user receives 10 complimentary reports, with additional reports available as an optional service for $1.85 each
- EMERGENCY SOS AND FALL ALERT - The watch automatically detects falls and sends alerts, while users can also manually trigger an SOS alert by pressing the panic button. Once activated, the user's location is generated instantly, and alerts can be sent to designated contacts via SMS, voice message, email, or the Audar Health Platform. Email alerts and platform dashboard notifications are available at no extra charge. Optional SMS and voice call message services are available for $1.40 per 4 SMS messages or $1.40 per voice message. Public emergency numbers (such as 999 or 911) cannot be used as alert contacts. Some landlines may not support SMS reception
- AUDAR HEALTH REMOTE PLATFORM - Connects the watch in real-time with unlimited data. Measurement schedules and watch settings can be altered remotely from any internet connected device and viewed by multiple carers in any locations. Vital signs deviations beyond set safety thresholds can be automatically sent as an alert to designated receivers. The wearer only needs to wear the watch for peace of mind of all of your family members
- STAY CONNECTED ANYWHERE - Audar E2 features a built-in SIM that connects directly to low-cost IoT networks in 20+ countries with no roaming charges - no smartphone, app, Wi-Fi, or Bluetooth required. Connectivity is included for the first year, $35 annual subscription thereafter, enabling remote monitoring and alerts wherever network coverage is available
- Is care through the AI pathway not inferior to conventional care?
- Does the system cause substantial bodily or psychological injury?
- Could it improve clinical quality, geographic access or affordability for Utah residents?
The standards call for voluntary, informed participation and clear disclosure that AI is involved and that authorization is temporary. Utah’s third-party evaluator page says the state has agreements with six independent organizations that may review proposals and audit pilots. Their described work includes testing for clinical errors, reviewing privacy, sampling live system activity and checking company reports.
A tamper-evident receipt can show that a specified safety check ran; Utah cautions that this alone does not prove an AI system is safe or that a company complied with the law. Zach Boyd, director of the Office of Artificial Intelligence Policy, summarized the state’s stated principle this way: “Our foundational approach is that technical innovation will never supersede patient safety.”

Why did Utah’s Medical Licensing Board object to Doctronic?
In an April 20, 2026 letter to the Office of Artificial Intelligence Policy, the Utah Medical Licensing Board said it learned of the Doctronic agreement only after implementation, when the system was already live. The board argued that even a refill can require reassessing dosage, side effects, contraindications, drug interactions and whether a treatment is working. It recommended: “It is the strong recommendation of the Utah Medical Licensing Board that this program be immediately suspended pending further discussion.”
That letter is the board’s formal position, not a neutral finding that the pilot caused patient harm. It also reflects a dispute about process and the clinical judgment involved in renewals; it should not be presented as the state’s endorsement or as a final determination about the program.
What is known about the pilots’ status and results?
The October 5 announcement describes the scope of the new agreements, but does not provide each pilot’s current phase, patient count, real-world performance results or detailed operational terms. It therefore does not establish that any pilot has reached autonomous delivery, or that any has demonstrated safety or effectiveness in practice.
